Inhalation induction with desflurane alone causes adverse airway events, such as coughing, bronchospasm, laryngospasm, and copious secretion of varying severity. The aim of this study was to determine whether desflurane minimize cardiovascular activation during induction. Methods: Sixty ASA I and II patients were randomized to receive 1 MAC or 1.5 MAC of desflurane during manual vernilation or not. Patients received propofol (2 mg/kg) to induce loss of consciousness (LOC). Rocuronium (0.8 mg/kg) was given at LOC and the trachea was intubated after 90 seconds of manual breathing support with or without inhaled anesthetics. Vital signs and adverse airway events were recorded until 10 minutes post-intubation. Results: A significant increase in blood pressure and heart rate were seen in no desflurane group. The stable vital signs were seen in desflurane groups. The adverse airway events were increased in 1.5 MAC group but 1 MAC group. Conclusions: Desflurane was able to be stable blood pressure and heart rate at 1 MAC but adverse airway events were increased at 1.5 MAC of desflurane. 2 김철홍 : 마취유도시 Desflurane 의효과 23 에게투여할수있다 (Akman, 2004). 최근흡입마취제만으로마취유도를시행하는방법이많이사용되고있는데, 특히외래마취시에후두마스크의사용과함께그빈도가증가하고있다. 흡입마취유도와유지 (volatile induction and maintenance of anesthesia, VIMA) 방법이단일약제를사용함으로서안전성과마취회복에장점이있다고알려져있다 (Ghatge, 2003). 이론상으로혈액-가스분배계수가가장낮은마취약제인 desflurane은가장이상적인흡입마취유도제이다. 그러나많은마취과의사들은 desflurane의자극성냄새와상기도를자극하는효과가있어서특히마취유도시에 desflurane의사용을기피하고있다. Desflurane 단독으로흡입마취유도를하는경우에기침, 기관지경련, 후두경련, 분비과다등의부작용을일으킨다고알려져있지만 (Kelly, 1993; Bunting, 1995) 이러한부작용은고농도의 desflurane을갑자기투여하는경우와 (Jones, 1990) 보조약물의부적절한사용으로유발된다고한다 (Kelly, 1993; Saros, 2006). 이실험에서저자는두시간이내의짧은구강외과수술을위한전신마취시주로사용되는 desflurane을이용하여, 마취유도시농도를달리투여하였을때마취유도시에발생되는혈역학적변화와기도합병증의발생빈도를조사하여적절한 desflurane 농도를찾고자하였다. 대상및방법본원에서구강악안면외과수술이예정된미국마취과학회신체등급 1, 2급에속하는환자 60명을대상으로하였다. 모든환자에게수술전방문시연구의취지에대한설명을하고동의를구하였다. 두시간이상소요될것으로예상되는환자, 기도유지가어려울것으로예상되는환자, 고혈압, 뇌혈관질환, 신경학적이상, 대사장애, 내분비이상, 신기능 / 간기능장애가있는환자는연구대상에서제외하였다. 모든환자는수술전 8시간이상금식시켰고, 수술실도착 30분전에마취전투약으로 glycopyrrolate mg/kg을근주하였다. 수술실도착후비침습적혈압측정기, 심전도, 체온계, 맥박산소포화도측정기를부착하여환자의활력징후를측정하였 다. 기준혈압과심박수를마취유도전에측정하였고이후매 1분마다혈압을측정하였다. 호기말이산화탄소분압과 desflurane 분압은호흡회로와연결된가스측정기 (Philips M 1026B; Philips, Germany) 를통해측정하였다. 모든환자에서사용된마취기 (Fabius GS; Drager, Germany), 환자감시장치 (Intelli- Vue MP70 Anesthesia; Philips, Germany), 기화기 (Drager D-vapor; Drager, Germany) 는동일하였다. 모든환자에게마취유도전안면마스크를통해 3분간 100% 산소를흡입시킨후 propofol 2 mg/kg을정맥내투여하고환자의의식이소실되는것을확인한후근이완을위해 roxuronium 0.8 mg/kg을정맥내주사하고 90초후에기관내삽관을시행하였다. 마취유지는산소와공기를각각 1.5 L/min의유량으로혼합하여 desflurane의호기말농도를 1 minimum alveolar concentration (MAC; vol%) 으로유지하였으며, 호기말이산화탄소분압은 mmhg를유지하도록환기량을조절하였다. 환자는무작위로세군으로나누어마취유도시에의식소실이후안면마스크를통해산소만투여 (Group 0), 90초간 desflurane 6 vol% 로투여 (Group 1), 그리고 9 vol% 로투여 (Group 1.5) 하였다. 마취유도및기관내삽관은동일한마취과의사가시행하였다. Desflurane을투여한군은의식소실후에 1분간 67% 아산화질소혼합가스 (N 2O 4 L/min + O 2 2 L/min) 를투여하여보다빠른마취유도를하고자하였고, 이후에는 100% 를산소를투여한후기관내삽관을시행하였다. Group 1, 1.5에서기침등의기도자극증상이있는경우, 혈압이기준혈압보다 30% 이상감소하는경우나심박수가기준심박수보다 30% 이상증가하는경우 desflurane 농도를 3 vol% 씩낮추어 1분간투여하였고혈압과심박수의안정 (30% 이내로유지되는경우 ) 을보이고기도자극증상이없어진경우에는다시 3 vol% 씩증가시켰다. Desflurane의효과를조사하기위해기준혈압과심박수, 기관내삽관을시행하기직전과직후, 그리고삽관후 10분에각각혈압과심박수를측정하였고, desflurane 투여후기침등의기도자극증상이나타나는빈도를기록하였다. 모든결과는평균 ± 표준편차로표시하였으며, 통계는 Prism ver 3.0 프로그램 (GraphPad Software Inc, California, USA) 을이용하여처리하였다. 각군간비

Desflurane을사용하지않은 Group 0에서기관내삽관시에혈역학적인안정성이확보되지않고기준혈압및심박수에비해현저히높은혈압과심박수를보였다가 10 분후에기준혈압으로떨어졌다 (Fig. 1, 2). 혈압은 Group 1과 1.5에서삽관을전후해서 Group 0보다낮게유지되었고 (Fig. 1), 심박수는삽 관직후에만낮게유지되었다 (Fig. 2). 기도자극으로인한기침의유발빈도는 Group 1.5에서높게나타났으며, Group 0과 1에서는각각 0명과 2명으로차이를보이지않았다 (Table 2). Group 1.5에서기도자극증상으로인한마취제농도조절은횟수는많았지만농도조절후에는기도자극증상이없어져서 1.5 MAC으로유지하는데어려움이없었으나 3명의환자에서지속적인기도자극증상이있어서 1 MAC으로유지하였다. 고찰이연구에서전신마취유도시에 desflurane의사용

4 김철홍 : 마취유도시 Desflurane 의효과 25 이기관내삽관에대한혈역학적과민반응을억제할수있음을알수있었다. 다만 desflurane의흡입농도를적절히조절하지않으면기도자극으로인한증상이발현할수있고이는심각한문제를초래할수있다. 고전적인마취유도방법인정맥마취제와속효성근이완제의혼합사용은기관내삽관으로인한혈압과심박수의상승을억제할수없었고, 현재많은연구에서흡입마취제나마약성진통제와같은보조약제의사용을추천하고있다. 흡입마취제를사용하여마취유도를하는것은정맥마취제를사용하지않기때문에이론적으로한가지약제를사용함으로써보다안전하고마취회복에유리한장점이있다 (Ghatge, 2003). 이러한관점에서짧은수술이나외래환자의전신마취는빠른마취유도와회복시간을보이는 desflurane이가장이상적인마취유도제이다. 하지만 desflurane은자극적인냄새와기도자극으로인한부작용때문에사용이제한적이다. Bunting 등은 (1995) desflurane을이용한흡입마취유도시에기침, 기관지경련, 후두경련, 분비물증가등과같은부작용을보고하였다. Kong 등은 (2000) desflurane을이용한흡입마취유도시에 1 vol% 에서시작하여 6번의호흡후에 1 vol% 씩증가시키는방법을사용하였으나이연구에서는실험전에미리조사한 desflurane 농도에서 1 MAC 이하에서기도자극증상이나타나지않은것을근거로 1 MAC을기준으로군을나누었다. Group 1.5에서기도자극증상이현저히증가하는것을볼수있었으나 1 MAC 이하의낮은농도에서는기도자극증상이거의나타나지않았는데미리정맥마취제로진정된상태이기때문일것으로생각된다. Copppens 등은 (2006) remifentanil, propofol and rocuronium을사용하여신속한마취유도를시행할경우흡입마취제를추가로투여하는것은기관내삽관시발생하는혈역학적반응을감소시키는데효과가없다고하였다. 이러한결론은이연구에서마약성진통제인 remifentanil을사용하느냐안하느냐에관한문제이다. Coppens 등은 (2006) 흡입마취제를마취유도중에사용하였을때수술실을오염시킬위험이존재하기때문에굳이흡입마취제를사용하여혈역학적안정성을도모할필요가없다고주장하지만마취중마약성진통제를사용하는것은다른문제를야기할수있다. 마약성진통제는주 로 fentanyl, remifentanil, alfentanil 등이사용되는데정맥으로주사한경우흉부경직이 (Eventov-Friedman, 2010) 나타날수있고, 다른정맥마취제와혼합사용시혈압이많이떨어지거나 (Coburn, 2005), 호흡억제가심하게나타날수있는단점이있다 (Bovill, 1984). 이연구에서 propofol 투여후일시적인호흡억제가나타난경우는있었지만지속적인호흡억제가나타난경우는없었다. Desflurane을사용한흡입마취유도는기도자극증상이나타나는데이러한부작용은갑자기고농도의 desflurane을흡입시켰을때나타난다 (Jones, 1990). 이연구에서도처음부터 1.5 MAC으로투여한 Group 1.5에서기도자극증상이많이나타났다. 하지만연구방법에서기술한대로 3 vol% 씩농도를조절하여기도자극증상이소실되는것을관찰하였으며, 이후 1.5 MAC으로투여하여도기도자극증상이계속나타나는경우는 15%(3/20) 이었다. 1 MAC의농도에서는기도자극증상이계속나타나는경우는한명도없었던것으로보아 Shin 등의연구에서와마찬가지로 1 MAC 이하의 desflurane은 Kong 등이사용한점진적으로증가시키는방법을사용하는것보다바로 1 MAC의 desflurane을사용하는것이빠른마취유도를위해유리할것으로생각된다. 결론적으로통상적으로사용하는정맥마취제와근이완제를사용한마취유도는기관내삽관에따른혈역학적안정성을유지하는데어려움이있고마취유지에사용되는 desflurane을보조적으로사용하면기관내삽관으로인한혈역학적반응을감소시킬수있다.
