Methods: Initial radiographs of the bilateral elbows in 40 baseball players with OCD (group I) were reviewed for the status of physeal closure of the lateral compartment; capitellum, radial head, lateral epicondyle. Forty baseball players with medial epicondylar apophysitis (group II) were enrolled as a control. Relative status of physeal closure of dominant elbow was defined as early, same, and delayed. Bilateral differences of the status of physeal closure were analyzed between groups, and according to the radiographic stages, extent of the lesions and demographic factors in group I. Results: Significant early physeal closures of dominant elbows were identified in group I in capitellum (group I, 55%; group II, 3%), radial head (group I, 53%; group II, 3%), and lateral epicondyle (group I 37%; group II, 5%). In group I, advanced stage and extended lesion showed early lateral compartment physeal closure especially in capitellum and radial head, and players with longer career length and limitation of motion showed early closure. Conclusion: Over the half of the adolescent baseball players with OCD demonstrated early radiocapitellar physeal closures of dominant elbow in initial presentation. Because premature physeal closure contributes to the development of arthritis without appropriate radiocapitellar remodeling, early detection of OCD is essential for prevention of arthritis and successful conservative management. 2 구정회외. 청소년기야구선수의박리성골연골염에서주관절외측구획성장판의조기폐쇄 서론주관절박리성골연골염은청소년기야구선수에서반복적인투구로인해주관절소두연골이연골하골에서분리되는투구장애질환으로, 투구동작의후기거상기와가속기에주관절외측구획에가해지는압박력과전단력이소두연골하골의허혈성변화를유발하여발생하는것으로알려져있다 1,2. 질환이진행되면요소두관절의연골이손상되고관절면의불일치로인해주관절의퇴행성관절염이합병될수있으므로조기발견과치료가중요하다 3. 주관절박리성골연골염의치료방법을결정하기위해서는성장판의상태, 병변의범위와진행정도, 연령, 주관절의운동범위등을고려해야한다. 특히방사선소견상상완골소두성장판의폐쇄정도는보존적치료나수술적치료를선택하는중요한단서가된다 4,5. 소두성장판이이미폐쇄된경우는병변의자연회복가능성이많지않으므로수술적치료를고려할수있는데, 초진시병변이상당히진행되어이미소두성장판이폐쇄된상태로내원하는경우도자주관찰된다. 박리성골연골염환자에서소두관절면의손상은인접하는요골두관절면의생역학적및형태적변화를일으킬수있으며 6,7, 성장판이열려있는소아에서성장판주위관절의손상은성장판의조기폐쇄를유발할수도있으므로 8,9, 박리성골연골염환자에서소두나요골두, 외상과성장판과같이주관절외측구획성장판의전반적인폐쇄정도를알아보는것은질환의자연회복가능성과예후를판정하는데도움이될수있다. 본연구의목적은청소년기야구선수에서주관절박리성골연골염으로진단된환자의초진시단순방사선소견상환측과건측주관절의외측구획의성장판폐쇄정도를비교하고성장판의조기폐쇄를유발하는원인및관련인자에대해알아보는것이다. 연구방법 1. 연구대상 2016년 1월부터 2018년 3월까지초진시단순방사선소견상주관절의박리성골연골염으로진단된청소년기남자야구선수 40명을실험군 (I 군 ) 으로선정하였다. 박리성골연골염병변은모두우세수에서확인되었고, 대상선정시우세수주관절의급성외상이나골절, 내원전주관절수술의병력이있는환자는제외하였다. 평균신장은 162.5±8.4 cm, 평균 체중은 59.9±9.8 kg, 평균체질량지수는 22.58±2.7 kg/m 2 ( 정상 22명, 과체중 10명, 비만 8명 ) 이었다. 평균연령은 12.8±1.0세였으며초등학생이 12명, 중학생이 28명이었다. 우세수는우측이 30명, 좌측이 10명이었고, 포지션별로는투수가 18명 (45%), 포수가 4명 (10%), 내야수가 14명 (35%), 외야수가 4명 (10%) 으로투야수비율은 18:22 이었다. 정식으로야구를시작한기간인야구력은평균 3.2±1.5년이었다. 외측구획성장판의폐쇄정도를비교하기위하여같은기간에내원하여단순방사선소견상박리성골연골염이없고우세수주관절의내상과견인골단판염으로진단된남자소아야구주환자 40명을대조군 (II 군 ) 으로선정하였으며, 평균연령은 12.2±1.0세, 평균신장은 159.8±8.8 cm, 평균체중은 54.3±11 kg, 평균체질량지수는 21.09±3.0 kg/m 2 ( 정상 29명, 과체중 6명, 비만 5명 ), 평균야구력은 2.77±1.2년이었다. 각군에서주관절이학적검사로양측주관절의가동범위를측정하였으며, 환측주관절의가동범위가건측에비해 20 o 이상의제한을보이는경우는 II 군에서는없었고, I 군에서는 11명에서관찰되었다 (Table 1). 2. 단순방사선영상 각군에서초진시촬영한환측과건측주관절의신전전후면, 45 o 굴곡전후면, 30 o 외회전사면단순방사선사진을이용하여외측구획성장판인소두, 요골두, 외상과성장판의폐쇄정도를확인하였다 (Fig. 1). 신전전후면상에서는외상과및요골두성장판, 45 o 굴곡전후면상에서는소두성장판, 30 o 외회전사면상에서는박리성골연골염의병기와병변의범위를판단하는데이용하였다. 환측주관절의외측구획성장판 Table 1. Radiographic stages and locations of lesion in osteochondritis dissecans (group I) Location of the lesions Radiographic stage Localized lateral (n=8) Central (n=14) Extended lateral (n=18) Radiolucent (n=17) Fragmentation (n=18) Free fragment (n=5) 외측형으로 구분하였고, 국소 외측형이 8예, 중앙형이 14예, 광범위 외측형이 18예이었다(Table 2). 모든 단순방사선 영상 의 판독에는 스포츠 의학을 전공한 정형외과 전문의 2명(HLC, KBP)이 참여 하였으며, 전문의 간 판독의 차이가 있는 경우 다수의 의견을 선택하였다. 3명 모두 다른 판독 소견을 보인 경우는 없었다. 3. 자료의 분석과 통계 I 군과 II 군에서 판정된 각각의 외측 구획 성장판의 폐쇄 정도를 각 군 전체 환자의 백분율로 표시하여 양 군 간의 차이를 비교하였고, 박리성 골연골염의 병기와 병변의 범위에 따라 성장판 조기 폐쇄를 보이는 환자의 빈도를 해당 병기와 범위 전체 환자 수의 백분율로 표시하여 차이를 비교하였다. 상기 항목에 대한 통계학적 검증 방법으로는 카이 제곱 검증을 이용하였다. Arrows indicate each lateral compartment physes; lateral epicondyle, capitellum, radial head. 와 성장판 상태가 동일한 환자의 연령, 신장, 체중, 체질량 지수, 야구력의 평균값은 독립 표본 t 검정을 통해 비교하였으 o 며, 투야수 포지션과 20 이상의 가동 범위 제한을 보이는 환자의 수는 카이 제곱 검정을 통해 비교하였다. 통계학적 자료의 분석은 SPSS ver (SPSS Inc., Chicago, IL, USA)을 가 0.05 미만일 때 통계학적 의의가 있는 것으로 판정하 였다. 폐쇄 정도는 건측에 비해 조기(early), 동일(same), 지연(delayed) 으로 판정하였고, 3가지 외측 성장판 중 이미 성장판 유합이 결 과 완전히 일어나 성장판 폐쇄의 선행 여부를 알 수 없는 경우나 외상과의 경우 화골핵이 아직 출현하지 않은 경우는 판정 박리성 골연골염 환자 군(I 군)에서 대조군인 내상과 견인 불가(not available)로 정의하였다. 단순방사선 소견 상 I 군 골단판염 환자군(II 군)에 비해 통계적으로 유의하게 환측 환자 40 명중 29명이 내상과 견인 골단염을 동반하고 있었다. 외측 구획 성장판의 폐쇄 정도가 건측에 비해 선행되어 있었다. 10 I 군에서 단순방사선 소견 상의 병기는 Minami 등 의 분류를 II 군에서는 환측 주관절 외측 성장판 각 부위별로 소두에서 이용하여 투과기, 분리기, 유리기로 구분하였으며 투과기가 1명(1/40, 3%), 요골두에서 1명(1/40, 3%), 외상과에서 2명(2/40, 17예, 분리기가 18예, 유리기가 5예이었다. 병변의 범위는 5%)이 건측에 비해 성장판 폐쇄가 선행된 반면, I 군에서는 11 Kosaka 등 의 분류를 이용하여 국소 외측형, 중앙형, 광범위 182 대한스포츠의학회지 소두가 22명(22/40, 55%, p=0.002), 요골두가 21명(21/40, 53%,

o 가 판정 불가로 판명된 경우는 없었으며 외상과의 경우 I 군에서 1명, II 군에서 2 명이 판정 불가로 판명되었다.

4 구정회 외. 청소년기 야구 선수의 박리성 골연골염에서 주관절 외측 구획 성장판의 조기 폐쇄 p=0.002), 외상과는 15명(15/40, 37%, p=0.003)에서 성장판 폐 연령, 신장, 체중, 체질량 지수, 포지션은 성장판 폐쇄가 선행된 쇄가 선행되어 있었다(Table 3). 환측이 건측보다 외측 구획 환자와 동일한 환자 간에 차이는 없었으나, 외측 구획 성장판 성장판 폐쇄가 지연된 경우는 I 군에서는 없었고 II 군의 외상과 의 폐쇄가 선행된 환자에서 야구력이 길었으며, 주관절 운동 에서 2명이 있었으나 통계적 유의성은 없었다. 소두와 요골두 범위가 20 이상으로 제한된 환자의 수가 많았다(Table 5). o 가 판정 불가로 판명된 경우는 없었으며 외상과의 경우 I 군에서 1명, II 군에서 2 명이 판정 불가로 판명되었다. I 군에서 고 찰 방사선학적 병기 상 투과기의 경우 소두는 5명(5/17, 29%), 요골두 5명(5/17, 29%), 외상과 3명(3/17, 17%)에서 건측에 박리성 골연골염은 요소두 관절염의 발생률이 50%까지도 비해 성장판 폐쇄가 선행되었으나, 양측이 동일한 경우가 많았 다(Fig. 2A). 병기가 진행될수록 소두(p=0.026), 요골두(p= 0.024), 외상과 성장판(p=0.021)의 폐쇄가 선행된 경우는 유의 하게 증가되었는데, 소두와 요골두의 경우 분리기에서는 각각 12명(12/18, 67%), 11명(11/18, 61%)에서(Fig. 2B), 유리기 환자 5명 모두에서(100%) 소두와 요골두, 외상과 성장판의 폐쇄가 선행되어 있었다(Fig. 2C). 병변의 범위와 관련하여 국소 외측 형인 경우는 외측 구획 성장판 폐쇄가 선행된 경우가 없었으며 (Fig. 3A), 중앙형인 경우 소두가 3명(3/14, 21%), 요골두가 4명(4/14, 30%), 상 외상과 3명(4/13, 31%)이 선행되어 있었으 나, 동일한 경우가 더 많았으며(Fig. 3B), 중앙형에서 외측 구획 성장판의 조기 폐쇄를 보이는 경우는 병변이 관절면의 50% 이상을 침범한 경우였다(Fig. 3C). 광범위 외측형인 경우 소두는 18명(18/18, 100%), 요골두는 17명(17/18, 94%), 외상과 는 11명(11/18, 61%)이 선행으로 판명되어 병변의 크기가 광범 위할수록 외측 구획 성장판, 특히 소두(p=0.035)와 요골두 (p=0.037)의 조기 폐쇄가 관찰되었다(Table 4, Fig. 3D). Comparison of lateral compart physeal closure according to demographic factors in group I Variable (n=22) Capitellum Radial head Lateral epicondyle* (n=18) (n=21) (n=19) (n=15) (n=24) Age (yr) Height (cm) Body weight (kg) BMI (kg/m 2 ) Career length (yr) Position 9:13 10: :12 10: :10 9: (pitcher:fielder) No. of LOM (>20 o ) BMI: body mass index, LOM: limitation of motion. *One case is not available; Indicates statistically significant difference. 상소두성장판이열려있는경우는 18명 (17%) 이라고보고하여내원당시이미 83% 의환자가소두성장판이폐쇄되어수술적치료를선택하는경우가많다고하였다. 본연구에서박리성골연골염환자군 40명중환측소두성장판이건측에비해폐쇄가진행된상태로내원한환자는 22명 (55%) 이었다. 이는본연구대상환자의평균연령이 12.8 세로비교적낮고, 환측소두성장판의절대적폐쇄여부를보는것이아니라건측대비상대적폐쇄정도를보는연구이므로그들의연구와는차이가있을수있다. 또한저자들은본연구에서소두뿐만아니라요골두 (21명, 53%) 와외상과 (15명, 37%) 의상대적인성장판폐쇄정도도건측보다상당히진행되어있음을관찰하였다. 소아주관절외측구획에존재하는화골핵은소두, 요골두, 외상과로각각의출현시기는남자의경우 1세, 4 5세, 10세경이며, 성장판폐쇄는 11세경외상과와소두가먼저유합되고 13 16세사이에소두와외상과순서로상완골골간단과유합된다 14. 요골두성장판의폐쇄는다른외측구획성장판보다약간늦은 14 17세에일어난다. 비록본연구가외측구획성장판의폐쇄순서를확인하는것은아니나, 연구자료의검토상박리성골연골염환자의환측주관절을제외하고는전술한순서대로폐쇄가진행되었고요골두성장판이가장늦게까지유지되어있었다. 박리성골연골염에서소두뿐아니라요골두성장판의폐쇄도건측보다빨리지는것은그만큼요골두의재형성능력도떨어진다는것을의미할수있으므로단순방사선에서소두를포함하여요골두나상완골외상과의성장판상태도세심히관찰해야할것으로생각한다. 박리성골연골염환자에서외측구획성장판의조기폐쇄가발생하는원인은불분명하나외측요소두관절면에가해지는반복되는압박력으로인한성장판손상이나박리성골연골염 으로유발된요소두관절면의부적합성 (incongruity) 이관여할것으로생각된다. 투구동작시요소두관절에가해지는압박력은선수체중의 2.4배가량되는것으로알려져있다 5. 또한소두는주로후방종말동맥에서혈액을공급받고골간단으로부터의측부순환이없으므로혈행이취약하며 15, 요골두보다연골의강도가떨어진다는연구도있으므로 16 요골두의반복적압박으로인한소두연골하골의허혈성변화는인접한소두성장판의조기폐쇄를유발할수있다. 소아에서성장판을통한골절이나성장판인접관절의외상은성장판의조기폐쇄를유발할수있는데 8, 특별한급성외상없이만성적으로성장판에가해지는압박력도 Salter Harris 제 5형성장판손상과유사한기전으로성장판의미성숙조기폐쇄를유발할수있다는보고들도있다 9,17. 본연구에서확인된바와같이박리성골연골염환자에서요소두관절에가해지는압박력은요골두성장판의조기폐쇄와도연관될수있다. Wu 등 7 은평균연령 13.8세의소두박리성골연골염환자 86명중 26명 (30%) 에서자기공명영상소견상요골두의골수부종이나낭포성병변, 요골두관절면함요성 (concave contour) 의소실이관찰되었고, 청소년은성인에비해요골두가내재적으로연약하므로요골두의반복적압박으로인해요골두에도병변이발생할수있다고하였다. 결국이러한요골두의압박성병변은소두와유사한기전으로요골두성장판의조기폐쇄에부분적인기여를할수있을것으로생각된다. 본연구에서외상과성장판폐쇄의선행정도는소두나요골두에비해비교적낮은빈도 (37%) 를보였는데, 이는외상과성장판이해부학적으로소두와연결되어있기는하지만직접적인압박을받는부위가아니므로손상의빈도가상대적으로낮은것으로판단된다. 또한대조군인내상과견인골단판염환자 제 36 권제 4 호

Lateral Compartment Physeal Closure of the Elbow in Osteochondritis Dissecans of the Adolescent Baseball Players 군에서도빈도는극히낮지만외측구획성장판의조기폐쇄가관찰되었는데, 이는내상과골단판염의발생으로외측압박력이상대적증가할수있으나대조군의경우요소두관절의적합성이유지되어조기폐쇄를보이는경우는극히드물게나타난것으로생각된다. 소두박리성골연골염병변의병기와침범정도, 병변의위치는요소두관절면의부적합성과관련하여외측구획성장판조기폐쇄의또다른원인이될수있을것으로생각한다. 본연구에서는병기가진행할수록외측구획성장판의폐쇄가빠른것으로나타났으며, 유리기에서는전예에서건측에비해소두, 요골두, 외상과성장판의조기폐쇄가관찰되었다. 또한, 광범위외측형인경우외측구획성장판, 특히소두 (100%) 와요골두 (94%) 의성장판조기폐쇄가매우높은빈도로관찰되었고, 중앙형인경우에도조기폐쇄를보인경우는관절면이 50% 이상을침범한경우이므로병변의크기도성장판의조기폐쇄와관련성이높을것으로보인다. 박리성골연골염병변은일차적으로소두에발생하므로소두성장판의조기폐쇄의원인은압박력으로인한소두자체의혈행장애나성장판의압박손상일수있으나박리성골연골염병변이없는요골두의성장판조기폐쇄는요소두관절면의부적합성으로유발되었을가능성이더클수있다. Matsui 등 18 은전산화단층촬영을이용한연구에서소두박리성골연골염병변범위가커지면요골두의각부위에가해지는스트레스의분포가달라지며, 관절면에가해지는힘의불균형으로인해요소두관절염을유발된다고설명하였다. 또한발달성고관절이형성증이나 Legg-Calve-Perthes 병에서대퇴골두성장판의조기폐쇄가유발될수있듯이 19,20, 소아활막관절에서외상없이유발된관절면의부적합성은성장판의조기폐쇄를유발할수있는원인이될수있을것으로판단된다. 본연구에서병변의범위와관련하여국소외측형 (8명; 소두 0%, 요골두 0%) 과중앙형 (14명; 소두 21%, 요골두 30%) 은광범위외측형에비해성장판조기폐쇄의빈도가높지않은것으로나타났다. 박리성골연골염에서소두외측부병변은중앙형병변보다예후가좋지않은것으로알려져있다 4,18. 특히외측부병변이중앙으로진행하여요소두관절면의 3분의 1 이상을침범하는경우요소두관절염의발생률이높다. 그러나, 본연구에서국소외측형 8명전예에서성장판조기폐쇄는관찰되지않았는데, 이는아직광범위외측형으로진행하지않아관절면의 3분의 2 이상이보존된것에기인한것으로생각할수있다. 그러나, 중앙형 (14명) 이나광범위외측형 (18 명 ) 에비해환자수가상대적으로적고, 병기상투과기가절반 (4명) 을차지하므로추가적인증례검토가필요할것으로생각한다. 박리성골연골염환자에서성장판조기폐쇄와관련된신체적특징의경우본연구에서는연령이나체중, 신장, 체질량지수, 포지션의영향은없는것으로나타났는데, 이는박리성골연골염의일반적인위험인자에대한연구들과유사한결과를보였다 21,22. 특히성장판조기폐쇄를보이는환자의평균연령이약간높은경향은있었으나통계적유의성은없었다. 이는연령이낮은환자에서성장판이열려있는빈도가높아양측성장판폐쇄정도가유사한경우가많을것으로판단할수있으나, 성장판폐쇄는골연령과의관련성이더욱많으므로 23 실제나이인역연령에따른차이는크지않을것으로생각한다. 또한연령이높을수록야구력또한길어진것에기인할수있을것으로생각된다. 본연구에서야구력이긴경우외측구획성장판의폐쇄가빠른것으로나타났는데, 박리성골연골염은역학적연구상스포츠노출기간과관련이있으므로 21,22 야구력이긴경우박리성골연골염의발생빈도가높아지고, 그에따른성장판조기폐쇄의가능성도함께높아질수있을것으로판단된다. 주관절운동제한은박리성골연골염에서흔히동반되며질환의예후와관련이있다 2,12,13. 특히건측대비 20 o 이상의운동제한은수술적치료의판단기준이될수있는데 4, 본연구에서 20 o 이상의주관절운동제한을보이는환자의대부분이외측구획성장판의폐쇄가진행되어있었다. 이는질환의진행으로요소두관절의부적합성이발생하면요골두성장판의조기폐쇄를유발하고, 요골두의재형성능력을감소시켜요소두관절염의발생과이에따른관절운동제한으로이어지는것으로생각된다. 따라서박리성골연골염환자에서요골두성장판의조기폐쇄가관찰되면관절염발생단계로진행하고있는것으로간주하여보다적극적인치료가필요할것으로생각한다. 외측구획성장판의조기폐쇄는장기적으로주관절외반변형을유발할가능성이있다. 그러나본연구의 40명의박리성골연골염환자중 29명 (72%) 에서내상과성장판의견인골단염을동반하고있으므로내측성장판의성장장애의발생도배제할수없어외반주발생여부와관련하여대조군을포함하여건측주관절과의장기적인비교추시가필요할것으로판단된다. 본연구는대상환자수가적으며, 야구를하지않는일반청소년대조군이없는단점이있다. 저자들은청소년야구선수에서흔히관찰되며비교적양호한예후를보이는내상과견인골단판염환자들을대조군으로설정하였으나, 대조군 186 대한스포츠의학회지

8 구정회외. 청소년기야구선수의박리성골연골염에서주관절외측구획성장판의조기폐쇄 역시정상주관절이아닌내측골단판의이상을가진선수인점을감안하면비록외측구획성장판이정상이라하더라도성장판폐쇄와관련된오차의유발가능성을완전히배제하기는어려울것으로생각된다. 또한본연구는성장판상태의판독과관련하여관찰자간의오차분석이없고, 전산화단층촬영이나자기공명영상을이용한성장판폐쇄정도의정량적분석을하지않았으며, 질환의진단이후치료방법이상이하여진단당시의성장판영상만을비교하였으므로주기적추시를통한건측및환측각성장판의실제폐쇄진행과정의차이를측정하지못한아쉬운점이있다. 결론적으로청소년기야구선수에서발생한골연골염환자는요소두관절면의손상이없는대조군에비해초진당시환측주관절의외측구획성장판의상대적인조기폐쇄가소두와요골두에서 50% 이상관찰되고, 조기폐쇄는병변의병기와범위, 야구력, 주관절운동범위와관련될수있다. 이러한성장판의조기폐쇄는요소두관절의재형성능력을저하시켜관절염발생으로진행할수있으므로질환의조기진단이중요하며, 진단시소두를포함한요골두나외상과성장판의상태도함께관찰하는것이치료방법선택이나예후판정에중요할것으로생각한다.
