Postural tachycardia syndrome (POTS) is another nomenclature of orthostatic intolerance. POTS characterized by a heart rate increase 30 bpm from supine to standing or >120 bpm at standing without orthostatic hypotension. POTS is a heterogenous in presentation with various pathophysiologic mechanisms. Important mechanisms are hypovolemia, denervation, hyperadrenergic and deconditioning state. There are presented as lightheadness or dizziness, palpitations, presyncope, sense of weakness, tremulousness, shortness of breath. POTS are classified under 3 groups that are neuropathic, hyperadrenergic, and deconditioning POTS. Most patients can be improved from a pathophysiologically based regimen of management. Key Words: Orthostatic intolerance, Postural tachycardia syndrome, POTS, Neuropathic, Hyperadrenergic, Deconditioning 서 론 기립성못견딤증 (orthostatic intolerance) 은기립 (stand) 시에교감신경의활성혹은대뇌관류의저하로인하여증상이나타나고, 앉거나누우면증상이호전되는경우를말한다. 1 기립성못견딤증을가진대부분의환자들은기립성저혈압이나분명한자율신경실조를가지고있지않으면서비특이적이고다양한증상을호소한다. 1,2 이러한증상들은음식을먹거나, 샤워를하거나, 경도의운동에서도나타날수있기때문에일상적인생활에서많은장애를초 Address for correspondence; Ki-Jong Park, M.D. Department of Neurology, School of Medicine, Gyeongsang National University, 90 Chiramdong, Jinju city, Gyeongsangnam-do, , Korea Tel: Fax: pkjong@gnu.ac.kr 래한다. 2 기립성못견딤증의정확한유병률을알수는없지만다양한질환의많은사람들에서관찰되고, 미국의한보고에서는대략 50만명이상에서관찰되는것으로추정하였다. 3 기립성못견딤증은질환이아닌하나의증후군으로다양한병태생리학적기전을가지고있으며, 다양한이질적인질환들로구성되어있다. 따라서명칭도환자가가지는특정부분에초점을맞추어지난수십년동안다양하게불려지고있으며, 최근에도완전히확립되지는않았다 (Table 1). 1 기립성못견딤증에서가장흔하고지속적으로관찰되는것이과도한빈맥이다. 4 또한빈맥은이들에게서초기에쉽게관찰할수있는지표로서기립성못견딤증을가장잘표현할수있는지표이다. 2 최근에는만성적인기립성못견딤증과기립성빈맥증후군 (postural tachycardia syndrome) 을대부분동일시하고있다. 본논문에서는기립성못견딤증에대한일반적인소견들과함께기립성빈맥증 Copyright 2009 by The Korean Society for Clinical Neurophysiology 1

Terms for Orthostatic Intolerance Chronic orthostatic intolerance Hyperadrenergic orthostatic hypotension Hyperdynamic β-adrenergic state Idiopathic hypovolemia Irritable heart Mitral valve prolapsed syndrome Neurocirculatory asthenia Orthostatic anemia Orthostatic tachycardia syndrome Postural tachycardia syndrome Soldier s heart Sympathotonic orthostatic hypotension Vasoregulatory asthenia 후군에대하여기술하고자한다. 기립성못견딤증의임상적소견 1 1. 기립성못견딤증의증상 현기증 (lightheadness), 어지럼 (dizziness), 운동못견딤증 (exercise intolerance), 심계항진증 (palpitations), 피곤감 (fatigue), 인지장애 (cognitive difficulties), 시각흐림 (blurred vision), 불안증혹은떨림증 (anxiety or tremulousness), 오심, 가슴조임 (tight chest), 불쾌할정도의축축함 (clamminess) 2. 기립성못견딤증의증상에영향을주는자극 갑작스런자세변화, 기립, 오랫동안누워있다가일어서는경우, 운동, 열, 음식, 일과중의시간, 약물 3. 기립성못견딤증의징후 빈맥 (tachycardia), 이완기고혈압, 과장된혈압진동 (exaggerated blood pressure oscillations), 망상피반 (livedo reticularis), 사지의냉감 (cold extremities), 발한운동기능장애 (sudomotor dysfunction), 원위부소섬유신경병증 (distal small fiber neuropathy) 기립시의심박동과혈압의변화기전 1. 기립 (stand) 시의심박동변화 5 기립시심박동은두개의정점을형성한다. 기립후대략 3초정도에첫번째심박동의정점이나타나고, 이후 5 초정도까지심박동수가감소한다. 대략 12초부근에심박동수가최대인두번째정점이관찰된다. 첫번째심박동수의증가는운동반사 (exercise reflex) 에의해유도된심장미주신경긴장의억제에의하여나타나고, 두번째심박 동의증가는동맥혈압수용체반사에의해매개된심장미주신경긴장의감소와교감신경긴장의증가에의하여나타난다. 대략 20초정도후에지연성서맥이관찰되는데, 역시혈압의과도한증가가동맥의혈압수용체반사를통하여심장미주신경을자극함으로써나타난다. 따라서기립성심박동에대한검사는심장미주신경의기능에대한검사이지만, 교감신경계가정상이어야정확한평가가가능하다. 이러한기립시의심박동변화는경사테이블을통한검사시에나타나는심박동의변화와본질적으로다르다. 경사테이블을위한검사시에는운동반사가작용하지않는다. 경사테이블을통한기립시에는대략 1초후부터심박동수가서서히증가하지만, 기립시에관찰되는심박동수의급격한증가가관찰되지않는다. 2. 기립시혈압의변화 6 자세변화시일어나는심박동과혈압의변화는주로자율신경계에의하여이루어진다. 정상인에서순환혈류의 25% 정도가흉곽에있다. 기립자세를취하면대략 500 cc 의혈류가복부나하지로이동하게된다. 이들중에서 50% 정도는기립후수초이내에재배치된다. 대략전체혈류량의 25% 정도가이과정에참여한다. 이때심장은충분한혈류량을받지못하기때문에심박출량이대략 40% 정도감소한다. 이들변화의과정을결정하는참조점 (reference point) 은 venous hydrostatic indifference point (HIP) 로알려져있으며, 늑막수준 (level) 의주위에있다. HIP는근육의활동에의한정맥컴플라이언스 (compliance) 의변화에의해서바뀔수있기때문에다소유동적이다. 기립후에건강한사람들은대개 1분이내에안정을찾는다. 기립시에는동맥압과심장충전압 (cardiac filling pressure) 이서서히감소하는데, 이것은목동맥팽대의고압력수용체와심장과폐의저압력수용체에의하여이루어진다. 기립시에정맥환류량 (venous return) 의감소는압력수용체반사를통하여교감신경을활성화시켜서전신혈관의수축을일으킨다. 또한동맥압의감소는고압력수용체를통하여심박동을증가시킨다. 이러한기전에의하여기립시에심박동은 10~15 회 / 분정도증가하고, 이완기혈압이 10 mmhg 정도증가하고, 수축기혈압은거의변화가없다. 기립성빈맥증후군 (Postural tachycardia syndrome, POTS) POTS 의정확한발병률을알수는없지만, 기립성저혈압보다 5~10 배정도로흔하게관찰할수있다. 7 미국의한 2 Korean J Clin Neurophysiol / Volume 11 / June, 2009
3 기립성못견딤증 : 기립성빈맥증후군 보고에서는대략 170/100,000 명정도로보고하였다. 대개 15~50 세사이에흔하고여자가남자보다 5배정도많이나타난다. 8 POTS 라는약어도 postural tachycardia syndrome, orthostatic tachycardia syndrome, postural orthostatic tachycardia syndrome 에서만들어진약어로서, 최근에는 postural tachycardia syndrome 이많이사용되고있다. Postural tachycardia syndrome 를그대로해석하면자세변화시의빈맥증후군이지만, 주로기립시에발생하기때문에 기립성빈맥증후군 으로의역하였다. 1. 진단기준 9,10 POTS 는기립시혈압의변화는없으면서, 1) 심박동이기립후 5~30분이내에 30회 / 분이상증가혹은기립시의심박동 >120 회 / 분, 2) 기립시증상이악화되고누웠을때증상이호전, 3) 증상이 3~6 개월이상지속, 4) 기립시혈장의노르에피네프린 (norepinephrine) 이 600 pg/ml (3.5 nm) 이상, 5) 급성출혈, 급성탈수, 약물등의기립성증상이나빈맥을일으킬수있는다른원인이없는경우에진단할수있다. 2. 임상적소견기립시의증상들은기립성못견딤증의증상들과유사하고, 열, 운동, 음식물, 생리에의해서악화된다. 8,11 기립과상관없이복부팽만감 (bloating), 오심, 구토, 복부통증, 변비, 설사, 방광기능장애, 홍체기능장애가관찰될수있다. 특히흔한증상들은현기증 (lightheadness), 심계항진 (palpitation), 전실신증상들 (symptoms of presyncope), 떨림증 (tremulousness), 하지의위약감혹은무거운느낌 (weakness or heaviness) 등이다. 그외에흔한증상으로짧은호흡 (shortness of breath) 과흉통이관찰되기도한다. POTS 환자들과기립성저혈압환자들을비교하였을때, 기립성저혈압환자들에서는현기증, 피곤감 (tiredness), 시각흐림 (blurred vision) 이흔하게관찰되었고, POTS 환자에서는이들과달리떨림증, 빈맥, 불안감, 오심, 호흡장애 (dyspnea), 발한 (diaphoresis) 이흔하게관찰되었다. 12 편두통, 수면장애, 만성피로증후군혹은피곤감, 섬유조직염 (fibromyalgia), 과민대장증후군 (irritable bowel syndrome), 염증성대장질환 (inflammatory bowel disease) 이 POTS와함께중복되어서관찰되기도한다. 9,11 피곤감은특히흔하게관찰되는데, 환자들은운동에대한내성이떨어진다. 운동후에회복하는기간이길어지고, 피곤감이수일에서수주정도지속되기도 한다. 13 가장특징적인징후는누웠다가일어났을때심박동이 30회 / 분이상증가하는것이다. 14 대개는기립후바로나타나지만 2분, 5분, 10분이내에나타나고, 그이후에나타나는경우도있다. 수축기혈압은 20 mmhg 이상감소하지않는데, 실제적으로는많은경우에기립시혈압이증가한다. 2 기립성빈맥은하루중에서검사시간에따라의미있는차이가관찰된다. 15 POTS 를가진 17명의환자에서혈압의변화는하루중에차이가없었지만, 기립시심박동수의변화는오후보다는오전에의미있게크게관찰되었다. 따라서검사의민감성을높이기위하여검사를오전에하는것이더타당하다. 청진시에는 mitral valve prolapsed 의잡음이들릴수도있다. 16 기립시와발살바검사를할때요골동맥의박동이잘만져지지않는 Flack sign 이관찰되기도한다. 2 POTS 의이학적소견으로서큰특징중의하나는 40~ 50% 의환자에서관찰되는말단청색증 (acrocyanosis) 이다. 17,18 오래서있는경우에다리의색깔이검은적-청색으로관찰되는데, 만지면차갑게느껴진다. 때로는발에서무릎위까지퍼져있기도한다. 그기전은정확하게알려져있지않지만, 정맥혈관에혈류가고여서발생하는것보다는피부에혈류가감소하여생기는것으로알려져있다 POTS 의병태생리기전 POTS 의병태생리기전은아직까지정확하게밝혀져있지않지만, 그명칭만큼다양한기전들이제시되었다 (Table 2). 1,19 4. POTS 의분류 POTS 는대개신경병증성 (neuropathy) 혹은부분적자율신경실조성 (partial dysautonomic) POTS, 고아드레날린성 (hyperadrenergic) POTS 로분류하는것은거의일반화되었다. 2,20 그외에연구자에따라서탈조건화 (deconditioning) POTS, 속발성 (secondary) POTS 등을추가하여분류하기도한다. 발생빈도는대략반정도가신경병증 Table 2. 4 성 POTS 이고, 1/3 정도가고아드레날린성 POTS 이다. 8 1) 신경병증성 (neuropathic) 혹은부분적자율신경실조성 (partial dysautonomic, PD) POTS 21 가장흔한형태로서이들환자들은경한말초자율신경병증에의해서말초혈관이중력에저항할정도로적당한혈관저항을유지하지못한다. 이것은기립시에다리, 아래쪽상지, 장간막혈관 (mesenteric vessel) 과같은아래쪽에있는혈관에정상보다많은혈액의저류 (pooling) 를일으킨다. 일정량의혈액이중심혈관을벗어나서아래쪽에저류되기때문에뇌관류 (cerebral perfusion) 를일정수준으로유지하기위하여보상적으로심박동과심근수축을증가시킨다. 심박동과심근수축의증가로인해서처음에는어느정도보상이가능하지만, 정맥의혈액저류는점차적으로증가하고, 이들에의한보상효과를초과하게된다. 환자들은점차적으로적당한혈압과심장으로들어오는정맥혈류량을유지하기위하여하지의골격근펌프 (skeletal muscle pump) 에의존하는것이증가하게된다. 그러나정맥혈류의저류가결국에는이러한보상작용을초과하게되어증상이나타난다. 하지에서는정맥컴플라이언스 (venous compliance) 가감소되어있다. 17 신경병증성 POTS에서는정량적발한운동축삭반사검사 (quantitative sudomotor axon reflex test) 8,22 와온도조절발한검사 (thermoregulatory sweat test) 8 에서원위부결절후발한운동신경의이상을시사하는소견이관찰되었다. 그리고갈바닉피부자극검사 (galvanic skin stimulation) 와피부조직검사에서도자율신경병증의소견이관찰되었다. 23,24 또한하지의정맥에 norepinephrien 과 phenylephrine 을투여하였을때과민성이관찰되었는데, 이는자율신경병증의탈신경성과민 (denervation hypersensitivity) 을시사한다. 25 노르에피네프린의과잉 (spillover) 을이용한검사에서상지에서는정상이었지만, 하지에서는아드레날린분비의장애를관찰할수있었다. 11 일부에서는자율신경에대한자가면역기전을시사하는결절성 (ganglionic) 아세틸콜린수용체에대한항체가저역가로발견되었다. 8 신경병증성 POTS 환자들은대부분바이러스감염으로추정되는급성발열반응이선행하며, 임신, 수술, 패혈증, 외상후에나타나기도한다. 8 2) 고아드레날린성 (Hyperadrenergic) POTS 2 고아드레날린성 POTS 환자들은종종기립성빈맥뿐만아니라기립성고혈압이같이나타난다. 19 경정맥성 isoproterenol 에대한반응이과도하게나타날뿐만아니라, 기립시 norepinephrine 이의미있게증가 ( 600 pg/ml) 되어나타난다. 8 이러한근거로고아드레날린성 POTS 의진단기준을 1) 경사테이블검사에서 10분이내수축기혈압이 10 mmhg 증가하고, 2) 기립시혈장노르에피네프린 600 pg/ml 으로제시하기도한다. 2 연접틈새내에 norepinephrine 을이송하는단백질의기능을저하시키는 single point mutation 도일부에서관찰되었다. 26 이러한 norepinephrine 이송의결핍은교감신경계의자극에대하여혈청내 norepinephrine 의과잉을초래한다. 이들환자들은증상이갑작스럽게나타나기보다는점차적으로진행하는양상으로나타난다. 27 그리고때로는가족력을가지고있기도한다. 대개는기립시에교감신경계활성에의한증상을많이호소하는데, 진전, 불안감, 사지의냉감, 심계항진을호소한다. 반수정도에서는편두통을경험할뿐만아니라, 기립시에짧은시간동안소변량이의미있게증가한다. 3) 탈조건화 (deconditioning) POTS 탈조건화는오랫동안누워있거나심한질환으로기립등의활동이거의없었던경우를말한다. 특히피곤감과섬유조직염을가진 POTS 환자들이탈조건화상태가되고, 운동내성이떨어진다. 2 POTS 환자에서불안감, 체성과각성 (somatic hypervigilance), 운동의기여정도를비교한연구에서 POTS 환자들은불안감이대조군보다많았지만, 기립시심박동의변화는불안감때문이아니라정맥저류동안에동맥압을유지하기위한생리적반응때문이었다. 13 이들은체성과각성 (somatic hypervigilance) 이적어도기립성증상을유도하는초기질환이나사건이될수있을것으로생각할수있으며, 이로인하여탈조건화가되고육체적활동이감소하면서이증상이과도하게나타나는형태가될수있는것으로보고하였다. 2 4) 속발성 (Secondary) POTS 20 심장에대한신경은유지하면서말초성혈관이나자율신경계의이상을초래하는질환에의하여발생한다. 대표적인질환으로당뇨병이있으며, 아밀로이드증 (amyloidosis), 사르코이드증 (sarcoidosis), alcoholism, 루푸스 (lupus), Sjogren syndrome, 항암제치료, 중금속중독등에의해서나타날수있다. 5. 5 기립성못견딤증 : 기립성빈맥증후군 가? 2) 갑자기혹은점차적으로발생하였는가? 3) 증상발현당시감염, 수술, 외상등의연관되는소견이있었는가? 4) 증상을악화시키거나호전시키는것이있는가? 5) 비슷한증상을가진가족력이있는가? 6) 편두통이있었는가? 하는병력이진단에중요하다. 증상, 기립시간, 일상생활에대한영향을고려하여기립성못견딤증의정도를등급화하여평가하는것이도움이될수도있다. 2,9 병력청취후에검사가필요한환자들에서는다음과같은검사를시행하는것이도움이될수있다 (Table 3). 2 1) 기립검사와경사테이블검사 (Fig. 1) POTS 의진단에는경사테이블검사가가장유용한검사로알려져있다. 그러나경사테이블검사는근육펌프에의한운동반사가작용하는기립검사 (stand test) 와는엄밀하게다르고실제생활에서일어나는현상과는차이가있다. 이러한이유로인하여기립검사에서는다른진단기준을사용하기도한다. 28 기립검사와경사테이블검사를비교한연구에서진단기준을 30회 / 분이증가한것으로하였을때기립검사에서는특이도 79%, 경사테이블검사에서는특이도 23% 로관찰되어서기립검사가 POTS 를배제하는데는 Table 3. POTS 환자에서추가적인검사 1) 기본적인자율신경반사검사 (autonomic reflex screen) 2) 온도조절발한검사 (thermoregulatory sweat test) 3) 결절성항체 (ganglionic antibody) 4) 운동검사 (exercising test) 5) 심장초음파와홀터모니터 더유용한것으로보고되었다. 29 POTS 아형에따라서심박동은고아드레날린성에서기립혹은경사테이블검사에서가장큰증가를보이고, 고아르레날린성에서는기립시에혈압이증가하는것을관찰할수있다. 2,8 2) 심전도, 홀터모니터 POTS 환자들이단지굴빠른맥 (sinus tachycardia) 만가지고있는경우가있다. 주로심전도와홀터모니터는심장전도에문제가있는부정맥이나심장리듬의장애를배제하기위하여일반적으로시행한다. 10 3) Norepinephrine 15분이상의누운상태와기립상태를유지하고시행한혈장의 norepinephrine 검사에서누운상태에서는정상범위이지만약간높게관찰되었고, 기립상태에서는 600 pg/ml 이상으로아주높게관찰되었다. 8,10 이는기립상태에서는교감신경계의활성이극대화되어나타난다는것을의미한다. 4) 자율신경검사심혈관계자율신경반사검사는대개는정상이거나약간과장되어관찰된다. 심호흡시검사에서미주신경기능은보존되어있으면서, 발살바검사에서는이완직전과직후의혈압회복과혈압의 overshoot 가과장되어관찰된다. 30 발한을검사하는자율신경검사는신경병증성 POTS 를진단하는데유용하게사용할수있으며, 주로하지원위부의무한증이관찰된다. 8,22 5) 기타 POTS 를가진많은환자에서혈액량이낮게관찰된다. 6 이것은적혈구나혈장량을검사할수있는핵의학검사를통하여확인할수있다. 31 어떤환자들은반복적인홍조를호소하기도하는데, 이것은비만세포 (mast cell) 질환과연관되어있다. 30 진단은 2~4시간동안의소변에서 methylhistamine 을검사하는것이도움이된다. 6. POTS 의감별진단 POTS 는아드레날린이높아서나타나는증상들때문에임상적으로갈색세포종 (pheochromocytoma) 과혼돈할수도있다. 갈색세포종을가진환자들은 POTS 환자들에비해서누워있을때증상이심하게나타난다. 혈장혹은소변의 metanephrine 를검사하여감별할수있다. 32 고아드레날린성 POTS와 inappropriate sinus tachycardia (IST) 는몇가지특징을같이가지고있기때문에감별이필요하다. 33 1) 자세변화시에 POTS 환자에서 IST 환자보다심박동의변화가더크다. 2) 누워있을때심박동수가 POTS 에서는 100회 / 분을넘는것이드물고, IST에서는흔히 100회 / 분을넘는다. 3) 고아드레날린성 POTS 에서는자세변화에따른 norepinephrine 의변화가 IST 환자에서보다크다. 만성피로증후군은뚜렷한질환없이 6개월이상반복되거나지속되는심한피로감을느끼는경우를말한다. 만성피로증후군환자에서는특히 POTS 가같이동반되면서기립성못견딤증이동반될수가있으며, 이는치료의방향이다를수있기때문에주의깊게관찰하여야한다 POTS 의치료 POTS 의아형에따라서치료의방법은다양하다. 1) 비약물적치료 2 (1) 가능한증상을악화시킬수있는약물은중지한다 (Table 4). 20 (2) 일주일에 3회이상 20~30 분정도의호기성운동을한다. (3) 골격근을강화시키기위하여복부와하지에적당한저항성운동을한다. 허리아래의근육을 30초정도씩반복적으로수축하는연습을한다. 이것은정맥용량을감소시키고전체적인말초혈관의저항을증가시키는역할을한다. (4) 복대를착용하는것도도움이되는데, 이는내장-창자간막 (splanchnic-mesenteric) 정맥의용량을감소시키고, 특히저혈량증 (hypovolemia) 이있거나정맥운동긴장이저하된환자에서도움이된다. Table 4. (6) 탄력스타킹이때로는도움이된다. (7) 잘때머리를 6~12 인치정도올리고잔다. 2) 약물적치료 2,20 약물중에서 POTS 의치료약으로 FDA 승인을받은약물은아직까지없다. 그러나증상호전을위해여러종류의약물들을사용할수있다. POTS 환자의 40~60% 정도에서약물에좋은반응이관찰되었다. 8 신경병증성 POTS 에서효과가있는것으로알려진약물들은 flucortisone acetate, midodrine (ProAmatine), desmopressin acetate (DDAVP), pyridostigmine bromide (Mestinone) methylphenidate (Ritalin, Mehytlin, Concerta), octreotide acetate (Sandostatin) erythropoietin (Epogan, Procrit) 등이있다. 고아드레날린성 POTS 에효과가있는것으로알려진약물들은 clonidine (Catapres), labetalol HCl (Trandate, Normodyne) 등이있다. Bupropion (Wellbutrin XL), Duloxetine HCl (Cymbalta), Escitalopram oxalate (Lexapro), Venlafaxine HCl 등은신경병증성과고아드레날린성모두에일부효과가관찰되었다. (1) 체액량을증가시키고, 말초혈관의저항을증가시키는약물을먼저사용한다 (fludrocortisones, DDAVP, 6 Korean J Clin Neurophysiol / Volume 11 / June, 2009
7 기립성못견딤증 : 기립성빈맥증후군 midodrine). (2) 대개아침에증상을많이호소하기때문에침대에서일어나기전에 midodrine 혹은 methylphenidate 를사용하기도한다. (3) 위의치료에반응하지않을때는 serotonin reuptake inhibitor (SSRI) 혹은 norepinephrine reuptake inhibitor 를사용할수있다. SSRI는신경심인성실신 (neurocardiogenic syncope) 에더도움이되고, norepinephrine reuptake inhibitor 는 POTS 에더유용하다. (4) 자율신경계신경절의신경전달을촉진시키는 pyridostigmine 이효과가있는것으로보고되었다. 34 이약물은바이러스감염혹은자가면역질환 ( 루푸스혹은 Sjogren syndrome) 에의하여 2차적으로오는 POTS 에효과적이다. (5) 아주심한 POTS에서는 erythropoietin 을사용한다. 21 (6) 난치성의환자에서는 somatostatin analog octreotide 를사용하기도한다. (7) 고아드레날린성 POTS에서는노르에피네프린의분비와효과를차단하는약물 (clonidine HCl) 을사용한다. Clonidine 는신경병증성 POTS 에서는도움이안된다. 순수한 β- 차단제는 α- 자극을막지못해서증상을악화시킬수있기때문에 α와 β- 차단제가혼합된약물을사용한다. Methyldopa, SSRI, norepinephrine reuptake inhibitor가몇몇환자에서도움이되기도한다. 3) 기타 (1) 속발성 POTS 에서는원질환의치료가도움이된다. (2) 당뇨병에의한 POTS 는부분적자율신경실조형태에준해서치료를하면된다. (3) 범종양성 POTS 는 pyridostigmine 에잘반응하고, 종양의치료후에증상호전이나타난다. 8. POTS 의예후신경병증성 POTS 환자들을추적검사하였을때 60% 정도에서기능적으로정상적으로되었으며, 80% 정도는상태호전이있었고, 90% 정도가일상적인업무로복귀할수있었다. 35 결 론 기립성못견딤증은비교적드물지않게관찰할수있는증상들을가지고있다. 특히 POTS 는기립성저혈압보다도더흔하게관찰할수있지만, 일반적으로많은관심을가지고있지않기때문에진단을하지못할수도있다. 당뇨병을동반하는고혈압 약물치료의최신지견 저자이경림 / 맹지혜이화여자대학교약학대학장약학정보원학술자문위원 / 이화여자대학교박사후연구원 개요 제2형당뇨병환자의 3분의 2 이상에서고혈압이발병하는데, 혈당이높은당뇨병환자에서혈압이상승하는원인에대해서는여러병태생리학적기전들이제시되고있다. 당뇨병환자에서고혈압은다른심혈관계질환의위험도를증가시키므로당뇨병환자는더욱더적극적인혈압조절이요구된다.
