Clinical characteristics of meningioma do not confirm to those of extracranial meningioma, hence they can present diagnostic challenges. We have experienced a case of primary ethmoid meningioma. The patient was treated by a left lateral rhinotomy with total removal of the tumor. Histopathologic examination confirmed the diagnosis of meningioma. follow-up after 15 months revealed no evidence of recurrence. () KEY WORDS:Meningioma Ethmoid sinus. 서 론 수막종은원발성두개내종양의약 15% 를차지하며 1) 대부분중추신경계에서기원한다. 이중약 20% 에서두개외로의진행을보이며안와, 두개골외판, 측두골, 중이, 비강, 부비동, 인두주위강에서발견된다. 2) 그러나일차적으로두개외에서기원한것은매우드물다. 3) 또한두개외에서발현되는수막종은국소적인증상만을나타내기때문에국소부위의종물로오인하고수술에임할수있다. 저자들은좌측사골동에서발생한일차성두개외수막종 1예를경험하였으며이를외비절제술을통하여제거하였기에보고하는바이다. 증 례 65세여자환자가 1개월전부터발생한좌측비강폐색을주소로내원하였다. 내원당시환자는후비루, 농성비루, 후각감퇴가동반되어있었다. 비내시경검사상좌측 논문접수일 :2008 년 1 월 3 일 / 심사완료일 :2008 년 5 월 9 일교신저자 : 김현준, 경기도수원시영통구원천동산 5 아주대학교의과대학이비인후과학교실전화 :(031) 전송 :(031) ENTKHJ@ajou.ac.kr 중비갑개와비중격사이로출혈성경향을보이는부드러운표면을가지는종물이관찰되었으며뒤로는비인두를채우고있었다 (Fig. 1). 외래에서실시한전산화단층촬영에서종괴는좌측비강전체를가득채우고후비공을지나비인두까지확대된양상을보이고있었으며불규칙하게조영증강이되었고두개내로의침범은없었다 (Fig. 2). 자기공명영상촬영에서좌측사골동에서확장되어나온 cm 크기의종물이 T1 강조영상에서저신호강도로, T2 강조영상에서고신호강도로보였으며두개내와는연결이없었다 (Fig. 3). 외래에서시행한종물생검시출혈이심하였으며, 지혈에수일간의패킹 (packing) 이필요하였다. 종물에대한조직학적결과는부신경절종 (paraganglioma) 이의심되어전신마취하에수술을계획하였다. 시야확보가어려웠고환자의나이가많고출혈을우려하여외비절제술 (lateral rhinotomy) 를통한절제술이시행되었다 (Fig. 4). 수술소견상종물은좌측비강상부에서부터기원하고있었으며, 수술시야에서중앙선을넘지않았고, 심한출혈소견을보였다. 종괴는 en bloc 으로제거하였으며출혈부위는전기소작으로지혈하였다. 종물의타부위침습여부를확인하기위하여추가로상악동골, 안와벽점막, 비중격점막, 하비갑개에서조직검사를시행하였 668

xial contrast enhanced image () and coronal contrast enhanced image () show heterogeneously enhanced tumor (arrows) at the left nasal cavity. 으나종물의침습은발견되지않았다.

술후두개내에원발성수막종이존재할가능성을배제하기위해시행한뇌전산화단층촬영에서는특이사항이발견되지않았다. 환자는술후 7일째퇴원하였으며, 별다른합병증이나재발없이 15개월간외래추적관찰중이다. 고찰 수막종은원발성으로두개내에서발생하는가장흔한종양중하나이다. 수막종은두개외로진행할수있으며이런경우안와, 두개골외판, 측두골, 중이, 비강, 부비동, 인두주 위강에서발견된다.

Preoperative enhanced PNS computed tomography. xial contrast enhanced image () shows an expansile soft tissue mass filling the left nasal cavity and nasophaynx. xial contrast enhanced image () and coronal contrast enhanced image () show heterogeneously enhanced tumor (arrows) at the left nasal cavity. 으나종물의침습은발견되지않았다. 조직소견상원형및난원형의종양세포들이섬유혈관조직으로둘러싸여있었고소양돌이모양의소엽을이루고있었으며호산성의균질하고풍부한세포질을가지고있었다 (Fig. 5). 면역조직화학적검사에서 vimentin, 상피막항원 (epithelial membrane antigen, EM) 에양성소견을보였으며 Pancytokeratin 에는음성을보여수막종으로진단하였다. 술후두개내에원발성수막종이존재할가능성을배제하기위해시행한뇌전산화단층촬영에서는특이사항이발견되지않았다. 환자는술후 7일째퇴원하였으며, 별다른합병증이나재발없이 15개월간외래추적관찰중이다. 고찰 수막종은원발성으로두개내에서발생하는가장흔한종양중하나이다. 수막종은두개외로진행할수있으며이런경우안와, 두개골외판, 측두골, 중이, 비강, 부비동, 인두주 위강에서발견된다. 비강및부비동으로침범할수있는수막종의두개내부위는후구, 전두개와, 전두하부위, 후각영역, 접형골능선등이있다. 4) 그러나원발성두개외수막종은전체수막종의 1~2% 를차지하며매우드물다. 이런두개외수막종을 Hoye 등은 4개로그룹화하였다 (Table 1). 5) 두개내수막종이두개외로진행된 Group 가가장흔하나본증례에서는방사선학적으로두개내이상소견이나두개와연결소견이없었고임상적으로도신경학적인이상소견이없었으므로 Group C에속하는일차성수막종으로사료된다. 수막종의기원에대한다음과같은 4가지가설들이있다. 첫째로는이영양성의수막조직구 (heterotrophic meningocyte) 가태아시기에중앙구조가닫힐때전위됨으로써기원한다는설과, 둘째로는두개주위나중추신경의신경주위집 (perineural sheath) 을따라있는수막조직구로부터기원한다는설, 셋째로는신경집세포 (schwann cell) 가수막조직구로의분화과정에서기원한다는설, 넷째로는다능성간엽세포 (multipotential mesenchymal cell) 에서기원한 669

Tumor exposures by lateral rhinotomy () and gross specimen shows a hemorrhagic irregular mass which is measured cm (). 다는설이다. 6-8) 본증례에서는수막종이단지비부비동에만존재하였고중추신경이나두개공 (skull foramina) 과연결이없었기때문에본증례의경우에는기원이세번째또는네번째설로설명될수있으리라사료된다. 수막종의임상적양상은비특이적이고다양하다. 일반적 670 으로비강및부비동종물에의한증상보다는두개내종물에관련된증상이더많은것으로되어있으나 9) 본예에서는두개내종물이없었으므로두개내종물에의한증상은보이지않았고비폐색과후각감퇴등이관찰되었다. 진단은방사선학적으로전산화단층촬영과뇌자기공명영상이중요한단서를제공하나, 확진은조직검사를통해서

T1 강조영상에서는등신호강도또는약간의저신호강도를보이며 T2 강조영상에서는종양의혈관분포상태와석회화에따라다양한신호강도를보인다. 10,11) 특히 T1 강조에서조영제에의한증강으로경막과부착되어있는징후인경막꼬리현상이잘관찰되나본증례에서는특별히관찰되지않았다. 조직검사는진단을위하여필요하며면역화학적염색은필수적이다. 수막종은면역조직화학적검사상 vimentin 에항상양성반응을보이며, 상피막항원 (EM) 에대해서는약 95% 이상에서양성을보인다. 12) 조직학적분류로는수막종은수막내피성 (meningotheliomatous), 섬유모세포성 (fibroblastic), 전환성 (transitional), 혈관종성 (angioblastic), 육종성 (sarcomatous) 의주요다섯가지아형이있다. 수막내피성, 섬유모세포성, 전환성, 혈관종성은재발과공격적인성장의위험성이적으며이중전환성과수막내피성이빈도가많은것으로되어있다. 육종성은전체수막종의 5% 미만으로재발과침습의위험성이가장높다. 13) 본증례에서는면역조직화학적검사에서 vimentin, 상피막항원 (epithelial membrane antigen, EM) 에양성소견을보였으며 Pancytokeratin 에는음성을보여수막종으로진단하였고조직소견상원형및난원형의종양세포들이섬유혈관조직으로둘러싸여있었으며종양세포의외형 이섬유모세포와유사하여섬유모세포수막종으로최종진단하였다. 감별해야할질환으로는상피성암종이나후각신경초종 (olfactory neuroblastoma), 골육종, 연골육종, 혈관종, 림프종등이있으며이들은뇌자기공명영상에서어느정도감별이가능할수있으나결과조직검사를통한확진이필요하다. 14) 치료는수술적제거가일차적인방법이며그외에보조적방사선치료와항암제치료를포함한다. 완전절제후의예후는좋은편이다. 15) 술후재발률은약 15% 정도이며, 재발기간은평균 5년정도로보고되고있다. 12) 가장흔한재발은불완전한종괴제거에있다. 14) 종양제거후두개저에결손부위가발생하는경우두개골막피판, 건막, 측두골근육및근막등을통해막아주어야비강으로부터의상행감염과뇌척수액비루을예방해야한다. 저자들은좌측사골동에서발생한일차성두개외수막종 1예를외비절제술을통하여제거하였기에문헌고찰과함께보고하는바이다. 중심단어 : 수막종 사골동.
