In this study, we evaluated the usefulness of a selective chromogenic agar medium and of multiplex PCR for detection of VRE, and both these techniques were compared with the conventional culture method for VRE detection. Methods : We performed the following 3 methods for detecting VRE infection in stool specimens: the routine culture method, culturing in selective chromogenic agar medium (chromid VRE, biomérieux, France), and multiplex PCR using the Seeplex VRE ACE Detection kit (Seegene Inc., Korea) with additional PCR for vanc genes. Results : We isolated 109 VRE strains from 100 stool specimens by the routine culture method. In chromid VRE, all the isolates showed purple colonies, including Enterococcus gallinarum and E. raffinosus, which were later identified using the Vitek card. All VRE isolates were identified by the multiplex PCR method; 100 were vana-positive E. faecium, 8 were vana- and vanc-1-positive E. gallinarum, and 1 was vana-positive E. raffinosus. Conclusions : For VRE surveillance, culturing the isolates in chromid VRE after broth enrichment appears to be an accurate, rapid, and easy method for routine screening test. Multiplex PCR is relatively expensive and needs skilled techniques for detecting VRE, but it can be an auxiliary tool for rapid detection of genotype during a VRE outbreak. VRE의정확한조기검출은병원내감염전파의추적과관리를위해서뿐만이아니라, 반코마이신내성황색포도알균 (vancomycin-resistant Staphylococcus aureus, VRSA) 의출현을막기위해서도중요하다 [2, 6]. 또한 VRE가분리되는환자에 631

2 632 Korean J Lab Med 2010;30:631-6 있어서는장내집락화의유무를조기에검출하여사람간의전파방지및격리를해야한다. 그러므로대변에서의 VRE 선별검사는 VRE의조기검출을위한여러가지선별배지를이용하는것과최근에신속증균 PCR법을사용하는방법등이소개되고있다 [1, 4, 6-11]. 이에본연구는 VRE 감시배양을위해의뢰된대변검체를고식적인배양법과더불어고체선택배지사용법, 그리고다중 PCR법으로신속동정을동시에시행한후각방법에대해비교평가를하고자하였다. 대상및방법 1. 대상검체본원에서 2009년 8월부터 2010년 2월까지 VRE 감시배양을위해진단검사의학과미생물검사실로의뢰된대변검체총 109 개를대상으로하였다. 2. 방법의뢰된대변검체를우선자가제조한액체배지 (6 mg/ml의 vancomycin이들어있는 enterococcosel broth) 에접종한후에37 에서증균배양후 1일째와 2일째각각배지의색상변화를판독하였다 [4, 8, 10]. 그중검게변한배지를선별하여 VRE 검출을위해아래와같은 3가지의방법을병행하였다. 먼저고식적인배양법을사용하였는데, 검게변한액체배지를혈액한천배지에접종한후 37 에서하룻밤배양하였다. 배지에자란균집락을그람염색을하여그람양성알균인것을확인하고 catalase 시험음성임을확인하였다. 이후 Vitek 2 GP 카드와 AST-P600 카드 (biome@rieux VITEK, Hazelwood, MO, USA) 를사용하여동정과항균제감수성검사를하였다. Enterococcus gallinarum 또는 Enterococcus casseliflavus이동정되었을때에는운동성과색소생성시험을시행하여감별하였다. 균동정시최종확인이필요할경우, 16S rrna 유전자에대한전체염기서열분석 (MACROGEN, Seoul, Korea) 을실시하였다. 두번째방법으로는 chromid VRE (biome@rieux, Marcy-l Etoile, France) 고체선택배지를사용하였다. 액체배지가검게변한것을확인한후내용물을 chromid VRE에접종하고 37 에서 1일배양후특정색소를띠는전형적인장알균집락을 VRE 로 1차동정하였다 ( 보라색 : E. faecium, 청록색 : Enterococcus faecalis). 그람염색과 catalase 시험후최종적으로 Vitek 2 GP 카드와 AST-P600 카드 (biome@rieux VITEK) 를사용하여생화 학적동정과항균제감수성검사를시행하였다. 세번째검출방법인다중 PCR은 vana와 vanb 유전자에특이적인시발체를이용하는 Seeplex VRE ACE Detection 키트 (Seegene Inc., Seoul, Korea) 를사용하였다. 검게변한액체배지 50 ml 와키트에포함된 DNA extraction solution 100 ml 를섞은뒤끓는물에 10분간중탕후 13,000 rpm으로 5분간원심분리하여 DNA를추출하였다. 이후 Seeplex VRE ACE Detection키트를사용하여총 PCR 반응액 20 ml (DNA 1 ml, 5 VRE primer 4 ml, 8-methoxypsoralen 3 ml, 2 Multiplex Master Mix 10 ml) 를제조한다음 PCR 반응을실시하였다. PCR 반응은 94 에서15분반응후94 30초, 60 1분, 72 1분의반응을 35회반복하였고최종연장반응을 72 에서 10분간시행했다. 최종산물은검체간상호오염을방지하기위해 20 분간자외선 (365 nm) 을조사한후에 ethidium bromide 를포함한 2% 아가로오즈겔에서전기영동하여 360 bp (vana) 와 250 bp (vanb) 에해당하는밴드의유무를판독하였다. 양성대조균주로 E. faecium BM4147 (vana), 음성대조균주로 E. faecalis ATCC 29212를사용하였다. 균동정결과 E. faecium, E. faecalis 이외의균 (E. gallinarum, E. casseliflavus, Enterococcus flavuscence) 이나왔을때는 Table 1에제시한시발체를추가로이용하여 vanc-1, vanc-2 및 vanc-3 유전자의유무를확인하였다 [2, 4]. PCR은추출한 DNA 0.5 ml, 시발체 (20 pmol) 2 ml, Smart Taq Pre-Mix (Solgent, Daegeon, Korea) 17.5 ml 를혼합한반응액을 94 에서 5분반응후 94 30초, 45 45초, 72 1분의반응을 30회반복하였고최종연장반응을 72 에서 5분간시행했다. 증폭된산물은전기영동후자외선조영하에판독하였다. 결 총 109검체가운데선택증균배지인 enterococcosel broth가검게변하지않은 8개의검체를제외한나머지 101검체를대상으로실험을진행하였다. 고식적인배양법으로최종동정한결과 Table 1. Vancomycin에감수성을가진 E. casseliflavus가동정된 1검체를제외한 100개의검체에서총 109균주의 VRE가분리되었다 (Table 2). 각균주에대한항균제감수성검사결과, vancomycin과 teicoplanin에대한 MIC는 E. faecium 88주 (80.7%) 와 E. gallinarum 8 주 (7.3%) 에서 32 mg/ml 이상으로고도내성을나타냈다. E. faecium 8주 (7.3%) 는 vancomycin에대한 MIC가 32 mg/ml 이상으로고도내성을보였지만, teicoplanin에대한 MIC는 16 mg/ml로중등도의내성을보였다. 나머지 4주의 E. faecium Table 2. (3.7%) 은 vancomycin에대한 MIC가 32 mg/ml 이상으로나타났지만, teicoplanin에대한 MIC는 4 mg/ml로감수성을보였다. E. raffinosus 1주의경우항균제감수성시험을 vancomycin 30 mg, teicoplanin 30 mg 디스크를써서실시한결과, 억제대의지름이 vancomycin과 teicoplanin 모두 6 mm였다. E. casseliflavus로동정된 1주의경우, vancomycin에대한 MIC가 4 mg/ml, teicoplanin은 1 mg/ml이었다. ChromID VRE 배지를이용한경우, 전혀균이자라지않은 1검체를제외한나머지 100검체 (99.0%) 에서모두 E. faecium 을의미하는보라색의전형적인균집락이관찰되었다. ChromID VRE 배지에서자라지않은 1검체는고식적인배양법에서확인한결과, vancomycin에감수성을보인 E. casseliflavus였다. Seeplex VRE ACE Detection 키트를이용한 PCR에서 109 주의모든 VRE 균주에서 vana 유전자형을나타내었다. 그중고식적인배양법으로동정하여 E. faecium 이외의균으로확인된 9주 (E. gallinarum 8주 +E. raffinosus 1주 ) 를대상으로 vanc-1, vanc-2와 vanc-3 유전자를포함한 PCR을추가로실시한결과, 8주의 E. gallinarum는 vanc-1 유전자도양성이었다 (Fig. 1). 각검사방법에따른총검사소요시간을비교하여보면, 고식적인배양법의경우선택액체배지에접종할때부터최종결과까지 3일정도소요되었고, chromid VRE는 2일째에 VRE 를검출할수있었다. 다중 PCR법은액체배지에증균후바로검사에들어가면 1일이내에검사결과를얻을수가있었다. 고찰 VRE는국내의중환자실이나혈액종양병동등에서토착화되 M M vanc-1 Internal control vana vanb A B Fig. 1. VRE의유행과전파를막기위한감염관리지침이있으나국내의모든병원에서적용하기가어려운실정이고다각적감염관리를시행하여도근절하기힘든병원감염의주요다제내성균이다 [12]. 특히 VRE는메티실린내성황색포도알균 (methicillin-resistant Staphylococcus aureus, MRSA) 으로내성유전자를전달할수있기때문에, MRSA가토착화되어있는국내병원에서는 VRE의신속하고정확하면서도비용효율적인검출이매우중요하다. VRE의신속한검출을위하여선택액체배지에증균을시키면그람음성막대균이나효모균등에의한오염이억제되므로검체를직접고형배지에접종하거나 PCR을시행하는것보다결과가우수하다고보고하였다 [1, 4, 8, 10, 13]. 이에본연구는모든검체를선택액체배지에서증균시킨후각방법의실험을진행하도록하였다. 선택액체배지에서검은색으로변한검체들중 1 검체에서고식적인배양법결과 vancomycin에감수성인 E. casseliflavus 이자라서위양성 VRE로분류하였다. 이검체를 chromid VRE에접종한결과음성이었는데, 이는선택액체배지와 chromid VRE의 vancomycin 함유량의차이 ( 선택액체배지 : 6 mg/ml, chromid VRE: 8 mg/ml) 때문이었다. 이번연구에서 VRE 양성률이통상적인 VRE 양성률보다높게나타난것은이전에 VRE가반복적으로동정이되었던환자의 VRE 선별검사가의뢰된검체만을대상으로하였기때문이다. ChromID VRE 배지를이용한경우, 보라색으로자란균주들은모두E. faecium으로생각되었으나, 혈액한천배지에계대배양하여고식적배양법으로동정한결과 9개의검체에서 E. faecium과 E. gallinarum 혹은 E. faecium과 E. raffinosus가같이존재하는것으로나타났다. Delmas 등 [4] 은 chromid VRE 배지에서 E. faecium 집락의모양이효모균과그람음성막대균의집락모양과비슷하여혼동될수있지만 E. gallinarum, E. casseliflavus, 그리고그람양성막대균의모양과는달라서구분이가능하다고하였다. ChromID VRE 배지에서자란균집락의모양을면밀히살펴본결과, 각장알균균종들간의차이를육안으로식별해내기는어려웠지만고식적배양법과다중 PCR 법을시행한결과 chromid VRE 배지에서보라색으로자란모든균주가 vana 유전자를가진 VRE로나타났다. 따라서혈액한천배지에계대배양하여고식적인배양법이나장알균간이동정법으로확인하지않고서도 chromid VRE 배지만으로 vancomycin 내성유무를빠르고정확하게선별하기에충분한것으로생각되었다. 한편, Seeplex VRE ACE Detection 키트를사용하여 PCR 을시행한결과모두음성으로나왔으나, Drews 등 [14] 이제시 한방법을인용하여핵산을다시추출하고주형 DNA 농도를지침보다 3배희석하여 PCR을시행한결과위음성문제가해결되었다. 본연구에서분리된 109주의 VRE 균주는모두 vana 유전자를가진것으로나타났다. 동시에분리된 E. gallinarum 8 주의경우, 모두 vana와 vanc-1 유전자형을함께지닌 vancomycin 고도내성균주로나타났다. E. gallinarum이분리된환자들을분석한결과, 대부분이장기입원한환자들로각각다른병동에입원하고있었으며 VRE에의한감염이없는상태이었으므로 VRE의집락화로판단된다. 또한, E. faecium이계속해서나오던상태에서 vana를가진e. gallinarum이분리되었으므로 Hsueh 등 [15] 과 Sung 등 [16] 의주장과같이종간에 vana 유전자의수평전파가이루어진것으로생각된다. 분리된 VRE 균주들의표현형을분석한결과, VanA가 97 주, VanB가 12 주로확인되었다. vana 유전자형에 VanB 표현형을가진 VRE가최근몇년간아시아에서증가하는추세를보이고, 국내에서는 vans 유전자의코돈영역에 IS1216V를가지는것으로인해 teicoplanin에대한내성변이가일어난것으로설명하는보고도있다 [17]. 본원의경우이번연구를통하여 vana 유전자형에 VanB 표현형을가진 VRE가꾸준히분리되는것으로나타났으므로추후에타병원을포함하여더많은검체들을대상으로한분자역학적연구를통하여지역적인확인이필요할것으로생각된다. 결론적으로, VRE 감시배양에서선택액체배지에서증균배양이후 chromid VRE 배지를이용하는방법은집락의색깔로 VRE를확인하므로쉽고정확하게구분할수있으므로검사실의통상적인선별검사로서비교적빠르고비용효율적인방법으로생각된다. 다중 PCR법은고비용, 인력소모적인방법이나검사결과의확인이하루만에가능하고민감한방법으로내인성저도내성장알균의표현형이고도내성을보일때와 VRE 대유행시내성유전자형의정확한확인으로신속한감염관리를대처할수있는방법으로유용하리라생각된다. 요약배경 : VRE의정확한조기검출은병원내감염전파의추적과관리를위해매우중요하다. 본연구에서는색소생산성고체선택배지와다중 PCR에대한평가를시행하였고각방법을 VRE 검출을위한고식적인배양법과비교평가하였다. 방법 : 대변검체들에대해세가지방법으로 VRE 동정시험을실시하였다. 고식적인배양법과 chromid VRE (biome@rieux, France) 배지를이용한색소생산성고체선택배지법, 그리고
