In this study, sediment btained frm ml fluid were cultured nt lysed bld agar(lb) and int ml thiglycllate brth, and ml uncentrifuged fluid were cultured int ml tryptic sy brth and thiglycllate brth. The bth results were cmpared t thse f previus studies. The psitive rate with ml centrifuged culture was. %, cmpared t. % and. % with previus studies, and indicated the vlume did nt influence the psitive rate. The prprtins f the islated bacterial species were similar t thse f ther studies. In cnclusin, ml centrifuged culture is effective as large vlume culture in the diagnsis f peritnitis f CPD patients. Key wrds : culture f dialysate, chrnic ambulatry peritneal dialysis( CPD ) I. 서론그러므로 CPD 환자복막염을적절히치료하기위 해서는세균배양과균종동정이매유중요하다고하겠 지속성복막투석 (cntinuus ambulatry peritneal 다. 그러나투석액중에는항균물질이있고 ll), 투석액 dialysis, CPD) 은혈액투석에비하여분자량이큰독의양이많아세균수가희석되어있으며 l), 또한세균 성물질을효과적으로제거할수있기때문에말기신이세포내에있는경우가있어서, 세균이나진균성복 부전환자의치료를위해널리사용되어왔다 -) 그러막염환자의모든검체에서양성결과를얻기는쉽지 나이치료를받는환자중일부는복막염을유발하게않다 ) 그래서배양양성을높이기위한연구가많았 되며이로인해사망하기도한다 -) 복막염은화학적다. 자극에의하여무균적으로생길수있지만, 세균이나 년 Dwsn l ) 등은투석액전량배양으로양성 진균에의한감염이더혼하다 -) 률이높아짐을보고하였다. 그후 Graevenitzl) 등은 - -

2 투석액의양과배양양성률이항상일치하지않으며, 검체 (. %) 에서 주의세균또는진균이분 투석액 ml 와 ml 의투석액배양에서같은결과리되었다 (Table ). 그람양성세균이 주 (. %) 로 를얻었음을보고하였다. 또한 Saublle ) 등도 ml 와 ml 의배양에서비슷한결과를얻었다고하였다 patients 세브란스병원에서는 - 년에는투석액전량 ( 약 L) 을배양하여 % 의양성률을보고하였고 l), 년 월 - 년 월에는 ml 를, 년 월 - 년 월에는 ml 를배양하여비슷한결과를 보임을보고하였다 ) 적은양의투석액배양은배지의 소모를줄일수있고, 검체의취급이용이해져서오염 률도줄일수있다. 이에이연구에서는소량의투석 액을배양하여도양성률이낮아지지않는지를검토하 고자하였다. II. 재료및방법 년 월 - 년 월에세브란스병원에서 CPD 치료를받던중복막염이의심되는환자로부터채취하 여의뢰된투석액 검체를대상으로하였다. 투석 액은 L 정도의주머니에담긴채로의뢰되었고, 가 능한한접수즉시접종하여배양하였으나, 접종이지 연될때는냉장고에보관하였다. 투석액주머니에서 무균적으로 ml 를취하여그중 ml 를 ml 의 tryptic sy brth(tsb, Difc) 와 thiglycllate brth (Thi, Difc) 에각각 ml 씩넣어 주일동안배양하 면서매일육안관찰하고세균증식이의심되면그람염 색과계대배양을하였다. 나머지 ml 는플라스틱원 침관에넣어서 rpm 으로 분간원침하여약 ml 의침사를 lysed bld agar(lb) 에 방울접종하고 나머지는 ml 의액체배지 (Brewer thiglycllate, Dif c) 에접종하였다. Lysed bld agar 는 tryptse bld agar (Difc) 에 % 혈액과. % sapnin(sigma Chemical C.) 을넣어만들었다. 접종된 LB 는 % C 항 온기에 시간, 액체배지는 주일간배양하였다. 증식 된세균은전통적방법으로동정하였고필요시에는상 품화된 kit 를이용하였다. Table. 3 α 없J -가장많이분리되었고그람음성세균은 주 (. %), 진균은 주 (. %) 가분리되었다. 균종별로는 cagulase 음성포도구균이 주 (. %) 로가장많았고, 그 다음으로는 P. aeruginsa 가 주, S. aureus 와 alpha 용혈 Streptcccus 가각 주, gamma 용혈 Streptcccus, E. cli 및. baumannii 가각 주이었다. 배양방법에따른분리균주수와양성률의분석에 있어서는오염으로생각되는 bacillus 주와 diph therids 주는제외하였다. 방법에따른분리균주수 는혈액배양용액체배지를사용한경우는 주 (. %) 가분리되었으나, ml 를원침한배양방법에서는 (. %) 가분리되었다 (Table ). 배지에따른분 Table. 4 리균주수를보면, LB 에배양된균주가 주, 혈주 (. %) 가분리되었으나, thiglycllate 배지에서는 액배양용액체배지에서는각각 주와 주였으나, 주 (. %) 가분리되었다. 진균은두배지모두에 ml thiglycllate 배지에배양된균주는 주로가서같이분리되었으나, LB 에서배양된그람양성구 장많았다 (Table ). 일배양후분리된균주수도균은. % 이었고그람음성간균은. % 인반면, ml thiglycllate 배지에배양한경우가가장많았 thiglycllate 배지에서는각각. % 와. % 로현 다. 저히높았다 (Table ). 또한 LB 에서배양된세균의 ml 투석액올원침후침사를배양한 LB와 thi- 증식을보면, 대부분의균주가소수의증식만을보였 glycllate 배지에서의배양률올보면 LB 에서는 다. LB ml Sme Few Ttal Thiglycllate N. 고찰 말기신부전환자가복막투석을시행하는동안에 세균과진균에의한복막염이한환자당 년에. --. 예가발생한다고보고되어있다 ) 투석환자의복 막염을일으키는균종은매우다양하다. 즉, 피부상재 균인 S. epidermidis 등의 cagulase 음성 Staphylcccus 를비롯하여, 장내세균, 환자의주변환경에있는포도당비발효세균등이다. Graevenitzl) 등은 cagulase 음성 Staphylcccus가 -- % 로가장많고, 그다음 으로는 S. aureus -- %, Enterbacteriaceae -- %, 포도당비발효그람음성간균 -- %, 그람양성 간균 -- % 순이며, 진균, Mycbacterium 및혐기성 세균에의한감염및중복감염은 % 미만이라고보 고하였다. 저자들의연구에서도 cagulase 음성 Staph~ lcccus 가. %, P aeruginsa. %, S. aureus. % 로많이분리되어이와비슷한결과를보였다. 투석환자의복막염을진단하기위해투석액침사의 그람염색이나 acridine range 염색이이용될수있으 나그양성률은 -- % 정도이다, 또한투석액의혼 탁도나백혈구수가검사되지만이들역시특이도나감 도가낮으므로배양이필수적이라고하겠다 ) 그러나 투석액중의미생물수가적고, 항균물질의존재등으 로인하여복막염환자의투석액배양에서양성률을 높이기는쉽지않다, ) 년이래로이에대한많 은연구가있어왔다. 저자들의병원에서도 년부 터여러가지배양방법을사용하여그결과를보고한 바있다. 그결과투석액양을, ml 에서 ml 까 지줄여서배양하여도양성률에는별다른차이가없어 다른연구자들의보고와유사하였다,, ) 최근여러연구자들은투석액배양을위해혈액배양 용액체배지를사용하여그양성률이매우높음을보 - -

5 고하였고, ), ml 투석액을원침하여침사를접종했 을때에도좋은성적을얻었다고하였다, ) 적은양의투석액배양은배지의소모를줄일수있고, 검체의취급이용이해져서오염률도줄일수있다. 따라서 저자들은이번연구에서소량의투석액을배양하여도 양성률이낮아지지않는지를검토하고자 ml 투석액 을원침하여 LB와 ml thiglycllate 배지에접종하였고, 또한 ml 투석액은혈액배양용배지에배양 하여. % 의양성률을얻었다. 저자들의지난번연 구결과를보면, 투석액전량배양시의 %, ml 배양시의. %, ml 배양시에. % 였다, ) 따 라서이양성률의차이는배양방법으로인한차이보다 는시기에따른차이였던것으로사료된다. 또 Saublle l ) 등이 ml 투석액을원침하여배양한것과 혈액배양용배지에배양한것이총양성검체중 % 와 % 의감도를보였는데이번연구에서각각. % 와. % 로원침후에배양한것이더나은양성 률을보였다. V. 결론 소량의투석액배양법은 ml 이상의배양으로얻 은양성률과의의있는차이가있지않으며, 이방법은 배지의양을절약할수있고, 술식이간편하여오염률 도줄일수있을것이라는결론을얻었다.
