Neuroendocrine neoplasms of the larynx canbe divided into those of epithelial or neural origin. The latter consists of paragangliomas while the group of the epithelial origin can be further divided into typical and atypical carcinoids, and small cell neuroendocrine, which consist of the oat cell type, the intermediate cell type and the combined cell type. There are now over 500 cases of neuroendocrine neoplasms of the larynx reported in the literature. The diagnosis is primarily based on light microscopy and; in some instances, it may be supported by special histochemical studies. It should be confirmed by immunocytochemical and/or ultrastructural investigation. The different biological behavior of neuroendocrine neoplasms of the larynx makes a specific diagnosis of paramount importance, since treatment depends on accurate diagnosis. We experienced 4 cases of neuroendocrine of the larynx, and which we report with a review of literatures. (Korean J Otolaryngol 2004;47: ) KEY WORDS:Laryngeal neoplasms Neuroendocrine tumors

후두에 발생한 신경내분비종양 4예 가 없었으나 1개월 후 다시 우측 경부 종괴가 발생하여 우 측 측경부청소술을 시행하였다. 수술 2개월 후에 우측 경부, 상완, 가슴 등에 피하 결절이 환자의 치료 경과에 따라 계측한 혈중 칼시토닌 수치는 병 의 재발에 따라 상승하고 수술 후 감소하는 양상을 보였다 (Fig. 3). 여러 개 발생하였으며(Fig.

과거력상 고혈압으로 약물치료 한 변화가 없이 병이 진행되는 경과를 보였고, 초 수술 후 중이었으며 40년간 매일 2~3갑의 흡연을 한 과거력이 있 1년 11개월에 추가치료를 포기하고, 증상 치료만 하기로 결 었다. 1998년 5월경 모 대학병원 방문하여 성문상부종양을 정하였다.

2 후두에 발생한 신경내분비종양 4예 가 없었으나 1개월 후 다시 우측 경부 종괴가 발생하여 우 측 측경부청소술을 시행하였다. 수술 2개월 후에 우측 경부, 상완, 가슴 등에 피하 결절이 환자의 치료 경과에 따라 계측한 혈중 칼시토닌 수치는 병 의 재발에 따라 상승하고 수술 후 감소하는 양상을 보였다 (Fig. 3). 여러 개 발생하였으며(Fig. 2), 그 중 상완에서 조직 검사 한 것이 전이성 종양으로 판명되었다. 보존적 항암화학요법 증 례 2 을 위해 혈액종양내과로 전과하여, CYVADIC(CTX, VCR, 61세 남자 환자가 1998년 5월경부터 시작된 연하곤란 ADR, DTIC) 항암화학요법을 3회 시행하였다. 하지만 특별 및 연하통으로 내원하였다. 과거력상 고혈압으로 약물치료 한 변화가 없이 병이 진행되는 경과를 보였고, 초 수술 후 중이었으며 40년간 매일 2~3갑의 흡연을 한 과거력이 있 1년 11개월에 추가치료를 포기하고, 증상 치료만 하기로 결 었다. 1998년 5월경 모 대학병원 방문하여 성문상부종양을 정하였다. 발견하고, 직접후두경하 조직검사를 시행하여 선암종(adeno)을 진단하였고, 1998년 9월까지 8주간 7020 cgy의 방사선 치료를 시행하였다. 하지만 환자는 방사선 치료에 반응이 없었고, 1998년 10 월 본원을 방문하여 성문상부암 4기(rT3N2bM0)로 진단 되었다(Fig. 4). 환자는 성문상부 후두 부분적출술 및 양측 제 3 형 변형적 경부곽청술을 시행하였고, 병리조직 검사상 신경내분비종양으로 확진되었다. 1998년 11월 수술 후 시 행한 혈중 칼시토닌치는 5.0 pg/ml 이하(정상범위 <30) 였다. 수술 후 2개월째 설근부와 성문부의 봉합부위가 분리되 어 응급으로 피판을 이용한 재수술을 권유하였으나 환자가 강력히 거부하고 귀가하였다. 수술 3개월째 호흡곤란 발생 하여 사망하였다는 연락을 받았다. Sagittal T2-weighted magnetic resonance image of larynx of Case 1 shows cystic lesion of epiglottis. 증 례 3 57세 남자환자로 1999년 5월부터 발생한 애성을 주소로 내원하였다. 환자는 과거력 상 심근경색이 있었고 결핵을 앓 은 경력이 있었다. 40년간 하루에 1갑 반의 흡연을 하였고 술은 30년간 일주일에 소주 1병 정도 마셨다. 1999년 9월 본원을 내원하였으며, 외부 전산화단층촬영 (Fig. 5)과 신체검사를 통해 성문상부암, 양측 경부전이로 4 SSL, both FND 800 Right FND Arm biopsy CYVADIC #1 500 CYVADIC # CYVADIC # Fig. 2. 형 변형적 경부곽청술을 지 총 6120 cgy의 방사선 치료를 시행하였다.

Supraglottic subtotal laryngectomy and bilateral neck dissection were done. 기 후두암(stage Ⅳ, T2N2cM0)으로 진단되었고, 성문상 부 후두 아전절제술 및 양측 제 3 형 변형적 경부곽청술을 지 총 6120 cgy의 방사선 치료를 시행하였다. 경과관찰 중 여러 차례 시행한 혈중 칼시토닌 수치는 모 18 시행하였다(Fig. 6). 병리 검사상 신경내분비종양(modera- 두 정상 범위였으나(Fig. 7) 치료 후 6개월에 시행한 tely differentiated neuroendocrine, atypical FDG-PET에서 국소재발 의심되었으나 전산화단층촬영에 carcinoid tumor)으로 보고되어 1999년 10월부터 12월까 서는 특별한 병변을 발견할 수 없었다. 그 후 추적 검사로 F 1339

Atypical carcinoid 선 수질성 암종으로 오인되기도 한다. 그 외 mucinous 변 는 전이를 잘하므로 치료 전 병기 판정이 중요하다. 새로운 화가 있을 수 있으며(mucinous carcinoid) 또는 spindle 전이 진단방법으로 MIBG(metaiodobenzylguanidine) scan cell이 특징적인 형태로 보일 수도 있다(spindle cell carci- 또는 Radiolabelled somatostatin receptor scintigraphy noid). Argyrophil 염색에서는 대부분 양성이며, argenta- 가 사용되고 있다.10) 치료는 수술적 치료가 일차적 치료이 ffin 염색에도 양성일 수 있다. 대부분의 상피성 표지자와 신 며, 경부청소술도 필요하다. 원격전이, 특히 피하전이가 흔 경내분비성 표지자에 모두 양성이다. 특징적으로 calcitonin 히 발생하는데(약 22%), 이에 대해서는 외과적 절제로 치 에 양성으로 보이면 진단 가능하다.
