1 대한고관절학회지제 18 권제 1 호 Vol. 18, No. 1, March, 2006 대퇴전자부골절환자의양극성인공골두치환술시전자골절편의내고정법 가톨릭대학교의과대학대전성모병원정형외과 목적 : 고령의대전자간골절편을포함한전자간골절이있는환자의인공골두치환술을시행하는데있어대전자부골절편에대한고정방법을후향적연구를통해이를비교하여보고하고자한다. 대상및방법 : 대전자부의부착이문제가될수있는 oyd-griffin 2형과 4형이었으며, 3가지내고정방법에대하여비교분석하였다. 첫번째인장강선을사용하였던예가 18예, 두번째로 K-강선및변형된인장강선을사용한예가 7예, 세번째로 GTRD (Greater Trochanteric Reattachment Device) 를사용한예가 15예이었다. 결과 : 첫번째방법은간편하게대전자부를고정시켰으며대부분좋은결과를얻을수있었고, 두번째방법은수술시단단한고정력을얻었으나술후조기보행시몇예에서는고정의실패뿐만아니라 K-강선의전위가초래되어 2예에서 K-강선을제거하는수술이필요하였다. 세번째인대전자부재부착기구를이용한방법은술기가어렵고수술시간이길어고령의환자에게부담이되는단점이있었지만술후조기재활이가능하였으며추시상임상적결과도우수하였다. 결론 : 전자간골절에서인공골두치환술후대퇴부골절편을내고정하는방법중인장강선방법과대전자부재부착기구인 GTRD를사용했던례에서는우수한결과를얻을수있었으며골다공증이심한대부분의환자에서권유될수있는방법으로술기가간단한인장강선을이용한방법이라고생각된다. 색인단어 : 전자골절편, 대퇴전자간골절, 대전자부재부착기구, 양극성치환술 서 대퇴골전자간골절이있는고령의환자에서있어서골유합술대신인공골두치환술의사용에대한관심이증가하고있다. 인공골두치환술은조기에체중부하를가능하게하여고령의환자에서일어날수있는전신합병증의발생을감소시키고골절부의불유합과대퇴골두무혈성괴사등의위험성을제거할수있다. 1970년대에 Stern과 Goldstein 18,19) 은고령의불안정한전자간골절을가진환자들에있어서조기체중부하를위해인공골두치환술을제안하였으며, Tronzo 20) 역시같은목적으로인공골두치환술에대해제안하였다. 대퇴골전자간골절에있어서인공골두치환술을시행 론 통신저자 : 지종훈대전광역시중구대흥동 가톨릭대학교대전성모병원정형외과 Tel: Fax: junwoo0220@yahoo.co.kr 하는경우에있어서 oyd-griffin 제 2형이나제 4형에있어서는대전자부골절에대한고정이필요하다. 현재까지소개된방법으로는 tension band wiring, modified tension band wiring, GTRD, Claw plate 7) 등이있다. 본연구에서는대퇴전자간부골절이있는환자에있어서인공골두치환술을시행하는경우에있어대전자부골편의고정방법에따른임상및방사선적결과를비교하였다. 대상및방법 1997년 5월에서 2002년 12 월까지 70 세이상의대전자간골절편을포함한전자간골절에서 6개월이상추시가가능하였던총 40 예의환자를대상으로하였으며술자는동일인이모두시행하였으며, 대퇴골전자간골절에서대전자부골절편에대한고정방법의비교에대한후향적연구를시행하였다. 수술시환자의평균연령은 78.1세로최소 70 세에서최고 93 세까지였다. 모든환자의최소추시기간은 6개월이었으며, 평균추시기간은 19개월이었다 ( 범위 : 6~40 개월 ). 67

2 대전자부의부착이문제가될수있는전자간골절분류에따른 oyd-griffin 2형이 28 예, oyd-griffin 4형이 12 예이었으며, 대전자부골절편의 3가지내고정방법에대하여 3군으로나누어그들의임상결과를비교분석하였다. 제 1군은 tension band wiring를사용하였던경우로 18 예가있었으며, 제 2군은 K-wire 를더불어이용하는 Modified tension band wiring을사용하였던경우로 7 예, 제 3군은 GTRD (Greater Trochanteric Reattachment Device) 기구를사용하였던예로 15예이 었다 (Fig. 1). 고관절수술시환자를측와위로위치한후절개법은 Direct lateral approach 8) 를사용하였다. 고관절측면에약 20 cm 절개후대둔근을중앙에서박리한후직접골절부위를노출시킨후, 대전자부의골절편을상방으로견인한후대퇴골골두를포함한두경부골절편을절제하여직접골절부위를노출시켰다. 본저자들은이것을편의상골절부위를통한접근법 (transfracture approach) 으로명명하였다. 이어서노출된근위대퇴부에대퇴스템을삽 C Fig. 1. 제 2군에서는대퇴골대전자부위에우선두개의 K-강선을이용한내고정을시행한후에추가로 wire를이용하 여고정하였고 (Fig. 3), 제 3군은대퇴골대전자부위의연부조직을박리하고 GTRD를이용하여대전자부를고정시켰다 (Fig. 4).

3 대퇴전자부골절환자의양극성인공골두치환술시전자골절편의내고정법 입하였으며대퇴부근위골간단에대퇴스템이압박삽입되어불안정성이없는 14 예에서무시멘트대퇴스템을사용하였고그렇지못한나머지 26 예는시멘트형대퇴스템을사용하였다. 제 1군에서이용한고정방법으로는소전자부의골절편이큰경우에서는골절편에직접철사를통과시키거나또는골절편이작을경우에는장요근 (iliopsoas muscle) 에철사를통과시킨후대퇴경부를감은후대전자부에부착되어있는중둔근 (gluteus medius muscle) 을 8자모양으로통과시켜고정시켰다 (Fig. 2). 제 2군에서는대퇴골대전자부위에우선두개의 K-강선을이용한내고정을시행한후에추가로 wire를이용하 여고정하였고 (Fig. 3), 제 3군은대퇴골대전자부위의연부조직을박리하고 GTRD를이용하여대전자부를고정시켰다 (Fig. 4). 고관절기능에대한임상적평가는전자간부위의통증과고관절기능을평가한 Merle d ubigne & Postel score 15) 를사용하였으며, 통증과보행정도에따라 11 점이상인경우를 very good, 10점인경우를 good, 9점인경우를 medium, 8점인경우를 fair, 7점이하인경우를 poor로분류하였다. 수술직후방사선사진을촬영하였으며, 전자간유합과 wire 또는 cable의파손, 금속물주위의골흡수, grip의이완등과같은합병증을평가하기위해서외래방문시마다 Fig. 3. 4 방사선촬영을시행하였으나고령인관계로거동이불편하여외래통원추적검사는힘든편으로 25 예 (62%) 에서는추적방사선촬영이가능하였으며나머지의경우는전화문의로결과를파악할수있었다. 결과대전자부의부착이문제가될수있는전자간골절분류에따른 oyd-griffin 2형이 28예, oyd-griffin 4형이 12 예이었으며, oyd-griffin 제 2형의경우 Postel score 상매우좋음 (very good) 2예, 좋음 (good) 5예, 중간상태 (medium) 11예, fair 5예, poor 5예이었으며, oyd-griffin 제 4형의경우에는매우좋음 0예, 좋음 0 예, 중간상태 6예, 양호 6예이었다. oyd-griffin 제 2 형에서보다나은결과를보였다 (Table 1). Tension band wiring을사용하였던예가 18예이었으며, 매우좋음 2예, 좋음 2예, 중간상태 9예, 양호 4예, 안좋음 1예이었다. Modified tension band wiring을사용한 7예의경우에서는좋음 1예, 중간상태 1예, 양호로 2예, 안좋음 3예이었으며, GTRD를사용한경우에는전체 15 예중좋음 2예, 중간상태 7예, 양호 5예, 안좋음 1예이었다. Modified tension band method using K-wires를사용한 7예가운데 2예에서는 K-강선의전위가초래되어 K-강선제거수술이필요하였다 (Fig. 3). 초기 short GTRD를사용한 1예에서는좌골신경부분 마비로제거수술을시행하였으며, 다른 1예에서는전위가있었지만환자는특별한증상을호소하지않아추시관찰중이며그후에는모든증례에서 long size의 GTRD를사용하였다 (Fig. 4). Tension band wiring을사용한 18예와 GTRD를사용한경우에서는추시소견상전자간유합이잘이루어졌으며, wire 또는 cable의파손, 분열, 부스러기의이동, 강선주위골흡수, 그립의이완등과같은합병증은발생하지않았다 5). Tension band wiring방법과대전자부재부착기구중 long size의 GTRD를사용했던예에서우수한결과를얻을수있었다 (Table 2). 이를통계적으로분석하기위해모든군에서매우좋음, 좋음, 중간상태를양호로, 양호, 안좋음을불량으로구분한후 Fischer's extract test를이용, 통계적분석을시행한결과 tension band wiring 방법과 GTRD 군에서는유의한차이를보이지않았으며 modified tension band wiring과 tension band wiring 군간에는유의한차이를보였다 (p<0.05). 고찰 불안정한대퇴골전자간골절이있는고령의골다공증환자에있어서조기체중부하를위해인공관절치환술을할것인지의여부는아직도정형외과의사에게숙제로남아있는문제이다 7,8,10,13,14,17). 많은저자들이불안정한대퇴골전자간골절의치료에있어인공골두치환술의사용에 Table 1. 5 대퇴전자부골절환자의양극성인공골두치환술시전자골절편의내고정법 관해좋은결과를보고한바있고 3,6,11,17,19,20), 대퇴골전자간부불유합이있는환자는고관절의외전시통증과동통을호소할수있으며전자간부불유합이있는환자는외전근의약화로고관절의탈구가발생하기쉽다 1,2,4,11,15,20). 몇몇연구에서는전자간부불유합을방지하기위한대전자부의고정방법에대한여러가지시도에대해서도보고된바있다 2,8,9,12,16). 저자는이번연구에서대전자부를고정하는방법인 Tension band wiring, Modified tension band wiring, GTRD 각각에대해비교하였으며 Tension band wiring 과 GTRD를사용한경우에보다좋은결과를얻을수있었고 modified tension band wiring을사용한경우에있어서는조기체중부하시 K-강선의전위가발생하였다. 1970년대에 William 등 1) 은 136 명의환자를대상으로 tension band wiring을이용한대전자고정술을시행한결과 18 개월추시상에서 100% 에서대전자부의전위없이골유합을얻었다고보고하였다. 본연구에서도 tension band wiring을사용한경우에서는 100% 에서전위없이골유합을얻을수있었다. Koichi Koyama 등 12) 은고관절인공관절재치환술을시행하는데있어서대결절의고정방법으로 Dall-Miles cable grip system을이용한연구에대해보고한바있는데, 전체 62 예가운데전자간부불유합이 19 예 (30.6%) 에서발생했으며, fragmentation이 18예 (29%) 에서, cable breakage가 4예 (6.5%) 에서, 케이블주위골흡수소견이 17 예 (27.4%) 에서발생하였다. 16예 (25.8%) 에서는자발통과압통이있었다. 본연구에서는추시기간이평균 19 개월 (range: 6~40개월 ) 로약간짧은차이는있지만상기합병증은아직발견되지않았다. GTRD를사용한경우에있어불유합의정도는 0~37.5% 까지다양하게보고되고있다 7). 본연구대상중 GTRD를이용한경우 short GTRD는적합하지않은것으로생각되며안정성이있는 long GTRD를써야만하는데이것자체가광범위한연부조직박리및장시간의조작이필요하여 tension band wiring과비교해보았을때특별한장점으로생각될수없다. GTRD는비록수술부위박리및수술시간의연장이다른방법보다는더욱필요하지만분쇄상이심하고보다견고한내고정이필요하다고판단될시에는그적응이될수있다고생각된다. Modified tension band wiring을시행한경우에서는조기체중부하를시행한경우 K-강선의이동이발생하였으며, 이로인한동통과농양이형성되었다. 환자를추시함에있어 K-강선이나케이블, wire의전위가있는경우에는주된증상을호소하지않더라도반드시추적관찰해야한다. 결 전자간골절에서대퇴부대전자부골절편을내고정하는방법중 tension band wiring방법과대전자부재부착기구인 long GTRD를사용했던예에서우수한결과를얻을수있었다. 또, K-wire나 S-pin 등의전위가가능한내고정물을이용하는 modified tension band wiring방법은적극회피하여야한다, 론 REFERENCES 01) mstutz HC, Mai LL and Schmidt I: Results of interlocking wire trochanteric reattachment and technique refinements to prevent complications following total hip arthorplasty. Materials and methods: Forty cases of an unstable femur pertrochanteric fracture were analyzed and followed up for average of 19(6~40) months. There were 18 cases fixed with tension band wiring (group 1), 7 cases treated using modified tension band wiring with K-wires (group 2) and 15 cases treated with the GTRD (Greater Trochanteric Reattachment Device) (group 3). Result: Group 1 was treated with simple a surgical procedure and the results were good. The second group had firm fixation postoperatively but required additional surgery in two cases to remove the K-wires because of wire migration after ambulation. Group 3 had relatively good results but required more dissection and a longer operating time. Conclusion: Tension band wiring or GTRD are good fixation methods for reattaching trochanteric fragments in pertrochanteric fractures. Moreover, tension band wiring is recommended for old osteoporotic patients due to a simple procedure and firm fixation. The use of modified tension band wiring using K-wire or a Steinmann-pin should not be used due to the possibility of distant migration.
