Materials and Methods: Twelve cases of patients who underwent corrective osteotomy of metatarsal bone for a Morton s neuroma from November 013 to September 014 were retrospectively reviewed. Corrective osteotomy was performed through a dorsal approach at the 3rd metatarsal bone base and distal metatarsal bone was displaced dorsally and proximally. erative and postoperative pain assessed using American Orthopaedic Foot and Ankle Society (AOFAS) score and radiographs were evaluated. Results: The mean age of patients was 41.4 years, and the mean follow-up period was 10.7 months. AOFAS score improved from preoperatively to postoperatively. The 3rd metatarsal bone was shortened by an average of 3.39 mm and elevated by.38 mm. Conclusion: Corrective osteotomy of metatarsal bone can be regarded as a new surgical option for Morton s neuroma without exposure around neuroma. Key Words: Metatarsal bones, Osteotomy, American Orthopaedic Foot and Ankle Society score, Neuroma, Metatarsalgia 서론 전족부통증의주요한원인중하나인지간신경종, 일명모턴씨신경종은족저공통족지신경에서발생하는비종양성의방추형비대로서실제신경종이아닌횡중족골인대와족저사이에서지간신경이압박되어생기는신경포착증후군으로이해되고있다. 1) 이러한지간신경종의보존적치료로스테로이드국소주사, 신발교정과중족골패드등의보조기를이용하여중족골간간격을넓혀신경압박을감소시키는방법들이이용된다 (Fig. 1). -4) 보존적치 료로증상이호전되지않는환자에게는수술적방법으로신경종제거술, 횡중족골인대유리술과신경박리술을통한감압술등이사용되며, 신경종제거술이흔히시행되고있다. 1,3,4,5) 하지만신경종제거술후신경종의재발및지속적인통증이남을수있으며, 3,6) 신경감압술의경우불충분한감압으로인한증상의지속및신경종주위의흉터조직의증식또는유착등의합병증이발생할수있다. 7-9) 본저자들은신경종주위의유착없이신경감압술을시행한다면좋은임상적결과를얻을수있을것이라고생각하였다. 이 Received April 1, 015 Revised April 7, 015 Accepted May 10, 015 Corresponding Author: Hyun-Woo Park Department of Orthopedic Surgery, Dankook University College of Medicine, 119 Dandae-ro, Dongnam-gu, Cheonan , Korea Tel: , Fax: , m3artist@hanmail.net Financial support: None. Metatarsal Osteotomy for Morton s Neuroma 에 이학적 검사상 멀더스 검사(Mulder s click test) 양성 소견을 보 치료에도 만족할 만한 증상 호전을 보이지 않는 환자들 중, 중족골 이며, 초음파 검사상 3 mm 이상으로 비후된 지간 신경이 확인되어 패드 착용 시에는 일시적인 증상 호전을 보인 환자군을 대상으로 지간 신경종으로 진단 받은 환자들에게 제 3중족골 교정 절골술을 중족골 교정 절골술과 신경종 절제술에 대해 장단점을 비교 설명 중족골 근위부에서 시행하여 신경종의 노출 없이 감압술을 시행하 을 하여 절골술을 선택한 환자를 대상으로 하였으며, 외상의 과거 였으며, 임상적 결과와 방사선학적 결과에 대해 분석 보고하고자 력이 있거나 무지 외반증, 소건막류 등 족부의 다른 질환이 동반된 한다. 환자를 제외하고 6개월 이상 추시관찰이 가능하였던 11명 1예를 대상으로 하였다. 평균 나이는 41.4세(5 세)였고, 남자는 8예, 대상 및 방법 여자는 4예였으며, 평균 추시관찰 기간은 10.7개월(6 6개월)이 었다. 방사선학적 검사로 술 전과 술 후에 체중부하 전후면 방사선 1. 연구 대상 사진을 촬영하였으며, Maestro 등10)의 방법을 이용하여 중족골의 013년 11월부터 014년 9월까지 단국대학교병원에서 지간 신 길이의 측정(Fig. ) 및 수술실에서 방사선 영상증폭장치(C-arm)를 경종으로 중족골 교정 절골술을 시행 받은 환자를 대상으로 하여 이용하여 술 전과 술 후 제 3중족골두의 상부 전위 정도를 측정하 후향적으로 연구하였다. 본원 외래를 방문하여 지간 신경종으로 였다. 또한 임상적 결과의 평가를 위하여 술 전과 술 후 6개월째에 진단 후 6개월 이상 중족골 패드 및 스테로이드 주사 등의 보존적 American Orthopaedic Foot and Ankle Society (AOFAS) 점수, foot function index를 조사하였다. 통계적 분석은 IBM SPSS Statistics version 1.0 (IBM Co., Armonk, NY, USA)을 사용하였으며, 유의 수준은 p<0.05로 하였다. 또한 본 연구는 단국대학교병원의 임상연 구윤리위원회(institutional review board)의 승인을 받았다.. 수술 방법 전 예에서 척추마취를 시행 받았으며, 마취 후 환자는 앙와위 를 취하게 하였고 대퇴부에 공기압 지혈대를 사용하여 수술 시야 를 확보하였다. 족배부의 제 3중족골 기저부를 방사선 영상증폭장 치를 이용하여 확인한 후 배부 종축 절개를 사용하여 접근하였다. 제 3중족골의 근위 골간단부 위치에서 족저면 원위부를 향하여 절 골을 시행하였다(Fig. 3). 절골 후 중족골의 골두를 족저면에서 촉 지하여 제 3중족골을 근위부 및 상부로 밀어 올려 전위시킨 후 절 골부를 K-강선을 이용하여 임시 고정하였으며, 방사선 영상증폭 장치를 이용하여 전위 및 K-강선의 위치가 적절한지 확인한 후 나 Figure. 60 Vol. 19 No., June 015 지지 않도록 단축시켰다. 방사선 영상증폭장치를 이용하여 제 3중 족골두의 상부 전위 정도를 종자골 상을 이용하여 확인하였으며 (Fig. 5), 족부 전후면 상을 촬영하여 단축 정도를 확인하였다(Fig. 6).

3 60 Vol. 19 No., June 015 지지 않도록 단축시켰다. 방사선 영상증폭장치를 이용하여 제 3중 족골두의 상부 전위 정도를 종자골 상을 이용하여 확인하였으며 (Fig. 5), 족부 전후면 상을 촬영하여 단축 정도를 확인하였다(Fig. 6). 고정한 금속물이 피부하에서 촉지되는지 확인하여 위치를 조 정한 후 피부 봉합을 시행하고 수술을 종료하였다. 결 과 신경종은 제 물갈퀴 공간에서 예, 제 3물갈퀴 공간에서 1예 로 관찰되었으며, 제, 3물갈퀴 공간 모두에 있던 경우는 9예였다 (Table 1). 술 전 AOFAS 점수는 평균 점에서 술 후 점으로 향 상되었고(p=0.005), 제 3중족골의 길이는 술 전 평균 mm에 A 서 술 후 평균 8.8 mm로 평균 3.39 mm 단축되었으며(p=0.000), 제 3중족골두의 상부 전위 정도는.38 mm였다(p=0.000; Table ). 술 후 창상 관련 합병증은 관찰되지 않았으나 나사의 이물감 이 있어 나사 제거술을 시행한 경우가 1예에서 있었으며, 제 3족지 B Figure 6. AOFAS: American Orthopaedic Foot and Ankle Society. 는완전회복이되었다. 그밖에전이성중족골통 (transfer metatarsalgia) 은발견되지않았으며, 불유합등의합병증은추시관찰기간동안관찰되지않았다. 고찰 지간신경종의진단을위해서는자세한병력과임상증상, 이학적검사가가장중요하다. 전족부의족지간을압박하여증상을유발시키는멀더스검사 (Mulder s click test) 를주로시행하며, 11,1) 국소마취제를병변주위에주사하여증상이나동통의소실여부를확인하는리도케인차단검사법등을통해임상적진단을할수있으나, 전족부통증을유발할수있는다른원인을감별하는것도중요하다. 최근에는초음파나자기공명영상과같은영상진단의민감도가높은것으로보고되고있어진단에도움을줄수있다. 13) 본저자들은멀더스검사와리도케인차단검사, 초음파검사를시행하여감별진단하였으며, 중족골패드를사용하여주관적인증상이호전되는환자군을수술의적응증으로생각하였다. 이는중족골패드를적용함으로써중족골두가상방전위되어중족골두간의간격이넓어짐으로써지간신경종의자극을피할수있는원리를이용하여환자군을선택한것이다 (Fig. 1). 실제로중족골패드적용시증상호전을보인환자군에서는교정절골술후즉각적인임상호전을보였으며, 이는중족골패드를적용한것과같이절골술후중족골두의상방전위로인한신경종의감압때문으로판단하였다. 하지만아직본술식은모든제, 3지간신경종에적용하기에는근거가부족하며, 이는환자마다중족골의관상면상의가동성에차이가있고, 신경종의정확한위치와크기, 이환기간등에따라환자의임상적결과에영향을줄것으로추측되기때문에중족골패드적용시증상이개선되는환자로적응증을제한하였다. 본술식은제 1저자인 Chu에의해처음시도되어본술식의이름을 Chu 술식 (Chu procedure) 이라고명명하고자한다. 지간신경종의수술적치료에는다양한방법들이보고되어있으나신경종절제술이주로시행되고있으며, 85% 의환자에서만족을보인다고하였으나 14) 여러합병증또한보고되고있다. Johnson 등 15) 은불충분한절제술로인한지속적인증상을보고한바있으며, Coughlin과 Pinsonneault 14) 는절제술후진성신경종이체중부 하부위에발생하여통증을유발할수있다고하였다. 또한 Akermark 등 16) 에의하면절제술로인해절제된지간신경의지배영역의감각감소및이상감각을유발할수있다고하였다. 절제술이외의수술적치료방법으로개방적심부횡중족골인대의절단술과신경박리술을통한신경감압술이사용되며, 최근에는내시경을이용하거나최소침습적인방법들이보고되고있다. 5,8) 하지만최근 Kim 등 17) 의심부횡중족골인대와지간신경종의발생위치에대한사체연구를통한해부학연구에의하면, 지간신경종이주변구조물의자극과증가된압력에의해발생된것이라면심부횡중족골인대의원위부가압박이되는부위일것이라고보고하였다. 지간신경종은걸을때중족골두와중족지관절사이위치에발생하는것으로중족지관절이심부횡중족골인대보다더중요한구조물이며, 따라서심부횡중족골인대를절단하는방법은지간신경의감압에있어충분하지못한방법일수있다고하였다. Hofstaetter 등 18) 에의하면횡중족골인대는상처조직에의해다시복구된다고하였으며, Fabié 등 7) 과 Zelent 등 8) 은횡중족골인대의불충분한감압으로지속적인증상이남을수있다고하였다. 이에본저자들은중족골두와중족지관절사이에존재하는지간신경종의감압을위하여근위부에서중족골교정절골술을시행하는것이근본적인치료방법이라고생각하였다. 중족골단축교정절골술 (Weil 절골술 ) 의경우중족골통또는치료가잘안되는족저부각화증, 중족지관절탈구등이있을때중족골의길이조절을위해사용된다. 18,19) Bauer 등 0) 은지간신경종의치료를위해중족골원위부에서 Weil 절골술을시행하였으며, 중족골의단축및중족골두의상부전위를통해중족골간간격을넓히고족저압력을감소시켜 % 이상에서좋은결과를보고하였다. 하지만본저자들은중족골의근위부에서중족골교정술을시행하였으며, 원위부절골술과달리신경종주위술후창상유착등에의한지간신경의포착이나지간신경의의인성손상을피할수있을것이라고생각하였다. 중족골간의압력변화로인해발생할것으로추측하였던전이성중족골통의합병증은발생하지않았으며, 이는제 4, 5중족골의관상면에서의충분한운동성때문에보상작용이일어났을것으로생각하였다. 1) 증례의약 % 에서발생한제 3족지의근력저하 (floppy toe) 는중족골단축에따른신전건의상대적길이가길어

5 6 Vol. 19 No., June 015 짐으로써발생하였으며, 이는 6개월이내에전예에서정상으로회복됨을확인하였다. 본연구의제한점으로는증례수가적으며, 추시기간이길지않다는점이다. 이를보완하기위하여전향적으로다수의증례를포함한연구를준비중이며, 기존환자군도추시중이다. 본추시기간동안에는합병증은관찰되지않았으나추후장기추시의결과에대하여는아직알수없다. 추후추적연구시중족골교정절골술을통해넓어진중족골간간격및족저압을정확하게평가할예정이다. 결 지간신경종의전통적이며유일한수술적치료인신경종절제술의심각한합병증인진성신경종의발생및증상재발을예방하기위한방법으로써, 본저자들의제 3중족골교정절골술이하나의좋은대안이될수있을것으로생각한다. 론 REFERENCES 1.
