We evaluated the performance of the new GENEDIA Rotavirus Ag Rapid test (Greencross Medical Science, Korea) immunochromatographic assay (ICA) for detecting human rotavirus in stool specimens, in comparison with ELISA and PCR assays. Methods: One hundred rotavirus-positive stool samples and 150 rotavirus-negative stool samples, confirmed by ELISA and PCR tests, were analysed using the GENEDIA Rotavirus Ag rapid test. The positive agreement (sensitivity), negative agreement (specificity), and total agreement rates of the ICA compared to ELISA and PCR were determined. To assess the analytical performance of the ICA, we tested its detection limit, reproducibility, and cross-reactivity. Results: The positive, negative, and total agreement rates of the ICA were 99%, 100%, and 99.6%, respectively, when compared with the results confirmed by ELISA and PCR. The total turnaround time of the ICA was less than 20 minutes. The lower limit of detection of the ICA for rotavirus was TCID 50 /ml, which was similar to that of ELISA but higher than that of PCR. No cross-reactivity was detected for 11 viruses and 19 bacteria. Conclusions: The GENEDIA Rotavirus Ag rapid test was easy to perform and provided rapid results, which showed high agreement with those obtained using ELISA and PCR. This test appears to be a useful tool for the diagnosis of rotavirus infection. ( J Lab Med Qual Assur 2013;35:107-14) Key Words: Rotavirus, Immunochromatography, Enzyme-linked immunosorbent assay, Polymerase chain reaction pissn: X eissn: Received October 30, 2013, Revision received November 16, 2013, Accepted November 18, 2013 서론 로타바이러스는전세계적으로영유아및 5세이하의소아에서심한설사를일으키는가장중요한원인균으로, Reoviridae 과에속하는지름 nm 크기의이중가닥 RNA 바이러스 (double stranded RNA virus) 이다 [1,2]. 로타바이러스는외피 (outer capsid), 내피 (inner capsid) 및핵심 (core) 의 3층으로이루어진바이러스이며, 핵심의 RNA는 11개의분절로이루어졌는데, 유전자의각분절은 6개의구조단백 (VP1-VP4, VP6, VP7) 과 6개의비구조단백 (NSP1-NSP6) 을만드는유전정보를가지고있다. 구조단백은내피단백 (inner capsid protein) 과외피단백 (outer capsid protein) 으로구성되며, 내피단백인 VP6 단백의항원성에따라 A-G군으로분류된다. 인체감염은주 로 A군에의해서발생하며, B군, C군에의해서도발생한다. A군로타바이러스는외피단백인 G (glycoprotein; VP7) 와 P (protease-sensitive protein; VP4) 의항원성에따라혈청형 (serotype) 으로분류되지만, 교차반응이없는 P혈청형-특이항체를구하기어려우므로최근에는분자생물학적방법으로 P유전형 (genotype) 과 G유전형을검출한다. 로타바이러스감염의진단은위장관염증상을보이는환자의대변에서로타바이러스를검출함으로써이루어지는데효소면역법 (enzyme immunoassay, EIA), 면역크로마토그래피법 (immunochromatographic assay, ICA), 라텍스응집법, 핵산검출법 (PCR: reverse transcription [RT]- PCR), 전자현미경등이검출에이용되며, 로타바이러스의유전형확인이역학적연구에이용되고있다. 이중 EIA, ICA, 라텍스응집법등로타바이러스항원검사법이가장널 Copyright 2013 The Korean Association of Quality Assurance for Clinical Laboratory 107

2 리사용되고있는방법이며, PCR법의경우민감도가높은방법이지만, 상대적으로고가의비용과많은노동력, 특수장비가필요하므로일반적인로타바이러스검출방법으로는잘이용되지않는다 [1]. 최근에검사법이간단하고 20분이내로신속하게결과를얻을수있는로타바이러스 ICA법이국내에서개발되었으므로환자검체를이용하여이 ICA법과 ELISA법, PCR법을비교해보고, 검출한계, 정밀도, 교차반응등분석적민감도를평가해보고자하였다. 대상및방법 1. 대상 2013년 1월부터 2013년 7월까지 ELISA법으로로타바이러스항원검사를시행한후보관하였던분변검체로 PCR 검사를시행하여로타바이러스 ELISA검사와 PCR검사모두양성인 100검체, 모두음성인 150검체를대상으로 ICA 를시행한후세검사법의결과를비교하였다. ELISA검사는로타바이러스검사가의뢰된분변검체로 72시간내에시행되었고, PCR검사와 ICA검사는 ELISA검사후 -70 o C 에냉동보관하였던분변검체를 2013년 7월에서 10월사이에해동하여검사를시행하였다. 먼저 ELISA 양성인검체 100개를선택하여 PCR검사를시행하였는데이중 PCR이음성으로나온검체가 14건이있어이검체는대상에서제외하고추가로 ELISA 양성검체에대하여 PCR검사를시행하여 ELISA와 PCR 동시양성 100검체를선정하였다. 환자의연령은중앙값 (median) 이 12개월 ( 생후 1일-83세 ) 이었으며, 5세이하의소아환자가 250명중 218명으로대상환자의 87.2% 를차지하였다. 2. 로타바이러스 immunochromatographic assay 제네디아로타바이러스항원래피드테스트 (GENEDIA Rotavirus Ag Rapid Test; Greencross Medical Science, Yongin, Korea) 를사용하여제조회사의지침에따라검사를시행하였다. 검사원리는 ICA법이며, 검사선 (test line) 에는다클론토끼항-로타바이러스항체가, 대조선 (control line) 에는염소항-마우스면역글로불린항체가흡착되어있다. 분변검체중의로타바이러스항원이검사키트의접합체 (conjugate) 패드에있는단클론마우스항-로타바이러스항체-금콜로이드접합체와반응하여면역복합체를형성하고, 이면역복합체가막을따라퍼지면서이동한후검출부의검사선에흡착되어있는항-로타바이러스항체와특이적으로결합하여적색선을형성하는것을육안으로확 인하는검사이다. 환자의분변검체를검사키트에포함되어있는검체채취봉으로찔러검체를채취한후키트에포함되어있는검체희석액에검체가채취된검체채취봉을넣고충분히혼합하였다. 검체채취봉을제거하고키트에서제공되는점적용마개를닫은후분변검체희석액 4방울을분석기의검체점적부위에떨어뜨렸다. 제조사의지침대로결과판독은 15-20분후에시행하였고대조선과검사선모두에적색선이생기면양성, 대조선에만적색선이보이면음성으로판정하고대조선에적색선이보이지않는경우는검사가정상적으로진행되지않은것으로보고재검을실시하였다. 3. 로타바이러스 ELISA RIDASCREEN Rotavirus 검사키트 (R-Biopharm, Darmstadt, Germany) 를사용하여제조회사에서제시한방법대로검사를시행하였다. 검사원리는샌드위치 ELISA 법으로 ELISA plate 의각 well 에로타바이러스항원에반응하는특이항체가부착되어있다. 분변검체 mg 또는 100 μl를키트에서제공되는검체희석액 1 ml에넣고잘혼합한후 5,000 rpm( 약 2,300-2,500 g) 에서 5분간원침하였다. ELISA plate 의 well에분변부유액상층액 100 μl를넣고여기에동량의비오틴과결합된단클론항- 로타바이러스항체를가한후실온에서 60분간반응시켰다. 세척액으로세척한후 streptavidin peroxidase 접합체 (conjugate) 시약 100 μl를첨가한후실온에서 30분간반응시켰다. 세척후기질시약 (tetramethylbenzidine) 을가하여실온에서 15분간반응시킨후반응정지액을가하고 450 nm에서흡광도를측정하였다. 제조사의지침대로결과판정을시행하였다. 계산된 cut-off( 음성대조의흡광도 +0.15) 를기준으로하는데, cut-off 의 +10% 이상일경우양성으로, -10% 미만인경우음성으로판정하며, cut-off 의 ±10% 이내의값을보이는경우 (equivocal) 는재검하며, 재검시에도 equivocal 로나오는경우는음성으로판정하였다. 4. 로타바이러스 PCR Seeplex Diarrhea V (Seegene, Seoul, Korea) 키트를사용하였으며제조회사지침에따라검사를시행하였다. Seeplex Diarrha V 키트는분변검체에서 A군로타바이러스뿐만아니라노로바이러스 GI와 GII, 아데노바이러스, 아스트로바이러스유전자를동시에검출하도록고안된 multiplex RT-PCR검사이다. 분변검체에서의핵산추출은 QIAamp DNA mini kit (QIAGEN, Hilden, Germany) 108 J Lab Med Qual Assur 2013;35:

3 와 QIAcube platform (QIAGEN) 을이용하여추출하였다. 핵산추출액 8 μl 와키트에서제공하는해당시약 (1 μl random hexamer, 3 μl DEPC treated water, 4 μl 5 RT buffer, 2 μl 10 mm dntp, 1 μl RNase inhibitor, 1 μl reverse transcriptase) 을섞어 20 μl 로맞추어 PTC- 200 thermal cycler (Bio-Rad, Watertown, MA, USA) 에서역전사과정 (37 o C 에서 90 분 ) 을거쳐 complementary DNA (cdna) 를합성하였다. 합성된 cdna 3 μl 를 master mix (4 μl 5 DV PM, 3 μl 8-Mop solution, 10 μl 2 multiplex master mix) 에혼합하여 20 μl 용량으로맞 춘후 PCR 과정 (94 o C 에서 15 분과정 1 회 ; 94 o C 에서 0.5 분, 60 o C 에서 1.5 분, 72 o C 에서 1.5 분과정 45 회 ; 72 o C 에서 10 분과정 1 회 ) 을진행한후아가로스겔에전기영동하여 541 bp 의 PCR 산물이확인되면양성으로판정하였다. 5. 정밀도및검출한계평가 A 군로타바이러스표준균주 (ATCC VR-2018) 를 MA- 104세포에접종하여얻은배양액을이용하여 ICA검사의정밀도와검출한계를평가하였다. 로타바이러스 TCID 50 /ml (50% tissue culture infectious doses per ml) 농도의로타바이러스배양액을생리식염수로 4,096 배수까지 2배수계단희석하였고 1:128 희석단계부터각농도를 3개의다른 lot (lot 번호 : EX130101, EX130102, EX130103) 의 ICA 키트로 3일간 10회씩반복측정 ( 각농도별총 90회측정 ) 하여정밀도를계산함과동시에이농도에서 ELISA와 PCR검사를시행하여 ICA, ELISA 및 PCR의검출한계를비교하였다. 6. 각종바이러스및박테리아와의교차반응평가로타바이러스 ICA법의교차반응성 (cross reactivity) 을평가하기위해사용된균주는 Table 1과같다. 세균은평판배지에접종후균집락을생리식염수에풀어 McFarland 0.5 혼탁도에맞춘후혼탁액으로로타바이러스 ICA를시행하였다. 바이러스의경우바이러스배양상층액원액을 Table 1. Comparison of the immunochromatographic assay results with ELISA and PCR results ELISA & PCR Immunochromatographic assay No. of specimen Positive Positive 99 Positive Negative 1 Negative Positive 0 Negative Negative 150 Total 250 Positive agreement rate of rapid antigen test: 99.0% (95% confidence interval [CI], 94.6% to 100%); negative agreement rate of rapid antigen test: 100% (95% CI, 97.6% to 100%); total agreement rate of rapid antigen test: 99.6% (95% CI, 97.8% to 100%). 율 ( 민감도 ), 음성일치율 ( 특이도 ), 총일치율은각각 99.0% (95% confidence interval [CI], %), 100% (95% CI, %), 99.6% (95% CI, %) 로나타났다 (Table 2). 전체 250개검체중불일치검체는 1건으로 PCR/ELISA 동시양성이지만 ICA에서음성으로나왔으며, 이는 28개월의남자환아의검체로 ELISA검사에서 optical density 가 1.534로중간정도의흡광도를보였고 PCR결과전기영동에서 band가약하게나타난검체였다. 2. 정밀도및검출한계로타바이러스배양액을이용한정밀도및검출한계평가에서배양액원액을 512배희석한검체를 3개의다른 lot로 3일동안총 90회반복측정하였을때반복측정의 97.7% 가양성결과를보였고, 1,024배희석한농도에서는 90회반복측정의 98.9% 가음성결과를, 그이상으로희석한농도와생리식염수에서는모두음성결과를보여검출한계는 TCID 50 /ml로계산되었다 (Table 3). 동시에시행한 ELISA검사도 ICA와마찬가지로 512배희석한농도까지양성결과를보였으며, PCR은 4,096배희석한농도까지양성으로나타났고이이상의희석은시행하지않았다 (Table 3). ICA의세 lot 간에정밀도및검출한계의차이는없었다. 3. 교차반응평가 ICA의교차반응평가에서 Table 1에나열된 11종류의바이러스와 19종류의세균에모두음성반응을보여시험한 Table 3. 5 균주에대한교차반응은없는것으로나타났다. 고찰 본연구에서로타바이러스 ELISA검사법과 PCR검사법모두양성인 100개의분변검체와두검사법모두음성인 150개의분변검체를대상으로신속항원검사를시행하였을때양성일치율 ( 민감도 ) 과음성일치율 ( 특이도 ) 은각 각 99.0% 와 100% 로매우높았다. 기존에보고된로타바이러스항원검사법의민감도와특이도결과를비교해보면 (Table 4) 동일한항원검사법이라도매우넓은민감도 ( %) 와다양한특이도 ( %) 를보였다. 본연구결과의민감도와특이도를기존연구들과비교하여해석할때고려해야할점은본연구가일정기간시행된모든분변검체를대상으로시행하지않았고 ELISA검사결과를기준으로검체를선정하여 ELISA검사와 PCR검사에동시 Table 4. 6 양성이거나동시음성인검체를대상으로평가하였다는점이다. 일반적으로 PCR검사법이항원검사법에비해분석민감도 ( 검출한계 ) 가우수하므로 PCR검사법양성인검체만을선택하여 ICA의 PCR에대한양성일치율을평가하였을경우에는 PCR에양성이지만항원검사법에음성인검체들이포함될수있으므로실제 ICA법의 PCR법에대한양성일치율은이보다낮게나타났을것으로생각된다. Table 4에서와같이보고자마다로타바이러스검사법의민감도나특이도를다르게보고하는이유는대상검체, 비교검사법및검사법이다르기때문일수있다. 항원검사법의경우어떤항체를사용했느냐에따라, PCR의경우어떤시발체 (primer) 와탐색자 (probe) 를썼느냐에따라검사결과가달라질수있다. 즉, 로타바이러스의 capsid 항원은많은항원결정기 (epitope) 를가지고있으며이항원결정기중어떤항원결정기와반응하는항체들을어떤조합으로제조하느냐에따라결과가다르게나타날수있고, 로타바이러스유전자수준에서다양한유전자형및변이가존재하는데사용한시발체와탐색자에따라검출할수있는로타바이러스의유전자형이조금씩다를수있어검사법간에불일치결과를보일수있을것이다. 한편, ELISA에서는양성이었으나 PCR에서는음성결과를보여분석대상에서제외되었던 14검체에대해서도 ICA를시행한결과, 14검체중 12검체에서 ICA 양성반응을보였고, 이는항원검사 (ELISA와 ICA) 의위양성또는 PCR검사의위음성일가능성을시사한다. 항원검사의경우시약에포함되어있는검출항체와비슷한항원결정기를가진다른물질이검체에포함되어있는경우교차반응으로인해위양성이나타날수있겠고, PCR검사의경우는시약의시발체가결합하는부위에유전자변이가생겼을때위음성으로나타날수있다. 본연구에서로타바이러스 ICA의검출한계는 TCID 50 /ml로측정되었고, ICA의검출한계를 ELISA 및 PCR과비교하였을때 PCR이가장최저검출한계가가장낮았고 ( 분석민감도가높았고 ), ICA와 ELISA의최저검출한계는비슷한것으로나타났다. 로타바이러스검사법의검출한계를 TCID 50 /ml로측정한연구는저자들이찾아본바로는거의없었으며, 최근에와서 real-time PCR법이개발되면서검출한계로 copies/reaction 까지검출되며환자검체에서의로타바이러스농도는 에서 copies/reaction 였다는보고 [3] 와, real-time PCR법으로검출한계가 5 copies/reaction까지검출되는데이는 10,000 rotaviruses/g stool에해당한다는연구 [4] 가있었고, real-time PCR법으로 10 copies/reaction 까지검출가능하다는연구 [5] 가있었다. 합성플라스미드 (plasmid) 를이용한바이러스유전자정량검사는표준화되어있지않고검사실마다실험조건이다르므로단순한수치비교만으로어떤검사의검출한계가더우수하다고단정하기어려우나, 본연구에서와같이단계적희석으로여러농도의검체를만들어서동시에비교해보면어떤검사의분석민감도가가장높은지확인할수있겠다. 장염증상이있는소아환자에서는 PCR법과 EIA법의양성률이유의한차이가없었으나증상이없는건강한환아의 18% 에서 EIA법으로는음성이었으나 PCR법에서는양성이었으므로환자의검사법으로너무민감도가높은 PCR보다항원검사법이더좋다고주장한논문도있다 [6]. ICA법, ELISA법및 PCR법의장단점을비교하여보면 PCR법은민감도가높은검사법이나검사과정이복잡하고검사소요시간이길며숙련된인력과고가의시약, 장비를필요로한다. ELISA법은 PCR에비해소요되는비용이적고다량의검체를동시에검사하기에편리하며흡광도값을통한상대적인정량이가능하다는장점이있다. ICA법은 PCR법에비해민감도가떨어지지만, 원침등의전처리과정이나특수장비없이간편한조작으로 20분이내에결과를 Table 5. 8 로타바이러스검출을위한면역크로마토그래피법의평가김현수 노지선 현정원 김한성 김재석 송원근 이규만한림대학교의과대학한림대학교동탄성심병원진단검사의학과 배경 : 로타바이러스는전세계적으로영유아및소아에서심한급성위장관염을일으키는주요원인균이다. 본연구는최근에개발된제네디아로타바이러스항원래피드테스트 (GENEDIA Rotavirus Ag Rapid test; Greencross Medical Science, Korea; immunochromatographic assay, ICA) 의검사수행능을평가하기위해이검사를 ELISA 및 PCR 법과비교하여평가해보았다. 방법 : ELISA 와 PCR 검사법으로확인된 100 개의로타바이러스양성대변검체와 150 개의음성대변검체를대상으로제네디아로타바이러스항원래피트테스트를시행하여양성일치율 ( 민감도 ), 음성일치율 ( 특이도 ), 총일치율을구하였다. 분석적성능평가로는검출한계, 정밀도, 교차반응검사를시행하였다. 결과 : ELISA 와 PCR 동시양성및동시음성검체에대한 ICA 의양성일치율, 음성일치율, 총일치율은각각 99%, 100%, 99.65% 로나타났고, ICA 의총검사소요시간은 20 분이내였다. ICA 의최저검출한계는 TCID 50 /ml 로 ELISA 와비슷하였으나 PCR 보다는높았다. 11 종류의바이러스와 19 종류의세균에대한교차반응은나타나지않았다. 결론 : 본연구에사용된제네디아로타바이러스항원래피트테스트는간편하고신속하게결과를얻을수있으면서 ELISA 및 PCR 법과비교하여높은일치율을보이므로로타바이러스감염진단에유용하게사용될수있을것으로생각된다.
