Hormone replacement in brain death patient 약한 자발 호흡이 있었으나, 곧 사라졌다. 당시 의식상태는 혼수상태 였으며 통증에 대한 반응이 전혀 없었고, 동공반사도 없는 상태였다. 흉부사진은 정상소견이었고, 심전도는 박동수 분당 96-183회의 심방세동 및 V4,5,6에서 ST분 절 하강소견을 보였다.

3 Hormone replacement in brain death patient 약한 자발 호흡이 있었으나, 곧 사라졌다. 당시 의식상태는 혼수상태 였으며 통증에 대한 반응이 전혀 없었고, 동공반사도 없는 상태였다. 흉부사진은 정상소견이었고, 심전도는 박동수 분당 회의 심방세동 및 V4,5,6에서 ST분 절 하강소견을 보였다. 뇌 컴퓨터 단층촬영 검사 및 자기공명영상에서 전반적인 허혈성 뇌손상의 소견이 관찰되어 상당한 뇌손상의 가능성을 예측 할 수 있었다. 인공 호흡기를 적용하면서 중환자실에 입원하 였고, 뇌손상을 최소화하기 위하여 입실 직후 저 체온요법을 시행하였다. 내원 1일 후 시행한 뇌 파검사상 전혀 의미있는 뇌파가 관찰되지 않았 고, 저체온 요법 종료후 입원 4일째 시행한 뇌파 검사에서도 역시 뇌파가 전혀 관찰되지 않아 뇌 사상태로 추정되었다. 이후에도 환자의 의식은 회복되지 않았으며 내원 1주 후에 시행한 뇌 컴 퓨터 단층촬영(Fig. 1)에서 전반적인 허혈성 손 상의 소견을 보였고, 이후 추적한 뇌파검사에서 도 의미있는 뇌파는 관찰되지 않았다(Fig. 2). 1차병원에서 시술을 받기위해 진정제 투여후 발생한 호흡정지 및 허혈성 뇌손상으로 진단하였 고, 뇌파검사 및 신경학적 검사상 뇌사상태로 판 단되었으나, 80세 이상의 고령 및 법적 분쟁의 가능성이 있는 환자로 장기 이식의 절차에 적합 하지 않았다. 아직까지 우리나라는 뇌사의 판정은 장기이식 에 관한 법률 안에서만 죽음 으로 간주되며, 이 외의 경우에는 사망으로 판정되지 않는 실정으로 우리는 최선의 치료를 유지해야만 했다. 입원 3 일째 시행한 혈액검사상 갑상선 관련 검사결과가 T3 0.36( ) ng/ml, TSH 0.10( ) μiu/ml, Free T4 0.84( ) ng/dl결과를 보였고, ACTH 자극 검사결과 ACTH 2.31( ) pg/ml, Cortisol 기저치 11.2 μg/dl-30분후 17.6 μ g/dl -60분후 17.3 μg/dl 의 결과보여 주사제 hydrocortisone 100 mg 하루 3회 보충요법을 시작하였다. 주사제 hydrocortisone은 감량하면 서 입원 2주째 추적검사를 시행하였고, T3 Fig.2. Brain death is most severe form of hypoxic brain damage. Survival of brain death patient has been reported about several days, usually within 7days. But we experienced an 82 year old patient who was diagnosed with brain death and survived for 85 days. In primary clinic, an 82 year old man received fentanyl and propofol for sedation to perform vertebroplasty. After 10min, respiratory arrest occurred. The physician and emergency medical technician did cardiopulmonary resuscitation, and he showed recovery of systemic circulation. He was tranferred to Chungbuk national university hospital and was admitted into intensive care unit(icu). We started normothermia therapy and maintained sedation for 3 days. After normothermia therapy, we performed electroencephalography but there was no meaningful wave in whole recording period. His mental status sustained comatous and self respiration wasn t noted in apnea test. He was diagnosed with brain death but we couldn t withdraw aggressive treatment due to possibility of legal problem. We checked his hormonal status and he showed panhypopituitarism secondary to brain damage. We replaced prednisolon, levothyroxine and vasopressin and he survived during 60 days in our ICU. We transferred him to Cheongju medical center and he died after 2 weeks.
