It was reported that a supplement of ecabet sodium to conventional triple therapy showed good results in Asia. The Aim of this study was to ascertain the efficacy of additional ecabet sodium on conventional triple therapy for eradication of H. pylori. Methods: We reviewed the cases of 111 patients (Group A) with H. pylori infection who received ecabet sodium with triple therapy (20 mg of rabeprazole, 1 g of amoxicillin, 500 mg of clarithromycin and 1 g of ecabet sodium, twice daily for 7 days). Another 186 patients (Group B) received PPI-based triple therapy (same as the above, except without the ecabet sodium). Eradication was evaluated 4 weeks later after completion of treatment by 13 C-UBT. Results: Eradication rates were 74.8% (83/111) in group A and 70.4% (131/186) in group B by intention-to-treat analysis (p=0.420), and 75.2% (82/109) in group A and 70.7% (128/181) in group B by per protocol analysis (p=0.405). Conclusions: The addition of ecabet sodium to conventional triple therapy did not increase the eradication rate of H. pylori in this study. These findings imply that ecabet sodium as an additional agent cannot overcome antibiotic resistance, which is the most important cause of failure of triple therapy. 2 제요법의효용성에대한의문이제기되고있다. 10,11 국내에서도이표준삼제요법의제균율은 1991년부터 1998년까지는 90% 이상으로초창기에는매우효과적인것으로인정받았으나, 년에서 2003년까지의여러연구에서 % 정도를보였고, 년에는 77.6% 정도로감소하였으며, 전반적으로 2000년이후의 H. pylori 제균율은 80% 미만으로알려져있다. 16 이러한제균율의감소는항생제내성률, 투약시의비순응, 충분하지못한치료기간, 약제부작용, 균의양, 흡연여부, 기저질환등의여러가지원인에의한것으로생각되고있다 여러가지원인중 H. pylori 제균율에감소에영향을미치는가장중요한원인으로항생제내성이고려되고있으며, 그중 clarithromycin에대한내성이가장중요한원인으로생각되고있다. 20 Ecabet sodium은 dehydroabeitic acid로소나무수지에서정제해낸물질이며 21 위점막보호효과가있어동아시아지역에서위염과위궤양의치료에널리사용되고있다. 22 Ecabet sodium은펩신활동성을억제할뿐만아니라 22 urease의활동성을억제하며, 23 산성환경에서 H. pylori의성장을억제하는것으로알려져있다. 24 최근들어동아시아에서 ecabet sodium 을 H. pylori의제균요법에추가로사용하여제균율을증가시켰다는보고들이있었다 본연구에서는한국인환자에서 ecabet sodium이프로톤펌프억제제를기반으로하는 1차삼제요법에추가하였을때제균율의호전에효과가있는지확인하고자하였다. 대상및방법 1. 대상 2008년 1월부터 2010년 1월까지분당서울대학교병원에서 amoxicillin, clarithromycin, rabeprazole을사용한환자에서 H. pylori 감염이진단된환자중 1차제균치료를시행한환자 525명을대상으로하여전자의무기록을통하여분석하였다. 연구제외기준은 1) 이전 H. pylori 감염에대하여치료를시행하였던경우, 2) 간경화, 신부전등의심각한동반질환이있는경우, 3) 이전위수술을받았거나, 점막절제술을시행한경우, 4) 치료 1달전까지항생제를투약한경우, 5) 임신한경우로하였다. H. pylori 감염의진단은위소만전정부에서얻은조직으로시행한 rapid urease test (CLO test; Delta West, Bentley, Australia) 양성이거나, 내시경으로위소만의전정부또는체부에서생검하여얻은조직에변형된 Giemsa 염색을시행하여 H. pylori에의한위염을확인한경우로하였다. 2. 방법 기존의프로톤펌프억제제를기반으로하는삼제요법에 ecabet sodium을추가하여투여한군 (Group A) 은 amoxicillin 1 g, clarithromycin 500 mg, rabeprazole 20 mg에 ecabet sodium 1 g을추가하여 1일에 2회씩 7일간투여하였고, 프로톤펌프억제제를기반으로하는삼제요법군 (Group B) 은 amoxicillin 1 g, clarithromycin 500 mg, rabeprazole 20 mg을 1일에 2회씩 7일간투여하였다. H. pylori 제균여부는치료가완료된 4주뒤 13 C-요소호기검사를통하여확인하였다. 투약순응도및부작용도함께확인하였다. 제균율은연구에참여한모든환자에서 intention-to-treat (ITT) 로분석하였고, 부작용등을견디지못하여 85% 이상의약제를투약하지못한환자는제외하여 per protocol (PP) 로따로분석하였다. 본연구의 1 차결과는양치료군의 H. pylori 제균율로하였고, 2차결과는각하위집단의제균율및부작용의발생으로하였다 C-요소호기검사 13 C-요소호기검사를시행하기위하여환자는적어도 4시간이상금식을한뒤첫호기샘플을얻었다. 100 mg의 13 C-요소가루 (UBiTkit TM ; Otsuka Pharmaceutical Co, Ltd, Tokyo, Japan) 를물 100 mg에녹여경구투여하였고, 경구투여 20분뒤두번째호기샘플을얻었다. 분기점은 2.5% 로하였다. 모아진샘플은동위원소질량분석기 (UBiT-IR300 R ; Otsuka Pharmaceutical Co. LTd) 를사용하여분석하였다. 4. 통계분석통계분석은 SPSS 18.0 for window (SPSS Inc., Chicago, IL, USA) 프로그램을사용하였다. H. pylori 제균율은 ITT와 PP로분석하였고, 양치료집단의제균율및부작용의발생정도는카이제곱검정이용한단변량분석을시행하여비교하였다. 하위집단의제균율을비교하기위하여코크란-멘텔-헨젤검정을사용하였다. 양군의인구통계학적차이를비교하기위하여카이제곱검정및피셔의정확성검정을통하여분석하였다. 분석결과 p 값이 0.05 미만일때유의하다고판정하였다. 결과 1. 연구집단 2008년 1월부터 2010년 1월까지분당서울대학교병원에서 amoxicillin, clarithromycin, rabeprazole을사용한환자에서 H. pylori 감염이진단된환자중 1차제균치료를시행한환자 525명중 167명의환자는기존의프로톤펌프억제제를기반으로하는삼제요법에 ecabet sodium을추가하여투여받았고 (Group A), 358명의환자는프로톤펌프억제제를기반으로하는삼제요법을투여받았다 (Group B). Group B에속한 358명의환자중심각한동반질환이있는 83명, 치료 1달전까지항생제를투여받은 4명, 이전위수술및점막절제술등의과거력이있는 12명, 추적관찰을시행하지않은 73명이제외되어 총 186명의환자에대하여분석하였다. PPI-based conventional triple p-value therapy (Group B) 70.4%(131/186) 70.7%(128/181) 로하는삼제요법에 ecabet sodium을추가한요법의제균율증가가 10% 이상차이가있는경우의미있는차이를보이는것으로가정하면유의수준 α=0.05 및검정력 (1-β) 을 0.8으로하였을때계산한최소대상자수는양군을포함하여 136 명이상이며본연구에참여한환자수를고려할때제균율증가의 10% 이상차이를확인하는검정력은 0.95이다. 기존의프로톤펌프억제제를기반으로하는삼제요법에 ecabet sodium을추가하여투여받은군 (Group A) 과프로톤펌프억제제를기반으로하는삼제요법군 (Group B) 에서성별은각각남자 / 여자 44/67명, 72/114명으로약 2:3의비율이었으며, 양군간의유의한차이는없었다. 연령범위는 22 82세, 전체평균연령은각각 53.53±12.88세, 52.42±12.16세로유의한차이는없었다 (Table 1). 양군에서집단의인구통계학적특성은음주력외에는유의한차이를보이지않았다. Group A 에서음주력이있는환자는 30명 (27.0%) 이었고, Group B에서는 58명 (31.2%) 으로 Group B에서좀더많았다 (p=0.020). 내시경적진단은 Group A에서십이지장궤양 41명 (36.9%), H. pylori 연관위염 54명 (44.8%), 기타병변이 16명 (14.4%) 이었고, Group B에서십이지장궤양 89명 (47.8%), H. pylori 연관위염 66명 (35.5%), 기타병변이 31명 (16.7%) 이었다. 2. H. pylori 제균율 프로톤펌프억제제를기반으로하는삼제요법에 ecabet sodium을추가하여투여받은군 (Group A) 에서제균에성공한환자는 intention-to-treat 분석에서 111명중 83명으로제균율은 74.8% 였다. 프로톤펌프억제제를기반으로하는삼제요법군 (Group B) 에서제균에성공한환자는 186명중 131명으로 70.4% 였다 (p=0.420). 중도탈락된환자를제외하고 per protocol 분석을시행하였을때 Group A에서는 109명의중 82 명의환자에서제균에성공하여제균율은 75.2% 였으며, Group B에서는 181명의중 128명의환자에서제균에성공하여제균율은 70.7% 로나타났다 (p=0.405). ITT, PP 분석에서모두프로톤펌프억제제를기반으로하는삼제요법에 ecabet sodium을추가하여투여받은군 (Group A) 에서제균율이높은경향성을보였으나통계적으로유의성은없었다 (Table 2). Table 3. Adverse Events during the Ecabet Sodium with Triple Therapy (Group A) and the PPI-based Conventional Triple Therapy (Group B) Nausea/vomiting Abdominal pain Epigastric soreness Constipation Diarrhea Metalic taste Oral mucositis Chest discomfort Dizziness Dyspepsia Total Ecabet sodium with triple therapy (Group A), n (%) 2 (1.8%) 2 (1.8%) 9.9% (11/111) PPI-based conventional triple therapy (Group B), n (%) 6 (3.2%) 4 (2.2%) 3 (1.6%) 9.7% (18/186) p-value 구내염, 흉부불편감, 어지러움증, 소화불량등의호소가있었으나양군에서모두각각 1 2% 정도로많지않았으며, 전체부작용의발생빈도도 10% 정도로양군에서차이는없었다 (p=0.682)(table 4). 고찰 H. pylori 감염은균주의다양성과숙주감수성에따라위궤양, 위염, MALT 림프종, 위암등다양한질병을유발한다. 이들질환의치료및위암등의더심각한질병으로의진행을예방하기위하여 H. pylori 제균치료가필요하며, 최근제균의적용범위가점차확대되어가는추세이다. 그러나이전표준요법으로추천되던프로톤펌프억제제를기반으로하는삼제요법의제균율은점차감소하고있으며, 최근우리나라에서도 80% 미만으로낮아졌다 이에따라다양한약제들의추가및다양한투여방법이시도되고있으며 1차제균치료에도적용되고있다. 10,11,28,29 Ecabet sodium은산성 ph를가지는위액내에서펩신의활동성을억제하여펩신에의하여당단백질이분해되어점막의중합구조가분해되는것을억제하며, 위점막의통합성을유지하게하고 21,22 H. pylori의위상피부착을억제하며, H. pylori가위의산성을중화하여위에서생존할수있도록하여가장중요한독성인자로작용하는 urease의활성을억제하는것으로알려져있다. 23,24 또한 NF-kB (nuclear factor-kappa B) 를억제하여 H. pylori에의하여야기되는 IL-8, MCP-1 (monocyte chemotactic protein-1) 유전자의전사를억제하여위점막상피세포의세포자살을억제한다는최근의연구도있다. 30 본연구에서는 H. pylori에대하여살균효과를가진것으로알려져있는 ecabet sodium을 24 프로톤펌프억제제를바탕으로하는기존의삼제요법에추가하여 1차치료를시행할때한국인에서제균율의증가여부를확인하는것이목적이었다. 그러나프로톤펌프억제제를기반으로하는삼제요법에 ecabet sodium을추가하여투여받은군 (Group A) 과프로톤펌프억제제를기반으로하는삼제요법군 (Group B) 에서제균율은각각 74.8%, 70.4% 로 Group A에서제균율이높았으나통계적으로유의성은없었다 (p=0.420). 이것은 ecabet sodium을추가한군에서제균율이증가했다는이전의연구들 과는다른결과였다. H. pylori 제균에영향을미치는요인으로는항생제내성, 환자순응도, 나이, 흡연여부등이관여하며, 이중항생제내성이가장중요하게작용할것으로알려져있다. 20 본연구에서살균효과를가진것으로알려져있는 ecabet sodium을추가하여도 H. pylori의제균율에차이가없는것으로나타났다. 이최근항생제내성의증가가크게작용하였을것으로생각된다. 여러보고에서 H. pylori에대한항생제내성률은지역에따라그정도가다르나전반적으로증가하는것으로확인되고있다. 20,29, 년대항생제내성율은 amoxicillin이 %, clarithromycin은 9 11%, metronidazole은 33 39% 가량이었으나, 19, 년대를지나며 amoxicillin이 1 2%, clarithromycin 15 27%, metronidazole이 40 65% 로증가한것으로보고되고있다. 33,35 우리나라의경우에도 1990년대에는항생제내성율이 amoxicillin 5 6%, clarithromycin 2 3%, metronidazole 50 60% 였으나, 2000년대에는각각 %, 12 15%, 55 65% 로증가한것으로확인되었고, 시간이지나면서점차증가하고있다. 31,32 H. pylori 제균에 1차치료로사용되었던프로톤펌프억제제를기반으로하는삼제요법에생균제, 비타민 C 등의여러보조제를추가하여항생제내성을극복하려고하였으나제균율이증가했다는연구결과가없어현재까지표준치료로인정되지않고있다. 36,37 이러한사실들을고려할때 ecabet sodium이 H. pylori에대하여항균작용을가지고있다고하더라도제균에주된작용을담당하는 amoxicillin 및 clarithromycin의내성을막을수없으며, ecabet sodium을추가하여사용한군에서제균율이호전되지않았던원인도이때문일것이라생각하였다. 또한시간이흐를수록내성율이증가하는것을고려할때 ecabet sodium을추가하여사용한군에서더효과가있었다는기존의연구들 에비하여본연구에서는대상시기가더늦어항생제내성이더높아졌을가능성도생각할수있다. 본연구의하위그룹분석에서 ecabet sodium의사용여부는제균율에의미있는영향을미치지못하는것으로확인되었다. 6 균에미치는영향은아직명확하지않으며, 본연구에서연관성분석을시행하였을때는음주여부는제균에영향을미치지않는것으로나타났다. 본연구는환자의수가충분히많고, ecabet sodium을제외하고양군에서동일약제를사용하였고, 비슷한환자군을대상으로시행하였으나후향성연구라는한계를지니고있다. 향후대상환자의수를증가시켜전향적연구를시행한다면더의미있는결과를얻을수있을것이다. 또한제균에실패한환자에서얻은 H. pylori 균주에서항생제내성이있는지측정하여제균실패의원인을확인하는것이필요하다. 결론적으로본연구는 H. pylori의 1차치료에있어프로톤펌프억제제를바탕으로하는기존의삼제요법에 ecabet sodium을추가한군에서제균율이통계적으로의미있게증가하지못하는것을확인하였다. 이것은 ecabet sodium의추가가최근의항생제의내성을극복하지못한것으로생각된다. 따라서항생제내성률의증가를고려하여새로운 1차치료요법에대한연구가필요하다. 요약목적 : Ecabet sodium은 Helicobacter pylori에대하여항균성을가지고있다. 최근아시아에서 ecabet sodium을기존의프로톤펌프억제제를기반으로하는삼제요법에추가하여제균율을증가시켰다는보고들이있다. 본연구에서는한국인환자에서 ecabet sodium을삼제요법에추가하여 H. pylori의제균율에대한영향을확인하고자하였다. 대상및방법 : H. pylori에감염된환자들중 111명은기존의삼제요법에 ecabet sodium을추가하여치료하였고 (rabeprazole 20 mg, amoxicillin 1 g, clarithromycin 500 mg, ecabet sodium 1 g, 1일 2회, 7일간 )(Group A), 186명은삼제요법으로치료하였다 (rabeprazole 20 mg, amoxicillin 1 g, clarithromycin 500 mg, 1일 2회, 7일간 )(Group B). 투약이완료된 4주뒤에제균여부를 13 C-요소호기검사로확인하였다. 결과 : 결과는후향적으로분석하였고, 제균율은 intention to treat 분석시 Group A에서 74.8%(83/111) 였고, Group B에서 70.4%(131/186)(p=0.420) 이었다.
