대한초음파의학회지 제25권 제1호 2006 유방촬영술에서는 [5] 특징적으로 정상 유방 실질과 동일한 모 양과 밀도로 보이며 유방초음파에서도 역시 정상 유선 조직과 동일한 고에코 부위가 피부층 바로 아래에 위치하는 것이 보이 게 된다 (Fig. 1).

이 병변들의 유방촬영술 소견 및 초음파 소견은 유방 안에서 생길 때와 동일하다 (Fig. 2)

초음 파에서 정상 림프절은 중심부 지방문이 고에코로 주변부 얇은 피질이 저에코로 보인다 (Fig. 3). 병적인 림프절은 좀더 둥근 형태로 되어 높이와 폭의 비율이 1.5 이하이고 크기가 커지며 (2 cm 이상) 유방촬영술에서는 밀도가 정상 림프절보다 증가 Fig.

2 대한초음파의학회지 제25권 제1호 2006 유방촬영술에서는 [5] 특징적으로 정상 유방 실질과 동일한 모 양과 밀도로 보이며 유방초음파에서도 역시 정상 유선 조직과 동일한 고에코 부위가 피부층 바로 아래에 위치하는 것이 보이 게 된다 (Fig. 1). 부유방에는 정상 위치의 유방에서 생길 수 있는 질환들은 모 두 생길 수 있다. 양성 병변들로는 섬유낭종성 변화, 섬유선종, 유선낭종, 유선염, 엽상종양, 비정형상피세포증식증 등이 보고 된 바 있다 [1-4, 6]. 이 병변들의 유방촬영술 소견 및 초음파 소견은 유방 안에서 생길 때와 동일하다 (Fig. 2). 2. 림프절과 관련된 양성 질환 1) 정상 림프절 유방촬영술에서 림프절은 내외사촬영, 액와첨부촬영 등에서 관찰되며 정상 림프절은 내부에 저밀도의 지방문을 보이는, 경 계가 명확한 난원형의 결절로 보이며 그 크기는 다양하다. 초음 파에서 정상 림프절은 중심부 지방문이 고에코로 주변부 얇은 피질이 저에코로 보인다 (Fig. 3). 병적인 림프절은 좀더 둥근 형태로 되어 높이와 폭의 비율이 1.5 이하이고 크기가 커지며 (2 cm 이상) 유방촬영술에서는 밀도가 정상 림프절보다 증가 Fig year-old woman with accessory breast in both axilla.. ilateral mediolateral oblique mammograms show symmetric glandular tissues (arrows) in both axilla without any discrete masses.. 2) 반응성 림프절 종대(Reactive hyperplasia) 양성 림프절 종대는 만성 혹은 급성 염증, 류마티스, 건선, 피 부경화증, 전신성 홍반성 루푸스와 같은 교원혈관질환(collagen vascular disease), 육아종성 질환, 후천성 면역결핍증, 실리콘 림프절병증등에 의해 생길 수 있다 [7].

전신성 홍반성 루푸스 환자에서도 림프 절 종대는 종종 나타나고 특히 젊은 연령에서 빈번히 보인다. 사르코이드증 등의 육아종성 질환도 전신성의 림프절 종대를 유발할 수 있고 약 반수에서 촉지성 림프절 종대가 생긴다.

유방에 실리콘 삽입물을 시술 받은 환자에 서 삽입물이 파열되거나 현미경학적 실리콘 유출이 있을 경우 액와부 림프절 종대가 생길 수 있고 이 경우는 림프절이 다른 림프절 종대의 경우보다 매우 고밀도로 보여 감별이 된다 [7]. Fig. 3. Normal axillary lymph node.

3 이지영 외 : 액와부의 촉지성 양성 병변의 영상 소견 되어 보이고, 초음파에서는 피질부가 두꺼워지고, 중심부 지방 문이 압박 또는 소실되어 보이지 않을 수 있다. 편심적 피질 비 후(eccentric cortical thickening)가 좀더 악성을 시사하는 반 면, 동심성의 피질 비후는 반응성 림프구 증식(reactive changes)에서 좀 더 보일 수 있다 [7, 8]. 2) 반응성 림프절 종대(Reactive hyperplasia) 양성 림프절 종대는 만성 혹은 급성 염증, 류마티스, 건선, 피 부경화증, 전신성 홍반성 루푸스와 같은 교원혈관질환(collagen vascular disease), 육아종성 질환, 후천성 면역결핍증, 실리콘 림프절병증등에 의해 생길 수 있다 [7]. 림프절 종대는 류마티 스 관절염의 초기 소견중 하나로 환자의 많게는 반수까지 액와 부의 림프절이 커져있고 (Fig. 4), 금침치료(gold therapy)를 받 은 경우 금속의 고밀도가 림프절내에 보여 미세석회화로 오인 되기도 한다 (Fig. 5). 전신성 홍반성 루푸스 환자에서도 림프 절 종대는 종종 나타나고 특히 젊은 연령에서 빈번히 보인다. 사르코이드증 등의 육아종성 질환도 전신성의 림프절 종대를 유발할 수 있고 약 반수에서 촉지성 림프절 종대가 생긴다. HIV 감염환자에서도 전신성의 림프절 종대가 초기에 반응성으로 종 종 보이며 후기에는 다른 감염이나 림프종에의한 림프절 종대 와 감별해야 한다 [7]. 전신성 질환에 동반된 반응성 림프절 종대는 일측성 보다는 양측성으로 생긴다. 유방에 실리콘 삽입물을 시술 받은 환자에 서 삽입물이 파열되거나 현미경학적 실리콘 유출이 있을 경우 액와부 림프절 종대가 생길 수 있고 이 경우는 림프절이 다른 림프절 종대의 경우보다 매우 고밀도로 보여 감별이 된다 [7]. 대한초음파의학회지 제25권 제1호 2006 3) 결핵성 림프절염(Tuberculous lymphadenitis) 모든 폐외 결핵의 30%-50%는 말초 림프절을 침범하며 이 는 소아에서 좀더 빈번히 나타난다. 액와부 림프절의 결핵은 경 부, 쇄골상부, 악하부의 뒤를 이어 약 12%에서 보인다 [9].

초음파에서는 다수의 림프절 을 침범하며, 융합하는 성향이 있다. 결핵성 림프절염은 흔히 파 열되어 농양을 형성하기도 한다 (Fig. 7).

그렇지만 림프절 종대의 원인을 정확히 규명하 기 위해서는 초음파유도하 조직검사등을 통한 병리적 진단이 필요하다. 3. 연부조직 종양 1) 신경 종양 Fig. 5. 49-year-old woman with rheumatoid arthritis and history of gold therapy.

신경집종(Neurilemmoma(Schwannoma))은 신경섬유종 (neurofibroma)과 함께 슈반세포(Schwann cell) 즉 말초신경 초(peripheral nerve sheath)에서 기원한 양성 종양이다. 중년 에 호발하며, 천천히 자라는 종양으로 대부분은 증상이 없다. 척 Fig.

4 대한초음파의학회지 제25권 제1호 ) 결핵성 림프절염(Tuberculous lymphadenitis) 모든 폐외 결핵의 30%-50%는 말초 림프절을 침범하며 이 는 소아에서 좀더 빈번히 나타난다. 액와부 림프절의 결핵은 경 부, 쇄골상부, 악하부의 뒤를 이어 약 12%에서 보인다 [9]. 유 방촬영술에서 결핵성 림프절염은 경계가 명확한 혹은 일부분에 서 경계가 불명확한 고밀도의 림프절 종대로 보인다. 이런 소견 만으로는 림프종이나 전이성 림프절 종대와 감별이 불가능하나 크기가 크고 거친 석회화가 동반된다면 결핵성 림프절염을 좀 더 시사할 수 있다 (Fig. 6) [10]. 초음파에서는 다수의 림프절 을 침범하며, 융합하는 성향이 있다. 결핵성 림프절염은 흔히 파 열되어 농양을 형성하기도 한다 (Fig. 7). 또한 림프절내 건락 괴사 내부의 유리질화나 석회화에 의한 고에코의 점(strong echo)이나 건락괴사(caseous necrosis) 주변의 육아종 조직층 에 의한 얇은 고에코층이 보인다면 좀더 결핵을 시사하는 소견 이 된다 [11]. 그렇지만 림프절 종대의 원인을 정확히 규명하 기 위해서는 초음파유도하 조직검사등을 통한 병리적 진단이 필요하다. 3. 연부조직 종양 1) 신경 종양 Fig year-old woman with rheumatoid arthritis and history of gold therapy. 5 이지영 외 : 액와부의 촉지성 양성 병변의 영상 소견 수신경뿌리(spinal nerve root)와 주요 말초신경(major peripheral nerve)이 흔히 생기는 위치이다. 신경집종은 피막이 있고, 신경이 종괴의 중심외 위치(eccentric position)에 있고, 종종 공동을 형성하는 것이 중요한 소견이다. 따라서 유방촬영 술에서는 경계가 명확한 난원형의 고밀도의 종괴로 초음파에서 는 역시 경계가 좋은 균질한 저에코의 고형성 종괴로 보이고 후 향에코증가를 동반하며 변성에 의해 내부에 낭종성 변화가 보 일 수 있다. 종괴내부에 콜라겐 침착에 의한 거친 에코나 국소 적으로 에코증가 부위가 보일 수 있으며 (Fig. 8) 드물지만 고 에코의 환이 종괴의 변연부가 아닌 내부에 보일 경우 진단적이 다 [2, 12-14]. 또 신경이 종괴의 내부로 지나가는 소견이 보 Rosen 등에 따르면 혈관종은 크게 미만성 혈관종 또는 혈관 종증(angiomatosis)과 국소성 혈관종으로 분류되며 후자는 다 시, 소엽주위형(perilobular type), 실질형(parenchymal type), 비실질 또는 피하형(nonparenchymal or subcutane- ous Fig year-old woman with tuberculous lymphadenitis and abscess formation due to spontaneous rupture. The echogenic area within the tumor is due to collagen deposits (arrowheads). 인다면 신경 종양을 진단하는데 중요한 단서가 된다. 자기공명 영상에서는 경계가 명확하고 T1강조영상에서 중등도 신호강도, T2강조영상에서 주변부는 점액 조직으로 인한 고신호강도, 중 심부는 섬유조직에 의한 불균질의 저신호강도를 보이며 조영증 강이 매우 잘된다 (Fig. 9) [15]. 2) 혈관종(Hemangioma) Fig year-old woman with a palpable abnormality in the left axilla.. 이지영 외 : 액와부의 촉지성 양성 병변의 영상 소견 1) 표피봉입낭종(Epidermal inclusion cyst) 표피봉입낭종은 피부층이나 피하지방층에 생기는 양성 병변 으로 모발이 있는 신체부위 어디서든 생길 수 있으나 두피, 얼 굴, 목, 몸통 그리고 등에 가장 흔히 생긴다.

유방촬영술에서는 경계가 좋은 구 형 혹은 난원형의 고밀도 혹은 등밀도 종괴로 보인다. 초음파에 서는 경계가 좋은 저에코의 종괴로 보이나 내부 성분에 따라서 다양하게 보일 수 있고 피부층에서 생기는 질환이므로 피부층 이 벌어지는 소견을 보인다면 진단적이다 (Fig. 12) [22-25].

Presumptive diagnosis of lipoma can be made on the basis of imaging findings and unnecessary invasive procedure can be avoided. Fig. 12.

7 이지영 외 : 액와부의 촉지성 양성 병변의 영상 소견 1) 표피봉입낭종(Epidermal inclusion cyst) 표피봉입낭종은 피부층이나 피하지방층에 생기는 양성 병변 으로 모발이 있는 신체부위 어디서든 생길 수 있으나 두피, 얼 굴, 목, 몸통 그리고 등에 가장 흔히 생긴다. 이 낭종의 벽은 표 피와 동일한 중층편평상피(stratified squamous epithelium)로 구성되며 피지샘(sebaceous gland)은 포함하지 않는다. 내부에 는 각질 층이 차있고 종종 석회화도 보인다. 이 낭종이 파열되 면 염증성 변화, 이물반응, 육아종성 염증반응을 일으키거나 나 아가 농양을 형성할 수 있다. 유방촬영술에서는 경계가 좋은 구 형 혹은 난원형의 고밀도 혹은 등밀도 종괴로 보인다. 초음파에 서는 경계가 좋은 저에코의 종괴로 보이나 내부 성분에 따라서 다양하게 보일 수 있고 피부층에서 생기는 질환이므로 피부층 이 벌어지는 소견을 보인다면 진단적이다 (Fig. 12) [22-25]. 2) 다발성 피지낭종(Steatocystoma Multiplex) 다발성 피지낭종은 드문 전신적인 피부질환으로 모발피지샘 관연결(pilosebaceous duct junction)에서 과오종성 기형 (hamartomatous malformation)으로 발생하며 내부는 지방성 Fig year-old-woman with nontender palpable mass in the right axilla.. However, various benign conditions in the axilla have been reported and some of them show characteristic imaging findings which allow presumptive diagnosis to be made without invasive procedures. We review various conditions of benign axillary mass and illustrate typical findings in this essay.
