Recent advances have led to the development of several HBsAg detection systems. Here, we evaluated the performance of 3 newly developed detection systems, which can detect HBsAg both qualitatively and quantitavely, and determined the concordance among their results. Methods: Four hundred and thirty two samples assigned to 4 groups-patient group, dilution group, weakly reactive group, and linearity groupwere subjected to qualitative and quantitative detection of HBsAg by using the 3 systems developed by 3 major manufacturers; Abbott, Roche and Siemens Centaur XP. Results: The results for the qualitative analyses were closely concordant among the three systems (98.3%) for all 432 samples. In 123 samples that were determined as HBsAg-negative, (76%) distributed frequently at the upper half level ( ) of negative reference range, compared with (11%) and Centaur XP (22%). In particular, in 65 samples that were diluted from the strongly positive samples to obtain weakly positive samples, the average index values obtained using (3.6 S/CO), (4.2 COI) and Centaur XP (11.4 index value) differed significantly (P <0.0001). In the antiviral treatment group and the post-liver transplantation group, no inconsistency was observed among the results of the qualitative and quantitative assays. In the 18-fold serially diluted samples, no linearity was observed. Conclusions: Because of the possibility of false-positive detection in the HBsAg-negative samples, regular management of equipment and appropriate selection of reagents are very important. In weakly positive samples, quantitative assay has not to be replaced for qualitative assay. Therefore, the qualitative assays should be used for screening the samples, whereas the quantitative assays should be used for monitoring the Hepatitis B virus (HBV) load in the samples determined as HBsAg-positive. The qualitative index value should not be interpreted as a quantitative measure of HBV load. 2 유자가감소하는추세를보였으나, 1998년보건복지부에서시행한실태조사에의하면한국인 10세이상남자의 5.1% 와여자의 4.1% 에서 HBsAg이양성으로여전히 HBsAg 만성보유자들이많이보고되고있다 [3]. HBsAg 검사는 HBV 감염후첫번째로나타나는표지자로서, 급성혹은만성B형간염의기본이되는검사이다. HBV 유병률이높은지역에서는 B형간염의선별검사로이용되고있으며만성B형간염환자에서는치료반응을예측하는인자로도이용되고있다. HBsAg 정량검사는간세포핵내의 covalently closed circular DNA (cccdna) 와상관관계가높다고보고되고있다 [4-6]. 따라서최근만성간염환자의추적검사에서 HBV DNA 검사와함께 HBsAg 정량검사가많이이용되고있다. HBsAg 검사로는효소결합면역측정법 (enzyme-linked immunosorbent assay, ELISA), 효소면역측정법 (Enzyme immunoassay, EIA), 미세입자효소면역측정법 (microparticle enzyme immunoassay, MEIA), 방사면역측정법 (radioimmunoassay, RIA) 등이사용되어왔고최근에는민감도가높고자동화가가능한검사법인미세입자효소면역측정법 (microparticle enzyme immunoassay, MEIA), 화학발광면역측정법 (Chemiluminescence immunoassay, CLIA), 전기화학발광면역측정법 (electrochemiluminescence immunoassay, ECLIA) 이개발되어사용되고있다. 하지만위음성결과들이보고되고있는데그원인에는 B형간염보균자의혈중에검사법의검출한계보다낮은농도로표면항원이존재하거나 [7], 바이러스의특정부위의변이로인한돌연변이로인해검사시약의항체와반응하지않는경우 [8, 9] 등을들수있다. 따라서보다정확하고민감한방법으로 B형간염진단검사를선택하는것이필수적이다. HBsAg 검사시약은포획항체 (capture antibody) 와결합항체 (conjugate antibody) 로이루어져있다. ARCHITECT HBsAg Qual Ⅱ (Abbott Laboratories, Delkenheim, Germany) 와 Elesys HBsAg Ⅱ (Roche Diagnostics, Mannheim, Germany) 시약의경우포획항체로 2개의단클론항체를사용하고결합항체로단클론및다클론항체조합을사용한다. ADVIA Centaur HBsAg Ⅱ (Siemens Healthcare Diagnostics Inc., Tarrytown, NY, U.S.) 시약의경우에는포획항체로 3개의단클론항체를사용하고결합항체로 2개의단클론항체를사용한다. HBsAg을포획하는항체의종류에따라돌연변이의검출률에차이가생기는데단클론 (monoclonal) 항체는돌연변이의검출률이낮고다클론 (polyclonal) 항체의경우돌연변이의검출률이높지만특이도가낮은단점이있다 [10, 11]. HBsAg 검사시약제조사들은이러한단점들을개선시키기위해노력해오고있는데우선 Abbott사는 ARCHITECT HBsAg 검사시약의민감도와특이도를높이고돌연변이의검출률을향상시켰다. Roche사는기존의시약이 1개의단클론포획항체와 1개의결합항체로이 루어져있었는데최근포획항체로 2개의단클론항체, 결합항체로단클론항체와다클론항체를사용하여돌연변의검출률을높였다. Siemens사는포획항체를 3개의단클론항체로증강하여특이도를높이고 a-determinant 에있는돌연변이검출률을높인시약을최근에개발하였다. 본연구는다양한환자군에서얻은검체를이용하여최근개선된주요세가지 HBsAg 시약을이용하여 HBsAg 정성검사와정량검사의검출능을비교하고자하였다. 대상및방법 1. 대상 실험에사용된검체는일반환자군으로 HBsAg 음성군검체 ( 이하, 음성군 ) 50개, HBsAg 약양성군검체 6개, HBsAg 강양성군검체 ( 이하, 강양성군 ) 50개, 고위험군바이러스혈청은행 ( knrrb.knrrc.or.kr) 에보관되어있는검체 ( 이하, 혈청은행검체군 ) 100개 (HSB ), 항바이러스제치료를받고있는만성 B형환자검체 ( 이하, 간치료군 ) 50개, 간이식군검체 ( 이하, 간이식군 ) 41개, HBsAg 양성을희석한검체 ( 이하, 희석군 ) 135개로총 432검체였다. 일반환자군검체의수집은 2011년 10월에서 2012년 1월까지울산대학교병원진단검사의학과로의뢰된검체와고위험군바이러스혈청은행에보관된검체를사용하였다. 장비로검사하여얻은지수값 (Signal cutoff value, S/CO) 의결과에따라음성군 (S/ CO<1.0), 약양성군 (1 S/CO<10.0) 강양성군 (S/CO 10.0) 으로임의로분류하였다. 약양성검체에서의차이를좀더알아보기위해 HBsAg 정성검사에서강양성이나온검체를약양성값이나오도록 10,000배희석하여 135개검체를임의로제조하였으며, 검사결과는 기준으로 65검체에서약양성으로확인되었다. HBsAg 약양성검체 ( 이하, 약양성군 ) 는환자검체 6개와약양성값이나온희석검체 65개를포함하여모두 71검체를대상으로하였다. 간치료군검체는만성간염환자가운데라미부딘약제치료를받고있는환자에서얻었으며, 간이식군검체는간이식후면역글로불린을투여중인환자에서얻었다. 직선성검증검체로는 HBsAg 정성검사의결과가강양성이나온검체 2개를 18단계로희석하여임의로제조하였다. 희석액은 HBsAg, Anti-HBs 및 DNA 검사에서음성인혈청을사용하였다. 2. 평가장비와시약검사에사용된장비는각각 Abbott사의 i2000sr analyzer (Abbott Laboratories, Abbott Park, IL, USA), Roche사의 Modular analytics system (Roche Diagnostics, Mannheim, Germany), Siemens사의 Centaur XP (Siemens Healthcare Diag

ARCHITECT HBsAg Qual Ⅱ는미세입자화학발광면역측정법을원리로하고있고화학발광이유도될때이신호를검출기로측정하여보정을통해구해진기준치와검체에서측정한신호를이용해계산된지수값 (S/CO) 의비율로결과를판정하며이값이 1.0 이상인경우양성으로판정한다. Elesys HBsAg Ⅱ는전기화학발광면역측정법을사용하는데전극에전류를가하는순간화학발광이유도되고이신호를검출기로측정하여보정을통해구한기준치 (cutoff index, COI) 와검체에서측정한신호를이용해계산된지수값으로결과를판정하며 0.9 이상 1.0 미만인경우 borderline으로판정하고, 1.0 이상인경우양성으로판정한다. ADVIA Centaur HBsAg Ⅱ는화학발광면역측정법을원리로하고있고지수값 (Index value) 이 1.0 이상인경우양성으로판정한다. 2) HBsAg 정량검사 HBsAg 정량검사의경우 Abbott사, Roche사두회사에서만전용시약을공급하고있어두회사에서만검사를시행하였다. Abbott사는 CMIA법을사용하고 0.05 IU/mL 이상을양성으로판정 하며측정범위는 IU/mL이다. 250 IU/mL를초과하면검체를희석하여결과를산출한다. Roche사는 ECLIA법을사용하고, 결과판정은 0.05 IU/mL 이상인경우양성으로판정하며검출한계치는 0.01 IU/mL이다. Roche사에서는정량검사측정시검체를 400배자동희석한후희석배수를보정하여결과를산출하며측정범위는 ,000 IU/mL이다. 3) 통계모든통계분석은 SPSS ver 17.0 을이용하였다. 세회사간의 HBsAg 양성검출률의차이를카이제곱검정을통해비교하였으며 HBsAg 정량검사간의양성검출률의차이는 paired t-test를통해비교하였다. P 값이 0.05 미만인경우통계적으로차이가유의한것으로판단하였다. 결과 1. 전체결과 총 432검체에서 HBsAg 정성검사를시행하였고음성군, 약양성군, 강양성군으로임의로분류하였으며세회사간의검사결과일치율은 98.3% 였다 (Table 2). 2. 음성검체결과총 432개의검체에서세회사의 HBsAg 정성검사에서모두음성을보인검체는 123개였으며, 음성검체를 0.25 구간씩네개의그룹으로나누어 Table 3에나타내었다. HBsAg S/CO 0.5 초과값을보 인검체는 (76%), Centaur XP (22%), (11%) 순이었다. 유의한차이를보였다 (P <0.0001). 약양성군에서 의양성 률이 92% 였고, 평균값은 IU/mL 이었으며, 은양성률이 83% 였고평균값은 IU/mL 였다. 결과값에서 는 범위의결과값이 64% 로가장많았고, Centaur XP 는 범위의결과값이 78% 로가장많았다. 3) 정성과정량검사 약양성군에서정성과정량간의차이를보인검체는 에 3. 그룹별 1) 정성검사 각검체별로구분하여검사한 362 검체들중에서음성군, 약양성 서 6 검체 (6/71, 8.4%) 였고, 에서 12 검체 (12/71, 16.9%) 였으며 (Table 6), 정성검사와정량검사간의비교에서 가 보 다불일치검사가상대적으로더적었다. 군, 강양성군, 혈청은행검체군, 간치료군, 간이식군의정성검사결 과는각군별로모두일치하였다 (Table 4). 약양성군에서의 HBsAg 분포를 Fig. 1 에나타내었다. 4. 희석검체 1) 정성검사 HBsAg 양성인검체를약양성군으로임의로희석하여제조하였 2) 정량검사각그룹별정량검사결과를 Table 5에나타내었다. HBsAg 검사에서음성군, 강양성군과혈청은행검체군, 치료군, 간이식군에서는유의한차이가없었으나약양성군에서는두회사간정량값에 던 135개검체에서정성검사의결과에따라음성군, 약양성군, 강양성군으로나누었고결과값을 Table 7에나타내었다. 의 HBsAg 그룹별농도는음성군에서 S/CO, 약양성군은 S/CO, 강양성군은 S/CO의범위에있었다

5 은음성군에서 COI, 약양성군에서 COI, 강양성군에서 COI 범위에있었으며, Centaur XP 는 음성군에서 index value, 약양성군에서 index value, 강양성군에서 index value 범위였다. 세회사 중음성률은 가가장높았고 (31%), (29%), Centaur XP (26%) 순이었으며양성률에서는 Centaur XP (74%) 가가장높 았고, (71%), (69%) 순이었다. 강양성률은 Centaur XP (40%) 가가장높았고 (25%), (21%) 순이었다. 약 양성군검체는 (48%), (46%), Centaur XP (34%) 순이 었으며평균값은 3.6 S/CO, 4.2 COI, Centaur XP 11.4 index value 였고 와 (P <0.0001), 와 Centaur XP (P <0.0001), 와 Centaur XP (P <0.0001) 모두유 의한차이를보였다. 강양성군검체는 Centaur XP (40%), (25%), (21%) 순이었으며평균값은 COI, S/CO, Centaur XP index value 였으며 Centaur XP 가상대적으로높은값을보였다. 세장비간결과값에는유의 한차이가없었다. 세장비간에 HBsAg 결과값에차이가나는검 체를 Table 8 에나타내었다. Linearity test with serial dilution of 2 HBsAg strongly positive samples. 3) 정성과정량검사정성과정량검사간의차이를보이는검체는 9검체 (6.66%), 13검체 (9.62%) 였다 (Table 9). 희석검체 135개로검사한정성검사와정량검사간의상관계수는 과 이각각 0.93 과 0.87이었다. 5. 직선성검증 HBsAg 정성검사에서강양성을보인검체 2개를 18계단희석하여 과 두장비의 HBsAg 정성검사와정량검사를시행하였다. 직선성검증을시행한결과두검체모두직선성을보이지않았다 (Fig.2). 정성검사와정량검사간의회귀분석을통해비교해본결과 는 R 2 값이 과 였고 은 와 였다 (Fig. 3). 고찰 우리나라는만성간염의유병률이높은지역으로 B형간염의선별검사및추적검사로 HBsAg 검사가자주이용되고있다. 하지만 HBsAg 검사는간혹위음성결과들이보고되고있는데그원인에는검사법의검출한계보다낮은농도로항원이존재하거나바이러스의특정부위의변이로인한돌연변이가검사시약의항체와반 응하지않는경우등이있다. 따라서민감도가높은검사시약으로 HBsAg을검사하는것이중요하다. 최근 Abbott, Roche, Siemens 에서는 HBsAg의민감도와특이도를높이고, 돌연변이검출률을높인시약들을개발하였다. 이전에도다양한시약에따른 HBsAg 항원검출률을비교한연구들이있지만최근개선된시약들의 HBsAg 검출률을비교해보았다. 총 432개의검체를이용하여검사를시행하였다. 전체검체의 HBsAg 정성검사결과에서양성검출률은 Centaur XP (72%) 가가장높았고 (70%) 와 (70%) 는동일하였다. 그중강양성결과를보인검체는 Centaur XP (60%) 에서가장많았고다음으로 (54%), (53%) 순이었다. 음성결과를보인 123개의검체를 4개의범위로나누어비교해본결과 index value 값들의분포가농도별로다양하게관찰되고특히 미만범위의값들도다수분포하였다. 이는측정시장비보정 (calibration) 의 baseline이일시적으로높아지거나검체요인으로인해 index value가높아질경우발생하며위양성을초래할가능성도있으므로검사시장비를잘점검할필요가있다. Centaur XP는 index value의낮은범위결과값이많고전체검체군에서양성을보인검체에서는강양성결과값이많은것으로보아저농도와고농도의결과값을타사제품들에비하여보다더잘구분해줄수있었다. 환자군에따른 HBsAg 결과값의차이를비교하기위해음성군 165

B 과약양성군, 강양성군, 혈청은행검체군, 간치료군, 간이식군으로나누어비교해보았다. 모든그룹에서 HBsAg 정성검사의결과는일치하였다. 이결과는비슷한연구에서도차이가없는것으로보고되어동일한결과를보였고 [12, 13] 외부신빙도조사결과보고에서도일치율이 95% 이상으로유사한결과를보였다 [14-17]. 하지만동일검체에대한비교실험을한외국의논문에서는서로다른결과를보였다는예도가끔있었다 [18, 19]. 환자군에따른정량검사에서는약양성군을제외하고음성률과양성률은일치하였다. 약양성군에서는 HBsAg 정성검사와정량검사에서불일치를보이는검체가 13개있었다 (Table 6). 차이를보인검체는정성검사에서모두양성결과를보인검체들로 는 6검체 (1.66%) 에서음성을보였고, 는 12검체 (3.32%) 에서음성을보였다. 차이를보인검체들은임상기록검토를통해만성B형간염환자로확인되었고 HBsAg 정성검사에서 S/ CO 농도값들이었다. 이것으로보아낮은농도값에서는정성검사가더예민하고검출률이높으며, 정량검사에서위음성가능성이더높다는것을알수있다. 따라서 HBsAg의스크리닝검사로는정성검사가더유용하고정량검사는만성간염환자에서추적검사로더유용하게쓰일수있으며정량검사결과를정성검사로서해석해서는안된다. 하지만본연구에서사용한저농도검체에는인위적으로희석하여제조한검체가포함되어있어실제환자검체에서는다른결과가보일수도있다. 불일치검체에대해서는 PCR 등의좀더예민한검사법이나참값을갖고있는확진법으로검증 을해주는것이더좋을것으로생각한다. 주로검사간차이를보이는약양성검체의결과를분석하기위해 HBsAg 강양성을보이는검체 135개를희석하여실험하였다. 희석군에서 HBsAg 정성검사결과값은음성군, 약양성군, 강양성군으로나누었다. 음성률과약양성률은 가가장높았고 (79%), 다음으로 (75%), Centaur XP (60%) 순이었다. 강양성군은 Centaur XP에서가장많았고 (40%) 다음으로 (25%), (21%) 순이었다. 135개의희석검체에서정성과정량간의차이를보이는검체는 9개 (6.66%), 13개 (9.62%) 의검체였고 S/CO 값이 사이의낮은농도값에서차이를보이므로낮은농도에서의결과값의해석에주의가필요하다. 희석검체를이용하여정성과정량검사간의관계를회귀분석한결과 R 2 = 0.93, R 2 = 0.87이었다. 따라서양성검체에서약양성결과를적게보이고강양성결과가많이보이는제품이더위음성의가능성을줄일수있으므로제품의선택에참고하여야한다. 직선성검증을위해 HBsAg 강양성을보인검체 2개를 18계단희석하여정성검사의예상농도와측정농도간의관계를비교해본결과 2개검체모두에서직선성을보이지않았다. 직선성검증에사용된검체의정성값과정량값을비교해본결과두회사모두정성과정량간에좋은상관관계를보였지만 HBsAg 고농도부위에서는직선성이현저히떨어져보였다. 이는정성검사의고농도검체에서발생하는 hook effect 때문으로생각된다. 이를배제하기위하여정량검사에서는 은 250 IU/mL 이상의값에서는 166

8 희석하도록권장하고있으며 에서는측정전 400배자동으로희석하여검사하고있다. 이결과로보아정성검사로정량검사의결과값을어느정도추정하는것은가능하지만환자마다동일하게적용할수없으므로정성검사로정량검사를대체할수는없을것으로생각한다. 요약 배경 : 간염검사법들은과거부터꾸준하게개선되어왔으나저농도혈청값, 비특이반응, 돌연변이등으로인하여위음성결과들이보고되어주요공급사들에서는특이도가높은단클론항체와다클론항체들을사용하고항체의종류를추가하여불일치반응들을줄이고자하였다. 따라서최근에새로이개발된주요검사법들을이용하여그분석능을비교하고자하였으며, 정성검사값을정량검사로써준용하여해석하려는사례들이있으므로정성검사 index value와정량검사농도값을비교하고자하였다. 방법 : HBsAg 검사에서음성, 약양성, 강양성결과를보인검체, 환자군별로구분한간치료군과간이식군의검체, 혈청은행에서양성으로확정된검체들을포함한총 432검체들을대상으로주요 3사의제품들로정성검사와정량검사를실시하였다. 약양성검체들은강양성검체들을희석하여약양성결과를보이도록제조하였으며, 이들을대상으로정성검사와정량검사를실시하였다. 강양성검체들을 18단계계대희석하여제조한검체들을대상으로정성검사와정량검사를실시하였다. 결과 : 세회사장비간의정성검사일치율은 425/432, 98.3% 로매우우수하였다. 음성검체에서정성검사의결과값은세회사장비에서모두일치하였으나 index 값을 0.25범위로나누어본결과 미만인검체들은 (2%) 와 Centaur XP (8%) 에비하여 (12%) 에서더많았다. 또한 3검체 (2.43%) 에서정성검사와정량검사간의차이를보였다. 양성결과값을보인 309검체에서 6검체 (1.94%), 14검체 (4.53%) 에서정성과정량간의결과값이달랐고 index 값은 S/CO범위에있었다. 약양성으로제조한 65검체들에서정성검사평균값은 3.6 S/CO, 4.2 COI, Centaur XP 11.4 index value였고 과 (P <0.0001), 과 Centaur XP (P <0.0001), 과 Centaur XP (P <0.0001) 에서유의한차이를보였다. 간치료군과간이식군에서의세회사간의정성검사와정량검사결과는모두일치하였다. 강양성검체들을 18계단희석한검체들에서직선성을보이지않았다. 결론 : 음성검체일부에서는 index value가상대적으로높게나타나서위양성을보이거나항원-항체모두양성결과를보일위험이있다. 이는장비의백그라운드값이높아지거나시약의비특이반 응으로나타날수있으므로정기적인장비관리와적절한시약의선택이필요하다. 약양성검체들에서정성검사에서는양성이지만정량검사에서는음성인검체들이보이므로정량검사에서는위음성으로판정될수도있다. 그러므로정량검사결과로정성검사를판정하여서는안되며단지모니터링에서만사용하여야한다. 강양성검체들을계단희석하여제조한검체들에서정성검사와정량검사들은검체별로서로상이한결과를보이므로정성검사에서보고되는 index value를정량검사의결과로준용하여해석해서는안된다. 감사의글 본연구는애보트, 지멘스의연구비지원과애보트, 로슈, 지멘스의학술및기술제공지원을받아수행되었습니다.
