The 3 coding region of the coagulase (coa) gene contains varying numbers of 81 bp tandem repeats. S. aureus produces a variety of extracellular protein toxins. Here, we typed S. aureus strains isolated from blood by coa gene restriction fragment length polymorphism (RFLP) patterns and toxin gene profiles. Methods : A total of 120 strains of S. aureus were isolated from blood cultures during at Kangdong Sacred Heart Hospital. The isolates were typed by PCR RFLP analysis of the coa gene and by multiplex PCR for detection of genes encoding enterotoxins (sea, seb, sec, sed, and see), toxic shock syndrome toxin-1 (tst), exfoliative toxins (eta and etb), meca and fema. Results : All the S. aureus strains were classified into 16 types on the basis of coa gene RFLP and could be further differentiated into 34 types according to the combined patterns of coa gene RFLP and toxin gene profiles. Of 85 methicillin-resistant S. aureus (MRSA) strains, 43 (50.6%) and 36 (42.4%) belonged to the RFLP pattern L5 and pattern L1, respectively. MRSA strains belonging to pattern L5 frequently carried tst (93.0%) or sec gene (81.4%), and strains belonging to pattern L1 frequently carried sea (88.9%) or see gene (44.4%). The rate of the pattern L5 in MRSA strains increased over the past few years and was higher in intensive care unit than in other wards. Conclusions : We typed S. aureus strains isolated from blood on the basis of coa gene RFLP and toxin genes. The strains belonging to coa gene RFLP pattern L5 and L1 appeared to be the major types of MRSA isolasted from bacteremia and revealed specific toxin gene profiles according to the coa gene RFLP patterns. 2 Coagulase Gene Typing of S. aureus 287 의한병원감염은전세계적으로중요한문제가되었으며, 국내전국의료기관에서분리된 S. aureus 중 MRSA는 68.8% 로보고된바있다 [1]. S. aureus에의한균혈증의경우그원인의삼분의이가병원감염과관련있는것으로알려져있으며 [2], 이러한병원감염을차단하고예방하기위하여검출되는균주들에대한역학적분석이요구된다. S. aureus 균주의역학적분석을위한기법으로는항균제내성양상, 혈청형, 리보타이핑, 파지타이핑, pulsed-field gel electrophoresis (PFGE) 및 PCR에기반한방법등이사용된다. PCR에기반한방법은 16S에서부터 23S까지의 rrna intergenic spacer region, coagulase 유전자및 protein A 유전자등을이용한다 [2]. Coagulase는모든 S. aureus 균주에서생산되는세포외단백으로검사실에서 S. aureus를동정하는데있어중요한기준으로사용되고있다 [3]. Coagulase는사람프로트롬빈을활성화시켜혈전을생성하는데, 그병독성에대해서는논란이있다 [4]. Coagulase를암호화하는유전자의 3 염기말단부위에는 81 bp의반복되는일련의염기서열들이존재하는데, 균주에따라그반복되는수와염기서열에차이를보인다 [5, 6]. 이러한 coagulase 유전자의다형성에기반하여유전형을분석하기위해 PCR 및제한효소절편길이다형성 (restriction fragment length polymorphism, RFLP) 법을사용한여러보고들이있다 [7-11]. S. aureus는 30 여개이상의외독소들을생산하며, 대표적인외독소로는장독소 (staphylococcal enterotoxin, SE), toxic shock syndrome toxin-1 (TSST-1) 및표피박리독소 (exofoliative toxin, ET) 등이있다 [12]. SE는다섯개의주요항원형 (SEA, SEB, SEC, SED 및 SEE) 을포함하여최근까지 19개의항원형이알려져있으며 [13], TSST-1는 1개의혈청형이알려져있다. ET는사람에게서 staphylococcal scaled skin syndrome (SSSS) 의원인이되며, 생물학적활성도는같지만그항원형에서 ETA와 ETB로분류된다 [14]. 이러한독소의검출을위한 DNA-DNA 교잡법이나다상중합효소연쇄반응 (multiplex PCR) 과같은분자유전학적방법들은통상적인면역학적방법의검사보다빠르고민감하며, 독소생성능의발현과상관없이균주의유전정보를제공하는장점을가진다 [15, 16]. 본연구에서는 2003년부터 2006년까지일개 3차병원입원환자의혈액에서분리된 S. aureus 균주들을대상으로 coagulase 유전자 3 말단의다형성부위를 PCR RFLP법으로분석하고, 독소유전자및 meca 유전자를다상중합효소연쇄반응법으로검출하여, 그유전형양상을파악하고자하였다. 대상및방법 1. 균주수집과검체정보 2003년 1월부터 2006년 12월까지한림의대강동성심병원의검사실에의뢰된혈액배양검체중동일환자에서 2번이상분리되어균혈증의원인으로의심되는 S. aureus 120주를대상으로하였다. 균주의동정은 coagulase, DNase 및 mannitol salt agar 검사와 Microscan WalkAway 96 (Dade Behring Inc., West Sacramento, CA, USA) 을사용하였으며, 항균제내성검사는 Microscan WalkAway 96 (Dade Behring Inc.) 을사용하였다. 분리된균주는탈지분유혼탁액에넣어 -70 C 에보관하였다. 검체가의뢰된환자연령의중앙값은 61.4세이며, 6개월에서 94세까지다양한연령층을포함하였다. 환자의성별은남자 69 명, 여자 51 명이었다. 일반병동에서 70주, 중환자실에서 47주가분리되었으며, 3주는의뢰처가확인되지않았다. 연도에따라 2003년에 21주, 2004년에 42주, 2005년에 24주, 2006년에 33 주가수집되었다. 2. DNA 추출 -70 C 에보관되었던균주를혈액한천배지에접종하여 37 C, 5% CO 2 조건에서 24시간배양후, InstaGene Matrix (Biorad, Hercules, CA, USA) 을사용하여제조사의지침대로 DNA 를추출하였다. 3. Coagulase 유전자 (coa) PCR RFLP 시발체는 coagulase 유전자의 3번염기말단부위내의반복되는부위들을포함하면서시발체내에다형성부위가포함되지않도록기존에고안된것을사용하였으며, 염기서열은 COA-1 (ATAGAGATGCTGGTACAGG) 과 COA-2 (GCTTCCGAT- TGTTCGATGC) 이다 [9]. 반응액은 10 buffer 5 μl, 각각 2.5 mm의 dntp 4 μl, 20 pmol의각시발체 (COA-1, COA-2), 1.25 U의 Taq polymerase (Roche Diagnostics, Indianapolis, IN, USA) 및 InstaGene (Bio-rad) 으로추출한 DNA template 10 μl 로구성하여총 50 μl 를만들었다. 반응은 Mastercycler gradient (Eppendorf AG, Hamburg, Germany) 를사용하여 94 C 에서 45 초간전변성후, 94 C 20초, 57 C 45초, 70 C 15초주기로 35회반복하였으며, 최종연장반응은 72 C 에

Multiplex PCR를통한독소유전자및mecA 유전자검출시발체는 Mehrotra 등 [17] 의방법에따라두가지의조합 (set A, set B) 으로구성하였다. Set A 조합의반응액은 10 buffer S M L XL 10 μl, 각각 2.5 mm의 dntp 4 μl, 0.5 mm의 MgCl 2 2 μl, 20 pmol의각시발체 (SEA, SEB, SEC, FEMA), 40 pmol의 SED 시발체, 60 pmol의 SEE 시발체, 1.25 U의 Taq polymerase (Roche Diagnostics), DNA template 10 μl 로구성하여총50μL 를만들었다. Set B 조합의반응액은 10 buffer 5 μl, 각각 2.5 mm의 dntp 4 μl, 0.5 mm의 MgCl 2 1 μl, 20 pmol 의각시발체 (TST, ETB, MECA, FEMA), 50 pmol의 ETA 시발체, 1.25 U의 Taq polymerase (Roche Diagnostics), Insta- Gene (Bio-rad) 으로 DNA template 10 μl 로구성하여총 50 μl 를만들었다. 반응은 Mastercycler gradient (Eppendorf AG) 를사용하여 94 C 에서5분간전변성후, 94 C 1분, 53 C 1분, 72 C 30초주기로 35회반복하고, 최종연장반응은 72 C 에서 7 분간실시하였다. 반응산물은 3.0% 아가로오즈겔에넣어 1 TAE buffer, 100 V에서 35분동안전기영동후확인하였다. 결 과 1. coa 유전자크기의다양성 S. aureus 120주모두에서 coa 유전자가검출되었으며, Ishino 등 [10] 이정한크기분류기준에따라 S, M, L, XL로분류하였고 (Fig. 1), 각각 6주 (5%), 13주 (10.8%), 97주 (80.8%), 4주 (3.3 %) 가해당하였다. 4 Coagulase Gene Typing of S. aureus 289 며, S 크기의 coa 유전자는 3가지 ( 유형 S1-S3), M 크기는 5가지 ( 유형 M1-M5), L 크기는 7가지 ( 유형 L1-L7), XL 크기의경우 1가지 ( 유형 XL 1) 의유형을나타내었다. 120주중 RFLP 유형 L1 과유형 L5에해당하는균주가각각 37주 (30.8%) 와 46 주 (38.3%) 로높은빈도를차지하였으며, 나머지유형들은 1주에서 8주까지해당하였다 (Table 1). 있었으며, sea는 49 주 (40.8%), sec는 40 주 (33.3%), see는 24주 (20.0%), tst는 47주 (39.2%) 에서검출되었다. seb, sed, eta 또는 etb가검출된균주는없었다. fema는 120주모두에서검출되었으며, meca는 85 주 (70.8%) 에서검출되었다. 4. 메티실린내성에따른 RFLP 유형과독소유전자양상 3. 독소유전자양상및 meca 유전자의검출 120 주중 94 주 (78.3%) 에서하나이상의독소유전자를갖고 Microscan WalkAway 96 (Dade Behring Inc.) 의항균제내성검사에서 MRSA는전체 120주중 85주 (70.8%) 였으며, 모두 meca 유전자가검출되었다. 5 290 김용균 김재석 김한성외 3 인 S2, L1, L2, L4, L5를나타냈으며, 각각 1주 (1.2%), 1주 (1.2%), 36주 (42.4%), 3주 (3.5%), 1주 (1.2%), 43주 (50.6%) 였다 (Table 1). MRSA 85주중 76주 (89.4%) 에서하나이상의독소유전자를갖고있었으며, sea, sec, see, tst가각각 35 주 (41.2%), 38주 (44.7%), 16주 (18.8%), 43주 (50.6%) 에서검출되었다. MRSA 중주요유형인 L1과 L5 에서각각특정독소유전자의빈도가높았는데, 유형 L1의 36주중 32 주 (88.9%) 에서 sea를갖고있었으며, 이중 16주 (44.4%) 에서는 sea와동시에 see를갖고있었다. 유형 L5의 43 주에서는 40주 (93.0%) 에서 tst를갖고있었으며, 이중 34 주 (79.1%) 에서 tst와동시에 sec를갖고있었다. Methicillin-susceptible S. aureus (MSSA) 35주는 RFLP 유형 S3, M1, M2, M3, M4, M5, L1, L3, L5, L6, L7, XL1를나타냈으며, 메티실린내성균주에비해상대적으로다양한분포를보였다 (Table 1). MSSA 35주중 18 주 (51.4%) 에서하나이상의독소유전자가검출되어 MRSA에비해낮은빈도를나타냈으며, sea, sec, see, tst가각각 14주 (40.0%), 2주 (5.7%), 8주 (22.9%), 14주 (40.0%) 에서검출되었다. MRSA와 MSSA가동시에나타난 RFLP 유형은 L1과 L5 였으며, 나머지유형에서는유형에따라균주들의메티실린내성이구분되었다. RFLP 유형 L1과 L5 균주들은대부분 MRSA였으며, L1의균주중 MRSA는 97.3%, L5의균주중 MRSA는 93.5% 였다. 5. 수집연도에따른분석수집연도에따라 MRSA의빈도는 2003년 57.1%, 2004년 69.0%, 2005년 79.2%, 2006년 75.8% 였다. MRSA 중가장주요한 RFLP 유형인 L5의균주들은연도에따라 MRSA에서차지하는빈도가증가하는추세를보였다 (2003년, 25.0%; 2004 년, 55.2%; 2005년, 47.4%; 2006년, 60.0%). 6. 분리된병동에따른양상일반병동에서 70주, 중환자실에서 47주가분리되었으며, 3 주는장소를확인할수가없었다. 일반병동에서분리된 70주중 MRSA는 45 주 (64.3%) 였으며, 주요한 RFLP 유형은 L1과 L5 로서각각 22 주 (48.9%), 20주 (44.4%) 가해당하였다. 중환자실에서분리된 47주중 MRSA는 38 주 (80.9%) 로일반병동에비해빈도가높았으며, 주요한 RFLP 유형은 L1과 L5 로각각 13주 (34.2%) 와 23 주 (60.5%) 가해당하였다. 고찰 S. aureus의 coa 유전자의 3 말단의다형성에기반하여유전형을분석하는방법으로는 PCR RFLP 방법이나직접염기서열분석이사용된다 [7-11, 18, 19]. 염기서열분석이높은해상도를보이는것으로알려져있으나 [18, 19], 본연구에서는실행과정의편리함과해석의용이함을이유로 PCR RFLP 방법을사용하였다. PCR RFLP 방법을사용한기존의보고들을보면, 시발체의고안과제한효소의선택에따라 coa 유전자의크기와 RFLP 유형에서차이를보이며 [7, 9], 현재까지표준화된방법은없다. 본연구에서는 coa 유전자의크기와 RFLP 유형이각각 4가지와 16가지가나타났으나, 본연구와동일한방법을사용한 Hookey 등 [9] 의보고에서는각각 4가지와 10가지, Ishino 등 [10] 의보고에서는각각 8가지와 31가지가나타났다. 연구에따라 RFLP 유형의수와종류가다르게나타나는것은대상균주들의수와분리된지역및역학적배경등의차이에서기인하는것으로생각된다. 본연구에서 coa 유전자의크기는 Ishino 등 [10] 이보고한크기에따라 S, M, L, XL로분류하였는데, RFLP 유형은실제데이터를비교하는데어려움이있어본연구에서임의로분류하였다. coa 유전자의 RFLP 유형에따른 S. aureus 균주들의분석은직접염기서열분석이나 PFGE 방법에비해분별력이낮은것으로보고된다 [18-20]. 하지만 RFLP 유형에따라대부분의메티실린내성균주를구분할수있고 [11], 혈청형분석에비해서는높은분별력을보이며, arbekacin-resistant MRSA를구분하는데유용하다는보고도있다 [10]. 영국에서분리된 MRSA를대상으로한보고에서는 coa 유전자 RFLP 유형분석이계통유전학적분석, 파지타이핑의방법과비교하여대등한분별력을나타내었다 [9]. 기존의보고들에서 MRSA 균주들의 RFLP 유형의수는 MSSA 에비해상대적으로적은것으로나타난다 [7, 9, 11]. 본연구에서도 MSSA의균주들에서는전체 16 가지의 RFLP 유형중 12 가지가나타났으며, MRSA의균주들에서는상대적으로적은 5 가지의유형이나타났다. 또한 MRSA 85주중 RFLP 유형 L1 과 L5 가각각 36주와 43주로 MRSA의대부분 (93.0%) 을차지하였는데, 이러한양상은 MRSA 균주들간의높은유전적연관성과관련있을것으로생각된다 [21]. 본연구에서독소유전자의빈도는 sea, sec, see, tst 각각 40.8%, 33.3%, 20.0%, 39.2% 였으며, seb, sed, eta, etb는검출되지않았다. MRSA에서의독소유전자의빈도 (89.4%) 는

6 Coagulase Gene Typing of S. aureus 291 MSSA에서 (51.4%) 보다높게나타났다. 이러한양상은국내에서분리된균주를대상으로보고한것과유사하였다 [22]. 하지만독일지역의혈액배양검체에서분리된균주에서는 sea (17.4%), seb (5.9%), sec (8.7%), sed (10.5%), see (0.5%), tst (18.3%), eta (0.5%), etb (0.0%) 의분포를보여상대적으로 seb, sed가높은빈도를나타내고, 메티실린내성에따라독소유전자의빈도가유의한차이를보이지않는것으로나타나는데 [23, 24], 이러한독소유전자의양상의차이는지역에따라분포하는균주들이서로다른역학적, 유전적배경을갖고있기때문으로생각된다. 본연구에서는 coa 유전자의 RFLP 유형과함께독소유전자의검출을병행하여유전형의양상을파악하고자하였다. 균주들은 coa 유전자의 RFLP 유형에따라 16가지로분류되었으며, 독소유전자의양상에따라 34가지의유형으로세분할수있었다 (Table 1). MRSA 85주중주요한유전형의균주는 sec와 tst 를갖고있는 L5 가 32주 (37.6%), sea를갖고있는l1이 16주 (18.8%), sea, see를갖고있는 L1이 16주 (18.8%) 였다. 특히, RFLP 유형 L5는 Ishino 등 [10] 이일본에서분리된 MRSA의 77% 를차지한다고보고한 RFLP 유형과동일한것으로생각되는데, 일본의 MRSA에서도 sec와 tst의높은빈도가보고된바있다 [25]. 연도에따라 MRSA 균주중유형 L5 균주의빈도는증가하는추세이며, 중환자실에서의빈도가일반병동보다높았다 (60.5% vs 44.4%). 이러한양상의원인을밝히기위한추가적인연구는필요할것으로생각된다. 본연구에서는환자의혈액에서분리된 S. aureus 균주를대상으로 coa 유전자의 RFLP 유형과독소유전자에따라유전형양상을파악하였다. coa 유전자 RFLP 유형 L5 및 L1 의균주가 S. aureus 균혈증환자에서분리되는주요 MRSA 균주로생각되며, RFLP 유형에따라특정독소유전자유형을보였다. 요약배경 : 모든 Staphylococcus aureus는 coagulase를생산한다. MRSA 85균주중 43균주 (50.6%) 는 RFLP 유형 L5이었으며, 36균주 (42.4%) 는유형 L1였다. 유형 L5의 MRSA 균주에서는 tst (93.0%) 와 sec 유전자 (81.4%) 가높은빈도로검출되었으며, 유형 L1의 MRSA 균주에서는 sea (88.9%) 와 see 유전자 (44.4%) 의빈도가높았다.
