The long life span of the parathyroid cells and the absence of a definite tissue marker for nodular parathyroid gland prohibit the timing of surgical intervention. The discrepancy between proliferation and apoptosis has been proposed as one possible cause of nodular development of the parathyroid gland in patients suffering with renal osteodystrophy. In the present study, we investigated the apoptotic labeling index of the parathyroid tissue in patients with renal hyperparathyroidism. Methods: The parathyroid tissues of 76 patients with renal hyperparathyroidism and those of 33 normal glands were used for determining the level of apoptosis by performing a Tdt-mediated dutp nick end labeling (TUNEL) assay. The patients information was collected by a review of the clinical charts. Statistical comparison was done via two tailed t-tests. Results: The averages of the TUNEL indices were 0.19 in the normal parathyroid glands and 1.84 in the hyperplastic parathyroid glands (P=0.00). The TUNEL index was higher in the oxyphil type of cells than in the chief cells and the water clear cell types (P=0.01). There was statistically significant correlation of the TUNEL index with the duration of the dialysis and less than 10 years dialysis showed a 2.23 index, which was higher than that of the longer term dialysis patients (P=0.00). (Korean J Endocrine Surg 2008; 8: ) Key Words: End stage renal disease, Secondary hyperparathyroidism, TUNEL index, Apoptosis 중심단어 : 말기신부전증, 이차성부갑상선기능항진증, TUNEL index, 세포자가사멸 Department of Anesthesiology and Pain Medicine, Asan Medical Center, University of Ulsan College of Medicine, 1 Department of Surgery, Korea University College of Medicine, Seoul, Korea 서 말기신부전증환자는장기간의신대체요법을시행하면서다양한합병증이발생하게되며, 빈혈, 고지혈증, 영양장애, 골이영양증 (osteodystrophy) 및심혈관질환위험도가증가한다.(1) 장기간의신대체요법환자에서발생하는골이영양증은이차성신성부갑상선기능항진증에의해발생하며, 심혈관계질환의발생과사망률의증가를가져온다.(2,3) 하지만부갑상선세포의성장을조절하는원인은정확히밝혀진것이없고, 미만성증식 (diffuse hyperplasia) 에서결절성증식 (nodular hyperplasia) 으로이행되는과정의세포표지자도알려진것이없다. 신성골이영양증발생시내과적치료로대부분치유되고최근 calcinomimetic의등장으로치료성적이향상되었다고는하지만, 공여신이항상부족한신장이식상황을고려할때부갑상선의수술적치료가필요한경우가약 5 10% 가량된다.(4-8) 최근말기신부전증의치료성적향상과환자들평균수명연장및삶에질에대한관심이높아지면서골이영양증의발생이전에적 론 250

2 이유미ㆍ이재복 : 말기신부전증에의한이차성부갑상선기능항진증환자에서부갑상선조직의세포자가사멸연구 251 절한치료가강조되어수술적치료가필요한환자를구분하려는노력이강조되고있다.(9) 부갑상선조직의증식과세포자가사멸이말기신부전증환자에서부갑상선질환의진행과어떠한연관이있는지는정확히알려진것은없으나이차성부갑상선기능항진증의발변원인의이해와치료성적을향상시킬수있으리라기대한다. 회복될수없는부갑상선의변화를확인할수있는방법을찾으려는노력의하나로본연구는말기신부전증환자에서수술후보관된부갑상선조직에서세포사멸정도를파악하기위해시행되었다. 방법 1) 실험대상고려대학교외과학교실에서 2004년 3월부터 2009년 4월까지말기신부전증에의한이차성부갑상선기능항진증으로치료받던중내과적치료에반응하지않아부갑상선수술을시행받은 76명의환자에서얻은부갑상선조직을이용하였다. 대조를위해갑상선수술중조직검사에서발견된 33개의정상부갑상선조직을함께염색하였다. 환자의연령, 투석기간 ( 년 ) 및재발유무를조사하였고, 진단검사의학결과인칼슘, 인및부갑상선호르몬 (intact PTH, ipth) 와병리조직검사지의세포형태, Ki 67결과를환자임상기록지를후향적으로검토하여조사하였다. 환자의수술적응증은내과적치료에도불구하고 K/DOQI 지침에해당되는 300 pg/ml 이상의 ipth 결과, 칼슘 9.5 g/dl 이상, 임상적으로지속되거나악화되는골증상, 소양증및골외석회화등이었다.(9) 수술은 59예에서부갑상선전절제술과전완부자가이식술을시행하였고, 17예에서국소마취하부분절제술을시행하였다. 국소마취하부분절제는직접피부와경부근막등에 2% 리도카인을주입하여수술전 Sestamibi scan, 경부초음파및컴퓨터단층촬영검사에서확인된증식성병변을일부절제하였고전신마취를시행할수없을정도의심장기능저하및마취과컨설트회신에의하여결정되었다. 2) Tdt-mediated dutp nick end labeling (TUNEL) assay 부갑상선조직의파라핀불록으로 Tissue microarray (TMA) 를 2 mm 두께로제작하였고, 3μm 두께의절편을제작하여슬라이드에부착시켰다. TUNEL assay는다음과같은방법으로시행하였다. 부갑상선조직의파라핀조직은탈파라핀화와탈수를시행하였다. 이후 3% H 2O 2 in methanol로실온에서 5분간블록을시키고, 50μl의 Labeling Reaction Mix (Trevigen, Inc, USA) 을각슬라이드에추가하고 37도씨습온챔버에 1시간동안반응시켰다. 슬라이드를 50μl의 Strep-HRP solution으로덮고실온에서 10분간반응시킨후 37도씨습온챔버에 10분간더반응시켰다. Converter POD로반응시킨후 3, 3-diaminobenzidine과 H 2O 2 로처리하자 horseradish peroxidase 염색된곳이확인되었고, methyl green으로 2분간대조염색하였다. 봉입하여슬라이드를완성한후현미경으로관찰하였다. 3) 면역조직화학염색결과의판정 분명한핵내면역염색을보이는경우만양성으로판정하였고, Image-Pro Plus program (Media Cybernetics, Inc. Silver Spring, MD, USA) 을이용하여슬라이드당최소 1,000개의세포를계수하여결정하였다. 양성염색된세포비율을 labeling index로표시하였고, 맹검법으로 10% 부갑상선조직의반복적계수를통해최초계수의오차를 5% 이내로조절되는것을확인하여타탕성을검증하였다. 4) 통계분석 TUNEL assay의 labeling index를정상부갑상선과증식성부갑상선에서비교하였고, 조직학적형태에서 chief cell, water clear cell과 oxyphil cell에따른비교, 투석연수와의비교, 수술전 PTH 측정치, 재발및병리조직검사지의 Ki 67 labeling index와비교하였다. 통계검증은마이크로소프트엑셀프로그램의 two tailed t test를이용하였고, P값이 0.05 이하인경우를통계적유의하다고판단하였다. 투석연수의비교는 5년이하의비교연구에서대부분의미있는차이를발견할수없고 10년이상의신대체요법환자에서의미있는차이를보이는문헌을근거로, 10년을기준으로하였다.(21) 1) 임상양상 결 부갑상선증식증환자는 29세에서 72세의연령분포를보였으며 ( 평균연령 52.1세 ±5.3), 남자 27명여자 49명이었다 (Table 1). 혈중칼슘측정치는평균 10.6 g/dl이었고, 혈중인측정치는 6.9 mg/dl이었다. 수술전혈중부갑상선호르몬 Table 1. Two tailed t-test for P value. 22명 (28.9%), water clear cell type 23명 (30.2%), oxyphil cell type 31명 (40.8%) 으로 oxyphil cell type이가장많았고, 결절성증식증은 57명 (75%), 미만성증식증은 28명 (25%) 이었다 (Table 2). 수술전투석은대부분혈액투석을시행하였고, 혈액투석기간은평균기간은 13.2년이었고 10년이상환자 57명, 10년이하환자 19명이었다. 병리소견상 Ki 67 labeling index는 1.3에서 6.3까지나타났으며, 정상갑상선조직의 Ki 67의평균은 1.2이었다. 본병원에서는부갑상선조직에대한 Ki 67 염색을모두시행하여이를이용하였고, 정상부갑상선에서의결과는갑상선수술중확인된것이어서갑상선암조직검사에 CK19, HMW-CK, Galectin-3, EGFR, p53, Ki 67 면역조직화학적검사를시행하고있어결과를확인할수있었다. 2) TUNEL index와임상양상의비교분석 TUNEL index (LI) 는정상부갑상선조직에서는 0.19, 증식성부갑상선조직에서는 1.84로확인되었다 (Table 2, Fig 1). 정상부갑상선조직 24예 (66.7%) 에서는양성염색이없었고, 정상갑상선과부갑상선조직에서의 LI 차이는통계적으로유의하였다. 조직학적분류에의한증식성부갑상선의결절성증식증과미만성증식증에서는미만성증식증에서 LI 2.31로결절성보다높았으나통계적으로유의한차이는아니었다 (P=0.12). 증식성부갑상선의세포형태에따른분류에서는 LI 측정치가 chief cell 2.21, water clear cell 1.85, oxyphil cell 1.57로 oxyphil cell type에서가장낮은결과를보였다 (P=0.05). 임상적특성상투석기간이 10년이상인증식성부갑상선조직의 LI는 0.92였고, 10년미만인경우 2.23으로투석기간이짧았던부갑상선절제조직의 LI가높았다 (P=0.00). 수술전 PTH 측정치가 1,000 pg/ml 이상, 재발이없었던경우및 Ki 67 labeling index가낮은증식성부갑 Fig. 1. 4 이유미ㆍ이재복 : 말기신부전증에의한이차성부갑상선기능항진증환자에서부갑상선조직의세포자가사멸연구 253 상선조직의 TUNEL index가높았으나통계적으로유의성은없었다 (P>0.05). TUNEL index는증식성부갑상선조직, chief cell type 및투석기간이 10년미만인조직에서유의하게높아세포자가사멸가증가된것을확인할수있었다. 본연구결과로도시하지않았으나미만형 18예, 결절형 57예로 5년간의임상적재발을비교하면미만형 5명, 결절형 20 명의 PTH 400 pg/ml 이상의상승이확인되었으나 60명 (80%) 의환자는현재내과적치료로유지되고있다. 현재치료는 cinacalcet복용및재수술 8예가시행되었다. 고찰말기신부전증환자는매년증가하고있고최근신대체요법의효과적치료로평균수명이증가하고있어 2006년도대한신장학회보고에의하면남자 5년생존율은 57.9%, 여자 61.2% 로과거에비해향상된생존율을보이고있으며,(8) 신장이식환자가가장좋은생존율을보이고있고당뇨성말기신부전증환자의수명이가장낮았다.(10) 2006년과 2007년의대한미국의신장이식수술현황은매년 20.8% 수준이고, 62.1% 혈액투석, 17.1% 복막투석을시행받고있다 (8). 신장은체내의인배설과비타민 D의 1 alpha hydroxylation을담당하는주요기관으로신기능의감소가말기신부전증환자의골이영양증주요원인이다.(1) 말기신부전증에의한이차성부갑상선기능항진증합병증발생을억제하기위해미국 2002년 K/DOQI 지침서 (9) 는혈중칼슘 g/dl, 혈중인 mg/dl 및혈중부갑상선호르몬측정치를 pg/ml로유지하기위해인산염결합제, 활성화비타민 D 및최근사용되는 carcinomimetics를권장하고있다. 발생한신성부갑상선기능항진증의치료의목표는낭포성섬유성골염과같은골질환진행의예방및이소성석회침착에의한심혈관계질환진행의예방에있다. 2007년 Tominaga 등 (6) 은증식성부갑상선병변은초기에는정상부갑상선엽상구조를유지하며미만성증식을보이고시간이지나면서결절을형성하게되는데결절성증식을이룬후에도초기다클론성를보이다가결국에는단일클론성을보인다고하였다. 결국결절성병변이이루어지면내과적치료로는회복이불가능하여부갑상선절제술을시행해야하는데수술전에임상적특성외에는효과적인구분법이없다는것이문제이다. 본연구는부갑상선조직에서의세포자가사멸과부갑상선증식증의연관성을찾고부갑상선증식증의임상적특성, 진단검사의학적특성, 병리조직학적특성과의연관성을분석하고자하였다. 세포자가사멸과세포증식성은종양의형성에서일정한역할이확인된것으로 2000년 Zhang 등 (11) 의보고에서정상부갑상선조직에서보다부갑상선선종및이차성부갑상선증식증에서높은세포자가사멸을확인하였다. TUNEL labeling assay와 Bcl-2/BAX 면역조직화학적염색에서정상 부갑상선 0.13, 일차성부갑상선선종 0.99, 이차성부갑상선증식증에서 1.20의세포자가사멸을보였고, Bcl-2 발현증가및 Bax 감소를증식성부갑상선조직에서확인하였다. 2008년 Wada 등 (14) 은 TUNEL assay를이용한부갑상선세포자가사멸조사결과정상조직에서가장낮았고미만성증식에서점차증가하고초기결절성증식때가장높았으며, 이후결절성단일클론성부갑상선증식증에서낮아지는점을확인하였다. Wada 등 (14) 은세포자가사멸과동시에세포증식능을 Ki-67로확인하였는데부갑상선증식증의진행에따라증식능의지속적상승과세포자사사멸의증가후감소되는양상을확인하고세포자멸사의변화와부갑상선증식병변의변화와연관이있다고하였다 년 Fukagawa 등은세포자멸사의역할에주목하면서 calcitriol pulse 치료가효과적인세포자가사멸을초래하여초기부갑상선증식증을치료할수있다고하였다. 본연구에서도정상부갑상선조직의낮은세포자가사멸을확인하였고, 증식성부갑상선조직에서도통계적으로유의한세포자가사멸증가를확인하였다. 그러나부갑상선증식증의조직학적분류에의한미만형과결절형에서는 LI의차이를발견할수없었다. 또한수술전부갑상선호르몬측정치, 수술후재발유무와도연관이없었는데이는수술이시행된시기가진행된결절성증식이이루어지고단일클론성을보이는시기로판단되어세포자가사멸이이미감소된상태에서비교가이루어진것으로생각된다. 세포자가사멸은매우짧은시기에이루어져보관중인부갑상선조직에서찾기가어려우며, 급속냉동보관된조직에서세포자가사멸이흔히발견된다는보고도있고, 정상부갑상선조직의부갑상선세포의생존이 20년까지된다는보고도있어세포자가사멸결과에대해서는아직이견이있다.(14,16) 정상부갑상선조직은하나가 mg의무게를가지며, 대부분의세포는 H&E 염색에서짙은핑크색을보이는 Chief cell로이루어져있다. 대부분 PTH를분비하며 glycogen을함유하고있고 glycogen양이증가하면 water clear 모습을보여 water clear cell이라불린다. 적은부분이지만분비과립을함유하지않는 oxyphil cell과 transitional oxyphil cell이존재하며소아시절에는 chief cell이전부이지만나이가들면서점차지방함량과 oxyphil cell이존재한다. 부갑상선증식증이발생하면부갑상선증식은일부부갑상선만커지는비대칭적인성장을보이며 chief cell 의증식, 보다많은 water clear cell 증식이나타나고지방세포는감소한다.(17) 결절성증식증발생과 chief cell과 oxyphil cell의비율이관련이없다는보고가많지만,(18,19) 2000년 Dreuke 등 (20) 은결절성증식병리소견시 oxyphil cell의증가를보고하였고, 2008년 Lomonte 등 (21) 은 calcinomimetic인 cinacalcet의등장후말기신부전증환자를 cinacalcet과정맥 calcitriol 사용군, 정맥 calcitriol 단일사용군및인산염결합약제만사용한환자군으로구분하여부갑상선절제조직의 oxyphil/chief

5 254 대한내분비외과학회지 : 제 8 권제 4 호 2008 cell 비율을비교하였는데 cinacalcet 사용군에서 oxyphil cell 증가및 chief cell 감소를확인하여 oxyphil cell의증가는내과적치료의실패를의미한다고하였다. Wada 등 (18) 도결절성부갑상선증식증에서 oxyphil cell의낮은세포자가사멸과높은세포증식능을확인하였으며, 본연구에서도 oxyphil cell 비율이 76예중 31예, 41% 로나타나결절성증식증에서 oxyphil cell비율이높을수록수술확률이높다는것을확인하였다. TUNEL index의비교에서도 oxyphil cell이 chief cell과 water clear cell type 보다통계적으로유의하게낮아세포자가사멸이감소한다는사실을확인하였다. 신대체요법시행기간과세포자가사멸의관계는일반적으로없다고알려져있으며,(22,23) 말기신부전증의진행에의한골이영양증의발생과시간이경과함에따라부갑상선병변의초기미만형증식에서말기결절성증식으로이행되므로투석기간이길수록진행된부갑상선병변의세포자가사멸이감소는가능하다하겠다. 본연구에서병리적형태인미만성과결절성의세포자가사멸차이는없었으나투석기간을 10년이상으로구분하였을때통계적으로유의하게세포자가사멸이감소하는것을확인하였다. 기간을 10년으로구분한것은본연구대상환자의평균투석기간이 13.2년이었고, 문헌고찰에서의의가없었던경우 5년이내를비교하였던점을고려하여결정하였다.(21) 미만형과결절형의부갑상선증식증의치료결과는의미있는차이는없으며, 재발과유의한차이를보인다는보고도있으나, 본연구에서는확인할수없었다.(6,24) 결 말기신부전증환자의부갑상선조직을이용하여세포자가사멸을나타내는 TUNEL labeling index를조사하였으며, 정상조직에서보다유의하게높은세포자가사멸을확인하였다. 증식성부갑상선조직의세포학적구분을하였을때 oxyphil cell type의비율이 40% 이었고, oxyphil cell type의 TUNEL labeling index가 chief cell type과 water clear cell type 보다높았으며, 투석기간이 10년이상인경우역시 TUNEL index가높았다. TUNEL labeling index는말기신부전증환자의초기부갑상선증식증과진행성부갑상선증식증을구분하는데유용하였으며, oxyphil cell type의부갑상선증식증을구분할수있었다.
