We compared Auto RPR Plus and Auto TPIM Plus with previously developed assays. Methods: The precision around the cut-off, linearity, and recovery rate of Auto RPR Plus and Auto TPIM Plus was evaluated using their positive/ negative control materials. The results of these two tests were compared with those of Mediace RPR and Abbott Syphilis TP using 431 remnant serum samples collected from people who underwent medical examinations. Results: The within-run precisions (coefficient of variation, CV values) of negative/positive control materials of Auto RPR Plus, Mediace RPR, Auto TPIM Plus and Abbott Syphilis TP were 15.7/2.3%, 2.4/2.3%, -/2.7%, and 8.5/2.3%, respectively; between-run precisions were 67.7/3.3%, 39.1/3.4%, -/4.%, and 7./1.5%, respectively. Auto RPR Plus showed better precision around the cutoff level (1. U) compared to Mediace RPR ( % vs %). The CVs of Auto TPIM Plus around the cutoff (1. U) were 13.5% at 1.5 U and 6.6% at 12.5 U. Agreement rates between Auto RPR Plus and Mediace RPR and between Auto TPIM Plus and Abbott Syphilis TP were 97.2% and 98.4%, respectively. However, twelve samples showed discrepant results for Auto RPR Plus (-)/Mediace RPR (+) and false-positive Mediace RPR results could not be excluded around the cutoff of 1. U. Conclusions: Auto RPR Plus showing good precision near the cutoff can be used for syphilis screening in health checkups. However, Auto TPIM Plus needs improvement in precision and adjusting the cutoff to be used for syphilis screening. T. pallidum 균체는일반적인세균배양법이 Corresponding author: Eun-Hee Nah Health Promotion Research Institute, Korea Association of Health Promotion, 35 Hwagok-ro, Gangseo-gu, Seoul 7653, Korea Tel: , Fax: , cellonah@hanmail.net Received: March 22, 217 Revision received: January 15, 218 Accepted: February 7, 218 This article is available from 218, Laboratory Medicine Online This is an Open Access article distributed under the terms of the Creative Commons Attribution Non-Commercial License ( which permits unrestricted non-commercial use, distribution, and reproduction in any medium, provided the original work is properly cited. 나염색법으로는분리되지않기때문에매독의진단에혈청학적검사가흔하게이용되고있다 [1]. 매독의혈청학적검사는 venereal disease research laboratory (VDRL), rapid plasma reagin (RPR) 등의비트레포네마검사와매독형광항체흡수검사 (fluorescent treponemal antibody absorption test, FTA-ABS), 트레포네마감작혈구응집시험 (T. pallidum hemagglutination, TPHA) 등의트레포네마검사가있으며이들검사를이용하여정확하고효율적으로매독을진단하는알고리즘들이개발되어왔다 [1-3]. 또한이러한매독혈청검사법중수기법검사는노동력소모가많고결과판독시검사자의주관이개입될소지가있어최근에는면역측정법, 화학발광면역측정법등의원리를이용한자동화검사들이널리쓰이고있다 [2, 4-6]. 본연구는면역혼탁법원리를이용한 RPR자동화검사시약 Auto RPR Plus (WiseMeditech, Anyang, Korea) 및트레포네마라텍스응 eissn

2 집 (Treponema pallidum latex agglutination, TPLA) 자동화검사시약 Auto TPIM Plus (WiseMeditech, Anyang, Korea) 의평가와기존시약들과의비교를그목적으로하였다. 재료및방법 216년 6월부터 8월까지건강검진으로 RPR검사가의뢰된사람의검체중남은혈청의양이 1. ml 이상인 431개의검체를대상으로하였다. 대상자들은모두남성이었고, 연령은 42.6±15.세였다. RPR자동화검사시약인 Auto RPR Plus 및트레포네마라텍스응집자동화검사시약인 Auto TPIM Plus의평가와기존의 Mediace RPR (Sekisui, Tokyo, Japan) 및 Abbott Syphilis TP (Abbott Diagnostics, Tokyo, Japan) 들과비교하였다 (Table 1). 1. 정밀도평가 Auto RPR Plus, Mediace RPR, Auto TPIM Plus, Abbott Syphilis TP시약의정밀도비교를위해각각의음성과양성정도관리물질을이용하여 5일동안하루에 1회검사, 1회검사시 5회반복측정 (5 5 design) 하여검체당총 25개의결과를얻어서검사내정밀도와검사간정밀도를구하였다. 추가적으로, Auto RPR Plus, Mediace RPR, Auto TPIM Plus 각각의음성과양성정도관리물질을 3:1, 1:1, 1:2로혼합하여판정기준치 (cutoff) 인 1. U와가까운세단계의검체 (Level 1 3) 들을준비하였다. 혼합하여준비된세가지물질의예측값 (assigned value) 은 Auto RPR Plus에서 U였으며, Mediace RPR에서 U이었다. Auto TPIM Plus의정밀도평가를위하여음성과양성정도관리물질을 9:1, 6:1, 5:1, 4:1, 3:1로혼합하여판정기준치인 1. U와가까운다섯단계의검체 (Level 1 5) 를준비하였으며그예측값은 U이었다. 정밀도의평가는 Clinical and Laboratory Standards Institute (CLSI) guideline EP15-A3에따라 2회반복측정하여시행하였다 [7]. 2. 직선성평가 Auto RPR Plus와 Auto TPIM Plus 시약의직선성평가를위하여, 건강검진을위해건강검진센터에내원한사람의검체가운데고농도 (H) 의양성검체를저농도 (L) 로단계적으로희석하여다섯가지농도 (L,.75L+.25H,.5L+.5H,.25L+.75H, H) 의시료로만들어직선성을평가하였다. 즉, 준비된시료를두번반복측정하여그평균값을이용하였으며, 그평가는 CLSI guideline EP6-A에근거하여시행하였다 [8]. 3. 기존시약과의비교평가 Auto RPR Plus와 Auto TPIM Plus의기존시약들과의결과비교를위하여 216년 6월 RPR검사가의뢰된사람의검체중남은혈청의양이 1. ml 이상인잔여검체 431개의검체를수집하였다. 수집된검체로 Hitachi 76 (Hitachi, Nakai, Japan) 에서의 Auto RPR Plus와 Mediace RPR 두시약의검사결과를비교하였다. 또한 Hitachi 76에서 Auto TPIM Plus 시약의검사결과와 Architect i2 analyzer (Abbott Diagnostics, Tokyo, Japan) 에서의 Abbott Syphilis TP 시약의결과를비교하였다. 또한, Auto RPR Plus와 Mediace RPR, Abbott Syphilis TP와 Auto TPIM Plus 결과의일치율을계산하고, 불일치하는검체의경우추가적으로 FTA-ABS검사 (Zeus Scientific, Raritan, NJ, USA) 를시행하였다. 또한, 정량적비교평가에있어, Excel 21 (Microsoft Corporation, Redmond, WA, USA) 을이용하여선형회귀분석과 SPSS 2. (SPSS Inc., Chicago, IL, USA) 을이용하여 Bland-Altman difference plot을시행하였다. 본연구의모든과정은기관연구윤리심의위원회의승인을받았다 ( HR-26). 결과 1. 정밀도 Auto RPR Plus의음성정도관리물질의검사내정밀도 ( 변이계수, coefficient of variation, CV) 와검사간정밀도 (CV) 는각각 15.7% Table 1. Mediace RPR의음성정도관리물질의검사내와검사간변이계수는각각 2.4% 와 39.1% 였으며, 양성정도관리물질의검사내와검사간변이계수는각각 2.3% 와 3.4% 였다. Auto TPIM Plus의양성정도관리물질의검사내와검사간변이계수는각각 2.7% 와 4.% 였다. Abbott Syphilis TP의음성정도관리물질의검사내와검사간변이계수는각각 8.5% 와 7.% 였으며, 양성정도관리물질의검사내정밀도와검사간변이계수는 2.3% 와 1.5% 였다. Auto RPR Plus시약의변이계수는예측값.66 U, 1.26 U, 1.65 U 에서각각 19.%, 7.2%, 7.3% 였다 (Table 3). Mediace RPR의변이계수는예측값.53 U, 1.3 U, 1.36 U에서각각 21.1%, 12.2%, 14.3% 로, Auto TPIM Plus의변이계수는 6.27 U, 8.96 U, 1.45 U, U, U의농도에서각각 25.9%, 16.%, 13.5%, 6.6%, 4.1% 로관찰되었다 (Table 3). 2. 직선성및측정가능범위 Auto RPR Plus 시약에서는.1 U로측정되는저농도의내원자검체와 8.7 U로측정되는고농도의내원자검체를순차적으로희석하여측정한결과, 회수율이 91.6% 에서 11.2% 사이로관찰되어직선성및측정가능범위 ( U) 를확인할수있었다 (Fig. 1). Auto TPIM Plus의경우 2. U로측정되는저농도의내원자검체와 27. U로측정되는고농도의내원자검체를순차적으로희석하여검사를시행하였으며, 그회수율이 94.5% 에서 1% 로관찰되어직선성및측정가능범위를확인할수있었다. 3. 비교평가 Auto RPR Plus의결과를 Mediace RPR의결과와비교한결과일 Table 3. Precision of Auto RPR Plus, Mediace RPR and Auto TPIM Plus around cutoff values Auto RPR Plus (WiseMeditech) 치율은 97.2% (419/431) 였으며 (Table 4), Auto TPIM Plus 와 Abbott Syphilis TP 의결과일치율은 98.4% (424/431) 였다 (Table 5). Auto RPR Plus 와 Mediace RPR 이불일치한 12 검체의트레포네마검사 결과는, Auto TPIM Plus 와 Abbott Syphilis TP 에서모두음성결과 를보인경우가 2 검체, 모두양성결과를보인경우가 1 검체였다. 이들불일치한 12 검체중검체량이부족한 1 검체를제외하고 11 개 의검체로 FTA-ABS IgM 과 IgG 검사를시행하였으며, 그결과 11 검 체모두 FTA-ABS IgM 음성, 1 검체는 FTA-ABS IgG 양성결과를 보였다 (Table 6). 연구에서시행한두가지의 RPR 검사의정량적비 교결과는 Fig. 2 와같다. TPIM Plus 양성 /Abbott Syphilis TP음성의결과를보였다. 불일치한 7검체의비트레포네마검사결과는 Auto RPR Plus 및 Mediace RPR의결과는모두음성이었다. 추가로시행한 FTA-ABS 검사에서, FTA-ABS IgM 결과는모두음성이었으며, FTA-ABS IgG 결과는 1검체에서약양성을보이는경우를제외하고모두음성이었다 (Table 6). 고찰 매독은 Treponema pallidum 이원인이되어생기는질병으로, 성접촉에의해전파되며여러장기에서만성적인증상을일으킬수있다 [1, 9, 1]. 매독의진단및치료반응의평가는일반적인배양이나염색법으로는어렵기때문에, 주로혈청학적진단에의존 9

Auto RPR Plus의판정기준치와가장가까운 1.26 U에서의변이계수는 7.2%, Mediace RPR의판정기준치와가장가까운 1.3 U에서의변이계수는 12.2% 였다. 따라서 Auto RPR Plus 에서상대적으로더나은정밀도를보이는것으로추정되었다. Auto TPIM Plus의판정기준치와가장가까운값인 1.45 U에서의변이계수가 13.5% 로관찰되었다. 따라서환자검체에서판정기준치와가까운결과값이측정되면결과해석시주의를요할것으로추정되었다. Auto RPR Plus 와 Auto TPIM Plus 의시약정보에따르면 Auto RPR Plus 의분석가능범위 (analytical measurable range, AMR) 는.5 8. U 이고, Auto TPIM Plus 의 AMR은 U이다 [12, 13]. 본연구에서는 Auto RPR Plus 의경우.1 8. U에서회수율이 9 11% 사이로측정되어, 검사구간내에서의직선성을확인할수있었다. Auto TPIM Plus 의경우역시측정구간 U에서 9 11% 이내의회수율을보여검사구간내의직선성을확인할수있었다. 비트레포네마검사인 Mediace RPR과 Auto RPR Plus 결과의비교에서, 두시약간일치율은 97.2% (419/431) 로관찰되었다. 일치하지않는 12개의검체는모두 Auto RPR Plus ( )/Mediace RPR (+) 의결과를보였으며, 이중 2검체는 Auto TPIM Plus ( )/Abbott Syphilis TP ( ), 1검체는 Auto TPIM Plus (+)/Abbott Syphilis TP (+) 의결 과를나타냈다. Auto TPIM Plus ( )/Abbott Syphilis TP ( ) 의결과를보인 2검체는 FTA-ABS IgM ( )/FTA-ABS IgG ( ) 으로나타나, Park 등 [2] 의연구에서와같이 Mediace RPR에서의위양성결과인것으로판단하였다. Auto TPIM Plus (+)/Abbott Syphilis TP (+) 의결과를보인 1개의검체에서는잔여검체량부족으로인해 FTA- ABS 검사를시행하지못한한개경우를제외하고 9개검체에서모두 FTA-ABS IgM ( )/ FTA-ABS IgG (+) 의결과를보여과거매독균에감염되어치료되었거나잠복매독으로생각되었다 [1, 2]. 이들 9개의 FTA-ABS IgM ( )/ FTA-ABS IgG (+) 인 Auto RPR Plus ( )/ Mediace RPR (+) 의불일치예의해석에서 Auto RPR Plus의위음성가능성을생각해볼수있었다. 비트레포네마검사는 1기및잠복매독에서는민감도가낮기때문에매독의유병률이낮은지역에서는매독의선별검사로트레포네마검사를권장하기도한다. 한편, Mediace RPR (+) 값의위양성도완전히배제할수는없었다. Auto RPR Plus ( )/ Mediace RPR (+) 의결과를보인 12검체들의 Mediace RPR의농도값이 U로 cutoff 1. U 근처의값이었다. Auto RPR Plus와 Mediace RPR 각각의회사에서제공한음성과양성정도관리물질을각각 3:1, 1:1, 1:2로혼합하여판정기준치 (cutoff) 인 1. U와가까운세단계의검체 (Level 1 3) 들을준비하여비교한정밀도평가에서 cutoff 1. U 근처의변이계수가 Auto RPR Plus의 CV는 7.2%, 7.3% 인반면에 Mediace RPR의 CV가 12.2%, 14.3% 로만족스러운정밀도는아니었다. 이전의자동화 RPR검사와수기법 RPR검사의비교에서, 자동화 RPR검사에서양성판정치로 1. U를 91

6 사용할경우수기법보다양성률이높고위양성이빈번하여양성례는반드시트레포네마검사의확인이필요하다고하였다 [14]. 보다정확한판단은정도관리물질이아닌실제임상검체로 cutoff 1. U 근처의값의변이계수를비교하는것이필요하리라생각되었다. Auto TPIM Plus와 Abbott Syphilis TP의결과일치율은 98.4%, 불일치검체는총 7검체였다. 7검체의결과는모두 Auto TPIM Plus (+)/Abbott Syphilis TP ( )/Auto RPR Plus ( )/ Mediace RPR ( ) 로관찰되었다. 추가적으로시행한 FTA-ABS IgM과 FTA-ABS IgG에서 6검체는 FTA-ABS IgM ( )/ FTA-ABS IgG ( ) 의결과를보였으며하나의검체에서만 FTA-ABS IgM ( )/ FTA-ABS IgG (weakly positive) 의결과를나타냈다. 추가검사의결과로미루어보건대, 6검체는 Auto TPIM Plus에서의위양성이, 다른 1검체는 Abbott Syphilis TP의위음성이의심되었다 [2]. TPIM은감염, 자가면역질환, 임신, 예방접종후, 당뇨병또는고연령에서위양성결과를보일수있다 [15]. 또한, Auto TPIM Plus에서의위양성결과로생각된앞의 6개의검체중절반에해당하는 3검체에서판정기준치와매우가까운값 (1, 11, 12 U) 을보여해석에주의를요하였다. 이러한경우, 반복측정해도판정기준치근처에서양성이면다른방법의트레포네마검사로확인하는것이필요하리라생각되었다. 본연구에는몇가지제한점이있다. 첫째, 건강검진을위해내원한사람을대상으로비교연구를진행하여매독에대한임상정보및과거력을알수없다는점이다. 둘째, 검체량및검사접근성의제한으로인해불일치를보이는검체의경우 Western blot을시행하지못하고위음성의가능성이있는 FTA-ABS만을시행한점이다. 셋째, 검체량의한계로인해 RPR 검사에서 prozone현상을확인하지못하였다는제한점도있다. 넷째, 정밀도평가에서제3자회사의정도관리물질이나내원자검체를이용할수없어각시약자회사의정도관리물질을이용하였다는점이다. 판정기준치와가까운값에서의정밀도비교역시동일한검체를이용하지못하였으며판정기준치근처의실제양성검체로반복측정하여그결과양성에서음성으로변화된비율로평가하지못하였다는점역시본연구의제한점이었다. 따라서정밀도의단순비교우위를판단할수없고그추정만가능하였다. 그럼에도불구하고현재까지 Auto RPR Plus와 Auto TPIM Plus 시약의판정기준치와가까운값에서의정밀도연구가많지않기때문에본연구를통해임상적으로중요한매독의혈청학적검사결과해석에도움을줄수있을것이라생각되며, 추후 Auto TPIM Plus의판정기준치에대한추가연구가필요할것으로생각된다. 요약 배경 : 매독의원인균은일반적인배양법으로분리되지않기때문 에, 혈청학적진단이기준방법으로고려되고있다. 본연구에서저자들은매독의새로운혈청검사시약을평가하고자하였다. 방법 : Auto RPR Plus와 Auto TPIM Plus의판정기준치근처농도의정밀도, 직선성, 회수율을양성 / 음성정도관리물질을이용하여평가하였다. 건강검진을시행한 431명의남은혈청을이용하여 Auto RPR Plus 와 Auto TPIM Plus 의결과를 Mediace RPR과 Abbott Syphilis TP의결과와비교하였다. 결과 : Auto RPR Plus, Mediace RPR, Auto TPIM Plus, 그리고 Abbott Syphilis TP의검사내정밀도는음성 / 양성정도관리물질에서각각 15.7/2.3%, 2.4/2.3%, -/2.7%, 7./1.5% 였다. 검사간정밀도는음성 / 양성정도관리물질에서각각 67.7/3.3%, 39.1/3.4%, -/4.%, 8.5/2.3% 였다. Auto RPR Plus는 Mediace RPR과비교하였을때, 판정기준치 (1. U) 부근에서더나은정밀도를보이는것으로추정되었다 (Auto RPR Plus 변이계수, %; Mediace RPR 변이계수, %). Auto TPIM Plus의판정기준치 (1. U) 근처의변이계수는 1.45 U에서 13.5%, U에서 6.6% 였다. 직선성은 Auto RPR Plus의경우 U, 그리고 Auto TPIM Plus의경우 U에서관찰되었다. Auto RPR Plus와 Mediace RPR 간및 Auto TPIM Plus와 Abbott Syphilis TP 간일치율은각각 97.2% 와 98.4% 였다. 결론 : Auto RPR Plus와 Mediace RPR 간, Auto TPIM Plus와 Abbott Syphilis TP 간일치율은우수했다.
