A prophylactic colectomy is recommended for patients with FAP to prevent colorectal cancer. Four surgical strategies are available for patients with FAP: proctocolectomy with permanent ileostomy, colectomy with ileorectal anastomosis, proctocolectomy with Koch s pouch continent ileostomy (Koch), and restorative proctocolectomy with ileal pouch-anal anastomosis (IPAA). Koch and IPAA, which make ileal pouch, have theoretical advantage of the elimination of the risk of colorectal cancer and adenomas and good functional outcome of reduced defecation frequency. However, recent reports have shown frequent development of adenomas and carcinomas in the ileal pouch after Koch or IPAA. We experienced 2 cases of multiple pouch adenomas after pouch surgery in FAP patients. Both patients were treated with endoscopic mucosal resection without complications. We report these 2 cases with a review of literatures. (C) Multiple ulcers due to polypectomy were noted. 34세여자환자가직장건강검진에서철결핍성빈혈이발견되어원인진단을위해내시경검사를시행하였고, 회장낭에다발성폴립이발견되어본원으로전원되었다. 과거력에서가족샘종폴립증으로 18년전에전대장직장절제술및 Koch 배변자제회장창냄술을시행받았으며, 이후환자는내시경감시검사없이지내왔다. 환자는무남독녀였고, 부친이 30대초반에가족샘종폴립증으로수술받은후사망하였다. 이외암을비롯한특이가족력은없었다. 말초혈액검사에서백혈구 9,000/mm 3, 혈색소 11.4 g/dl, 혈소판 296,000/mm 3 이었고, 혈청생화학검사에서 AST 25 IU/L, ALT 23 IU/L, 크레아티닌 0.5 mg/dl, 혈중요소질소 12 mg/dl, 총단백 6.1 g/dl, 알부민 3.6 g/dl, 총빌리루빈 0.7 mg/dl이었으며, 종양표지자검사에서 CEA 0.32 ng/ml이었다. 복부검사에서장음은정상이었고압통이나반동압통은없었으며, 촉지되는종괴나간및비종대의소견은없었다. 입원하여시행한상부위장관내시경에서는이상소견은없었다. 소장조영술에서는회장낭에다수의폴립이발견되었으며, 보다근위부소장에는이상소견이없었다. 회장창냄술부위를통해시행한회장내시경 (ileoscopy) 에서회장낭에 3-15 mm 사이의다양한크기를보이는 50여개의폴립이관찰되었고 (Fig. 1A), 대부분의폴립을점막절제술로제거하였다 (Fig. 1B, C). 절제된폴립들은조직검사결과모두대롱샘종 (tubular adenoma) 의소견을보였다. 환자는내시경점막절제술과관련된특이합병증이없어입원 3일째퇴원하였고, 추후외래에서주기적인회장내시

과거력에서가족샘종폴립증으로 12년전에외부병원에서전대장직장절제술및회장낭- 항문연결술을시행받았으며, 이후내시경감시검사없이지내왔다. 부친이가족샘종폴립증으로 42세무렵에수술받았으나대장암으로사망하였고, 누나와남동생이가족샘종폴립증으로각각 38세와 42세에전대장직장절제술및회장낭-항문연결술을시행받았다. 말초혈액검사에서백혈구 8,100/mm 3, 혈색소 15.

(B) Snare polypectomy was performed after submucosal injection of normal saline. 경감시검사를하며추적관찰하기로하였다. 고 찰 증례 2 38세남자환자가건강검진으로시행한내시경검사에서이상소견이발견되어치료를위해입원하였다. 과거력에서가족샘종폴립증으로 12년전에외부병원에서전대장직장절제술및회장낭- 항문연결술을시행받았으며, 이후내시경감시검사없이지내왔다. 부친이가족샘종폴립증으로 42세무렵에수술받았으나대장암으로사망하였고, 누나와남동생이가족샘종폴립증으로각각 38세와 42세에전대장직장절제술및회장낭-항문연결술을시행받았다. 말초혈액검사에서백혈구 8,100/mm 3, 혈색소 15.1 g/dl, 혈소판 219,000/mm 3 이었고, 혈청생화학검사에서 AST 41 IU/L, ALT 38 IU/L, 크레아티닌 0.7 mg/dl, 혈중요소질소 12 mg/dl, 총단백 7.7 g/dl, 알부민 4.7 g/dl, 총빌리루빈 1.1 mg/dl이었으며, 종양표지자검사에서 CEA 0.73 ng/ml이었다. 복부검사에서장음은정상이었고압통이나반동압통은없었으며, 촉지되는종괴나간및비종대의소견은없었다. 항문을통해회장낭쪽으로시행한회장내시경에서항문연 7 cm 및 4 cm 상방회장낭에각각 1 cm 및 7 mm 크기의융기형폴립이관찰되었고이외에도 1-2 mm 크기의폴립이다수회장낭안에서관찰되었다 (Fig. 2A). 이들중크기가큰 2개의폴립을내시경점막절제술로제거하였으며 (Fig. 2B), 절제된폴립들은조직검사결과모두대롱샘종으로진단되었다. 회장낭보다근위의회장에는폴립이발견되지않았다. 환자는내시경점막절제술과관련된특이합병증없이퇴원하였고, 추후외래에서주기적인회장내시경감시검사를하며추적관찰하기로하였다. 전대장직장절제술후회장낭-항문연결술을하거나 Koch 배변자제회장창냄술을하는술식은직장기능을대신할회장낭을만들어줌으로써, 가족샘종폴립증환자의수술후기능적편이를제공한다는관점에서흔히시행되어왔다. 특히, 전대장직장절제술후회장낭-항문연결술은장루의불편함도없으므로표준술식으로받아들여지고있다. 이러한술식은대장점막을모두제거하는것이므로이론적으로는하부위장관에서샘종또는샘암종의발생가능성을완전히차단한다는장점도있다. 그러나, 이들술식에서만들어지는회장낭에샘종의발생위험도가낮지않음이보고되고있다. 3-8 이전연구들에의하면수술후 4-6년째회장낭샘종의유병률은 13-57% 에이르렀다. 한연구에서는회장낭에발생하는샘종의유병률이나이가 1세증가함에따라 6.6% 증가하며, 추적관찰기간이 1년증가함에따라 20% 증가한다고하였다. 7 다른연구에서는 8년의추적관찰기간동안샘종발견율을 74% (17/23) 로높게보고하였는데, 인디고카민 (indigocarmine) 등을이용한색소내시경이편평하거나크기가작은샘종의발견에도움이되었다. 6 한편, 가족샘종폴립증에대한수술후회장에서의샘종발생에는회장낭자체가중요한역할을한다. 수술후추적관찰에서회장낭과회장낭근위부의회장점막을비교하였을때, 회장낭근위부의회장점막에는회장낭에비해샘종발생이드물기때문이다. 3,4,7 한연구에서는 14.7년을추적관찰하였을때 Koch 배변자제회장창냄술을받은경우회장낭샘종유병률은 50%, 전대장직장절제술및회장낭-항문연결술을받은경우회장낭샘종유병률은 75% 로서, 회장낭에서의샘종발생위험도가수술후 5년, 10년, 20년째각각 13%, 43%, 72% 에이른다고하였다. 반면, 전대장절제술및회장-직장연결술을받은경우말단회장에서의샘종유

4 52 The Korean Journal of Gastroenterology: Vol. 56, No. 1, 2010 병률은 0% 로나타나, 샘종발생에있어회장낭의역할이중요함을보고하였다. 8 이번증례들은회장낭을만드는수술을시행하고각각 12 년및 18년이경과한후회장낭에다발성샘종폴립이진단되어, 시간이경과함에따라발생률이높아진다는기존의서양보고들과일치하였다. 또한회장낭보다근위회장에서는폴립이발견되지않아, 회장낭자체가위험인자라는점에서도기존의보고들과동일하였다. 이번증례의특이점으로가족샘종폴립증환자의회장낭조성수술후회장낭에발생한샘종을내시경절제술로치료한국내첫보고라는점에서의의가있다. 회장낭에샘종이발생하는기전으로는다음과같은가설이제시되고있다. 회장에서는대변의이행속도가빠르지만회장낭에이르게되면대변이정체되어오래머물게되고, 회장낭내용물의성질이변하게된다. 즉, 회장낭에는단쇄지방산이대장의수준까지증가하고, 정상세균총보다는혐기성세균이증가하며, 혐기성세균에의한담즙산의담즙산염탈결합 (deconjugation) 과탈카르복실작용 (decarboxylation) 이증가한다 그결과회장낭에는특히혐기성세균에의해형성되는발암물질인 deoxycholic acid와 lithocholic acid가높은농도로존재하게된다. 10,11 이들이회장점막과오래접촉하면서회장점막은대장점막으로화생성변화를거쳐형성이상 (dysplasia) 이유발된다. 대장점막으로의화생성변화는회장낭점막에서흔히관찰되며, 이를회장낭이새로운직장으로적응하는과정으로설명하기도한다. 12,13 이렇게회장낭내대변정체로인한회장점막의화생성변화가회장낭의샘종발생에중요한역할을하는것으로생각된다. 회장낭에서암종이발생한경우도보고되고있다. 8,14-18 국내에서도가족샘종폴립증으로전대장직장절제술및회장낭- 항문연결술을시행받은환자가 5년만에배변곤란으로내원하여항문연상방 7-10 cm의회장낭에샘암종을진단받은증례와, 19 가족샘종폴립증으로전대장절제술및회장-직장연결술을시행받은환자가 6년뒤직장암을진단받고직장의점막절제술을포함한회장낭-항문연결술을시행받은후, 7년뒤항문출혈로내원하여항문연상방 5 cm의회장낭에샘암종을진단받은증례가보고되었다. 20 회장낭의샘종이암종으로변화하는기전은대장에서와유사하며, 샘종의개수, 샘종의크기, 형성이상의정도, 융모구조의비율등이위험인자이다. 8,14-19 회장낭의샘종은나이가증가할수록, 수술후기간이길수록증가하므로 7 수술후주기적인내시경감시검사가필요한데그기간과간격에대해서는아직일치된의견이없다. 한연구에서는회장낭수술후 6개월, 1년, 2년째내시경을시행하고이후매 2년마다내시경감시를시행하라고제 안하였다. 이들은또한, 크기가크거나 (>1 cm) 고도형성이상을보이면내시경절제술혹은아르곤플라즈마응고술로제거한후매 6개월마다내시경감시를하도록권고하였다. 8 한편, 샘종이 5개이하이고, 저도형성이상을보이면매 3년마다내시경감시를시행하고, 샘종이 5개이상이고중등도이상의형성이상을보이거나융모구조가동반되면내시경절제술혹은아르곤플라즈마응고술을시행한후 6-12개월마다내시경감시를하도록제안한보고도있다. 7 그런데, 감시검사에서샘종이발견된경우이상의제안들과같이크기가크거나조직학적으로진행된경우에한해내시경절제술을시행하고그렇지않은경우에는추적관찰하는것이최선인지에대해서는이견이있을수있다. 회장낭에발생한샘종이샘암종으로발전할위험도가얼마나높은지정확히알려져있지않은상태에서, 회장점막이얇으므로능숙한술기능력이요구되는내시경절제술을일률적으로적용하는것이어려울수있다. 그러나발견된샘종을절제하지않을경우보다잦은빈도로추적검사를시행해야할가능성이있어비용-효과측면에서불리할수도있다. 또한, 최근내시경및주변기기의눈부신발전은치료내시경술기를수월하게해주었다. 따라서, 절제가능한정도의샘종폴립이라면크기나조직학적진행정도에관계없이내시경절제술을시행할수있겠다. 저자들은두증례모두에있어발견된폴립을가능한모두절제하려고시도하였으며, 특별한합병증없이절제할수있었다. 결론으로, 가족샘종폴립증환자에서전대장직장절제술후회장낭-항문연결술또는 Koch 배변자제회장창냄술등의회장낭을만드는술식을시행할경우회장낭에샘종및샘암종발생이증가할수있다는서양증례보고들과일치하는회장낭의다발성샘종 2예에대해국내에서처음으로성공적인내시경절제술을시행하였다. 가족샘종폴립증환자에서회장낭조성수술후에는주기적인내시경감시가반드시필요하다. 참고문헌 1.Bussey HJ, Veale AM, Morson BC.
