Moreover, there is a lack of knowledge of the radiological findings of the joints, and thus, the extent of joint ligament damage may be underestimated, leading to improper treatment. This paper reports three cases of Chopart joint injury and seeks to reconsider the importance of Chopart joint evaluation and treatment. Chopart 관절손상에대한임상적의구심과이에따른즉각적이고정확한영상의학적평가라고보고한바있다. 본저자들은드물게발생하는 Chopart 관절골절및탈구 3예를경험하였기에이에대해보고하고자하였고, 더불어 Chopart 관절손상에대한평가와치료에대한중요성에대해환기하고자하였다. 본증례보고는단국대학교병원임상시험윤리위원회 (Institutional Review oard) 의승인을받아연구를진행하였다. 증례보고 1. 증례 1 17세의남자환자로 4층높이건물에서추락하며수상하여응급실을통해내원하였다. 응급실에서시행한이학적검사상우측중족부로통증, 압통, 부종이있었으나감각신경이상및운동신경이상은없었고개방성창상은관찰되지않았다. 단순방사선촬영상우측족부거주상골관절및종입방골관절로골절및탈구소견이관찰되었고 (Fig. 1), 우측족부컴퓨터단층촬영 (computed tomography) 상에서도동일한소견이관찰되었으며 (Fig. 2), Zwipp 분류 (Zwipp classification) 상 (Fig. 3) 2) transnavicular와 transcuboid 의 combination type의 Chopart 관절손상으로생각되었다. 동반손상으로우측대퇴전자하골절 (subtrochanter fracture) 및좌측 Copyright c2018 Korean Foot and nkle Society. ll rights reserved. C 족관절 원위 경골 골절(distal tibia fracture), 천골 골절(sacrum)이 관찰되었다. Transnavic Transcub 수상 4일째 수술을 시행하였고 우측 족부 거주상골 관절 골절 및 탈구에 대하여 관혈적 정복술(open reduction) 및 경피적 K-강 선 고정술(K-wire fixation; diameter 1.6 mm), 우측 족부 종입방골 Translig Translig 관절 골절 및 탈구에 대하여 비관혈적 정복술(closed reduction) 및 외고정술(external fixation)을 시행하였다(Fig. 4). 동시에 우측 대 Transtalar Transcalc 퇴 전자하 골절 및 좌측 족관절 원위 경골 골절에 대해서도 수술 을 시행하였다. 수술 후 환자는 특이사항 없이 안정적이었으며 수 술 후 시행한 이학적 검사에서도 감각신경 이상 및 운동신경 이상 Figure 3. Zwipp classification2) of fracture and dislocation at the Chopart joint. 은 관찰되지 않았다. 수술 후 2주 뒤 우측 족부에 대해서 전환수술 (conversion operation)을 시행하였다. 우측 족부 거주상골 관절 골 절 및 탈구에 대하여 첫 번째 수술에서 고정한 K-강선 1개를 제거 한 후 주상골 골절(navicular fracture)에 대해 K-강선(diameter 1.2

Dislocation of talonavicular jointand flat foot deformity were observed (, ). 시행하였고, 우측 족부 종입방골 관절 골절 및 탈구에 대해 고정 한 외고정 장치를 제거한 후 가교 금속판(bridging plate) 내고정술 주상골 관절 탈구로 인한 편평족 변형(flat foot deformity) 등의 부 (internal fixation)을 시행하였다(Fig. 5). 전환수술 직후 우측 족부 정유합(malunion) 소견이 관찰되었고, 수술 후 7개월째 가교 금속 감각신경 이상 및 운동신경 이상은 관찰되지 않았다. 전환수술 후 판 제거술을 시행하였다(Fig. 6). 환자에게 특별한 불편감이 없어 4주째 나머지 경피적 K-강선을 제거하였고, 8주째까지 단하지 석 인솔(insole)을 적용하여 추시관찰하였다. 수술 후 10개월째부터 고고정을 시행하였다. 이후 관절 범위 운동 및 목발을 이용한 부분 환자는 보행 시 거주상골 관절 부위로 통증을 호소하였고 이후 지 체중부하 보행부터 점진적으로 전 체중부하 보행을 시행하며 특 속되는 통증으로 수술 후 1년 3개월째 거주상골 관절 탈구 정복 후 이 불편감 없이 일상생활을 하였다. 수술 후 6개월째 우측 족부 체 자가 장골 이식술(auto iliac bone graft)을 이용한 거주상관절 관절 중부하 단순 방사선 촬영 추시상 골유합 소견이 관찰되었으나 거 유합술(arthrodesis)을 시행하였다(Fig. 7). 관절 유합술 후 6주째까

Preoperative simple radiographs of the foot anteroposterior () and lateral () show that 3rd to 5th metatarsal bone fractures, talar head and cuboid bone fractures were observed and dislocations were observed in talonavicular joint, calcaneocuboid joint. 지 단하지 석고고정을 시행하였고, 술 후 10주째 목발을 이용한 부 었다. 동반 손상으로는 좌측 하지 근위 경골 골절(proximal tibia 분 체중부하 보행 및 술 후 3개월째부터 전 체중부하 보행을 시행 fracture) 및 좌측 오금 동맥 파열(popliteal artery rupture)이 관찰되 하였다. 추시관찰상 환자는 통증 호소 없이 일상생활을 하였으며 었다. 응급실에서 시행한 이학적 검사상 우측 족부로 통증, 압통, 우측 족부 단순 방사선 촬영 추시상 특이사항은 없었다. 심한 부종 및 타박(bruise) 소견이 관찰되었으나 감각신경 이상 및 운동신경 이상은 관찰되지 않았고 개방성 창상은 없었다. 좌측 하 2. 증례 2 지에 대해 수상 당일 응급 수술을 시행하였다. 수상 8일 후 환자 상 39세 남자 환자로 건설현장 노동 시 금속 H-빔(H-beam)에 양하 태가 안정화된 이후 우측 족부에 대하여 수술을 시행하였고, 우측 지 압궤되며 수상하여 응급실을 통해 내원하였다. 응급실에서 시 족부 거주상골 관절 탈구에 대하여 관혈적 정복술 및 경피적 K-강 행한 단순 방사선 촬영상 우측 족부 제 3 5번 중족골 골절(meta- 선(diameter 1.6 mm) 고정술, 거골 골두 골절(talar head fracture)에 tarsal bone fracture), 거주상골 관절 및 종입방골 관절로 골절 및 대해 관혈적 정복술 및 피질골나사못(diameter 3.5 mm) 고정술, 탈구 소견이 관찰되었고(Fig. 8) 우측 족부 컴퓨터 단층촬영상 동 우측 족부 종입방골 골절 및 탈구에 대하여 비관혈적 정복술 및 경 2) 일 소견이 관찰되었다(Fig. 9). Preoperative simple foot anteroposterior radiograph shows talonavicular joint dislocation. 며 응급실에서 시행한 이학적 검사상 우측 족관절 및 중족부로 통 증, 압통, 부종, 타박상이 있었지만 감각신경 이상 및 운동신경 이 을 시행하고(Fig. 10) 단하지 부목고정 후 수술을 종료하였다. 수 상은 없었고, 개방성 창상은 관찰되지 않았다. 단순 방사선 촬영상 술 후 우측 족부 감각신경 이상 및 운동신경 이상은 관찰되지 않았 우측 족부 거주상골 관절의 탈구 소견이 관찰되었고(Fig. 11), 우 다. 수술 1주 후 수술 부위의 부종이 완화된 것을 확인하고 단하지 측 족관절 컴퓨터 단층촬영상 동일한 소견이 관찰되었다(Fig. 12). 석고고정을 시행하였다. 수술 후 5주째까지 추시관찰 시 환자에게 우측 족부 거주상골 관절 탈구 소견에 대해 Zwipp 분류상(Fig. 3)2) 통증 등 불편감의 호소가 없었고 단순 방사선 촬영 추시상 Chopart transligamentous type의 Chopart 관절 손상으로 생각되었다. 동반 관절이 잘 정복되어 있었으며 금속물이 특이사항 없이 유지되고 손상으로 우측 족관절 원위 경골 골절 및 거골 체부(talar body) 후 있다. 방부 골절이 관찰되었다. 수상 2일째 수술을 시행하였으며 우측 족부 거주상골 관절 탈구 3. 증례 3 에 대해 관혈적 정복술 및 경피적 K-강선(diameter 1.8 mm) 고정 35세 남자 환자로 자가용 운전 중 단독으로 가로수를 들이받으 술을 시행하였다(Fig. 13). 동시에 우측 족관절 원위 경골 골절 및 며 수상하여 응급실을 통해 내원하였다. 타과 관련 문제는 없었으 거골 체부 후방부 골절에 대해서도 수술을 시행하였다. 수술 후 우

Chopart Joint Fracture and Dislocation 125 Figure 12. xial view of computed tomography shows talonavicular joint dislocation. 측족부감각신경이상및운동신경이상은관찰되지않았다. 수술 직후단하지부목고정을시행하였고수술 4 일후수술부위부종 이완화된후단하지석고고정시행과함께목발을이용한비체중 부하보행을허용하였다. 수술후 2 주째부분체중부하보행을허 용하였고, 수술후 6 주째석고고정제거후관절범위운동및수술 후 8 주째전체중부하보행을허용하였다. 수술후 3 개월째 K- 강선 제거술을시행하였으며단순방사선촬영추시상 Chopart 관절이 잘정복되어있고불편감없이일상생활에복귀하였다. 고 찰 Chopart 관절은거주상골관절과종입방골관절로구성된관절 로, Chopart 관절손상은드물게발생해진단을놓치거나부정확하 게진단하기쉬우며치료를간과할수있는부위이다. 2-4) Chopart 관절은거골하관절 (subtalar joint) 과밀접한상호작용을하고있 다. 따라서하나의관절이라도기능이저하되면인접한 2 개의관 절들에부정적인영향을줄수있고전반적인족부기능의저하를 초래할수있다. 6) Chopart 관절골절및탈구는약 40% 에서축성부하 (axial load) 에의해발생하는것으로알려져있다. 회전력 (rotational force) 에 의해서도발생할수있으며이는회전탈구 (swivel dislocation) 라고 한다. 전족부의내전 (adduction) 과함께중족부내측으로외력이 작용하는경우거주상골또는거골골두골절이발생할수있고, 반대로전족부의외전 (abduction) 과함께중족부외측으로외력이 작용하는경우에는종골전방돌기 (anterior process) 와입방골의골 절이발생할수있다. 2) 증례 3 의경우가회전력에의해발생한회 전탈구이며전족부가내전되면서중족부내측으로외상력이작용 하여거주상골관절탈구가발생했다고생각된다. Zwipp 7) 은손상된해부학적구조물들에따라 Chopart 관절골절 및탈구에대해 1 transligamentous, 2 transtalar, 3 transcalcaneal, 4 transnavicular, 5 transcuboidal, 6 type 2 5 의 combination 으로분류하였고, 이중 transligamentous type 은드물다고하였다. 또한 Zwipp 7) 은 Chopart 관절손상은기본적으로족부전후 면 (anteroposterior view), 측면 (lateral view), 45 사선면 (oblique view) 을촬영하고, 특히족부측면상정상적으로 Chopart 관절은 cyma 선 (cyma line) 이라고하는조화로운이중물결모양 (double wave appearance) 을이루고있음을확인해야하며컴퓨터단층촬 영을통해정확한골절해부학을이해하고적절한치료계획을세 우는것이중요하다고하였다. Kutaish 등 5) 은 Chopart 관절손상에 있어최선의치료를위한핵심은 Chopart 관절손상에대한높은 수준의임상적의구심과즉각적이고정확한영상의학적평가라고 하였다. 특히컴퓨터단층촬영이나자기공명영상촬영 (magnetic resonance imaging) 은뼈와연부조직손상에대한정확한평가를 위해권고하였다. 저자들은 3 예중 combination type 2 예, tranligamentous type 1 예를경험하였고, 모두족부측면단순방사선상 cyma 선의파괴가관찰되어컴퓨터단층촬영을시행후 Chopart 관절의골절및탈구된구조물의해부학적관계를정확하게파악 해치료계획에반영하였다. Chopart 관절골절및탈구에대한치료는비관혈적정복술과석 고고정을이용한보존적치료나내고정술을통한수술적치료가 있다. 보존적치료는탈구가재발되거나심한통증을동반한부정 유합등의합병증이발생할가능성이높아대개 K- 강선, 흡수성핀 (resorbable pin), 금속나사와금속판을이용한내고정술을시행한 다. 2,3) Rammelt 등 3) 과 Richter 등 8) 은 Chopart 관절손상치료후좋 은예후를얻기위한중요한예측인자로해부학적정복과정확한

7 126 Vol. 22 No. 3, September 2018 내고정술이중요하다고하였고, 부정적인예후인자로비관혈적정복, 지연또는부정확한고정, 동반된족부나하지로의골절여부, 압궤손상 (crushing injury) 등이있다고하였다. Chopart 관절골절및탈구후발생할수있는합병증으로는연부조직감염이 10% 정도로보고되고있으며대개개방성손상 (open injury) 이나치료지연 (delayed treatment) 으로인해발생한다고알려져있다. 그외에도피부괴사, 비골신경 (peroneal nerve) 또는비복신경 (sural nerve) 손상, 심부정맥혈전증 (deep vein thrombosis), 구획증후군 (compartment syndrome), 복합부위통증증후군 (complex regional pain syndrome) 등도발생할수있다. 2,8,9) 부정유합과불유합 (nonunion) 은 20% 40% 로보고되고있으며, Chopart 관절손상을간과하거나초기진단시중족부염좌나족근골, 중족골단독골절로오진한경우, 수술시정복의부정정열 (malalignment) 등에의해서발생할수있다고보고되고있다. 2,3,10) Rammelt 등 10) 은 Chopart 관절손상후통증을동반한부정유합발생시관절유합술이통증완화및기능적재활에있어서효과적치료방법이라고보고하였다. 증례 1의환자에서도내고정술후 7개월째경과관찰상부정유합소견이관찰되었고환자보행시심한통증을호소하여통증완화및기능적회복을위해관절유합술을시행하였다. 본저자들은드물게발생하는 Chopart 관절골절및탈구 3예를경험하였고, 드문손상이기때문에진단을간과하거나진단을놓치거나오진할수있는것에대해주의가필요할것으로생각된다. 외상환자에있어중족부통증이있는경우 Chopart 관절손상에대한임상적인의구심을가지고즉각적이고정확한영상의학적평 가및손상형태분류가이루어져정확한해부학적정복과내고정 술을시행하는것이중요할것으로생각된다.
