Outcome of restoration has been dependent on dental technician's wax up proficiency, dentists can design restoration for themselves now. This study aims to investigate the outcome of restoration designs, according to handling skill of CAD-CAM design tool. Materials and methods: A patient's mandibular right st molar was prepared. After taking impression, stone model was made, scanned the stone model with 3 shape intra-oral scanner, stereolithography (STL) file was extracted. With 3shape dental designer, one dental technician with more than 5 years work experience (designer 0) and three dental technicians with less than years work experience (designer,, 3-group DT) and 4 st year residents (designer 4, 5, 6, 7-group RT) designed gold crown on the same STL file. Designed crown's MD (mesio-distal) and BL (bucco-lingual) diameter, height of crown, inter-cuspal distance, number of occlusal contact points were compared. Statistical analysis was carried out, test of normality within each group, using independent t-test. Number of contact points were compared, using Wilcoxon signed-rank test. Results: There was no significant difference between group DT and group RT. Number of contact points also resulted in no significant difference. Conclusion: The outcome of each designed crowns showed no statistical differences, in values which can be expressed as numbers. Subjective factors were different. With increasing proficiency in handling designing software, fabrication of restorations according to each designer's occlusal concept can be made easy. (J Korean Acad Prosthodont 06;54:34-8) Keywords: Computer aided design-computer aided manufacturing (CAD-CAM); Design software; Proficiency 서론 CAD-CAM (Computer aided design-computer aided manufacturer) 시스템은 957 년자동차, 항공산업분야에처음이용되기시작하였다. CAD-CAM 은컴퓨터의발전과함께발달하였고, 987 년 Mörmann 과 Brandestini 가 CEREC 시스템을처음개발함에따라치과분야에소개되었다. 단계의경우최근치과용디지털소프트웨어의 tool 의다양화와직관적인디자인 tool 의발전에따라치아의빠르고정확한디자인이가능해졌다. 6 Cerec system 의등장과함께구강내스캐닝과밀링이가능해졌으며, 컴퓨터를이용한디자인과클리닉내에서밀링을하게됨에따라치과기공소를거치지않는당일보철물수복도가능해졌다. 6 또한임상가의기공과정에의참여또한용이해지고있다.

2 CAD-CAM 시스템은 3 가지단계로구성되어있다.. 지대치형성된치아와주변의구조물들을가상모형형태로데이터를수집하는단계,. 가상모형상에가상수복물을디자인하고, 밀링매개변수를설정하는단계, 3. 고형블럭을디자인된형태에따라밀링하는단계로구성되어있다. 4,5 데이터수집단계와가상수복물디자인은 CAD 단계, 고형블록밀링은 CAM 단계로, 단계는구강내스캐닝 (intra oral scanning) 과인상체스캐닝 (impression scanning), 모형스캐닝 (model scanning) 방식이있으며, 세방식모두기존의방식에비해기공단계를줄임으로써오차의발생가능성을줄일수있다는장점이있다. 단계의경우최근치과용디지털소프트웨어의 tool 의다양화와직관적인디자인 tool 의발전에따라치아의빠르고정확한디자인이가능해졌다. 6 Cerec system 의등장과함께구강내스캐닝과밀링이가능해졌으며, 컴퓨터를이용한디자인과클리닉내에서밀링을하게됨에따라치과기공소를거치지않는당일보철물수복도가능해졌다. 6 또한임상가의기공과정에의참여또한용이해지고있다. 7 따라서본연구에서는가상수복물디자인단계에서치과기공사와임상가의 CAD- CAM 디자인소프트웨어를이용한보철물디자인의차이를알아보고자한다. 재료및방법 한환자의하악우측제 대구치를금관지대치형성한후폴리이써 (Impregum Penta, 3M ESPE AG, Seefeld, Germany) 로통상적인방법으로인상을채득하였다. 초경석고 (Moldastone CN, Heraeus Kulzer, Hanau, Germany) 를이용하여석고모형을만든후 Trios intra-oral scanner (3shape, Copenhagen, Denmark) 로석고모형과대합치를스캔한후교합관계를스캔하였다. 동일한가상인상모형상에 5 년이상의 CAD-CAM 디자인경력을가진 명의치과기공사 (Designer 0), 년이하의경력을가진 3 명의치과기공사 (Designer,, 3), 4 명의보철과 년차전공의 (Designer 4, 5, 6, 7) 가 3shape dental designer (3shape, Copenhagen, Denmark) 를이용하여단일금관을디자인하였다. 디자인한금관의 STL (stereolithography) 파일을추출하였다. 3shape dental designer 상에서각금관디자인의 M-D diameter (MD) 및 B-L diameter (BL), 치관의높이 (CH), inter-cuspal distance (CD) 를측정하였다 (Fig. ). Designer 0 의측정값을기준으로각 designer 의금관측정치의차이를구하였다 (Table ). 치과기공사 3 명의그룹을 group DT, 보철과전공의 년차그룹 4 명을 group RT 로하여각측정값의차이의평균을구하고 (Table ), SPSS 통계프로그램 (version 0, A B Fig.. Shapiro-Wilk test 결과 group DT 와 group RT 간에 MD, BL, CH, CD 는유의수준 0.05 하에모두정규분포및등분산을이루었으며, 이에 student s t-test 를시행하였다. 결과 Designer 0 을기준으로한 MD, BL, CH, CD 의차이는 Table 과같았다. 다른결과들에비해 D0-CD 가전반적으로큰경향을보였다. Table 의수치를 group DT 와 group RT 로나누어평균값을 낸결과 D0CD-CD 가다른결과들이비해큰경향을보였다 (Table ). Student s t test 시행결과 group DT 과 group RT 의 MD, BL, CH, CD 는모두통계적으로유의할만한차이가없었다 (Table 3). 디자인된금관을교합면에서보았을때구의깊이의명확성이나교두의형태가윗줄의 group DT 가밑줄의 group RT 와형태적으로차이가있다 (Fig. ). 4 고찰 CAD-CAM 을이용한보철물의디자인과치과기공사가통상적인 wax up 방식으로제작한보철물을비교한최근의연구에따르면 CAD-CAM 을이용한보철물이자연치의외형을재현하는데있어더용이한것으로알려져있다. 8 현재 laboratory scanning 과 porcelain firing 을이용한보철물제작과정은매우정확하고재생가능하며, long span & short span 고정성보철물에서임상적으로적합한결과를보여준다. 9 디자인 tool 은직관적이고다루기쉽게제작되어통상적방법인 wax up and dental casting 으로제작한보철물과비교했을때치과기공사와임상가의결과물의차이가적어지고있다. 본실험의결과로미루어볼때금관의근원심크기나금관의높이와같이제한된공간내에서디자인해야하는요소들은큰차이가없고, 크기가제한되지않는협설크기또한큰차이가없다. 이는디자인소프트웨어의디자인초기단계에서자동으로형성하는형태에의한것으로보인다. 교합면의형태. 교두의높이와구의깊이등보철물의이용편의성에큰영향을주지만수치화하기어려운요소들은술자의교합에대한개념에따라차이가있다. CAD 시스템은 library 에저장된치아의형태를이용하거나자동화된디자인으로숙련자와비숙련자간에큰차이없이빠른시간내에이상적인결과물을얻어내기위한목적으로만들어졌고, 비슷한결과물을기대할수있으나, 개인별로차이가여전히존재하는것을볼수있다. 하지만 CAD 시스템은소프트웨어를다루는숙련도가높아진다면디자이너가가지고있는교합에대한개념이나저작효율개선을위한디자인을구현하기가훨씬용이하다. 통상적인 wax up 의경우숙련도가결과물이나시간을결정하기때문에 wax up 에익숙하지않은임상가는교합에대한지식을구현하기가어렵다. 현재개발된 CAD 소프트웨어들은제조사에따라제공하는 library 와 tool 에차이가있기때문에제품에따른학습곡선이존재한다. 하지만 design tool 간에호환이가능해지고비슷한기능을제공한다면교합에대한치과의사의철학을구현하기가용이해짐에따라통상적인 wax up 기법에비해기능적, 심미적으로만족스러운보철물을제작할수있을것이다. 5 ORIGINAL ARTICLE 김태현 김종은 이아름, 박영범, * 연세대학교치과대학치과보철학교실 연세대학교치과대학구강과학연구소, BK 플러스통합구강생명과학사업 목적 : 최근치과영역에서 CAD-CAM (computer aided design-computer aided manufacturer) 시스템의이용과디자인소프트웨어가늘어남에따라쉽고빠른수복물의디자인이가능해졌다. 과거에는치과기공사의 wax up 기술의숙련도에따라금관의외형이결정되었으나, CAD-CAM 시스템에서는디자인소프트웨어에대한숙련도가더중요한요인이되어가고있다. 이에치과기공사와치과의사간의결과물의차이를알아보고자한다. 재료및방법 : 대합치와인접치가양호한환자의하악우측제 대구치를금관지대치형성한후 polyether 로인상채득하였다. 인상재에초경석고를이용하여모형을제작하였으며, Trios 구강내스캐너를이용하여 STL (stereolithography) 파일을추출하였다. 3shape dental designer 상에서 5 년이상경력의치과기공사 명, 년이하경력의치과기공사 3 명, 4 명의보철과 년차전공의가동일한 STL 파일을이용하여금관을디자인하였다. CAD- CAM 시스템경력이가장오래된기공사의디자인을기준으로하여치과기공사그룹 3 명 (group DT) 과 년차전공의그룹 4 명 (group RT) 의금관외형을비교하였다. 디자인된금관의근원심폭, 협설폭, 높이, 교두간거리, 대합치와의접촉점의수를측정, 비교하였다. 독립표본 t 검정을시행하여그룹간의차이를비교하였고, 대합치와의접촉점의수는 Wilcoxon signed-rank test 를이용하였다. 결과 : 정규성검정결과근원심폭, 협설폭, 높이, 교두간거리는정규분포를보였다. Group DT 와 group RT 의금관디자인상에서근원심폭, 협설폭, 높이, 교두간거리에서통계적으로유의할만한차이가없었다. 대합치와의접촉점수는정규분포를보이지않았으며, wilcoxon signed-rank test 결과통계적으로유의한차이를보이지않았다. 결론 : 금관의외형의크기에있어서는 group DT 와 group RT 사이에통계학적으로유의할만한차이가없었지만수치화하기어려운금관의해부학적형태나주변치아와의형태적유사성등은차이가있었다. 디자인소프트웨어에대한숙련도가높아진다면각술자가가지고있는교합에대한개념을더구현하기쉬울것이다.
