Methods: A retrospective examination that is studied with patients who were treated with gefitinib as 1 st line therapy to the elderly patients more than 70 th years old with EGFR mutation. Results: Fifty-one patients were enrolled from January 2011 to December The median follow up period was 38 months (3-85 months). Their median age was 78 years old (70-88). ECOG performance grade 1 patients were 25 (49.0%), grade 2 and 3 patients were 19 (37.2%), 7 (13.8%) respectively. 48 patients (94.1%) had adenocarcinoma, 41 patients (80.4%) were at stage IV and 34 patients (66.6%) were never smokers. Exon 21 mutation was found in 23 patients (45.0%) and 28 patients (55.0%) had exon 19 deletion. Response rate was 21.5% and disease control rate was 68.7%. Progression free survival was 7.9 months (95% CI; months) and overall survival was 13.0 months (95% CI; months). Few hematologic and nonhematologic toxicites, skin rash were reported in this study. Conclusion: EGFR-TKI, gefitinib was effective and tolerable for the elderly patients more than 70 th years old with epidermal growth factor receptor (EGFR) mutation as 1 st line targeted chemotherapy. Key Words: Aged, Epidermal growth factor receptor tyrosine kinase inhibitor (EGFR-TKI), Gefitinib 서론 비소세포폐암 (Non-small cell lung cancer) 은전체폐암의 80-85% 를차지하고있으며, 1 진단당시증세가뚜렷하지않아대부분조기발견이힘들다. 다행히진단되더라도발견 책임저자 : 김용태 경기도고양시일산동구일산로 100 국민건강보험일산병원종양혈액내과전화 : (031) , 팩스 : (031) ytkim@nhimc.or.kr * 본연구는국민건강보험일산병원의연구비지원으로이루어졌음 (NHIMC 2016CR054) 시이미 3기후반부나, 4기의진행성또는전이성비소세포폐암 (NSCLC) 인경우가많아고식적항암화학요법 (palliative chemotherapy) 이치료의표준으로여겨져왔다. 2 현재진행성비소세포폐암의표준적인 1차항암화학요법은 platinum 을기반으로하고, 3 여기에다가 gemcitabine, docetaxel, paclitaxel 혹은 CPT-11 을같이사용하는병용화학요법이다. 4 그러나 platinum 항암제들이신장독성, 이독성, 심장독성등이심하고구토를유발하는등일반적으로쉽게치료를받기가어려운항암제이다. 더구나최근들어평균수명의연장과동반질환이많은고령인상태에서새로이진행성비소세포폐암으로진단받는경우, 기존 platinum 기반의항암치료가이러 134 Korean Journal of National Health Insurance Service Ilsan Hospital

EGFR 변이가있는 70 세이상의고령의진행성 NSCLC 대상으로 1 차치료로써 Gefitinib (Iressa ) 단독요법의치료효과 한환자들에게는힘든치료방법일수밖에없다. 그러나다행히최근들어분자생물학적분석의발달로항암치료도기존의화학치료에서표적치료제로바뀌어가고있다. 과거고령이거나전신상태가불량하여항암화학치료가불가능하였던환자들도분자생물학적유전자분석후표적치료제사용이가능하게된것이다. 두경부암위암과더불어비소세포폐암에서도 epidermal growth factor receptor (EGFR) tyrosine kinase의세포돌연변이가발견되면서비소세포폐암치료에있어서도표적치료 (target therapy) 가기존의항암치료와같이시도되기시작하였고, 이러한표적치료가기존의 platinum을기반으로한항암치료와비교하여부작용은상대적으로적고여성, 선암 (adenocarcinoma), 비흡연자등의특정환자군에서좋은치료효과를보인다는연구결과 5 들이발표됨에따라 EGFR 변이가있는 NSCLC 환자를대상으로 1차, 2차구제요법 (2 nd line salvage chemotherapy) 으로현재임상에서활발히사용중이다. 6 본연구의목적은 EGFR mutation 이있는 70세이상의고령진행성비소세포폐암환자를대상으로 1 st line으로의 gefitinib 의치료효과와투약의안정성을평가하고자한다. 이를통하여동반질환이많고기저질환으로항암치료의위험성이상대적으로큰고위험군의환자를대상으로첫치료로써 gefitinib의치료성적을조사하고자한다. 이연구결과를근거로지금까지알려진항암치료성적과비교하여반응률과생존기간도조사할예정이다. 대상및방법 1. 대상 2011년 1월부터 2016년 12월까지국민건강보험일산병원혈액종양내과에서 stage IIIB-IV로진단받고, EGFR mutation 이발견되었던 70세이상의고령의환자를대상으로환자의의무기록을대상으로후향적으로분석하였다. 대상은 1차항암치료로표적치료제 gefitinib 을사용한환자로예상생존기간이 12주이상인경우를대상으로하였다 2. 방법 Gefitinib 은 1일 1회 250 mg 을매일복용하였고, 한주기를 28일로정의하였다. 약물투여는진행성병변을보이거나심각한부작용이관찰될때, 환자의전신상태 (performance status) 가약물을투여하기어려울정도로악화되거나, 환자가항암치료를원하지않을때중단하였다. Gefitinib 투여후치료효과 (response eval- uation) 판정을위해서 2, 3개월간격또는병의진행이의심될때마다흉부전산화단층촬영 (chest CT scan) 과병변이포함된타부위 CT scan 및필요시전신골주사검사나 PET CT 검사를진행하였다. 뇌전이를의심할만한증상이생겼을경우 brain MRI 검사를시행하였다. 대상환자는 ECOG scale 1-3인환자를포함하여진행하였고, 치료반응평가는 Response Evaluation Criteria in Solid Tumor (RECIST) 1.1 version 을기준으로평가하였으며, 중앙관찰기간은치료시작시부터마지막자료분석시점까지로정의하였다. 반응평가는 RECIST 1.1 기준으로완전관해 (complete response), 부분관해 (partial response), 안정질환 (stable disease), 진행질환 (progressive disease) 로분류하였고, 질병조절률 (disease control rate, DCR) 은완전관해, 부분관해, 안정질환으로유지되는것으로정의하였다. 무진행생존기간 (progression free survival, PFS) 은치료시작일로부터질병의진행일이나, 질환으로인한사망일로, 전체생존기간 (overall survival, OS) 은치료시작부터사망까지의기간으로정의하였다. 항암치료중생겨나는독성판정은매주기마다 National Cancer Institute- Common Toxicity Criteria (NCI-CTC) version 3.0 에따라조사하였다. 3. 통계분석 70세이상의고령의환자를대상으로무진행생존기간 (progression free survival, PFS), 전체생존기간 (Overall survival, OS) 를 Kaplan-Merier method로분석하였다. 결과 1. 환자의특성 2011년 1월부터 2016년 12월까지총 51명의자료를후향적으로분석하였다. 전체환자중남자는 24명 (47.1%) 여자는 27명 (52.9%) 이본연구에조사되었다. 환자의중앙나이는 78세 (70-88세) 였다. 전신상태는 ECOG 1인경우가 25명 (49.0%) 2인경우가 19명 (37.2%) 3인경우가 7명 (13.8%) 였다. 진단당시병리학적인소견은 adenocarcinoma 가 48명 (94.1 %) 였고, large cell carcinoma 가 3명 (5.9%) 로조사되었다. 진단당시병기 (stage) 는 IIIb가 10명 (19.6%), IV가 41명 (80.4%) 였다. 대상환자의과거흡연력은비흡연자가 34명 (66.6%), 흡연경험이있었던환자가 17명 (33.3%) 이었다. EGFR mutation 종류는 exon 21 이상이 23명 (45.0%) exon 19 deletion 이 28명 (55.5%) 였다 (Table 1). Volume 16 Number 2 December

Clinical Effects and Safety of Gefitinib as 1 st Line Therapy to the Elderly Patients with EGFR Mutation 2. 치료반응전체환자 51명전부가 1차항암치료이상을시행하였으며, 치료에의한반응평가가가능하였다. 전체적인치료반응률은완전관해는없었고, 11명에서부분관해를보여 21.5% 였다. 안정병변은 24명으로 47.2% 였으며, 질병조절률은 68.7 % 였다 (Table 2). 3. 생존기간 51명의중앙무진행생존기간은 7.9개월 (95% CI ) 이었고, 중앙전체생존기간은 13.0개월 (95% CI ) 로조사되었다 (Fig. 1, 2). 관련된부작용으로사망한환자는관찰되지않았다. 심각한독성으로치료를포기한환자도없었다. 혈액학적인독성을조사해보았을때 4명 (7.8%) 에서 2단계백혈구감소증을보였고, 2명 (3.9%) 에서혈소판감소증이관찰되었다. 비혈액적독성에서는피부발진이대표적이었으며, 전체 51명의환자중 9명이관찰되었고, 1단계가 4명 (7.8%) 2단계가 4명 (7.8%), 3단계가 1명 (1.9%) 로조사되었다 (Table 3). 고찰 폐암은다른장기의고형암과는달리증상이거의없으며, 증상이있어도기침, 가래등단순한호흡기감염으로오인하 4. 치료독성 치료독성은전체 51 명모두가평가가능하였다. 치료와 Table 1. Adverse events of gefitinib Hematologic toxicities Neutropenia Thrombocytopenia Febrile neutropenia Non Hematologic toxicity Skin rash No (%) 2(3.9%) 1(1.9%) 는경우가많아진단당시이미수술을하지못하는진행성병기 (advanced stage) 에서발견되는경우가대부분이다. 그리고폐의해부학적인특성상혈관분포가발달되어있고, 많은혈류가폐를관류하게되므로다른고형종양에비하여특히재발을잘하는특징이있어항암치료후에도완치가사실상어려우며, 주로고식적 (palliative) 인목적의항암화학요법이표준치료로사용되고있다. 많은임상시험결과현재진행성비소세포폐암의표준적인 1차항암화학요법은 platinum 을기반으로하는병용화학요법이다. 7 그러나이러한 platinum 을포함한항암치료는독성이심하고쉽게재발하는한계점이있으며, 환자가고령화함에따라장기적인치료가불가능하여전체적인항암치료의효과를저하시키고, 장기생존이어렵게하는원인이되고있다. 이러한이유로독성이적으며상대적으로안전하게장기간투여가가능한표적치료제가폐암치료에시도되고있는데, 대표적인약제가 EGFR tyrosine kinase 의작용을억제시키는표적치료제다. 8 이러한표적치료가기존의 platinum을기반으로한항암치료와비교하여좋은치료효과를보인다는연구결과 9 들이발표됨에따라현재임상에서활발히사용중이다. 이중대표적인것으로 EGFR TKI 인 gefitinib (Iressa ) 9,10 이다. 특히 70세이상의고령의환자의경우일반적인 platinum 기반의항암치료를받기가어렵다는점을감안할때이러한표적치료제의시용은또하나의희망적치료방법으로평가할수있다. 본연구에서 EGFR 변이가있는 70세이상의고령의환자를대상으로 1차 치료방법으로 gefitinib 을사용한결과를보면치료반응률이 21.5% 였다. 이는 Pilotto S 등이발표한외국문헌 ( 치료반응률 25.7%, 질병조절률 60.0%, 무진행생존기간 5.19개월, 전체생존기간 14.4개월 ) 과비교해서유사한반응률을보여주고있다. 11 본연구에서나타난질병조절률은 68.7% 였고, 이수치는참고문헌 11 과비교하였을때높은질병조절률로평가된다. 이러한결과는본원의환자들이 70세이상의고령인것을감안한다면우수한치료결과로사료된다. 무진행생존기간 (progression free survival, PFS) 결과는 7.9개월 (95% CI ) 로역시참고문헌 11 과비교할때증가된결과를보여주고있다. 전체생존기간 (Overall survival, OS) 의경우본원연구결과는 13.0 개월 (95% CI ) 로참고문헌 11 과비슷한기간을나타낸다. 이러한결과를볼때 70세이상의고령의환자라도전신상태가양호하면 gefitinib 과같은 EGFR-TKI 제제는성인과거의같은효과를낼수있으며, 생존기간을연장하고삶의질을증가시켜줄수있음을보여주고있다. 항암치료와관련된부작용면에서도비교적심하지않아항암치료와연관된감염으로사망하거나치료를중단한환자는없었으며, 심각한혈액학적독성도최고단계가 2단계로 7.8 % 에서생긴백혈구감소증, 3.9% 의환자에서생긴혈소판감소증이었다. 이들은대증적치료후항암치료를계속해서시행할수있었다. 비혈액학적독성인피부부작용은최고로심한독성이 3단계로스테로이드연고사용후호전되어항암치료를유지할수있었다. 그외에 2단계독성이 7.8% 1단계독성이 7.8% 로각각조사되었으나, 대증적치료후항암치료를예정대로진행하였다. 결론적으로 70세이상의고령이면서진행성비세포성폐암으로진단된환자의경우, 유전자분석상 EGFR 유전자변이가있으면 1차치료로써표적항암제인 gefitinib은치료반응률과질병조절률에서효과를기대할수있다. 또한생존기간면에서무진행생존기간과전체생존기간이참고문헌과비교할때고령이라도충분히생존기간연장에기여할수있음을보여주고있다. 이러한결과를고려한다면고령으로환자가항암치료를거부하거나, 소극적인태도일때 EGFR 유전자변이가있는환자인경우적극적인항암치료권유와장기적치료및추적관찰이필요하리라사료된다. 다만본연구는단일기관연구인관계로등록된환자수의제한되었다는점과후향적분석이라는한계점을가지고있지만, 본조사결과는특히고령의환자가많은본원에서치료반응률및생존기간예측과부작용을평가하여고찰하는데좋은자료가될것으로생각된다.
