Recently, target-controlled infusion (TCI) has begun to be used widely to administer and titrate propofol and remifentanil during sedation. To investigate the effect and safety of the pharmacologic agents used in anesthetic department, we performed a retrospective study. Methods: Retrospective study of a series of dental procedure under intravenous sedation performed in department of anesthesiology in Dental Hospital of Dankook University was carried out with propofol or propofol/remifentanil between January and August 2011 and January and April All patients received oxygen by nasal cannula. The average propofol and remifentanil target was 0.5 µg/ml and 1.0 ng/ml, respectively using a TCI pump. The average peripheral oxygen saturation (SpO 2), heart rate, blood pressure, respiratory rate, nasal end-tidal CO 2 were recorded at 5-10 minute intervals. The age, gender, weight, procedure and sedation time, type of procedure were also recorded. Results: We included 22 cases of 19 adults (group A) and 6 cases of children (group B). In group A, 4 patients received propofol (group A-P), and 15 patients received propofol with remifentanil (group A-PR). In group B, 6 patients received propofol only. The mean age of group A was 41.1 years old and that of group B was 9.5 years old. No clinically significant complications were noted. There were no case of de-saturation <90%. The median respiratory rate was 13.1 (range 6 to 36) in group A and 19 (range 13 to 25) in group B. The median end tidal CO 2 was 36.7 mmhg(range 8 mmhg to 56 mmhg) in group A and 41.7 mmhg (range 30 mmhg to 53 mmhg) in group B. Conclusions: Based on our results, dental sedation using propofol/remifentanil in adult and propofol in children with TCI pump seems to appear as a safe and effective procedure while performing dental procedure. 이는진정제단독으로도진정과불안해소, 기억상실증과같은효과를나타내지만 [4], 아편유사제와함께사용시에는적은용량으로도효과가있으며, 또한상승작용을나타내기때문이다 [5]. 이러한약물들중에서회복력과약리학과약동학적인면을생각해봤을때 propofol 과 remifentanil 이진정치료시이상적인약물로생각된다 [6,7].

Monitor (Datex ohmeda). 불안조절을위해정맥내로투여되는약물의중추신경억제제가기억상실을유발한다는부가적인장점도지닌다. 치의학에서널리쓰이는정주진정제로는 benzodiazepine 이나 propofol 이있으며때로는아편유사제를 propofol 이나 benzodiazepine 과결합해사용하도한다 [4]. 이는진정제단독으로도진정과불안해소, 기억상실증과같은효과를나타내지만 [4], 아편유사제와함께사용시에는적은용량으로도효과가있으며, 또한상승작용을나타내기때문이다 [5]. 이러한약물들중에서회복력과약리학과약동학적인면을생각해봤을때 propofol 과 remifentanil 이진정치료시이상적인약물로생각된다 [6,7]. 최근에는혈중목표농도조절주입법 (target controlled infusion; TCI) 이진정치료에널리사용되고있다. TCI 는마취및진정의유도및유지를위한컴퓨터보조정맥주입법이다. 이경우약물의주입속도는약역학적표준에의해자동적으로바뀌게된다 [8]. propofol 과 remifentanil 은신속하게진정이발현되며, 과진정에의한의식소실및호흡저하등의위험성이있으므로적절한용량의투여가필요하다. 따라서이번연구는 propofol 과 remifentanil 을 TCI 로투여하는진정상태에서의치과진료시의효과와안정성을조사하기위해서시행되었다. 대상및방법 2011 년 1~8 월, 2012 년 1~4 월사이에단국대학교치과대학부속치과병원에서 propofol 단독또는 propofol 과 remifentanil 을병용하여시행된정주진정하치과진료사례에대해서무작위로차트리뷰를통한후향적연구를실시하였다. 이번연구는 25 명의환자를대상으로하였으며, 이중성인이 19명 ( 여성 10명, 남성 9명 ), 어린이가 6명 ( 여성 3명, 남성 2명 ) 이었다. 성인을그룹 A로분류하였으며이그룹의평균연령은 41.1 세 (21~63 세 ) 였다. 어린이를그룹 B로분류하였으며이그룹의평균연령은 9.5 세 (6~13 세 ) 였다. 환자들은진정치료시의주의사항에대하여설명을듣고치료에동의하였다. 정맥천자를시행하고, 탈수를방지하기위해서 5% D/W 를계속공급하였다. 모든환자는경비캬뉼라를통하여 3~4 L의 100% 농도의산소를공급받았다. TCI 장비 (Ochestra Infusion Workstation, Fresenius Vial, France) 를이용하여 propofol 과 remifentanil 의목표농도는각각 0.5 μg/ml, 1.0 ng/ml 로정주를시행하였다 (Fig. 1). TCI system 에서 propofol 에는 Schnider 값이 remifentanil 에는 Minto 값이사용되었다. 시술이끝난후환자는회복실에서충분한휴식을취한뒤, 귀가하였다. 수집된정보는 5~10 분간격으로측정된평균산소포화도, 심박수, 혈압, 호흡수, 호기말이산화탄소의농도, 나이, 성별, 치과진료와진정치료시간, 진료의종류이다 (Fig. 2). 결과성인 19 명의 22 개의사례와소아환자 6명의 6개의사례를대상으로하였으며성인을그룹 A, 소아를그룹 B로구분하였다. 이들의특성을 Table 1에나타내었다. 그룹 A에서는 2 대한치과마취과학회지제 13 권제 1 호 1-7

B 그룹에서는불안장애를보이는환아가 3명이었으며, 1명이언어발달장애였다. 진정치료동안심각한합병증은없었다. 평균호흡률은그룹 A에서 13.1 (range 6 to 36) 이었고 (Fig. 3, 4), 그룹 B에서 19 (range 13 to 25) 였다 (Fig. 5). 평균호기말이산화탄소의농도는그룹 A에서 36.7 mmhg (range 8 mmhg to 56 mmhg) 이었으며 (Fig. 6, 7), 그룹 B에서는 41.7 mmhg (range 30 mmhg to 53 mmhg) 이었다 (Fig. 8). 호기말이산화탄소농도가 50 mmhg 이상인사례가 A-PR 그룹에서 3건, B 그룹에서 1건있었으며, A-P 그룹에서는발견되지않았다. 호기말이산화탄소감시장치의 waveform 이사라진사례는없었으며, 산소포화도가 90% 이하인경우는없었다. 보조호흡을시행한경우는없었다. 환자의심박수는그룹 A-PR 에서 72.9 (50~104), 그룹 A-P 에서 77.8 (59~99), 그룹 B에서 86.4 (73~107) 였다. 상실성빈맥 ( 심박수 > 120 회 / 분 ) 은관찰되지않았다. 이완기혈압은그룹 A-PR 에서 69.7 mmhg (range 41 mmhg to 110 mmhg), 그룹 A-P 에서 64.4 mmhg (range 58 mmhg to 80 mmhg) 그룹 B에서 56.2 mmhg (range 45 mmhg to 72 mmhg) 였다. 저혈압 ( 혈압 < 90 mmhg) 은 A-PR 그룹에서 1건, B 그룹에서 3건이었다. 환자의평균진정시간은그룹 A-PR 에서 62.3 (20~120) 분, 그룹 A-P 에서 42.5 (20~60) 분, 그룹 B에서 58.3 (30~90) 분이었다. 진료의종류는외과적인수술, 우식치료, 근관치료, 치주치료, 보철치료등이었다. 고찰 심한스트레스로유발되는과환기나실신과같은과거력이있는경우, 치료하는동안스트레스를최소화하여환자에게발생하는위험성을감소시키고, 치료중있을지모를통증에대해고지해주어환자를더편안하게해줄필요가있다 [9]. 이러한방법으로사용되는의식하진정은환자의불안을경감시키고편안하고안전한상태를유지해줄수있을뿐만아니라, 치료중환자의예상치못한움직임이나반응을경감시키는장점이있다 [10]. 일반적으로만 3세이상, 체중 15 kg 의어린아이의경우에는구강투여를통한진정시효과가미약하여, 효과적인진정을통하여치과진료를마무리하기가어렵다. 따라서정주진정하에서치과진료를시행하게되는데, 이번연구에서 propofol 의정주투여를통해 6~12 세사이의아동을효과적으로치료할수있음을알수있었다. 이번연구에서 TCI 의식하진정자료와산소포화도의변화, 호기말이산화탄소의농도, 이완기혈압과심박수등이 JOURNAL OF THE KOREAN DENTAL SOCIETY OF ANESTHESIOLOGY 2013 Mar ; 13(1) : 1-7 3

4 Jeong Eun O, et al : A Study of Intravenous Sedation in Dankook University Dental Hospital 후향적으로분석되었다. 일반적으로혈역학지수와산소포화도가정상변위범위내 (± 20%) 있는것이추천된다 [11]. 이번연구에서포함된환자들에서심장의리듬이상이나구토, 흡인, 부정맥등의임상적인부작용은나타나지않았다. 또한추가적으로산소를공급해야하거나기도재확보를필요로하는등의호흡억제를나타내는임상적인징후도보이지않았다. 실제임상에서는약리학적, 약동학적다양성으로인해약의용량에따라많은변화가나타날수있다. 이번연구에서 TCI 의사용은과진정의부작용을예방하면서불안과동통조절에적합하다고볼수있다. TCI 는진정시조절가능한낮은농도로마취제를공급하면서과진정의경우를줄여주며, 비슷한효과를얻을수있게한다 [12]. 일정하게약물의농도를유지하는것은수동적으로약물을투여할때발생하는과투여도발생하지않으면서빨리일정한속도에이르게된다. 또다른장점은혈장과효과기관에서의약물의농도가계산되어있다는점이고자동적인계산을통해서혈중농도를맞춘다는점이다. 혈역학적으로불안정한환자의경우에 TCI Fig. 3. Respiratory rate in group A-PR. 는낮은농도에서진정치료를시작하여, 점차높은농도로농도를조절할수있으며, 안정적인환자에서는안전범위중가장높은값의세팅을사용하여진정치료를더빨리유도할수있을것이다. 또한기존의체중을바탕으로한방법과비교할때심혈관계의안정성이더뛰어난것으로밝혀졌다 [13]. 초기목표혈중농도인 Propofol 0.5 μg/ml 와 remifentanil 1.0 ng/ml 는치과진료시의진정농도로적절해보인다. 방보영등의 [14] 연구에서매복지치수술적발치시에사용한 propofol 의목표혈중농도는 0.54 ± 0.11 µg/ml, remifentanil 은 1.11 ± 0.30 ng/ml 였으며, 이농도에서안전하게의식하진정을시행하였다. 그외의과적인분야에서도의식이있는상태에서의진료시 propofol 이나 remifentanil 또는두가지를병합한진정을시행하였는데, Fiber optic procedure 를시행하였을때 propofol 과 remifentanil 의 TCI 농도는 1.5~3.5 μg/ml 와 1.0~5.0 ng/ml 였다 [15]. Propofol 과 remifentanil 을사용한 nasotracheal intubation 에서초기농도의값은 propofol 이 2.5 μg/ml, remifentanil 이 1.5 ng/ml 였다 [16]. Propofol 은빠른재분포와대사로회복이빠르다 [17]. Propofol 의장점은잔여효과가없는회복이가능하다는점이다. Propofol 로인해서심혈관계의억제가나타날수있으나적정과 TCI 의사용으로최소화될수있다 [18]. Propofol 사용시부작용인주사시작열감은 remifentanil 을함께투여하여줄일수있었다. Remifentanil 은최소한의잔여효과를지니며, 매우짧은기간작용하는아편유사제이다 [6]. Remifnetanil 과 propo- Fig. 4. 5 오정은외 2 인 : 단국대학교치과대학부속치과병원에서시행된정주진정에대한연구 fol 을함께사용할경우마취의유지시 propofol 의용량을적게사용할수있게하며, 회복에걸리는시간도짧다 [19]. 이들의조합은안전하고효과적이나, 호흡부전을초래할수있기때문에기도확보가중요하다 [20]. 이번연구에서 propofol 과 remifentanil 의농도는 propofol 단독군보다 propofol 과 remifentanil 을함께사용한군에서더낮았으며이는다른연구결과와일치한다 [19]. 진료시간은 propofol 과 remifentanil 을함께사용한군에서더길었는데, 이는치과시술이외과적수술부터우식치료, 근관치료까지다양할뿐아니라환자사례에따라치료에소요되는시간이차이가있기때문으로생각된다. 또한 propofol 과 remifentanil 을함께사용할때회복시간이짧았다는기존의연구결과를바탕으로진료시간이길어질것이라예상되는경우혼합약제를사용한것으로생각된다. 그러나아동의경우술후마취제효과및과진정의위험성에대한가능성으로 remifentanil 의사용에대해여전히논쟁중이므로 propofol의단독투여를시행하였다 [21]. 환자에따라동일한용량의약제에도진정심도가깊어져 Fig. 6. End tidal CO2 in group A-PR. 의사소통이불가능하고, 기도보호반사가둔화되고, 호흡이저하되어저산소혈증에의한손상가능성이있을수있기때문에활력징후감시, 진정상태의감시가필요하다 [22]. 그중에서호기시이산화탄소를탐지하여무호흡을모니터링하는방법은진정이나마취상태에서환자의호흡상태를평가하는데유용하며 [23], 호기시비침습적으로이산화탄소를측정할수있다. Anderson 등은 [24] capnography 는임상적인검사나산소포화도의감시이전에호흡부전을탐지할수있는방법이라고결론지었다. ETCO 2 waveform 의상실은준임상의호흡억제의즉각적인징후가된다. 이번연구에서 ETCO 2 의 waveform 을사용하여호흡억제를객관적으로측정할수있었다. James R 등에 [25] 따르면호흡억제는 ETCO 2 가 50 mmhg 이상이거나 ETCO 2 의 waveform 이없거나산소포화도가 90% 이하일경우로구분할수있는데, 본연구에서는 ETCO 2 가 50 mmhg 이상인사례가 A-PR 그룹에서 3건, B 그룹에서 1건있었으며, A-P 그룹에서는발견되지않았다. 호흡억제범주에들어가는환자들에서 ETCO 2 로탐지된환자들중맥박산소측정기로탐지된사례는없었다. 이번사례와같이환자가추가적으로산소를공급받고있다면맥박산소측정기는준임상적인호흡억제를탐지하지못할것이다. ETCO 2 는호흡억제에민감한감시장비이다. ETCO 2 의증가는저환기 (hypoventilation) 를의미하며, 반면에저산소증 (hypoxia) 은환자가추가적으로산소를공급받는경우에는상대적으로늦게발견된다 [25]. Hypoxia 가일어나기전에 hypoventilation 을탐지하는것은임상가가환자를자극하는것을막아주며, 최종적으로추가적인진정제의투약여부및기도의평가를할수있도록한다. 따라서이번연구에서 Fig. 7. End tidal CO2 in group A-P. Fig. 8. End tidal CO2 in group B. 6 Jeong Eun O, et al : A Study of Intravenous Sedation in Dankook University Dental Hospital ETCO 2 를감시함으로써아편유사제의부작용으로발생할수있는호흡부전에대해즉각적인탐지를할수있었다고여겨진다. 이번연구에서의한계점은연구가차트의후향적인분석으로이루어졌기때문에차트에기재되어있지않은진정치료에대한환자의만족감이나진정치료시의진정의깊이에대해서는알수없었다는점이다. 또한샘플의수가적어서이표본이전체환자로일반화하기에도무리가있을수있다. 추후이런부분을보완하여, 미래의연구에서는진정치료시의환자의신체적인스트레스정도를수치화하여진정의깊이, 환자의만족감을포함한진정법에대한비교및평가가필요하지않을까생각된다. 이번연구결과를바탕으로했을때 TCI 를사용하여성인에서 propofol 단독또는 propofol 과 remifentanil 을병용하고, 소아환자에서 propofol 을단독으로사용한진정치료시적절한모니터링을통하여안전하고효율적으로치과진료를시행할수있음을알수있었다. 참고문헌 1.
