These foreign bodies result in pain, bleeding, and infection and are immediately removed in most cases. However, the foreign body without serious symptoms can remain for several years without treatment. Recently we experienced the penetrating metallic foreign body retained in the nasal cavity over 30 years and complicating orbital abscess. The foreign bodies were two pieces of broken metallic chopsticks. One piece was retained in both ethmoid sinuses and the septum and another in the nasomaxilloorbital cavity resulting in orbital abscess. The foreign bodies were removed successfully with nasal endoscope, sublabial approach and lower eyelid approach to the orbit without rhinoophthalmological sequelae. 기왕력으로 당뇨병에 대한 약 micin(400 mg) 등의 항생제를 투여하였으나 안증상이 호 물 복용 중이었으며 4년 전 직장암 진단 받아 직장 부분절 전되지 않고 더욱 악화되며 안압 상승 및 시력 저하가 심해 제술 및 항암치료를 받은 경력이 있었다. 져 이물 제거 및 안와농양에 대한 치료를 위해 이비인후과 입원시 안소견상 시력은 우안, 좌안 1.

It is about 5 cm long, corroded, and easily broken. 촬영한 단순 부비동 X선상에서 비강내 이물이 있음을 처 안과에서 비강과 안구 이물 및 안와 봉와직염 또는 안와 음 확인한 후 수술 권유 받았으나 이물로 인한 증상이 없어 농양을 의심하여 입원하에 3일간 cefotetan(2.0 g)과 astro 수술을 받지 않았다고 하였다. 기왕력으로 당뇨병에 대한 약 micin(400 mg) 등의 항생제를 투여하였으나 안증상이 호 물 복용 중이었으며 4년 전 직장암 진단 받아 직장 부분절 전되지 않고 더욱 악화되며 안압 상승 및 시력 저하가 심해 제술 및 항암치료를 받은 경력이 있었다. 져 이물 제거 및 안와농양에 대한 치료를 위해 이비인후과 입원시 안소견상 시력은 우안, 좌안 1.0이었으며 외 로 전과되었다. 안근운동 검사상 좌안의 외하방 주시시 경도의 제한과 좌 이물 제거를 위해 전신마취 하에서 우선 비내시경을 이 측 주시시 복시를 호소하였으며, 좌측 안와주위 부종 및 하 용하였다. 우측 사골동과 비중격을 통과하여 좌측 사골동까 안검에 종창 및 동통 소견 관찰되었다. 동공 반사 및 안압 지 걸쳐 있는 금속 이물은 비중격 부위에서 부식된 상태로 은 소견이었고 세극등검사상 좌측 결막하에 경도의 충 쉽게 절단이 가능하여 우측 비강의 이물은 염증 소견을 보 혈 및 안저 검사상 좌안 하비측에 미세 초자체 출혈 외에는 이는 주변 연부조직과 함께 우측 비공으로 제거하였고 잘려 특이 소견 관찰되지 않았다. 술전 시행한 단순 부비동 X 진 좌측의 이물 조각은 좌측 비공을 통해 제거하였다. 적출 선 및 부비동 컴퓨터 단층촬영상 우측 비강 상부로부터 비 된 이물은 쇠젓가락으로 판명되었다(Fig. 3). 좌측 비강을 중격을 가로 질러 좌측 안와에 걸쳐 있는 금속성 물질과 좌 경유 상악동 및 안저에 있는 금속이물의 경우 경중비도 상 측 비강상악동안와에 걸쳐 있는 금속성 물질 등 두 조 악동 개방술(middle meatal antrostomy)을 시행하여 제거 각의 금속성 이물이 관찰되었다(Figs. 1 and 2). 하려 하였으나 기구 삽입이 어려워 구순하 절개(sublabial 178 Korean J Otolaryngol 2004;47:17781

After the incision on periorbita, the metallic foreign body (arrow) is noted (F orbital fat, O inferior orbital wall, P periorbita). 존 금속이물을 안구에서 제거한 다음(Fig. 5), 창상부위를 incision)를 가한 후 상악동을 통한 접근법을 시도하였다. 되어 ceftriaxone(2.0 g/일)과 sulbacillin(3.0 g/일)을 7일 상악동 및 좌측 안와하벽에 박혀 있는 금속 이물을 제거하 간 경정맥 투여하였다. 수술 후 4일 째 좌안의 안구운동 저 고 나서 portable Xray 촬영 결과 안와 아래외측에 부 하 및 안와 주위 동통이 감소하고 부종이 호전되었으며 시 러진 금속이물이 잔존하여(Fig. 4), 확장된 상악동 자연구 력과 안압은 소견을 보였고 수술 후 8일째 퇴원하였 및 상악동 앞벽을 통해 내시경적으로 제거하려 하였으나 부 다. 수술 후 6개월에 추적검사상 두통 및 안와 주위 동통은 러진 조각을 찾는데 실패하였다. 이를 제거하기 위해 좌측 없었으며 안압 및 안구운동은 이었다(Table 1). 세척하고 배액관을 삽입한 후 피부절개를 봉합하였다. 수술 후 세균 배양검사에서 Klebsiella pneumoniae가 검출 아래 속눈썹에서 3~4 mm 하방의 피부 주름에 절개(하안 검 절개 lower eyelid incision)를 가한 다음 안륜근(or 고 찰 bicularis oculi muscle) 일부를 절개하여 안와하연에 도달 한 후 안와골막을 절개하였다. 절개된 안와골막의 아래로 안 비강 및 안구내 이물이 의심되는 환자의 초진시 자세한 병 구를 하방에서 상방으로 박리한 다음 안와골막에 존재하는 력 청취와 신체 검사로 쉽게 이물 여부를 진단할 수 있으나 179
