Authors experienced a case of avulsion fracture at the femoral attachment of ACL in a skeletally mature, a 21-year-old male associated with ipsilateral femoral shaft fracture. Here, authors report on the case with a literature review. Care should be taken because an avulsion fracture at the femoral attachment of ACL can be accompanied by ipsilateral femoral shaft fracture in skeletally mature patients. Conflict of interest: None. 착부의견열골절이더많이발생하는경향이있다. 1,2) 골성숙이완료된환자에서도부착부의견열골절이발생하기는하나주로경골부착부에서발생한다. 3,4) 저자들은골성숙이완료된환자에서동측의대퇴골간부골절에동반된전십자인대대퇴골부착부견열골절을경험하였기에문헌고찰과함께보고하는바이다. 증례보고 25 세남자환자가내원당일이륜차교통사고후발생한우측대퇴부통증을주소로내원하였다. 응급실에서시행한단순방사선검사에서 AO/OTA 분류 32-A3 형의대퇴골 Copyright c 2016 The Korean Fracture Society. 이에 슬관절에 대한 이학적 검사를 시행하였으나, 통증으로 인하여 부종 외의 소견을 확인할 수 없었다.

술 후 8일째 진단적 관절경을 시행하였으며, 관절경검사상 지 침자를 이용하여 확인한 결과 전십자인대의 긴장이 유지되 고 불안정성을 보이지 않아 보존적 치료를 시행하기로 하 였다(Fig. 4).

And initial knee anteroposterior (C) and lateral (D) image without a sign of fracture. 간부 골절이 확인되었다(Fig. 1). 신체검사에서 우측 대퇴 부에 부종이 관찰되었고 압통이 있었으며, 슬관절부에 창 상은 없었다. 응급실에서 시행한 슬관절 단순 방사선 검사 에서는 특이 소견을 발견할 수 없었다(Fig. 1). 대퇴부의 통증 및 변형으로 슬관절부의 이학적 검사는 시행할 수 없 었다. 내원 다음날 전향적 골수내정( mm)을 이용 하여 내고정을 시행하였다(Fig. 2). 수술장에서 슬관절 이 학적 검사를 시행하였으며, 슬관절의 부종 외에 불안정 등 특이소견을 발견하지 못하였다. 수술 후 3일째부터 환자는 대퇴부 통증보다 동측 슬관절 통증을 더 심하게 호소하였다. 이에 슬관절에 대한 이학적 검사를 시행하였으나, 통증으로 인하여 부종 외의 소견을 확인할 수 없었다. 이에 혈관절증으로 판단하여 자기공명 영상 검사를 시행하였으며, 검사 결과 전십자인대 대퇴골 부착부의 불완전 견열 골절 소견을 확인하였으며, 골절편 의 확인을 위해 전산화 단층촬영을 시행하였다(Fig. 3). 술 후 8일째 진단적 관절경을 시행하였으며, 관절경검사상 지 침자를 이용하여 확인한 결과 전십자인대의 긴장이 유지되 고 불안정성을 보이지 않아 보존적 치료를 시행하기로 하 였다(Fig. 4). 환자는 전십자인대 보조기를 8주간 착용하에 슬관절 관절범위 운동을 진행하였다. 술 후 6개월 추시 시 방사선 검사상 대퇴골 간부의 골유합을 확인할 수 있었으 Fig. 2. Arthroscopic images show a grossly normal looking anterior cruciate ligament (A) and an intact anterior cruciate ligament with applied tension using probe (B) (black arrows). 고 찰 전십자인대의 대퇴골 부착부 견열골절은 드물고, 주로 소아환자에서 보고되었다.1,2) 골성숙이 완료된 환자에서의 전십자인대 대퇴골 부착부 견열골절은 소아환자에서보다 드물어 저자들은 오직 1예만을 문헌 검색을 통하여 확인할 수 있었다.5) 전십자인대 견열골절의 치료에 있어서 전위가 없는 경우 보존적 치료, 경도의 전위가 있는 경우 보존적 치료나 수술적 치료를 할 수 있으며, 전위된 경우에는 수 술적 치료를 하는 것이 권장된다. 수술적 방법으로는 주로 관절경적 방법으로 K-강선이나 나사못, 봉합술 등을 이용 하여 해부학적 위치에 고정하고 관절운동을 시작하여야 한 다.3,6) Griffith 등3)은 불완전 견열골절의 경우에 온전한 부 착부에서 전십자인대가 박리되는 경우가 있어 관절경검사 를 통하여 확인하여야 한다고 하였다. 본 증례에서는 경도 의 전위를 보이는 골연골 골편이 전산화 단층촬영과 자기 공명영상을 통하여 확인되었으나, 대퇴골 부착부의 일부만 이 골절되어 안정성이 유지된 것을 관절경검사로 확인한 후 보존적 치료를 시행하였다. 본 증례에서 특기할 만한 것은 전십자인대의 대퇴골 부 착부 견열골절이 대퇴골 간부 골절에 동반되어 발생하였다 는 점이다. 전십자인대 대퇴골 부착부 견열골절의 발생기 전은 명확하게 밝혀지지 않았으며, 회전력이 동반된 과신 1,2) 전 혹은 과굴곡이 손상 기전으로 제시되고 있다. 본 증 례의 경우 대퇴골 간부 골절이 발생할 때 원위부인 슬관절 이 신전되어 과신전 손상에 의해 전십자인대 대퇴골 부착 부의 견열골절이 발생한 것으로 볼 수 있다. 대퇴골 간부 골절은 고 에너지 손상인 경우가 많아 동측

10) 본 증례에 서도 수술장에서 시행한 마취하 검사에서 불안정성이 확인 되지 않았으며, 수술 후 환자가 대퇴골보다 슬관절의 동통 을 호소하여 검사를 시행하고 진단이 되었다. 따라서 대퇴 골 간부 골절 환자에서 슬관절 손상이 동반될 가능성을 항 상 염두에 두는 것이 중요하다고 하겠다.

And postoperative 6 months knee anteroposterior (C) and lateral (D) image show no abnormal findings. 슬관절의 인대 손상이 동반될 수 있으며, 그 빈도는 5%-48% 정도로 다양하게 보고되고 있다.7,8) 그러나 대퇴골 간부 골절로 인하여 신체 검진에 어려움이 있어 진단이 쉽 8,9) 지 않아 주의를 요한다. 또한 자기공명영상 결과 인대 손상이 확인된 환자에서도 마취하 시행한 이학적 검사에서 이상이 발견되지 않는 경우가 많은 것으로 보고되고 있어 이학적 검사에서 특이 소견이 발견되지 않은 경우라도 환 자가 호소하는 증상에 주의를 기울여야 한다.10) 본 증례에 서도 수술장에서 시행한 마취하 검사에서 불안정성이 확인 되지 않았으며, 수술 후 환자가 대퇴골보다 슬관절의 동통 을 호소하여 검사를 시행하고 진단이 되었다. 따라서 대퇴 골 간부 골절 환자에서 슬관절 손상이 동반될 가능성을 항 상 염두에 두는 것이 중요하다고 하겠다. 저자들은 드물게 발생하는 성인 환자에서 발생한 동측의 대퇴골 간부 골절에 동반된 전십자인대 대퇴골 부착부 견 열 골절을 경험하여 문헌고찰과 함께 보고하는 바이다. 대 퇴골 간부 골절 시 슬관절의 과신전으로 인하여 전십자인 대 대퇴골 부착부의 견열골절이 발생할 수 있어 대퇴골 간 부 골절의 진단 및 치료 시 주의를 요한다. 2) Kawate K, Fujisawa Y, Yajima H, Sugimoto K, Tomita Y, Takakura Y: Avulsion of the cartilaginous femoral origin of the anterior cruciate ligament in a threeyear-old child.
