In addition, updated anti-emetic guidelines that include the aprepitant regimen have been published by NCCN and ASCO. However there is scarce clinical data in Korea. The prospective study was performed to evaluate the prevention of high dose cisplatin induced nausea and vomiting in all patients who started high-dose cisplatin-based chemotherapy at our hospital. We checked the nausea severity and vomiting episodes by calling patients within 4 to 5 days after chemotherapy. The retrospective study was performed to compare the prevention of CINV in solid tumor patients who switched their anti-emesis regimen from the standard regimen to the aprepitant regimen. In aprepitant regimen, aprepitant was added to the same anti-emetic regimen used during previous cycles. We checked the nausea, vomiting grades and adverse events in electronic medical records (EMR). In prospective study, 195 patients were included in the analysis. 88.2% of patients achieved a complete response (no emesis and no rescue therapy). In retrospective study, 54 patients were reviewed. With aprepitant regimen, nausea and vomiting grades were improved in 22 patients (40.7%) and in 9 patients (16.7%), respectively. Compared with standard regimen, addition of aprepitant provided superior prevention against CINV in Korean patients receiving highly emetogenic cisplatin-based chemotherapy. Moreover, aprepitant significantly prevented CINV in patients who received the standard regimen to prevent CINV in previous chemotherapy cycles. Key words - aprepitant, NK1-receptor antagonist, antiemetic Chemotherapy-induced nausea and vomiting (CINV) 은항암화학요법시동반되는가장흔한부작용중의하나로환자에게신체적, 심리적인고통을줄뿐아니라대사과정및영양상태를악화시키고항암화학요법시행을어렵게만들어치유의기회를박탈하기도한다. 1-3) CINV를예방하기위해 1980년대이전에는주로 phenothiazine계제제가사용되었고, 그후 antihistamines, anticholinergics, corticosteroids, dopamine antagonist 등이사용되었다. 1990년대초반에는 5- Correspondence to : 손기호삼성서울병원약제부서울시강남구일원동 Tel: , Fax: bluesea.sohn@samsung.com hydroxytryptamine 3 (5-HT 3 ) antagonist 의효과가확인되어 CINV 발생비율이상당부분감소하였다. 4,5) 하지만 5-HT 3 antagonist 투여에도불구하고고용량 cisplatin을투여받는환자의약 50% 는여전히 CINV를경험한다. 6-8) 최근새로운기전의항구토제인 neurokinin-1 (NK 1 ) receptor antagonist가개발되어관심이모아지고있다. 9) 이약물은지연성구토에관여한다고알려진 substance P가 NK 1 receptor에결합하는것을저해하여항구토효과를나타낸다. 10) 2003년경구 aprepitant (Emend, Merck and Co, USA) 가처음으로 FDA 승인을받았고, 다른 NK 1 receptor antagonist가 CINV 예방효과를검증하는과정에있다. 11,12) 기존 5-HT 3 antagonist와 corticosteroid를병용하는 2제요법 (Standard regimen) 에비해 aprepitant를추가한 3제요법 (Aprepitant regimen) 에서 CINV 예방효과가더높게나타나 17

Pharm., Vol. 20, No. 1, 2010 는것으로보고되고있으며, 이를바탕으로하여 2006년 American Society of Clinical Oncology (ASCO) 와 2007년 National Comprehensive Cancer Network (NCCN) 에서는 Aprepitant regimen을표준치료법으로인정하였다. 13,14) 우리나라에서는 2007년 4월부터경구 aprepitant가발매되었고현재대부분의 3차병원에서 CINV 예방에 aprepitant를투여하고있다. 삼성서울병원에서는 2008년 4월부터심한구토를유발하는항암화학요법에서 CINV 예방을위한표준치료법으로 aprepitant regimen을적용하였다. 하지만국내환자를대상으로한 aprepitant의효과및안전성에대한연구는아직충분하지않다. 본연구에서는본원에서항암치료를받는고형암환자를대상으로 aprepitant의효과와부작용을확인하여기존 5-HT 3 antagonist와비교하고, 앞으로의 CINV 예방에대한선택방향을제시하고자하였다. 연구방법 Aprepitant regimen의효과연구대상환자의선정및제외기준삼성서울병원통원치료센터에서 2008년 4월부터 9월까지 6개월동안고용량 cisplatin ( 50 mg/m 2 ) 을포함하는복합화학요법을시행받은고형암환자중 CINV 예방을위해항암치료첫주기부터 aprepitant를투여받은 18세이상의환자를대상으로하였다. 대상환자중항암치료를받기전 24시간이내에구토를경험했거나다른항구토약물을복용하고있는환자, 복부또는골반에방사선치료를받는환자와뇌에종양이있는환자는제외하였다. 15) Aprepitant 대사에영향을줄수있는 CYP3A4 효소를강력하게유도또는억제하는약물 (eg. phenytoin, carbamazepine, itraconazole) 16-18) 을복용하는경우, 연구를진행하는도중복약이행을하지않은경우, 항암치료 1주기진행과정에서다른병원으로전원했거나치료를포기하는등의이유로항구토제에대한효과를확인할수없는경우도제외하였다. 항구토제투여및부작용확인항암화학요법제 1일에서제 4일까지 aprepitant regimen ( 제 1일 aprepitant 125 mg, 5-HT 3 antagonist, dexamethasone 10 mg 정맥투여, 제 2일과제 3일 aprepitant 80 mg 경구복용, 제 2일부터제 4일까지 dexamethasone 8 mg 경구복용 ) 을투여하였다. 제 1일에투여된약물은삼성서울병원통원치료센터에서관리하였고제 2일부터투여하는약물은환자스스로복용하거나보호자에의해투여되었다. Cisplatin 투여후 4~5일이지났을때전화설문으로복약이행도와 CINV 정도및발생한부작용을확인하였다. 제 2일부터제 4일까지 aprepitant와 dexamethasone의복용여부를환자또는보호자에게확인하였다. 오심의정도를평가하기위해 11 point verbal rating scale (11-VRS, 오심의정도를 0에서 10까지의숫자를이용하여표현, 숫자 0은 오심이전혀없었음., 숫자 10은 오심이견딜수없을정도로심함. 을의미함.) 을사용하였다. 19,20) 분석방법 Aprepitant regimen의유효성은 complete response (CR) 를나타내는비율로평가하였다. Cisplatin 투여후 5일동안오심을전혀경험하지않아 11-VRS score를 0으로평가한 no nausea에해당하는환자와, 0에서 4까지의숫자를선택한 no significant nausea에해당하는환자를확인하였다. 성별, CINV history, 멀미여부, 음주력등의 CINV 위험요인 4,5) 에따른 CR율을비교하였으며분석방법은 chi-square test를이용하였다. 유의수준은 p < 0.05로하였다. Aprepitant regimen 의안전성은발생한부작용의빈도로분석하였으며, 항암치료첫주기에서발생한부작용중 3% 이상의환자가경험한부작용을확인하였다. Standard regimen과 Aprepitant regimen의비교연구대상환자의선정및제외기준삼성서울병원통원치료센터에서 2008년 4월부터 9월까지 6개월동안고용량 cisplatin ( 50 mg/m 2 ) 을포함하는복합화학요법을시행받은 18세이상의고형암환자중 CINV 예방을위해 standard regimen ( 제 1일 5-HT 3 antagonist, dexamethasone 20 mg 정맥투여, 제 2일부터제 5일또는제 6일까지 5-HT 3 antagonist 1정경구복용, 제 2일부터제 5일까지 dexamethasone 8~16 mg 경구복용 ) 을투여하다가 2008 년 4월 aprepitant 처방이가능해지면서두번째주기이상의항암치료진행중에 CINV 예방을위해 aprepitant regimen(( 제 1일 aprepitant 125 mg, 5-HT 3 antagonist, dexamethasone 10 mg 정맥투여, 제 2일과제 3일 aprepitant 80 mg 경구복용, 제 2일부터제 4일까지 dexamethasone 8 mg 경구복용 ) 을투여한환자를대상으로하였다. 대상환자중항암치료를받기전 24시간이내에구토를경험했거나다른항구토약물을복용하고있는환자, 복부또는골반에방사선치료를받는환자와뇌에종양이있는환자는제외하였다. 15) Aprepitant 대사에영향을줄수있는 CYP3A4 효소를강력하게유도또는억제하는약물 (eg. phenytoin, carbamazepine, itraconazole) 16-18) 을복용하는환자도제외하였다. 유효성및안전성비교전자의무기록검토를통해오심, 구토및부작용의발생과정도를확인하였다. 전자의무기록은항암치료전외래진료시작성된것으로환자가항암치료후에경험했던부작용의정도가 Common terminology criteria for adverse events (v3.0) 에근거하여기록되어있다. 오심, 구토 grade는항암치료주기마다확인하였고, 다른부작용은 Standard regimen을

3 Aprepitant Regimen 의고용량 Cisplatin 유발오심및구토예방효과 19 사용한첫번째주기와 Aprepitant regimen을사용한첫번째주기에서발생한것을확인하였다. 분석방법분석방법은 paired-test의일종인 generalized estimating equation (GEE) 으로하였고, 유의수준은 p < 0.05로하였다. Aprepitant regimen의안전성은 McNemar's test로분석하여 regimen 변경전과후에유의하게차이가나는부작용이있는지확인하였다. 연구결과 Aprepitant regimen의효과연구대상환자군의특성대상환자는총 205명이었으며, 이중오심및구토발생정도가확인되지않은환자또는항구토제를제대로복용하지않은환자를제외하고최종 191명이평가대상이되었다. 총 686 주기의항암치료가진행되었고환자당평균 3.6주기의항암치료를받았다. 남성이여성보다많았고, 50% 이상은비소세포성폐암또는위암환자였다 (Table 1). 유효성 1주기에서 6주기까지항암치료를진행했으며평균 CR율은 88.2% 였다 (Table 2). Chi-square test분석결과, CINV의위험요인중성별 (p<0.0001) 과음주력 (p = ) 에따른 CR율이통계적으로유의한차이를보였다 (Table 3). 안전성항암치료 1주기에서 82.2% 의환자가한가지이상의부작용을경험하였고, 가장빈번하게발생한부작용은변비 (20.4%) 와피로 (16.2%) 였다 (Table 4). Standard regimen에서가장많이사용된 5-HT 3 antagonist는 granisetron (Kytril, Roche) 이었다 (Table 5). 유효성의비교기존 standard regimen을사용했을때의오심및구토 grade와 aprepitant regimen으로전환한후의오심및구토 grade의변화를보면, 대부분의환자에서 grade가낮아지거나유지되었다. 이런변화는 GEE로분석했을때통계적으로유의한차이를보였다 (Fig. 1). 오심과구토 grade가호전된경우는각각 40.7%, 16.7% 였고, 유지된경우는각각 55.6%, 75.9% 였다. Standard regimen에비해 aprepitant regimen으로전환했을때 CR율과 no nausea를나타내는비율이모두증가하였다 (Fig. 2). 안전성의비교 Standard regimen과 aprepitant regimen을사용한경우모두에서가장많이발생한부작용은피로였다. Ondansetron과 dexamethasone을병용하는 standard 군과 aprepitant를추가투여하는군에서의 CR을확인하였고그결과 standard 군의 CR율은 52.3% 로 aprepitant 군의 72.7% 와통계적으로유의한차이를보였다. 이와유사한연구가 2006년 European society for medical oncology에서진행되었다. 11) 이연구역시 70 mg/m 2 이상의 cisplatin을투여받은환자를대상으로하였으며 5-HT 3 antagonist로 ondansetron을 찰

Cisplatin 투여후 5일간의 CR율은 ondansetron 투여군에서 63.1%, aprepitant 투 여군에서 74.0% 로통계적으로유의하게높았다. 본연구는 50 mg/m 2 이상의 cisplatin을투여받은한국인고형암환자를대상으로하였으며, CINV 예방을위해 aprepitant regimen을사용한경우 CR율이 88.2% 로나타났다. 투여된 cisplatin 용량을 50 mg/m 2 이상으로설정한것은 ASCO와 NCCN의 antiemesis guideline에서심한구토를유발하는고용량 cisplatin의기준과동일하게하기위한것이었으나앞에서소개된연구와는그기준이다르다. 본연구는대조군없이진행된관찰적연구이며, cisplatin 투여 4~5일후전화설문을통해항구토제의효과를확인했다는한계를가지고있다. 또한본연구에서는급성 CINV 와지연성 CINV에대한효과를구분하여평가하지는않았으나, aprepitant가지연성 CINV의예방에도효과가있을것이라고생각되기때문에 18,21) 지연성 CINV에대한효과를구분하여평가하는추가연구가필요할것으로사료된다. 본연구와같이한개체안에서 CINV 예방 regimen을변경했을때의효과를비교한자가대조연구는많지않다 년 Osorio-Sanchez 등의연구 22) 에서 26명의환자를대상으로항암치료 1주기에서는 standard regimen을, 2주기이상에서는 aprepitant regimen을투여한경우효과를비교하였고, 오심과구토의 grade 평균이 regimen 변경시통계적으로유의하게낮아지는것을확인하였다. 2006년 Karin Oechsle 23) 는 standard regimen의투여에도불구하고 CINV를경험하는 34 명의환자에게 aprepitant regimen을투여했을때의효과를확인하였다. 이러한환자군에서 aprepitant regimen으로전환한경우, 항암제투여 4일후오심을경험한환자는 24명 (71%) 에서 4명 (12%) 으로, 구토를경험한환자는 26명 (77%) 에서 0명으로감소하였다. 본연구에서항구토 regimen 변경후구토 grade의호전을보인환자는 9명 (16.7%) 에불과하였는데, 이는기존 standard regimen을사용할때부터이미구토를경험하지않았던환자가많았기때문으로생각된다. 경제적측면에서기존 standard regimen과 aprepitant regimen은유사했으며, 복약이행도는 aprepitant regimen 사용

Aprepitant는 5-HT 3 antagonist에비해특히지연성 CINV 에높은효과를보일것으로예상되기때문에, dexamethasone에의한부작용을경험하는환자에서는 5-HT 3 antagonist와 aprepitant 투여만으로도 CINV가효과적으로예방될수있는지확인해볼필요가있다. Steroid의병용투여없이 CINV를예방할수있다면면역억제, 혈당상승등의부작용에의해 aprepitant regimen의사용이제한된환자에서의 CINV 예방에도움을줄수있을것이다. 결 본연구에서는통원치료센터에서고용량 cisplatin을투여받는환자를대상으로 aprepitant regimen 투여시효과및안전성을확인함과동시에기존 standard regimen에서 aprepitant regimen으로전환한경우의자가대조군에서의 aprepitant 효과및부작용변화를확인하였다. 연구결과 aprepitant regimen을투여받은환자의 88.2% 가 CR을보여 CINV 예방에어느정도의효과를나타내는지확인할수있었다. Aprepitant regimen의경우특히심하게나타나는부작용은없었다. Standard regimen을투여하다가 aprepitant regimen으로투여를변경한환자에서오심과구토 grade가호전된경우는각각 40.7%, 16.7% 였고, 발생한부작용은통계적으로유의한차이가없었다. 이상과같은연구결과는한국인고형암환자에서도 aprepitant regimen이 CINV 조절에효과적으로사용될수있다는것을의미한다.
