The characterization of the SHV ESBLs producing Klebsiella pneumoniae strains present in clinical isolates is time-consuming processes. We describe here in the development of a novel system, which consists of a real time PCR. We found 11 K. pneumoniae strains to be presumptive strains ESBLs producers by clinical and laboratory standards institute (CLSI) guidelines. The double disk synergy test showed 8 ESBL positive and conventional PCR showed 10 SHV ESBL positive, which were K. pneumoniae strains isolates. By real time PCR analysis, SHV gene in 11 of 11 strains were identified. When sequencing analysis was compared with real time PCR, both analysis were presented 99% similarity. In this study, we used a rapid, sensitive, and specific real-time PCR (RT-PCR) method for detection of the assay SHV ESBL producing K. pneumoniae strains in clinical isolates. Klebsiella pneumoniae 는 2~5% 정도가원내감염의원인균이며, 특히하부호흡계와비뇨기계에감염을일으키는것으로잘알려져있다 (Gazouli 등, 1997; Kil 등, 1997). K. pneumoniae가 cephalosporins계에다제내성을나타내는것은 1980년대초에보고되었으며 (Bingen 등, 1993), 그추세가점점더 153

ESBL 유전형검사에일반 PCR 방법들이널리이용되고있기는하지만 PCR을수행하는시간이많이소요되고증폭후전기영동으로확인하고사진촬영하는작업이필요하다 (Karch 등, 1989). ESBL 생성 K. pneumoniae 의 typing에있어, 분석력뛰어나고빠르며, 재현성을나타내는방법이필요하다 (Monica 등, 2004). Real-time PCR 방법을이용한 ESBL 생성 K. pneumoniae의유전자형검출방법은 ESBL 생성 K. pneumoniae 임상분리균주의검출과분석의기술적인어려움이나, 시간적인소모되는문제점을보완하고, 빠르고, 감수성이좋은방법으로잘알려져있다 (Fortin 등, 2001; Mckillip 등, 2000). 본연구에서는 K. pneumoniae균주를대상으로 ESBL 검출법에대해서 double disk synergy(dds) 시험법과, real-time PCR법의비교및염기서열을규명하였다. II. 재료및방법 1. 재료 2009년 7월부터 10월까지대전에있는 S 대학병원에서 VITEK(Biomerieux, Marcy-1 Etoile, France) 로동정된 ESBL 생성 K. pneumoniae 11 균주를대상으로실험하였다 (Table 1). 2. Double disk synergy 시험세균부유액을 Mueller-Hinton 우무 (BBL Microbiology systems, Cockeysville, MD., USA) 에고르게접종한후, 배지의중앙에는 ticarcillin-clavulanic acid(tim) 또는 cefotaxime-clavulanic acid(ctx/cla) 를, 주위에는 30 μg의 cefotaxime(ctx), ceftazidime(caz) 및 aztreonam (ATM) 디스크를놓되, 중앙과주변디스크의가장자리간격은 2 cm가되도록하였다. 접종배지는 37 항온기에 18시간배양후결과를판독하였다. CTX, CAZ 및 ATM 디스크에의한억제대가 TIM 또는 CTX/CLA 디스크쪽으로상승작용에의해억제대의크기가증가하였을 Table 1. List of SHV ESBL producing K. pneumoniae isolates used in this study AST Genotype Strains FOX DDST PCR RT-PCR A1 A2 A3 S A4 S A5 A6 S A7 A8 A9 A10 A11 AST, antibiotic susceptibility test; FOX, cefoxitin; DDST, double disk synergy test; R, resistance; S, sensitive 때양성으로판정하였다 (Table 1). 3. 균주로부터의 DNA 분리 K. pneumoniae 11 균주를 QIAamp DNA Mini Kit (QIAGEN) 를이용하여 DNA를추출하였다. 순수분리된 11개균주를 LB media(bd, Detroit, USA) 를이용하여 37 shaking incubator에서하루동안진탕배양하였다. 배양액을 13,000 g에서 3분간원심후상등액은버리고침전물을이용하여키트가제시한방법으로 DNA를추출하였다. 4. PCR 을위한 Primer 의합성 SHV ESBL 생성 K. pneumoniae 균주의특성을분석하기위한 primer(bioneer Co. Daejeon, Korea) 를다음과같이제작하였다 (Table 2). 5. 일반 PCR 을이용한 ESBL 유전자의증폭 유전자증폭을위한 PCR 반응액은 HotStart PCR PreMix(Bioneer Co.) 에 10 pmol primer와 1 µl DNA를 Table 2. 5 µl의 SYBR premix Ex Taq(TaKaRa), 10 µm 각각의 primer(shv F, SHV R), 2 µl DNA를혼합하여총 25 µl 로 cuvette에분주후마개를닫은후 500 g에서 30 초간 spin-down 후 Smart Cycler를이용하여반응을실시하였다.

Real-Time PCR을이용한 ESBL 유전자의증폭임상검체에서분리된 ESBL 생성 K. pneumoniae 균주를종특이적 (species specific) 유전자인 SHV 유전자의존재를확인하기위하여 SYBR Green Ⅰ을이용한 real-time PCR을실시하였다. PCR 반응은 cuvette(smart cycler, TaKaRa) 에 25 µl 반응액을기준으로 870 bp의 SHV 특이유전자를다음과같은조건에서증폭하였다 µl의 SYBR premix Ex Taq(TaKaRa), 10 µm 각각의 primer(shv F, SHV R), 2 µl DNA를혼합하여총 25 µl 로 cuvette에분주후마개를닫은후 500 g에서 30 초간 spin-down 후 Smart Cycler를이용하여반응을실시하였다. 95, 10초간 polymerase 활성화를실시한후, shuttle PCR 30회반복반응 (95 10초, 60 10초 ) 하여목적 DNA를증폭하였다. 증폭된 DNA는 SYBR Green Ⅰ의결합으로특유의형광을발하게되어이형광의발생정도를 Smart Cycler software v. 2.0으로분석하였다. 이때얻어지는 melting peak는증폭된 DNA의 GC% 에따라특이적으로나타내었다. 7. PCR 산물의확인 PCR 증폭산물을 2% agarose gel에 1 TAE buffer 에서 70 volt, 100 ma로 1시간동안전기영동을실시한다음, agarose gel을 ethidium bromide (0.5 μg /ml) 수용액에서 20분간염색하고증류수에 10분간탈색하여 UV transilluminator로관찰하고, polaroid camera로사진촬영하였다. III. 결 1. Double disk synergy 시험 과 Double disk synergy 확인시험에서는 A3균주, A4균주, A6균주가음성으로나타나총 8균주가양성으로판명되었다 (Table 1). 2. 일반 PCR 및 Real-Time PCR 분석 SHV ESBL 생성 K. pneumoniae 는일반 PCR 의경우 A3 균주가음성으로나타났으나 real-time PCR 분석결과 11균주모두양성으로나타났다 (Fig. 1, Fig. 2). Lane M, 1Kbp DNA ladder; lanes A1 to A11 K. pneumoniae isolates; N, negative control. 8. 염기서열분석임상검체에서분리된 ESBL 생성 K. pneumoniae에서 real-time PCR법으로증폭된 DNA 절편이 SHV 유전자인지확인하기위하여 DNA 염기서열을확인하였다. PCR 반응후전기영동을실시하여 870 bp 크기의 DNA 절편을 agarose gel에서잘라낸후 QIAquick PCR Purification Kit(QIAGEN) 을이용해 agarose gel에서 DNA를유출후 SolGent(Daejeon) 에의뢰하여염기서열을확인하였다. Fig. 2. Sequencing and DNA BLAST results of PCR amplified SHV gene(a1 strain). from K. pneumoniae isolate. 3. 염기서열분석 SHV ESBL 생성 K. pneumoniae real-time균주의 PCR 염기서열분석결과를 National Center for Biotechnology Information(NCBI, Bethesda, Md.) 의 Basic Local Alignment Search(BLAST) 을이용하여염기서열의유사성을비교분석하였다. 검색결과모든균주가이미보고된균주와 SHV 유전자가 99% 의유사성을보였다 (Fig. 3). IV. 고찰 최근에 β-lactam 항균제에내성인세균에의한감염의증가가중요한문제로대두되고있다. 세균이 β-lactam 항균제에대한내성을획득하는기전은 β-lactamase 생성, penicillin-binding protein의변성, β-lactam 항균제의세포막투과성저하, 세포밖으로의항균제유출등다양하며, 장내세균은흔히 β-lactamase 생성에의하여 β -lactam 항균제에대한내성을획득한다 (Silva 등, 2001). 호흡기감염, 요로감염증, 패혈증, 수막염등다양한감염증의발생원인이며, 화학요법제에저항성이강하기때문에기회감염증과균교대증의원인이되는 K. pneumoniae 는현재지난 20년동안가장현저하게증가한원내감염원인균으로대두되고있으며, 3세대 cephem계다제내성을나타내는 ESBL이증가되고있는추세이다 (Galani 등, 2002). ESBL생성 K. pneumoniae의검출에는표현형특성을이용하여검출하는방법이널리알려져있다. 그러나, 표현형방법은검출에시간이많이소요된다는것과, 접종량이정확하지않으면정확한결과를얻기가힘들다. 또한, ESBL 매개화학요법제내성유전자의정확한동정은, 병원에서의원내감염의감시와역학에대단히중요한역할을할수있다 (Pnaiara 등, 2000; Silva 등, 2001). K. pneumoniae의 ESBL 생성유전자를검출하는데 realtime PCR 방법을이용하면유전자들을검출하는데있어서가장정확하고민감하고또한빠른시간에검사가가능한방법이라하였다 (Chia등, 2005). 본실험결과표현형적방법으로분리된 ESBL 생성 K. pneumoniae 11균주중 A3, A4, A6균주의경우 DDS결과가음성으로나타났다. 일반 PCR결과 A3균주만음성으로나타났고나머지균주는 SHV만유전자를가지고있었고, real-time PCR결과모두양성으로나타났다. A4, A6균주의경우 DDS결과는음성으로나타났으나 SHV유전자검출에서양성으로나타났고 real-time PCR에서모두양성으로나타나다른 ESBL 유전자자형에대한연구가필요하다. 156
