I evaluated the cases referred for HLA-B27 antigen using flowcytometry (FCM) to find the clinical characteristics of the patients and the diagnostic utilities of median fluorescence intensity (MFI) in HLA-B27 program. Methods:I evaluated 443 subjects of HLA-B27 cases using FACScan flowcytometry, consisted with software for automated calibration and analysis, calibration beads, and the anti-hla- B27 fluorescein isothiocyanate (FITC)/anti-CD3 phycoerythrin (PE) monoclonal antibodies (all from Becton Dickinson, San Jose, CA). Results:Of the total 443 cases, the positive rate in male cases was 44% (132/300) and it was higher than that of female cases (22.4%, 32/143). The gating procedure was failed in one sample of 443 (0.23%). The positive rates in each diagnostic criteria were as follows; AS 61.6%, gout 20.0%, herniated intervertebral disc 20%, lower back pain 25.6%, polyarthritis 16.0%, psoriatic arthritis 20.0%, rheumatoid arthritis 28.3%, reactive arthritis 26.9%, SpA, undifferentiated 31.8% and uveitis/iritis 23.8%. In AS group, 89 cases (95.7%) showed MFI values higher than 150. Conclusion:About 62% of AS group showed HLA-B27 positivity using FCM and the positive rates of other diseases group in SpA categories were around 20-30%. If we considered MFI value 150 as differential value, about 95% of HLA-B27 positive AS cases might not need further confirmatory study to differentiate HLA-B7. Key Words:Flowcytometry, HLA-B27, Ankylosing spondylitis, Spondyloarthropathy 서론 HLA-B27 은척추관절병증 (spondyloarthropathies; SpA) 과강력한연관성을보이고있으며이중대표적인사례는강직성척추염 (ankylosing spondylitis:as) 이다 [1-3]. 유세포분석기와단클론항체의발달은 Human Leukocyte Antigen (HLA) 연구에새로운방법을제시하였으며, HLA-B27 항원의진단에도편리함을주었다. 유세포분석기를이용한 HLA-B27 항원검사의진단적유용 성은세포독성검사 (micro-lymphocytotoxicity test) 와비교되어입증되었고 [4,5], 현재통상적으로유세포분석기를이용하여 HLA-B27 검사를시행하는기관도적지않다. 이에저자는약 3년간서울백병원에 HLA-B27 검사가의뢰되었던환자군을대상으로후향적으로이들의진단명을조사하여 HLA-B27 양성을보인환자군의분포와특징을찾아보고자하였다. 또한유세포분석기로 HLA-B27 검사시사용되는 median fluorescence intensity (MFI) 에대해진단적유용성을찾을수있는지여부도조사하였다. 본논문은 2002 년도인제대학교학술연구조성비보조에의한것임. 교신저자 : 신보문우 ) 서울중구저동 2 가 85 번지서울백병원진단검사의학과전화 : , FAX: 대상및방법 1. 대상서울백병원진단검사의학과에 HLA-B27 검사가의뢰되

2 210 신보문 었던 443 예의환자군을대상으로하였으며남자는 300 명 (67.7%), 여자는 143 명 (32.3%) 였다. 임상적진단명은후향적으로의무기록지를참조하였다. 이들환자군은 physical examination 및 rheumatoid factor, anti nuclear antibody 검사및방사선과적검사를시행하여류마티스성관절염 (RA) 과혈청음성척추관절병증 (seronegative spondyloarthropathies) 을감별하였으며, 이단계에서도감별이어려운환자및 AS 를의심하는환자의확진과반응성관절염및모양체염환자처럼예후판정을위한추가적검사로 HLA-B27 검사를의뢰한사례들이다. 2. 방법 FACScan 유세포분석기를이용하여 anti-hla-b27 fluorescein isothiocyanate (FITC) 와 anti-cd3 phycoerythrin (PE) 을단클론항체로이용하여 2 color 형광염색을시행하였다 (Becton Dickinson, San Jose, CA). HLA-B27 검사를시행하기전에 CaliBRITE beads 와 AutoCOMP software 를이용하여유세포분석기를 setting 하였다. 검사를위해각각의시험관에 30uL 씩의 anti- HLA-B27 fluorescein isothiocyanate (FITC) 와 anti- CD3 phycoerythrin (PE) 단클론항체를 50uL 의 EDTA 혈액과혼합하여실온암소에서 15분간반응시켰다. 이후 2mL 의 FACS Lysing Solution (Becton Dickinson) 을더하여진탕후실온암소에서 10분간반응시켰다. 이반응검체를실온에서 300g 로 5분간원심분리후가라앉은세포층을 2mL phosphate buffer saline (PBS) 로재부유시키고같은원침과정을다시반복후재부유된세포 ( 림프구 ) 를이용하여검사를시행하였다. 매검사당 10,000 개의세포를분석하였으며분석대상은프로그램상 forward scatter (FSC) 와 fluorescence 2 (FL2) 를이용하여 T 림프구만이따로 gating 된후 (Fig. 1), 이 T 림프구들의 fluorescence 1 (FL1) 에서의 MFI 값이 lot 에따라미리정해진 특이적인결정치 (decision marker value) 보다크거나같으면양성으로판정하였다 (Fig. 2). 결 1. HLA-B27 양성예의 MFI 분석 HLA-B27 양성 AS 환자군의 MFI의평균값은 160이었으며이중 4예 (4.3%) 에서만이 MFI 값이 150 이하였다 (147, 147, 148 및 149). 요통환자군, 비특이적 SpA 환자군및모양체염 / 홍채염환자군중 HLA-B27 양성을나타낸경우는모두 150 이상의 MFI 값을보였다. HLA-B27 양성 RA 환자군에서는단지 1예 (6.7%) 만이 150 이하의 MFI값을나타냈으며 HLA-B27 양성반응성관절염군에서는 3예 (42.9%) 에서 150 이하의 MFI 값이관찰되었다. 고찰 HLA-B27 항원은 SpA 의다수에서나타나며이중가장중요한질환은 AS 이다. AS 는염증성질환이며남자에흔 하고척추와 pelvis 의관절에가장손상을주는질환이다. 본연구의각질환군에서도남자환자의 HLA-B27 양성율이여자에비해유의하게높았으며특히 AS 환자군에서는남자의양성율이여자의약 5배로나타났다. AS 와 SpA 질환군에서의 HLA-B27 항원의높은발현율은이들질환군과 HLA-B27 의강한연관관계를시사하고있다. 일반적으로 AS환자군에서의 HLA-B27 의양성율은 90-95% 이상인것으로알려져있다 [1-3]. 반응성관절염, PSA 및비특이적 SpA 등은모두 SpA 질환군에포함되는질병군이다 [6-9]. AS 와반응성관절염 /Reiter's disease 가 HLA-B27 과높은연관관계를보이는것에비하여 [1-3,6], HLA 와 psoriatic arthritis 와의연관성은뚜렷하지않다 [7,8]. HLA-B27 양성인이반응성관절염 /Reiter's disease 으로발병할가능성은실질적으로매우낮아일부보고에서는위험도를약 0.67% 로보고하였다 [6]. 그럼에도불구하고 HLA-B27 항원은반응성관절염 /Reiter's disease 환자의 60-80% 에서관찰되며이는인구집단의유전자발현빈도에따라양성율이변할수있다고한다 [6]. 한국인에서의 HLA-B27 의유전자발현빈도는 3.6% 로알려져있다 [10]. 본연구의 AS, RA, psoriatic arthritis, 다발성관절염, 반응성관절염, 비특이적 SpA, 모양체염 / 홍채염질환군에서의 HLA-B27 양성율은다른연구결과들과비교시낮은편이다. 이러한원인으로는진단명이 HLA-B27 검사의뢰시와다르게차후에바뀔여지가있기때문으로여겨진다. 예를들면요통으로인해 HLA-B27 검사를의뢰한경

4 212 신보문 우 AS 라고확진할수는없었으나 HLA-B27 검사와추후경과관찰에의해 AS, 혹은 SpA 질환군중하나로확진될수있기때문이다. 또한 AS, SpA, 반응성관절염등과모양체염 / 홍채염의상호관계처럼 HLA-B27 과연관된질환군들은질병의경과에따라질병이겹치거나중간에나타나는경과를취하기도하기때문이다. 모양체염환자 236 명을조사한연구에서이들중 13% 에서 SpA 를기저질환으로갖고있는것이밝혀진연구도있으며 [9], AS 환자군의 1/3 정도와반응성관절염환자군의 20% 는질환의진행시기중어느때든모양체염 / 홍체염이발현되나다른 SpA 질환군에서는이러한발현은드문것으로알려져있다 [1,3,6]. 우리는 HLA-B27 프로그램에따른유세포분석기를사용하여 HLA-B27 분석을할때일반적으로 MFI 값은유념해서보지않는경향이있다. 그러나 MFI 값이 HLA-B27 과 HLA-B7 을감별하는데중요하다는것을보여주는연구들이있다 [4,5]. 유세포분석기를이용하여 HLA-B27 항원유무를검사하는데있어서일반적으로위양성을나타내는데는 HLA-B7 항원에의한것이가장많은것으로알려져있다 [4,5]. 때로는 HLA-B22, HLA-B37 및 HLA-B39 항원이위양성결과를초래하기도한다 [5]. 그러나이들교차반응항원들의 MFI 값은일반적으로 HLA-B27 의 MFI 값보다낮게나타났다. HLA-B27 양성예의 95% 이상에서강한 MFI 치를나타냈고대부분의 HLA-B7 예들은음성이거나양성이어도약한 MFI값을보여주었다. MFI값이중복된경우는단지 3% 였다 [4]. HLA-B27 진단프로그램에서의 HLA-B27 양성은 lot 에따라특이적으로결정된 FL1 decision marker 값과같거나이보다높은 MFI 값을나타내는경우로정한다. 본연구에서는 lot 특이적 FL1 decision makers 의값은 142에서 144였다. 본연구에서는 MFI 값이 150 이상을보이는경우는강양성으로판정하고 150 이하에서양성을보이는경우는약양성으로정하였다. 이약양성예는위양성가능성을배제하기위해림프구독성검사나 DNA 검사를실시하여야했는데본연구에서는시행하지는못하였다. 따라서본연구에서 HLA-B27 이양성이나 MFI 값이 150 이하를보였던 4예의 AS 환자들은 HLA-B7 혹은다른교차항원에의한양성일가능성도전혀배제할수없을것으로여겨진다. 이것은다른연구결과들과도유사한것으로, Hulstaert 등은유세포분석기를이용한 HLA-B27 진단시민감도, 특이도는각각 100% 와 97% 로보고하였다 [5]. 본연구에서 gating 이실패한것은 1예로전체의 0.23% (1/443) 였다. 이환자의당시백혈구수치는 6700/uL 였으며백혈구백분율상림파구는 37% 로정상범위에속하였으나, 이들림파구의몇 % 가 T림프구였는지는밝히지못하였다. Hulstaert 등은그들의연구에서 0.28% 의 gating 실패율을보고하였으며, T 림프구감소증이원인이라고하였다 [5]. 요약배경 :HLA-B27 은강직성척추염을포함한척추관절염질환군과밀접한관계를가지고있는조직적합성항원이다. 유세포분석기를사용하여 HLA-B27 을검사한경우각질환군별연관성및 median fluorescent intensity(mif) 의유용성을살펴보고자하였다. 방법 : 서울백병원진단검사의학과에 HLA-B27 을의뢰한 443 명의환자군을대상으로각질환군별로분류하였으며, 유세포분석은 anti-hla-b27 FITC/anti-CD3 phycoerythrin 을사용하여 FACScan (Becton-Dickinson, USA) 로시행하였다. 결과 : 전체 443 명중남자의 HLA-B27 양성율은 44% (132/300) 로여자의 22.4%(32/143) 에비해통계학적으로유의하게높았다 (p<0.01, chi-square test). 강직성척추염은 151 예 (34.1%) 였으며, 양성율은 61.6%(93 예 ) 였다. 양성자의 82.8%(77예 ) 가남자였다. 이외의질환군에서의양성율은류마티스성관절염 28.3%, 비특이척추관절염 31.8%, 모양체염 / 홍채염 23.8%, 통풍 20.0% 등이었다. 분석시 gating 이실패한예는 1예 (0.23%) 였으며, HLA-B27 양성인강직성척추염환자군중 95.7%(89 예 ) 가 150 이상의 MFI 값을나타내었다. 결론 : 유세포분석을이용한 HLA-B27 검사상강직성척추염환자군의약 62% 에서양성을보였으며, 강직성척추염이외의모든척추관절염환자군의 20-30% 에서도양성반응을보였다. 유세포분석시 150 이상의 MFI 치를기준값으로사용한다면강직성척추염환자중약 95% 의양성반응군은유세포분석만으로도검사가가능하리라고사료된다. 참고문헌 1.
