The disease invariably occurs in diabetics, usually with ketoacidosis or immunocompromised patients. Typical initial symptoms of a patient with rhinocerebral mucormycosis are facial pain and swelling, headache, fever, and blood-tinged rhinorrhea. Symptoms rapidly progress to facial or orbital cellulitis, proptosis, and visual loss. Patients often may have infraorbital numbness, corneal anesthesia and facial nerve palsy. Recently, we experienced two cases of rhino-orbito-cerebral mucormycosis with atypical initial presentations. One case is a 54-year-old male, who had suddenly developed left facial palsy and periorbital swelling. The other is a 61-year-old female, who had right periorbital painful swelling with visual loss for 1 day. Herein we report the clinical courses and symptoms of these patients with literature review. Initial symptoms in making early diagnosis of rhino-orbito-cerebral mucormycosis are emphasized. Korean J Otolaryngol 2001;44: KEY WORDSMucormycosis Facial paralysis Blindness. 666

The soft tissue density is noted in left maxillary sinus and nasal cavity. 증 례 2 61세 여자가 내원 하루 전 갑작스럽게 생긴 우측 시력 소 실로 98년 6월 본원 안과에 입원하였으나, 급성 부비동염 내부를 채우고 있었고 좌 상악동의 연부조직은 자연 개구부를 에 의한 안 합병증이 의심되어 본과로 의뢰되었다. 내원 5 통해 비강과 하비갑개까지 침범하고 있었으며 좌측 협부로도 일 전부터 우측 전두부 두통 및 안와주위 동통과 부종이 있 조영 증강되는 미만성의 연부조직 소견을 보였다(Fig. 1). 모 었으며 과거력상 3년 전 당뇨병을 진단받고 인슐린으로 자 균증 의심 하에 내원 2 병일 째부터 amphotericine B를 1일 가 조절을 하는 중으로 가족력상 특이 소견은 없었다. 이학 60 mg씩 총 3.0 g이 되도록 투여하였고 내원 10 병일 째 본 적 소견상 우측 안와 주위에 경도의 발적성 부종이 관찰되 과로 전원되어 12 병일 째 전신마취 하에서 내측 상악동 절제 었으며 그외 안면 부종이나 피부의 괴사는 없었다. 비강 검 술을 시행하였다. 피부 절개 후 좌측 상악동 골막에서 염증성 사에서도 특이 소견은 관찰되지 않았다. 안과적인 검사상 조직과 함께 농양이 보여 국소적으로 배농시킨 후 상악골 절 우측 시력은 소실된 상태였으며, 안구 운동은 중등도의 장 개를 시행하였다. 상악동 내부의 점막은 검은색의 가피로 둘러 애를 보였다. 안압은 24 mmhg였고 안저 검사상 박동을 관 싸여 있었고 암적색의 분비물이 관찰되어 병변 부위와 함께 찰할 수 없었다. 실험실 검사상 혈당량은 249 mg/dl, 소변 한 덩어리로 내측 상악골을 절제하였다. 수술 검체의 병리 조 검사에서 당 1000 mg/dl, ketone 50 mg/dl 였다. 안와 및 직학적 검사상 hematoxylin-eosin(h-e) 염색에서 격막이 뇌 자기공명영상 검사(MRI)상 우측 전(全) 부비동에 조영 없고 불규칙한 균사가 신경을 침범하는 소견을 관찰할 수 있 증강되는 소량의 점막비후가 관찰되었고 우측 안구 주변 및 었다(Fig. 2). 수술 후에도 당뇨 조절과 함께 amphotericine 안구 내부에 조영 증강되는 선상 및 망상의 음영이 관찰되 B를 투여하였다. 술후 2 병일 째 수술 부위가 발적 소견을 보 어 안와 봉와직염을 시사하고 있었다. 부비동개구연합(os- 이고 안면마비는 House-Brackmann grade VI로 더욱 심해 tiomeatal unit) CT상 우측 부비동개구연합 주위를 채우고 져서 수술 부위를 개방하고 wet dressing을 하여 창상을 치료 있는 연부조직과 우측 전부비동에 경한 점막 비후가 관찰되 하였다. 술후 45 병일에 근전도 검사를 실시했으며 이때 자발 었다(Fig. 3). 우측 급성부비동염에 의한 안 합병증 의심 하 활동 전위는 관찰되지 않았고 안면마비의 정도는 호전을 보이 에 진료 당일 우측 내시경하에서 전 부비동 근치술을 시행 지 않았다. 술후 60 병일째 내시경 검사상 좌측 상악동의 내 하였다. 수술 소견에서 중비도의 비점막은 부종 및 용종성 부는 육아조직으로 채워져 있었으며 재발의 소견은 보이지 않 변화를 보이고 있었고 상악동과 사골동에서는 소량의 점액 았으나 안면마비는 grade VI로 변화 없는 상태에서 퇴원하였 성 비루가 관찰되었다. 술후 1 병일 째 우측 안면의 안신경 다. 수술 4개월 후 외래에서 전산화단층촬영을 다시 실시하였 및 상악신경 지배 영역에 이상 감각이 발생하였고 부종이 더 667
