1 서무규 : 한국의색소분아진균증 대한의진균학회지제 15 권제 3 호 2010 초청원고 한국의색소분아진균증 동국대학교의과대학피부과학교실 서무규 = Abstract = Chromoblastomycosis in Korea Moo Kyu Suh Department of Dermatology College of Medicine, Dongguk University, Gyeongju, Korea The dematiaceous fungi, characterized by having melanin in their cell walls, have become significant due to an increasing cause of human skin disease in Korea. Chromoblastomycosis is subcutaneous mycotic disease that contains sclerotic cells or muriform cells and histologically show pseudoepitheliomatous hyperplasia with microabscess, and phaeohyphomycosis is a group of mycotic infections that contain dematiaceous yeast-like cells, pseudohypae-like elements, hyphae or combination of these form in tissue. Ten Korean cases of subcutaneous chromoblastomycosis have been reported to date, four males and five females, ranging in age from 37~68 (mean 53.8 years). Causative organisms of subcutaneous chromoblastomycosis were eight of and one of Phialophora verrucosa. Nine cases of erythematous or verrucous and one case of ulcer were present skin lesion, and most of exposed areas of the body except one case. Patients were treated with itraconazole, terbinafine, 5-flucytocine or amphotericin B. [Kor J Med Mycol 2010; 15(3): ] Key Words: Chromoblastomycosis, Korea 서론색소분아진균증은자연계에부생하는흑색진균 (dematiaceus fungi) 이외상을통해서피부및피하조직에감염을일으키는만성질환으로주로열대또는아열대지역에서보고되고있으며, 원인균으로는 (F.) 가가장흔하고그외에 Cladosporium(Cladophialophora) carrionii, Phialiphora verrucosa, Rhinocladiella aquaspersa, F. compacta, Exophiala(E.) dermatitidis, E. jeanselmei, E. spinifera 등이있다 1~7. 본증은심재성진균에의해서초래되는드문피부질환으로같은동북아지역인일본의보고들 8~10 에비하여국내에서는상대적으로매우드물게보고되었는데, 1990년안과이 11 는임상적조직학적소견으로본증을진단하여보고하였으며, 1996년서등 12 이처음으로원인균을동정하여보고한이래로지금까지총 10예가보고되고있다 11~20. 저자는주로국내에보고된색소분아진균증을중심으로소개하고자한다. 접수일 : 2010 년 6 월 3 일, 수정일 : 2010 년 8 월 28 일최종승인일 : 2010 년 8 월 30 일 별책요청저자 : 서무규, 경북경주시석장동 , 동국의대부속경주병원피부과전화 : (054) , Fax: (054) mksuhmd@hanmail.net 본 론 색소분아진균증은대게피부및피하조직에국한되어발생하는만성진균질환으로구성성분

2 대한의진균학회지제 15 권제 3 호 2010 으로세포벽에멜라닌색소가있는공통된특징을가진흑색진균에의해서야기된다. 감염은외상을통한피부에균의침범으로발병하며비록일부증례에서다른동물에대한감염이보고되고있지만대부분의감염은인간에게발생한다 2,21. 본증은모든대륙에서진단되고있지만주로아메리카, 아시아, 아프리카의습한열대와아열대지방에서더빈번히보고되고있으며 1~3, 원인균으로는 (F.) 가가장흔하고그외에 Cladosporium(Cladophialophora) carrionii, Phialiphora verrucosa, Rhinocladiella aquaspersa, F. compacta, Exophiala(E.) dermatitidis, E. jeanselmei, E. spinifera 등이있다 1~7. 국내에서도 F. 가 7예 12~16,19,20 로가장흔한원인균이었으며 Rhinocladiella aquaspersa 17 와 Phialiphora verrucosa 18 가각각 1예보고되었다. 본증은일반적으로여자보다남자에서더흔하며모든연령층에서발생이가능하나주로 30~50대에호발하며농사일에종사하는남자에서주로발생한다 1~4,7. 지역에따라성별과연령의분포에차이를다소보이는데, 남미의보고예 22,23 에서는 30~50대의남자에서주로발생하는것과는달리일본의보고예 8,9 에서는남녀의비율이거의동일하게보고되고있으며 50대이상에서더빈번하게보고되고있다. 국내에서는본증례를포함하여남자 4예여자 5예로남녀비가비슷하였으며 2예를제외하고는모두 50대이상에서발생하여남미보다는일본과유사한연령및성별분포를나타내었다. 본증은일반적으로상지보다는주로하지에발생하며피부발진은초기에는대게홍색구진으로시작하여점차우췌형결절이나판과같은다양한형태를나타낸다 7 (Fig. 1). 국내에서는하지보다는상지에서더 Table 1. (SDA, 3 weeks, 25 ). 많이 발생하였으며, 남자에서는 하지에서 더 많 이 발생하였고, 여자에서는 상지에서 호발하였다. 임상양상은 각각 홍반성 판형이 4예, 우췌양 판 형이 4예로 대부분 홍반성 또는 우췌양 판형이었 다 (Table 1)11~20. 이는 중남미의 보고들22,23보다는 일본의 보고들8,9의 보고와 비슷한 것으로 지역에 따라 차이가 있는 것으로 여겨지며, 이런 차이는 각 나라의 산업화와 삶의 양식의 차이에서 기인 하는 것으로 생각된다. 병리조직학적 소견은 표피의 위상파종성 과형 성 및 미세농양과 진피내 염증세포와 거대세포 Fig. 5. (Lactophenol cotton blue stain, 400). 로 구성된 만성 육아종성 염증을 보이고 특징적 으로 진피내 거대세포 혹은 그 주변에 큰 구형의 (Fig. 2, 3). 두터운 벽을 가지고 분할을 보이는 소위 경화세 진균학적검사 소견은 KOH 검사상 경화세포가 포를 보이면 본 증으로 진단한다. 또한 경화세포 나타나며 생검조직은 사부로 배지에 25 에서 는 무염색 표본에서도 쉽게 관찰할 수 있다12,24 수 일간 배양하면 서서히 발육하는 흑갈색 집락

4 대한의진균학회지제 15 권제 3 호 2010 을볼수있고이를검경하면포자형성에따라 3형, 즉분생자가생겨서연쇄상으로연결되는 Cladosporium형, 분생자병의끝이나옆을따라포자가형성되는 Rhinocladiella형, Phialide라는플라스크형의분생자병의끝에포자가형성되는 Phialophora형을관찰할수있다 2,3 (Fig. 4, 5). 최근에 de Hoog 등 25 은 속을진균핵내의 ITS 부위의염기서열및계통발생분석으로 F. 를 F. monophora로나누어 F. monophora 를처음명명하였으며 A, B, C 3군으로나누었고, 2007년 Yaguchi 등 10 은 ITS부위의염기서열및계통발생분석으로 F. 와 F. monophora를각각 A1과 A2의 2군으로그리고 B1, B2, B3의 3군으로더세분하여 A1군은태국, 호주, 남미, 아프리카균주, A2군은중남미균주, B1군은중남미, 아프리카균주, B2군은일본, 중국균주, B3군은미국, 영국균주들이속하여지역적으로분류하였다. 치료는외과적절제술과피부이식을하는것이일차적인방법이며그외국소온열요법, 냉동수술, itraconazole, 5-fluorocytosine, 또는 amphotericin B 등의투여가있다 1~5. 효과적인치료는대게국소재발을예방하기위해작거나국소적인병변에넓은마진을동반한외과적절제이지만본증의깊거나광범위한침범으로종종적합하지못하다 5. 결론그동안국내피부과에서는심재성진균증으로대부분스포로트리쿰증 (Sporotrichosis) 으로생각하였고흑색진균에의한감염증에대한인식이부족하였고또한흑색진균동정에관심이적었던것이사실인것같다. 최근 1996년이후부터국내에서도색소분아진균증이많이보고되고있으므로향후보다많은증례를발견할수있을것으로생각된다.
