1 Clinical Features and Prognostic Factors Influencing Long-term Survival in pt2 Gallbladder Carcinoma Patients Purpose: The prognosis of gallbladder carcinoma is unfavorable, and the depth of invasion, the T-factor, is the most important prognostic factor. T2 tumors are not easily diagnosed preoperatively, and they have unpredictable behavior. In this study, we reviewed the clinicopathologic features of pt2 gallbladder carcinoma and identified prognostic factors. Methods: We enrolled 64 pt2 gallbladder carcinoma patients who underwent surgery in Kyungpook National University Hospital between January 1992 and July The clinicopathologic features were retrospectively reviewed, and univariate and multivariate analyses were performed using the Kaplan-Meyer method and Cox regression hazard model to identify factors influencing long-term survival. Results: Twenty-one patients (32.8%) were asymptomatic, with seven tumors (10.9%) found incidentally. The gallbladder body was the most commonly involved site (45.3%, 29/64). Although CA19-9 was not sensitive, it was helpful for predicting recurrence. The 5-year diseasespecific survival rate in R0 resection was 75.9%, whereas no patients in the R1 or R2 resection groups survived more than 18 months.änone of the clinicopathologic factors was prognostic. The unfavorable prognostic effect associated with lymph node involvement highlights the importance of complete lymph node dissection. Partial liver resection favorably affected survival, but not to a statistically significant degree. Conclusion: Absence of symptoms and specific imaging findings in patients with considerable pt2 gallbladder carcinoma warrants high disease suspicion, and every effort should be taken to achieve R0 resection with extensive lymph node dissection, since it may improve long-term survival in pt2 carcinoma patients.

R0 절제수술을 받은 58명을 대상으로 장기생존율을 비교한 결 과 성별, 응급수술을 받은 환자군과 계획수술을 받은 환자군 담낭암은 특이한 증상이 없는 경우가 많아서 진단이 늦게 되며 주변에 위치한 간, 십이지장, 간 문맥 등의 중요한 기관 으로 쉽게 침투하기 때문에 담낭암의 예후는 불량하다. 암의 침윤깊이가 담낭의 점막에 국한된 pt1a 또는 고유근층까지 침범한 pt1b 담낭암은 담낭 절제술로도 높은 장기 생존율 이 보고되고 있다. pt1 담낭암은 담낭암을 시사하는 영상학 적 소견이 없는 경우가 많으며 담석증, 담낭 용종과 같은 양 성 질환으로 수술한 후 우연히 발견되는 경우가 많다. 이에 비해 pt2 담낭암은 흔히 비특이적 증상을 수반하며 영상학 적으로 담낭벽의 비후, 크기가 큰 담낭용종을 나타내는 경우 가 많아서 수술 전에 담낭암에 대한 의증을 가지고 수술 하 는 경우가 많다. 이 연구에서는 전체 64예의 pt2 담낭암 환 자중 67.2%(43/64)가 증상을 나타내었으며 환자가 상복부 동통을 호소할 때는 증상의 지속기간이 우상복부 동통보다 더 오랫동안 (10.7 Vs 4.2 weeks p=0.039) 지속되었다. 또한 증상이 없이 우연히 발견된 경우가 32.8%(21/64)이 었는데 이는 담낭암이 진단당시에 진행된 암이 많이 발견되 는 것과 관련이 있다.@@ pt2 담낭암은 수술 전에 영상학적으로 T3 담낭암으로부 터 비교적 쉽게 감별할 수는 있으나 T1 담낭암으로부터 감 별은 어렵다. 이 연구에서는 64예의 담낭암중 수술 전에 영 상학적으로 담낭암에 대한 의증을 가진 경우는 85.9%(55/64)이었다. 한편 영상학적으로 담낭암을 의심하 지 못했던 경우는 14.1%(9/64)예로 수술의 적응증은 급성 담낭염 7예, 담석증 1예, 이었고 다른 수술중 우연히 발견된 간의 장기 생존율은 다르지 않았으며 담낭 축농증이 동반된 경우와 동반되지 않은 경우에도 환자의 장기 생존율은 다르 지 않았다. 이외에 담낭암의 유형, 담낭암의 위치, 조직학적 분화도, 임파절 전이, 신경주위 침윤의 유무, 수술전 혈장 CA19-9 수치, 임파절 곽청술, 간 상부 절제술등은 환자의 장기 생존율에 유의한 영향을 미치지 않았다. 임파절 전이 유무가@장기생존률에 미치는 영향을 알기 위해 임파절 곽청 술을 시행 후 병리학적으로 전이 유무가 확인된 42명의 환 자 중에서 임파절 전이가 없는 29예와 임파절 전이가 있는 13예의 생존율을 비교한 결과 5년 생존율은 각각 76.3%와 81.5%로 유의한 차이가 없었다. 또한 상기한 인자들로 Cox 회귀모형을 이용하여 시행한 다변량 분석에서도 유의한 예 후인자는 없었다(Table 3).
