Most common cause was hepatocellular carcinoma, showing 49 out cases (86%) 2. Sonographic findings revealed echogenic th rombus in the dilatf'd portal vein. Echogenecity 01 thrombus was similar to the surrounding liver parenchyma. Thrombus was mainly located in the intrahepatic branch and main portal vein 3. The location 01 th rombus in hepatocellular ca rcinoma was rel ated to the location 01 tumor, and Irequentl y extended to the main portal vein (68.9%). Incidence 01 th rom bosis in hepatocellular carcinoma revealed 49 out cases, but prospective study showed 42 out cases (50%). Thrombosis was more common in large sized ill-delined diffuse inliltrative hepatocellular carcinoma 4. Computed tomography (onl y 15 cases analyzed) lindings revealed low density in the portal vein in al l the cases and periportal enhancement in 10 cases on post-enhanced scan 5. Collateral circul ations were largely detected at the porta hepatis and gal lbladder lossa. Age & Sex Distribution Age Sex M F Total (%) (10.5) (3 1.6) (35. 1) (15.8) over (77.0) 2. 방법 초음파진단기는 D iasonic WideVue Sector Real Total (%) 54 (94.7) 3 (5.3) 57 (1 000) time S canner 로 3.5 MHz 의탕촉자를사용히였다. 초음파검사상혈전의이 l 묘양싱, 분포, 문정맥의확장정도및기타소견들을분석하였다. 간암이원인이었던경우는종괴의위치, 크기, 에코양상, 외면및유형을면밀히관찰하여혈전의분포및빈도와의관제를조사하였무며일부판지에서시행된내시정검사, 전산화단층 2. 원인질환문맥혈전증을초래하였던 57 예를원인질환별로보띤간암이 49 예 (86 %) 로대부분이었고 ( Fi g. 1, 2) 그외에간경화, 웨장염 CF ig. 3), 위암의전이 (Fig.4) 및원인불 " 닝이각 2 예였다 (Tab l e 2). Portal Vein T hrombosis in Hepatocell ular Carcinol1la Period Cases of Cases of PVT* Hepatoma (%) T otal Period (3 1.2) Feb Dec ( 9.6) Jan 85-Sept (50.0) PVT* : Portal Vein Thrombosis 간안에서문맥혈전증의빈도는동일기간내에초음파 검사상진단된간암 1 57 예중 4 9 예 (3 1. 2%) 플나E때 었다. 그 c. 1 나온논운을꺼 l 획힌 년 1 월이후에는 간암이있는환자에샤전향상 S로문정맥을초사하였고, 그 결과 84 예종 42 예 (50 % ) 에샤 혈전증이관참되 었 며이는후향성으로조사하였던 년과비교시 떤저한차이윤보였다 ( Tabl e 3) 3. 임상증상 환자 57 예 종 47 예에서복부동흥과불쾌감을호소했 a벼 그외에피로깎, 소화불링, 토현 및 황달등을호 소하였 나꺼 111~ 얀신즙의진단이 l 득이한증싱 및소견은 없었다 4 초음파소건 가. 혈전의에코양상, 분포및문정액의직정혈전의에코는주위간싣질과비교하였으며동등한에코플보인경우가 41 예 (72 % ) 였고고에코성이 1 2 예, 지에코성이 4 에였다. 혈전의분포는간내분지에만있던경우가 1 8 예 (31. 6 % ) 였으며 3 8 예 (66.7 %) 에서는간내분지와함께주문정맥까지신전을보였고, 상장간악정맥및하공정맥까지파급된예 ( Fig. 5 ) 도있었다 ( Fig. 6). 이플종합하연, 혈전은우측간내분지에 4 7 예, 좌측간내분지에 28 예빛주운정맥이 1 38 예로주로간내분 ;< 1 와주문정맥내에존재하였다 (F i g. 7). 혈전증이있는운정맥의직경은정상엠위인 1.3 cm 이히인경우는단지 8 예 (1 4 % ) 로, 대부분에서확장되었고 2 cm 기념는경우도 1 4 예에서관찰되었다 (Ta b l e 4) 나문맥혈전증과간암과의관계

The first nll ll1ber below each figure means the nllmber of cases, ancl the second nll ll1ber ll1eans the percentage 계가없었으냐간암의외연이불분병하거나미만성침윤 성유형을보이는종 % 에서반도가높음을보였고종꾀의크 기가직경 5 cm 이하인경우는혈전의빈도가떨어지나 5 cm 을넘는경우에는훨씬증가함을보였다 (Table 5). 다. 기타소음파소견 간경화로인한비종대나복수의동반이많았으며측부 혈행은 1 5 예에서관찰되었고주로운맥주위나당낭와에 서관찰되었다 (Table 6) (Fig. 3 -B). 내시경검사를 시행한 21 예중 16 예에서식도정맥류가발견되었으나 이들중초음파검사셰서는단지 7 예만이측부혈행이관 찰되었고, 전산화단층촬영술상양성 7 예중 3 예, 혈관 조영술상양성 5 예중 I 예에서관찰되어측부혈행의 진단율은초음파검사가다소낮음을일수있었다 (Ta Fig. 7. A lb acete 등 4, 은 32 % 의밴도를보고하였으며저자의전향성연구에 서는 50 % 의빈도을냐타내 한 57 % 와비슷하였다. 간암의수울에의한절제 이는 Mathieu 등 2, 이보고 가능성을결정하는데는몇가 지의요소가따르며종양의범위, 정맥침습의유무및간 외로의침숨유무등을아는것이중요하다. 종양이위치 하는간내문맥이상으로신전된혈전증이있을경우는이 미수숭의긍기사항이되며. Shinakawa 등 2 2 l 은간암의 70 % 에서간내문맥정맥을. 13 % 에서간정맥까지의침 습을보고하였고. Albaæte 등은간암의 32 % 에서주운 정펙의침속이있다고하였무나 Nakashima 등강 ) 은이 보다훨씬높아주문정맥은 64.7%. 간정액은 % 에서침슴한다고하였다저자들의연구에서도문맥헬 전증이있던간암 49 예중 35 예 C 71.4 %) 에서이미주 운정맥을침습하여발견당시매우진전된상태임을알 결과를보였다. 간암을제외한악성 압딱및 간종양에서는운정맥및간정액의 전위소견을냐타내는문펙강내로의침습은매우 드물며 7, 따라서간종양이있는흰자에서문맥현전증의 유무를보는것은종양의감! 블진단에있어서도중요한 소견이다 문정맥의조영을위해과거에는경비문맥조영술이나 동문액조영술이이용되었으나, 게부담을주며위험이따르기때문에 이는침슴적이고환자에 최근에는초음파 검사나전산화단층촬영울을이용한비침습적인방법으로 진단을하게되었다. 이들중초음파검사는여러가지장 점이있어집단검진검사 ( Screening test ) 로널리이용되고있다 10. 문정맥을초음파겸사로인지할수있는비율은상당히 높아 97 % 까지관찰할수있무며 10,24) 문정맥의장축 을쉽게추적할수있 a 므로전산화단층촬영솔보다유리 하며, 초음파검사상문맥혈전증을의심한경우에가양성 은없다고보고한사람도있다 11, 문맥혈전층의 초음파소견으로는문맥강내에반향성인 혈전의에코를보는것이중요하며그밖에측부혈행, 문 정맥의확정, 해띤종성변형 (cavernomatous transformation ) 등의소견을보는것이도웅이된다 11, 16,25> 혈전의에코양상은대부분에서주위간실질의에코와거 의동등한에코를보이기때운에 구벨이되는경우가않으며, 운정맥의혈관벽으로만 간휴양에서종괴가있는부 위의운정맥분지가보이지않을경우는혈관벽을파괴시 키는종양의침슈이있음을뜻한다 2, 일반적으로측부혈행이발달되기까지는어느정도의시 간이요하므로측부혈행은간경화증등양성질환인경우가 간암갇은악성질환보다많이 나타나며 11, 문정맥폐색 이서서히진행되연 90 % 에서측부헬행이형성된다고한다 13, Ga nsbeke 등 11, 은문맥혈전증때초음파소견을시기에 따라 3 개의연속되는단계로구별할수있다고하였는 데, 우선운펙강내에반향성의헬전이보이다가, 혈전과 힘깨작은측부혈행을규명할수있고, 오리 양성질환이어서 생존하는경우문정액의해면종성변형을볼수있 다고히였으며이는간암에서는드문소견이라고하였다. 해면종성변형의형성은보통 개월이소요되며 26, 수년후에초유파검사냐전산화단층촬영숭에영맥히나

9 大훼!iX M~ 옆짝 ~t,ht. : mz2 천짜 3 ~JÆ 타난다고하나 25, 27, 저자들의연구에서측부혈행이규명며, 크고외연이불분명한미만성침윤성종양에서호발 되었던예을중다수에서해면종성변형을규영할수있었 하였다 다 4. 전산화단층촬영숲은 15 예에서병행되었는데조영 문정맥직경의정상상한선을 Weinreb 등!7, 의보고에 제투여후모든예에서문정맥내에서저음영혈전의소 의한 13mm 로보았으며, 이때문정맥의확장은저자들견을보였고 1 0 예에서문맥주위의초영제증강소견을 의경우 86 % 에서보였고. Gansbeke 등 11 은 15 mm 를냐타냈다. 상한선으로하여 38 % 정도에서보였다고하였다 5. 측부혈행은간문부및담낭와에서주로관찰되었 전산화단층촬영술상소견은저음영의혈전과함께혈 전이있는문정맥의확장을보이며그밖에조영제증강 무로명확히구멸되는혈관벽을볼수있다 16. 혈전의 음영은응고혈액의시간파연관성을보이므로 28, 29,30,3 1> 신선한혈전은혈액과비슷한고음영을보일수있다. 혈관벽은조영제주입후에는액관의혈관 (Vasa vasor - uml 에의한증강작용에의해비교적고음영의테두리를 보여 선로와같은소위 rail sign 을보이며 2 문정맥 내의종양침습에의한혈전은간동맥과문정맥의맥판혈 관으로부터혈액공급을받으므로혈관조영술이나전산 화단층촬영술을시행하띤초음파검사에서일수없었던양성과악성혈전을감 l 설할수있다고한디 32. 초음파검사는다른어떤검사보다도문정맥의영상을 관찰하는데도움이되나, 초음파검사의시술지 - 의숙련도 및시행당시의띤밀한검사가요구되므로운맥혈전증의 가장중요한원인인간암이의심될때에는전향성으로문 정맥을자세히관찰하떤진단뿐만아니라환자의치료방 침결정및예후판단에많은도움을줄것으로사료된다.
