Lugol s iodine method used following formalin-ether concentration (MGL), a most widely used protozoa examination. The trichrome staining method was performed after preserving the specimen in fixative solution. During the period from April 1986 to February 1987, a total of 390 samples from patients of Wonju Christian Hospital were collected and examined. The results are summarized as follows : 1. The detection rate of intestinal protozoa was 4.4 % by Lugol s iodine method compared with 14.2% by trichrome staining method. 2. Four species of protozoan cysts were observed by Lugol s iodine method, the detection rate of each intestinal protozoa was 1. 8% for Endolimax nana, 1. 0 % for Entamoeba coli, 0.8% for Entamoeba histolytica and 0.5% for Giardiα lamblia. 3. Using the trichrome staining method, four species of protozoan trophozoites and cysts were detected, and the detection rate of each intestinal protozoa was 6.5 % for Endolimax nana, 3.6% for Entamoeba coli, 3.1% for Entamoeba histolytica and 1. 4% for Giardia lamblia. 4. The sensitivity was 28 %, Lugol s method, 92% by trichrome staining and false negative 72% and 8% respectively. Negative predictive value was 88% by Lugol s method and 99% by trichrome staining. 2 1 서론 Kessel (1925) 9) 이한국에서는처음으로장내원충 감염에대하여조사보고한이래, 이용되어온원충겸 사법을보면최 (1926) 가직접도말법 6 회반복검사 로 60% 의장내원충류감염율을보고하였고, 정등 (1963) 은 formalin-ether 원심침전법에하 MGL 법 이라고함 ) 후 iodine 용액과흔합하는 Lugol s 염색 법으로 53.4% 의양성율을보고하였다. 안등 (1966) 은 MIF (Merthiolate-Iodine-Formalin) 고정후침 전법을이용하여 39.5% 의양성율을보고하고김등 (1971) 7) 은직접도말법, 유산아연부유법, MGL 볍 시행후의 Lugol s iodine 염색법으로 34.9 % 의감 염율을보고하였다. 이와같이우리나라에서는장내 원충검사법으로서직접도말하거나, MGL 법, 유산 아연부유법으로집란후 iodine 용액을혼합염색하 는 Lugol s 염색법을이용하여주로원충의포낭형 을검출하여왔다 7, 8) 그러나장내원충류의정확한감별확인과원충의 영양형과포낭형을모두관찰하여검출율을높이 기위해서는 MGL 법과더불어 PV A (Polyvinyl alcohol ; 이하 PVA 라함 ) 고정액이나 Schaudinn s 용액을사용하여가검물을보존시킨후 trichrome 염색법이나 iron-hematoxylin 과같은영구염색법을 실시하여원충을검사하도록추천하고있다. 녀 4, 5,13,15,17,18) 이에본연구에서는 1986 년 4 월부터 1987 년 2 월 까지원주기독병원에내원하는환자를대상으로고 정액사용후의 trichrome 염색볍과지금까지우리 나라에서원충검사법 A 로가장많이이용해온 MGL 법후의 Lugol s iodine 염색법을동시에실시 하여두방법간의원충의검출율을비교하고각검 사법의감수성과위음성및예측도등을비교검토 하였다. 1. 연구대상 11. 연구대상및방법 에게주었다. 2 ) 일반채변용기에받아진가검물은 MGL 법후 2 % iodine 용액 1 방울과혼합하여 (Lugol s 염색 법 ) 400 배에서원충을경겸하였다. 3) Scholten (1972) 15) 의방법에따라 Schaudinn S 고정액에 19ram 이상의신선가겸물을고정 가검물을잘섞고원심분리시험관에약 2ml 가량거즈 (gauze) 에통과시킨후생리식염수 (0.85% NaCl) 로 10 ml 정도되게채운다. CID 2, OOOrpm/ 2 min 정도원침시킨후상청액을 버리고, 침전물을잘섞어동량의 Mayer s albumin( 계란흰자와 glycerin 을동량섞어여 과시켜만듬 ) slide 에 2 방울정도떨어뜨린후 2cm 가량 실온에서 1-2 시간말린후 trichrome 염색 을한다. 이때도말이잘펜 slide 는금속광택 (gloosy sheen) 이보인다 2, 12, 14) 4) Trichrome 염색법 2,18,19) CD 70% alcohol 에 5 분간담근다. CID 70% alcoh 이에노란색을띄울때까지 iodine 을넣은용액에 5 70% alcoh 이에 2 다른 70% alcoh 이에 2 Trichrome 염색액에 분간두는데이때 염색이잘되도록염색액을 3TC 정도따뜻하 게하고, glacial acetic acid 1-2 방울을넣 90% acid alcohol 에 1 분간두고 (J) Acid 를제거하기위해 100% alcohol 에 5 초 정도담근후, 다른 100% alcoh 이에 1 분간담 Xylene 을이용하여청명하게한후 canada balsam 으로봉입한다. 3. 자료의분석 두방법의검출율간의비교와예측도비교는 1986 년 4 월부터 1987 년 2 월사이에원주기독병 McN emar test 를이용하고, 유의수준을 a=0.05 로 원내원환자중장내원충류검사및통상대변검사정하였다. 를의뢰한환자 390 명을대상으로하였다. 2. 검사방법 1 ) 일반채변용기와 Schaudinn S 고정액이들어 있는용기맺가검물채취에관한설명서를대상자 결과 1. 성별, 연령별장내원충류갑염율 성별장내원충류총감염율은남자 233 명중 31

3 m 없명 (13.3%), 여자 157 명중 24 명 (15.2%) 이였으며 (Table 1), 성별에따른각원충류감염율간에는 유의한차이를볼수없었다 (P>0.05). 연령별장내 원충류감염율은 10 세이하군에서와 61 세이상군에 서다른연령군에비해왜소아메바의감염율이높게 나타났고, 1l ~20 셰군에서 3 명의원충감염자중 이질아메바 2 명 (12.5%), 51~60 세군에서 7 명의원 충감염자중대장아메바 5 명 (12.5%) 으로각각의 연령군에서높은양성율을보였으나 (Table 2), 연 령에따른각원충류감염율간에는유의한차이를 볼수없었다 (P>0.05). 2. 검사법간의원충류검출율의비교 390 명의검사건수중 MGL 법을시행한후의 Lugol s 염색법에서 17 건 (4.4%) 의원충이관찰되었 으며, trichrome 염색볍을이용했을때 55 건 (14. 2%) 의원충이검출되었다 (Table 3). 이두방법의 원충류의검출율간에는통계적인유의한차이가있었다 (P<0.05). 4 3. 원충류겁사법의신뢰도비교 MGL 법을실시한후의 Lugol s 염색법과 trichrome 염색법을동시에실시하였을때두검사법 에서어느한방법으로라도원충이검출된경우 60 건중, Lugol s 염색법으로 17 건의원충이검출되어 28% 의감수성을, trichrome 염색법은 55 건으로 92 % 의감수성을나타냈다. 위음성에있어서 Lugol s 염색법 72 %, trichrome 염색법 8% 로 trichrome 염색법의위음성이낮았으며, 음성예측도는 Lugol s 염색법 88%, trichrome 염색법 99% 였다 (Table 4). 4. 검사법에따라관찰된장내원충류 장내원충류중 4 종의원충이검출되었으며, MGL 볍시행후의 Lugol s 염색법으로는 17 건의원 충의포낭형만관찰되었으나, trichrome 염색법으 로는영양형 9 건과포낭형 37 건, 영양형과포낭형 이함께검출된경우는 9 건이었다. 이질아메바는 13 건이관찰되었는데 Lugol s 염색법으로 3 건의포 낭형이, trichrome 염색법으로는 4 건의포낭형과 5 건의영양형, 포낭형과영양형이함께검출된경우는 1 건이었다 (Table 5).. IV. 고 찰 우리나라에서는처음으로장내원충류검사법으 로서고정액사용후 trichrome 염색법과, MGL 법 시행후, Lugol s iodine 염색법을실시하여원충의 검출성적을비교하여보았다. Lugol s 염색법과 trichrome 염색볍 간의원충류 검출율을비교하면 Lugol s 염색법으로 4.4% (390 건중 17 건 ), trichrome 염색법으로 14.2% (390 건중 55 건 ) 가검출되어 trichrome 염색법의검출율이월등히좋은결과였는데이는 MGL 법으로검출이불가능한원충류영양형의검출이가능했기때문이라고볼수있다. 이질아에바의경우, 총 13 건이발견되었는데 Lugol s iodine 염색법으로포낭형만 3 건 -194-

5 냉검출되었고, trichrome 염색법으로는포낭형 4 건, 가 San Francisco Bay 지역주민을대상으로장내 영양형과포낭형이함께검출된경우 1 건, 영양형 원충류검사에 trichrome 염색법을이용하여 97% 의 만발견된경우는 5 건으로이질아메바의영양형겸 감수성을보고한바와같이, trichrome 염색법이원 출성적이좋았다. 충류검사법으로서높은감수성을나타냈다. 또한 검사법의감수성은 Lugol s 염색법은 28%, tri- 위음성은 Lugol s 염색볍 72%, trichrome 염색볍 chrome 염색법은 92% 로이는 Takahashi(1982) 17) 8% 이며, 음성예측도는각각 88%, 99% 로 trichro- Table 6. '-j 같은기간동안원주기독병원내원환자의장내 윤충류와원충류의감염율은표 6 에서와같이윤충 류 5.5%, 원충류 14.2% 로원충류의감염율이더 높은것을알수있었으며, 따라서원충류에대한감 수성이높은검사법을실시하여정확한진단을통한 치료가필요하다하겠다. 표 7 에서와같이 MGL 볍시행후의 Lugol s 염색 법을이용한보고자료들의 70 년대와 80 년대의원충 류감염율을비교하면, 본연구에서 trichrome 염색 법의이용으로원충류의높은검출성적을얻을수 있었으나, trichrome 염색과정은 Lugol s 염색법보 다복잡하고시간도오래걸리며, 또가겸물의고정 상태, 고정된정도빛도말한두께등에따라정상 색조의변화가생길수있어 3, 4) 검사자의원충감별 능력의숙련이요구된다. 이에원충검사법으로서 감수성높은 trichrome 염색법을간편하게수정하고 감별확인이용이하도록하여일반병원이나검사실 에서통상검사로이용되도록하는노력이앞으로 추구되어야할과제라생각한다. V. 결론 Lugol s iodine 염색법과 trichrome 염색법간의 장내원충류검출성척을비교하고자, 1986 년 4 월부 터 1987 년 2 월까지원주기독병원내원환자 390 명 을대상으로두검사법을실시하여두방법간의원 충류검출율, 감수성, 위음성및예측도등을비교 한바그결과는다음과같다. 1. Lugol s iodine 염색법으로는 4.4%, trichrome 염색법으로는 14.2% 의원충류감염율 을보였다. 2. Lugol s 염색법으로 4 종의원충류영양형이 관찰되었으며, 왜소아메바 1. 8%, 대장아에바 1. 0%, 이질아메바 0.8%, 람블편모충 O. 5% 의순으로감염율을나타냈다. 3. Trichrome 염색법으로는 4 종의원충류영 양형과포낭형이검출되었으며, 왜소아에바 6.2%, 대장아메바 3.5%, 이질아메바 3.1%, 람블편모충 1. 3% 의순으로감염율을보였다. 4. 감수성은 Lugol s 염색법은 28 %, trichrome 염색법은 92% 였으며, 위음성은각각 72%, 8% 였다. 또한음성예측도는 Lugol s 염색법 88 %, trichrome 염색볍 99% 였다. 이상의결과로 trichrome 염색법이장내원충류 검사법으로서신뢰도높은방법임을알수있었다. 참고문헌 1.
