Extended-spectrum-β-lactamase (ESBL) producing K. pneumoniae infection was associated with a significantly longer duration of hospital stay and greater hospital charges. The purpose of this study is to investigate the antibiotic resistant patterns and the DN fingerprint types of extended-spectrum β-lactamase (ESBL) producing K. pneumoniae. 223 K. pneumoniae strains were collected from three general hospitals with more than 500 beds in Busan, Korea from September 2004 to October The minimum inhibitory concentration (MIC) of antibiotics was measured using the Gram negative susceptibility (GNS) cards of VITEK (Vitek system, Hazelwood Inc., MO). Random amplified polymorphic DN method was used to detect DN fingerprint of the organisms. Of the 226 K. pneumoniae isolates 65 ESBL-producing K. pneumoniae strains were detected by the Vitek system and confirmed by the double-disk synergy test. ll the 65 K. pneumoniae strains were resistant cefazolin, cefepime, ceftriaxone and aztreonam, and 83.0% of the organisms were resistant to ampicillin/sulbactam, 66.1% to tobramycin, 67.6% to piperacillin/tazobactam, 61.5% to ciprofloxacin, and 47.6% to trimethoprim/sulfamethoxazole and 43.0% to gentamicin. The RPD patterns were distincted as 10 types by three random 10-mer primers (208, 272, 277). mong ten type patterns, three types (Ic, IIb, IIIe) were remarkably represented at patient of internal department, nerve surgery department, general surgery department, and neonatal room. These results indicate that RPD can be useful for DN of strains typing in the epidemiological investigations. Therefore more investigation are needed in order to prevent the ESBL type-producing K. pneumoniae from spreading resistance to oxyimino cephalosphorin antibiotics. Key Words: Klebsiella pneumoniae, Extended-spectrum-β-lactamase (ESBL), Random amplified polymorphic DN (RPD) 서 Escherichia coli와 Klebsiella pneumoniae는장내세균속 (family Enterobacteriaceae) 에속하는그람음성간균이며인체장내정상상재균이다 (bbott, 1999). K. pneumoniae는병원감염의중요원인균중하나로요로, 창상및혈액등에서분리되고있다. 또한, 병원감염폐렴의흔한원인균으로알려져있다 (Quintiliani et al., 1999). 그람음성간균치료에임상적효능이있는첫 penicillin계항균제인 ampicillin이소개된이 * 논문접수 : 2006 년 8 월 1 일수정재접수 : 2006 년 9 월 15 일 교신저자 : 김윤태, ( 우 ) 부산광역시금정구부곡 3 동 9 번지, 부산가톨릭대학교보건과학대학임상병리학과 Tel: , Fax: ytkim@cup.ac.kr 론 후, β-lactam 항균제는이들균종에의한감염증의치료에광범위하게사용되어왔다 (Quintiliani et al., 1999). mpicillin이임상에서사용된후 β-lactamase 생성에의해서이항균제에대한내성을획득한균주가등장하였다 (Livermore, 1995). 이러한내성세균의감염증치료에병원에서는 Extended spectrum β-lactam 항생제를흔히사용하는데이항생제를 oxyimino cephalosporin이라불리며, 제 3세대 cephalosporin 항생제인 ceftazidime, cefotaxime, ceftriaxone 등이여기에속한다 (Jacoby, 1997). 그런데 1980년대초반에 extended-spectrum β- lactam제까지분해하는효소인 extended-spectrum β-lactamase (ESBL) 를생성하는균주가처음보고되었다 (Livermore, 1995). 그후세계여러나라에서이러한내성세균이보고되고있고, 그빈도는계속증가하고있다 (rlet et al., 1990). Extendedspectrum β-lactamase (ESBL) 는주로 E. coli와 K. pneumoniae 가생성하며이들효소를생성하는유전자에따라 TEM형,

2 SHV형, CTX-M형등으로불린다. 또한이들효소를 encode 하는유전자는 plasmid에매개되므로다른균주로의내성전달이용이하다 (Du Bois et al., 1995). 그렇기때문에 ESBL 을생성하는균주들은병원내감염의주요원인이되고있다. Extended-spectrum β-lactamase (ESBL) 는 Penicillin, 협범위 cephalosporin 뿐아니라제 3세대 cephalosporin과 monobactam도가수분해할수있다. 그러나 cephamycin과 carbapenem에는활성이없으며, clavulanic acid에의해서활성이억제되는특성이있다 (Jacoby, 1994; Bauernfiend et al., 1989). 국내에서는 1990년대초부터이효소를생성하는그람음성세균의분리가보고되었으며, 그빈도는 2000년대들어서현저하게증가하고있는것으로알려졌다 (Lee et al., 2001). Random amplified polymorphic DN (RPD) PCR 방법은균의 DN 염기서열에대한특별한정보없이 1개의 arbitrary primer를이용하여중합효소연쇄반응을실시하여그산물을관찰함으로써균의 DN 다형성을관찰할수있는법으로, 과정이간단하고신속하여균의분류와역학조사에널리이 용되기시작하였다 (Robert et al., 1995). RPD 분석은일반세균뿐아니라, 진균인 Candida species의동정과역학적분류에유용하다고보고되었다 (Robert et al., 1995). Eshwar 등은그람음성간균인 Pseudomonas aeruginosa에감염된 cystic fibrosis 환자의역학적조사에이용하여그유용성을증명하였다 (Eshwar, 1996). 따라서 RPD 분석은여러가지세균에의한감염증에서동일균의전파를알아내는분자역학적조사에도많이이용되어왔다. 이에본연구에서는부산의종 합병원에서분리된 ESBL 생성 K. pneumoniae 균주를대상으로항균제내성현황을조사하고, 그내성유전자의형별 Table 1. ntimicrobial agents and resistance breakpoint used in this study ntimicrobial agents mikacin mpicillin Resistance breakpoint, µg/ml 64 mpicillin/sulbactam /16 ztreonam Cefazolin Cefepime Cefoxitin Ceftriaxone Ciprofloxacin Gentamicin Imipenem Piperacillin/tazobactam 128/4 Tobramycin 16 Trimethoprim sulfamethoxazole: SXT 4/76 을분류하고자하였다. 또한이들균주들이원내감염을일으키는주요원인인지를조사하기위하여 DN를추출한후, RPD 분석을실시하여같은균종내에서동일균주인지를밝힘으로써 ESBL 생성균주의원내감염에대한역학적조사에이용하고자하였다. 1. Bacterial strain 재료및방법 2004년 9월부터 2005년 12월까지부산시내 500병상이상의 3개종합병원임상검체에서분리된 223개의 K. pneumoniae 균주로부터 ESBL 생성 K. pneumoniae 65 균주 (병원: 52주, B병원 : 7주, C병원 : 6주 ) 를실험대상으로하였다. K. pneumoniae는 VITEK (Vitek system, Hazelwood Inc., MO) 의 GNI card에시험균을접종하여생화학적시험을하고동정하였다. 2. Minimum inhibitory concentration (MIC) measurement by VITEK system Disk 확산법에의한항생제감수성시험결과 ESBL 균주로추정되는 K. pneumoniae를 MacConkey agar에서 18시간배양하여멸균된증류수에부유시켜 5,000 rpm에서 3회원심분리하여세척시킨다음, 멸균된 0.45% NaCl 1.8 ml에현탁하여 McFarland No. 0.5로조정하였다. 이균액 200 µl를다시멸균된 0.45% NaCl 1.8 ml에가하여항균제최소억제농도자동측정기인 VITEK의 GNS card에접종하여 MIC를측정하였다. 측정된항균제는 14종으로 Table 1과같았고, 각약제에대한최소억제농도는 NCCLS (National Committee for Clinical Laboratory Standards) 의기준에따랐다 (NCCLS, 2000). Double disk synergy test McFarland No. 0.5 농도에맞춘신선한균부유액을멸균된면봉으로적셔미리만든 3 mm 두께의 Mueller-Hinton (MH) 평판배지에고르게도말하였다. 이평판의중앙에 ticarcillin/clavulanate (75/10 µg, Becton Dickinson, US) disk를얹여놓고그주변 1.5 cm 간격에 cefotaxime (30 µg), ceftazidime (30 µg), ceftriaxone (30 µg) disk를올려놓아 37 에서 18시간배양한후에억제대의형태를관찰하여상승효과 (synergism) 를판정하였다 (Fig. 1). 4. ESBL gene detection by PCR (polymerase chain reaction) ESBL 효소를생성하는유전형을분석하기위하여중합효소연쇄반응을실시하였다. 시험균주를 TSB 액체배지 (tryptic soy broth) 에접종하고 37 에서 18시간배양하였으며, 10,000 rpm에서 10분간원심분리한후침전물을 ccprep plasmid extraction kit (Bioneer Seoul Korea) 를이용하여 plasmid DN 를추출하여 template로사용하였다. 사용한 primer는 Table 2에나타내었고시약은 ccupower PCR Premix kit (BIO- NEER Co. Ltd. Seoul, Korea) 를사용하였다. Primer (10 pmol) 를각각 1 µl, template DN 8 µl, 3차증류수 10 µl를가하여총량이 20 µl 되게맞춘후사용하였다. PCR 기기는 Genemp PCR system 2400 (Perkin-Elmer Centus Corp., Norwalk, Ct., US) 를사용하였다. TEM type의 PCR 반응조건은 predenaturation 94 에서 5분간시행후 denaturation 94 에서 30초, annealing 45 에서 90초, extension 72 에서 1분으로하여 30 cycle을중합하였다. Final extension은 72 에서 3분으로하였다. Random amplified polymorphic DN (RPD) PCR 원내감염의원인이동일균주에의한것인지를분석하기위하여 Random amplified polymorphic DN (RPD) PCR을실시하였다. 균주를 TSB에접종하여 37 에서 8~10시간배양하였다. 균배양액 1.5 ml을 microcentrifuge tube에옮긴후, 12,000 rpm에서 5분동안원심분리하여상층액은버리고하층펠렛만남겼다. 여기에 STET buffer [Sucrose 8 g (8%), Triton X ml (5%), EDT g (50 mm), Tris g (50 mm)] 300 µl를넣고 vortex 하였다. 여기에다시 10 mg/ ml의 lysozyme을 20 µl 넣고상온에서 1분간방치하였다. 95 로맞춘 hot block에꽂아서 5분간 heating 하였다. 12,000 rpm에서 10분간원심분리후상층액을새 tube에옮겼다. 여기에 99.9% ethanol 600 µl를넣고 -70 에서 30분이상정치시켰다. 12,000 rpm에서 10분간원심분리후상층액은버렸다. 70% ethanol 300 µl를가한후가볍게 inverting 한후다시 12,000 rpm에서 5분간원심후상층액을완전히제거하였다. 이때피펫으로조심스럽게제거한후 vacuum이나 dryer 로건조하였다. 30~60 µl TE buffer 또는멸균증류수에녹인후, PCR template로사용하였다. RPD PCR의 primer은 NPS Unit list of standard primers ( 중에서 No. 208, No. 272, No. 277 세가지를사용하였다 (Eshwar et al., 1996). PCR에사용된시약은 ccupower PCR Premix Kit (BIO- NEER Co. Ltd. Seoul, Korea) 를사용하였다. PCR product 제조는 premix kit에 primer 1 µl와 template DN 6 µl를넣고멸

4 균증류수 13 µl를넣어서총량이 20 µl되게하였다. 사용한 primer는 Table 3에나타내었고 primer 농도는 40 pmol로맞추어사용하였다. PCR 조건은 94 에서 5분, 36 에서 5분, 72 에서 5분간 4 cycles한후 94 에서 1분, 36 에서 1분, 72 에서 2분간 30 cycles 하고나서 72 에서 10분간 1 cycle로마무리하였다. 1.5% agarosegel을사용하여 100 V에서 90분동안전기영동한후 EtBr로 20분동안염색한후 U.V에서 DN 분획을확인하였다. DN major band의개수와위치를파악하여분석자료를 Excel에기록한후 MVSP (Multi variate Statistical Package) 의프로그램을이용하여 Dendrogram을그려서분류를하였다 ( mvsp.html). 결과 1. ntibiotic susceptibility test by VITEK system 항생제감수성시험결과 100% 내성으로나타난항균제는 ampicillin, cafazolin, cefepime, ceftriaxone, aztreonam이었고, amicillin/sulbactam은 79.6%, tobramycin은 83.0%, piperacillin/ tazobactam은 67.6%, ciplofloxacin은 61.5%, trimethoprim/sulfamethoxazole과 gentamicin은각각 43.0%, amikacin은 23.0%, cefoxitin은 16.9% 의내성을나타내었으며, imipenem은모든균주가감수성을보였다 (Fig. 2). 2. ESBL 생성균주확인 VITEK (Vitek system, Hazelwood Inc., MO) 에서 ESBL 생성균주로판정된 K. pneumoniae 65주중에서 65주 (100%) 모두 ceftazidime 디스크의 double disk synergy 시험에서양성이확인되었다. 그러나 cefotaxime, ceftriaxone 디스크의 double disk synergy 시험에서는 65균주중 2균주를제외한 63주 (96.9%) 가양성으로확인되었다. 3. bla TEM, bla SHV, bla CTX-M gene detected by polymerase chain reaction ESBL 생성 K. pneumoniae 65균주를대상으로 bla TEM, bla SHV, bla CTX-M 유전자검출을위한 PCR을시행한결과 bla TEM 유전자는 42주 (64.6%), bla SHV 유전자는 46주 (70.7%), bla CTX-M Table 4. 5 유전자는 29주 (44.6%) 가양성반응을보였다. 65균주중 bla TEM 만가지고있는균주는 4주 (6.1%), bla SHV 만가지고있는균주는 7주 (10.7%), bla CTX-M 만가지고있는균주는 8주 (12.3%), 그리고, bla TEM, bla SHV 두가지 유전자를가지고있는균주는 22주 (33.8%), bla TEM, bla CTX-M 두가지유전자를가지고있는균주는 4주 (6.1%), bla SHV, bla CTX-M 두가지유전자를가지고있는균주는 5주 (7.6%) 였고, bla TEM, bla SHV, bla CTX-M 유전자세가지를모두가지고 Fig. 4. mplified RPD products with primer 208 from 65 clinical isolates of K. pneumoniae. 6 있는균주는 12주 (18.4%) 였다. 그리고세가지유전자를모두가지고있지않은균주는 3주 (4.6%) 였다 (Table 4, Fig. 3). 4. Random amplified polymorphic DN (RPD) analysis RPD 양상을분석한결과여러가지희미한 band를제외하고뚜렷하게나타난것으로 dendrogram을그린결과크게세가지형으로분류되었고 band의양상과위치를고려하여다시 10가지 band pattern으로세분하였다. DN 분획이 500 bp, 600 bp, 700 bp, 1050 bp, 10 Kbp에서 5개의 band가뚜렷한것을 Type Ia라고하였다. Type Ib는 500 bp, 600 bp 900 bp, 1050 bp에서 4개의 band가나타난것을, Type Ic는 500 bp, 600 bp에서 2개의 band가나타난것을, Type IIa는 600 bp, 700 bp, 900 bp, 1050 bp에서 4개의 band가나타난것을, Type IIb는 600 bp, 1050 bp, 10 Kbp에서 3개의 band가나타난것을, Type IIIa는 450 bp, 550 bp에서 2개의 band가나타난것을, Type IIIb는 450 bp, 550 bp 600 bp, 700 bp, 900 bp, 10 Kbp에서 6개의 band가나타난것을, Type IIIc는 900 bp, 1050 bp에서 2개의 band가나타난것을, Type IIId는 450 bp, 550 bp, 700 bp, 900 bp, 1050 bp에서 5개의 band가나타난것을, Type IIIe는 500 bp, 600 bp, 900 bp, 1050 bp에서 4개의 band가나타난것으로분류하여 dendrogram으로나타내었다 (Fig. 4, 5). The lane numbers are the sample numbers. 그람음성간균은임상검체에서흔히분리되는균속인데, 이세균에의한감염증치료에는흔히 β-lactam제가이용되어왔다 (Du Bois et al., 1995). 그러나근래중증감염을일으키는 K. pneumoniae 중에 ESBL 생성균이증가하고있으며, ESBL 유전자는 plasmid에의해다른균종으로전달될수있고원내감염을일으킬수있기때문에심각한문제가되고있다 (Dumarche et al., 2002). 송등 (Song et al., 2000) 의전국적인조사에의하면우리나라의 ESBL 생성 E. coli와 K. pneumoniae도중환자실환자에서분리율이높았고 (Son et al., 1997), 다른항균제에대한내성률이 ESBL 비생성균주에비해높다고하였다 (Epai, 1998). 본연구에서도항생제감수성시험결과 ampicillin, cafazolin, cefepime, ceftriaxone, aztreonam이 100% 내성으로나타났고 cefoxitin의내성률은 13.0% 로가장낮았으며, imipenem은모든균주가 100% 의감수성을나타냄으로써 ESBL 생성균주의전형적인양상을나타내었다 (Fig. 2). 본연구에서는아직까지 carbapenem 제제인 imipenem에내성을보이는 ESBL 생성균주가없다는것을확인할수있었다. 병원의규모가크거나대학병원일수록 ESBL 생성균주의분리율이높다는보고가있다 (Hong et al., 2004). 우리나라의경우그비율은균종, 병원및분리연도에따라달랐지만, 대체로 E. coli는 9.1%, K. pneumoniae는 29.2% 로적지않게보고되었다 (Hong et al., 2004). 본연구에서는 500병상정도의병원환자에서분리된 K. pneumoniae 중 ESBL 생성균주의비율이 28.7% 로전국평균과거의비슷하였다. Double disk synergy 시험은 TEM 및 SHV형 ESBL 이 clavulanic acid에의하여활성이억제되는특성을이용한것으로, 이효소를생성하는세균의검출에널리사용되고있다 (Coudron et al., 1997). 본연구에서는 ceftazidime cefotaxime, ceftriaxone 디스크와 clavulanic acid 디스크를사용하여 double disk synergy 시험을시행하였다. 위의 3세대 cephalosporin 디스크중한가지디스크에서라도 synergy 현상이

7 나타난균주를양성으로판정하였다. 실험한결과 Vitek system에서 ESBL로분리된 65균주중 cefotaxime, ceftriaxone 디스크의 double disk synergy 시험에서는 63주 (96.9%) 가양성으로나타났다. 그렇지만 ceftazidime 디스크의 double disk synergy 시험에서는 65균주모두가양성으로나타났다. 따라서 Vitek system에서의 ESBL 생성균주검출과 ceftazidime 디스크의 double disk synergy 시험과의일치율이 100% 를나타내어 ceftazidime 디스크를사용하여 double disk synergy 시험을하는것이가장효과적이었다. TEM형은 1960년대초에그리스에서 Temoniera 라는패혈증환자의검체에서분리된 K. pneumoniae로부터처음으로검출되어보고되었다. 그래서이환자의이름을따서 TEM형이라고불려졌다 (Bradford, 2001). SHV형은 sulphydryl variable의약자로표시한것인데주로 K. pneumoniae의 chromosome에존재하지만 plasmid에매개되어자주 E. coli에서발견된다. SHV형은 1980년대에독일에서분리된 Klebsiella ozaenae 균주에서처음검출되었다 (Bradford, 2001). CTX-M 형 ESBL은 1989년독일의임상에서발견되는 E. coli 균주에서검출되기시작했는데이효소는다른항생제에비해 cefotaxime에강한내성을나타내었다. 이 cefotaxime 항생제의이름을따서 CTX-M이라고명명하였다 (Bonnet, 2004). CTX-M-β-lactamases는 TEM, SHV형 ESBL과는다소연관이적은것으로보고되고있으며, TEM, SHV형 ESBL gene과는 40% 이하의 homology를갖고있는것으로보고되었다 (Moland et al., 2003). 현재전세계적으로 ESBL 생성균주가많이나타나고있는데주로 TEM형, SHV형, CTX-M형등이주류를이룬다. 국내에서도 ESBL 생성균주가상당히많이검출되고있는데분리되는 ESBL 생성장내세균중에는 SHV형을생성하는세균이많았다는보고가있다 (Lee et al., 2003). 본연구에서도 TEM형 42주, SHV형이 46주로나타나서 SHV형이많이증가함을나타냈다. 그리고 CTX-M형이 29주가검출되었는데이는우리나라에서도 CTX-M형이많이나타나고있다는것을알수있었다. 또한 TEM, SHV, CTX-M형의한가지 gene을생성하는균주는적게나타났고, 두가지또는세가지의 gene을동시에보유하는균주가많이확인되었다. 특히, TEM과 SHV를동시에가지는균주가 65균주중에서 22주 (33.8%) 로가장많았고그다음으로 TEM, SHV, CTX-M 세가지를동시에가지고있는균주가 12주 (18.5%) 로확인되었다 (Table 4). 이상과같은결과에서최근에는한가지의내성유전자를가지고있는균주보다여러가지내성유전자를복합적으로가진균주가많이출현한다는것을알수있었다. RPD PCR 방법은최근균종을좀더정확하고신속하게동정하는것이외에도, 동일균의전파를알아내는분자역학적조사에도이용되었다 (Loudon et al., 1993). 본연구는실험에사용된균주를대상으로간단한방법으로 DN를추출한후, RPD 분석을실시함으로써, 원내감염을일으키는동일균주의검출을위한역학적조사에이용하고자하였다. 연구에사용한균주들은대학병원이아닌 3 개의일반종합병원에서분리한것들이었는데다양한검체에서균이분리되었다. 각병원별로분석하였고검체를수거한병동별로도분석하였다. 동일한병원의같은병동에서분리한균주들의 band pattern이유사하게분류할수가있어서같은병동내의감염이동일균주에서비롯되었음을추정할수있었다. 하지만같은병동내에서분리한균주라도 band pattern이다른것도다수나타났다. 또한, 이들 band pattern 을검출된 ESBL 유전형들과도비교분석해보았으나이들의연관성은찾아낼수가없었다. 총 10가지의 band type 중에서 1-3균주정도를포함한일부 type들은 band의양상이정확하게분석되지않아서분류의한형태로보기는어려울것같았다. 이는 PCR이제대로되지않았거나전기영동이정확하게실행되지않아서나타난현상같았다. 10가지분류중에서 Ic, IIb, IIIe형의 3그룹에포함되는균주가전체 65균주중에서 48균주였는데 90% 이상의높은유사성을보였다. 가장많이나타난 IIIe형에는 30균주가포함되었는데이는 병원의내과중환자실 12주와신경외과 12주로나타났다. IIb형에는 7균주가포함되었는데 B병원의소아과 4주, 병원소아과 3주로나타났다. Ic형은모두 11균주가포함되었는데 C병원외과에서분리한 5균주가같은형태였다. 이같은분석결과는 ESBL을생성하는균주가원내감염으로서같은병동에입원한환자들에게전파되고있다는것을증명해주는것같다. 앞으로항생제에내성을가지는균주의확산을방지하기위해서 ESBL gene을생성하는균주의유전적특징과항생제내성기전을연구하고또한이러한균주의원내감염확산을방지하기위하여꾸준한조사와감시가필요할것으로생각된다.
