It is used therapeutically as a racemic mixture, composed of S- and R-enantiomers. However, its calcium channel blocking activity is confined to S-amlodipine; R-amlodipine has 1-fold less activity than the S-enantiomer. Objective: The objective of this study was to compare the pharmacokinetic characteristics and safety profiles of the newly developed S-amlodipine formulation, composed of different additives, with those of the previously developed formulation. Methods: This randomized, double blind, two periods, two sequence crossover study was conducted in 24 healthy male volunteers at Seoul National University Hospital Clinical Trials Center. All subjects were randomly assigned to one of two sequence groups: (1) a single dose of the new S-amlodipine formulation (5 mg, 2.5mg/tablet) in the first study period, followed by a single dose of the reference S-amlodipine formulation (5 mg, 2.5mg/tablet) in the second study period, or (2) vice versa. A 14 day-washout separated the study periods. Blood samples for pharmacokinetic analysis of S-amlodipine were collected just before study drug administration and at predefined time points up to 168 hours post dose. Safety profiles including hematology, biochemistry, electrocardiography, and urinalysis were assessed throughout the study. Pharmacokinetics of the two formulations were characterized using noncompartmental model and compared between the formulations. Results: The pharmacokinetic profiles of S-amlodipine were comparable between the formulations. The mean [SD] values for C max in reference and test formulation (3.4 [.8] ug/l vs 3.3 [.88] ug/l, respectively) and AUC from time to the last available measurement (AUC last) (159.2 [45.6] ug*h/l vs 16.6 [48.9] ug*h/l, respectively) were similar. The calculated 9% confidence interval of the corresponding ratios of log-transformed C max and AUC last, were.9~1.3 and.93~1.8, respectively. Neither formulation caused any serious adverse events. Conclustions: Both formulations were safe and well tolerated in tested dose, 5 mg of S-amlodipine. This study provides evidence for similar pharmacokinetic characteristics of the newly developed formulation of S-amlodipine to a previously prescribed formulation in healthy Korean male volunteers. 2 최윤정외 : 기존제형 S-amlodipine gentisate 에대한변경제형 S-amlodipine gentisate 의생물학적동등성연구 서론 Amlodipine 은디하이드로피리딘계의 3 세대 calcium channel blocker 로써 calcium ion 이심장및말초혈관평활근의세포막으로유입되는것을차단하는기전으로혈압강하작용을나타내는고혈압및협심증치료제이다 1. Nifedipine 과같은초기의 calcium channel blocker 들은일반적으로작용시간이짧아 1일 2~3 회복용해야했으나, amlodipine 은혈중반감기가 35~5 시간으로 1일 1회복용으로치료역내의혈중약물농도를유지할수있다는장점을가지고있다. 또한약효지속측면에서도 amlodipine 최대효과가투여 4~6 시간후에나타나 24 시간까지작용이유지되는것으로알려져있다 2,3. Amlodipine 은비대칭탄소 (chiral center) 를가지는 chiral 화합물이다. 일반적으로 chiral 화합물의약효는순수한이성질체화합물이이성질체혼합물보다우수한것으로알려져있다 4. 또한이성질체화합물의입체배치상태에따라또는이의염형태에따라서로상이한약물학적특성을갖기도하는데, amlodipine 의경우 S-이성질체는강력한 calcium-channel blocker 로써활성을나타내는반면, R-이성질체는 calcium-channel blocking 효과가거의없는것으로밝혀졌다 5. 또한 S- amlodipine 단독투여시, 낮은용량에서 racemic amlodipine 과거의동일한치료효과를보이면서, amlodipine besylate (Norvasc ) 와인과관계가있는대표적부작용인말초부종이감소하였다는결과가보고되었다. 이에 S-amlodipine 은 racemic amlodipine 을대체할효과적인약물로많은연구및약제로의개발이이루어져왔다 6. 일반적으로부형제자체에는약력학적작용이없지만약제의흡수, 대사, 분포및배설에영향을줄수있다. 부형제가변경될경우, 부형제가약물과반응하여수용성또는불용성의혼합체를형성할수도있으며, 약물의흡수속도와양을증가시키거나감소시켜나타나는약동학적변화에의해약물의작용에도차이가발생할수있다 7,8. 따라서, 본연구에서는건강한성인남성을대상으로, 기존에개발된제형의 S-amlodipine gentisate 와유효성분은동일하지만제제학적연구를통하여새로개발된, 보다안정된제형의 S-amlodipine gentisate 를단회교차경구투여한후약동학적특성을연구함으로써인체에서두약물의안전성을포함한생물학적동등성을평가하고자한다 (Table 1). Demographic data of study subjects 항목 대조약 / 치료약 (N=12) 투여순서군 치료약 / 대조약 (N=12) 순서군에따른 P-value * Age (year) 24.3 ± ± Height (cm) ± ± Weight (kg) 69.3 ± ± Results are presented as mean ± standard deviation * Kruskal-Wallis test 연구대상및방법 1. 연구대상 19세이상 45세이하의건강하며본연구에적합하다고판단된 24명의성인남성피험자가연구에참여하였다. 본연구는서울대학교병원의학연구윤리심의위원회 (institutional review board, IRB) 의승인을얻은후시작되었고, 피험자들은모두사전에연구의목적, 내용등에대하여충분한설명을듣고자의에의하여참여를결정한후, 동의서에서명하였다 (Table 2). 2. 연구방법 1) 임상시험방법본연구는시험약과대조약을단회교차투여하는무작위배정, 이중맹검, 교차시험설계 (crossover design) 로써각순서군 ( 대조약-시험약투여순서군또는시험약-대조약투여순서군 ) 당 12명의피험자가배정되었다. 대조약-시험약투여순서군의피험자는제 1기에대조약인 5 mg 의 S- amlodipine 기존제형을복용하고휴약기를가진 후, 제 2기에시험약인 5 mg 의 S-amlodipine 변경제형을복용하였다. 시험약-대조약투여순서군은반대의순서로임상시험용의약품을복용하였다. 기존에문헌에보고된 S-amlodipine 의혈중반감기가 35~5 시간임을고려하여 1,9, 1기에투여받은약이체내에서충분히소실될수있도록 14 일의휴약기를가진후 2기에시험약또는대조약을투여하였다. 피험자에게투약 3일전부터임상시험약인 S- amlodipine 의흡수, 분포, 대사및배설에영향을줄수있는카페인함유음료의섭취, 음주, 흡연그리고심한운동을금지시켰다. 피험자는투약전날밤 1시이후금식한후임상시험용의약품투약당일오전공복상태에서시험약또는대조약을복용한후, 4 시간동안공복상태를유지하도록하였다. 안전성은피험자의자발적인보고와문진, 활력징후, 신체검사, 임상실험실검사및심전도검사를통하여확인하였다. 2) 약동학적분석약동학적특성평가를위해약물투여전상완정맥에생리식염수로채워거치한정맥내카테터를통해투약직전, 투약후 1, 2, 4, 6, 8, 1, 12, 39

4 최윤정외 : 기존제형 S-amlodipine gentisate 에대한변경제형 S-amlodipine gentisate 의생물학적동등성연구 14, 24, 48, 72, 96, 12, 144, 168 시간까지각각약 8 ml 의혈액을채취하였다 1. 채취한혈액은즉시 heparinized tube 에담은후원심분리하고, 농도분석시까지 2 C 이하에냉동보관하였다. 약동학파라미터는비구획모델 (noncompartmental method) 로최고혈중농도 (C max ), 측정가능한마지막채혈시점까지의혈중농도-시간곡선하면적 (AUC last ), 측정가능한마지막채혈시점에서무한대시간까지외삽하여계산한혈중농도-시간곡선하면적 (AUC inf ), 최고혈중농도도달시간 (T max ) 및소실반감기 (t½) 를구하였다. 채혈시각은예정된시각이아닌, 실제채혈이수행된시각으로분석하였다. C max 와 T max 는관측된값을사용하였고, AUC last 및 AUC inf 는혈중농도-시간곡선그래프로부터혈중농도상승구간은선형사다리꼴방식계산법 (linear trapezoidal rule) 에의하여계산하였으며, 혈중농도감소구간은대수사다리꼴방식계산법 (logarithmic trapezoidal rule) 에의하여산출하였다. 시간에따른혈중농도의 log-linear plot 에서 C max 이후농도중정량한계이상의마지막농도측정치를포함하여최소한 3개의점을포함한보정된결정계수 (adjusted r 2 ) 값이최대가되도록선형회귀하여소실속도상수를구하였으며, 선형회귀분석에서의가중치는측정농도의역수로설정하였다. 또한, ln(2) 를소실속도상수로나누어소실반감기 (t½) 를산출하였다. 3) 혈장암로디핀농도측정 S-amlodipine 표준품을메탄올에녹여 1. mg/ml 의표준원액을만들어냉장보관하였고, 이용액을순차적으로희석하여 spiking standard 용액을이동상으로조제하였다. 표준검량선을작성하기위하여공혈장.2 ml 에.1,.2,.5, 1, 2, 5, 1 및 2 ng/ml 의농도가되도록첨가하고내부표준물질인 terazosin (1 ng/ml) 2 ul 와 1M NaOH 2 ul 를가하여 1초간잘섞었다. 여기에 tert- butyl methyl ether 1 ml를가하여 2분간진탕한후원심분리하고상층액을질소기류로완전히건조시켰다. 건조후이동상 2 ul 에재용해시키고원심분리한후, 상층액을취하여 LC- MS/MS(Liquid chromatography joined with tandem mass spectrometry) 에주입하였다. 여기에서얻은내부표준물질의피크면적에대한 S- amlodipine 의피크면적비로검량선을작성하였다. 7 C 에보관했던임상시험혈장시료를실온에방치하여녹인후 1초간탁상용혼합기에서잘섞은다음이혈장시료.2 ml 를취하여시험관에옮기고위의시험방법과동일한방법으로전처리하고 LC-MS/MS 에주입하였다. 얻어진크로마토그램에서내부표준물질의피크면적에대한암로디핀의피크면적비를구하여미리작성한검량선으로부터혈장중암로디핀의농도를구하였다. 최소정량한계는 1 ng/ml 이었으며, 일내 (intraday), 일간 (inter-day) 분석의정밀도및정확도는이론치의 85%~115% 이내였다. 4) 통계분석대표적인평가변수인연령, 키, 몸무게에대한두순서군간의차이여부를 Kruskal-Wallis test 를사용하여평가하였다. 또한, 변경제형 S-amlodipine 과기존제형 S-amlodipine 간의약동학적파라미터의비교는유의수준.5 에서혼합모델 (mixed effect model) 을이용한분산분석 (ANOVA, analysis of variance) 으로하였고, 계산된 C max, AUC last 및 AUC inf 를로그변환하여기존제형 S-amlodipine에대한변 4

C max 는기존제형의경우 3.4 ±.8 ug/l, 변경제형의경우 3.3 ±.9 ug/l ( 평균 ± 표준편차 ) 를보였다. t ½ 는기존제형의경우 44.4 ± 8.6시간, 변경제형의경우 42.3 ± 11 ( 평균 ± 표준편차 ) 시간이었다. AUC last 는기존제형과변경제형에서각각 ± 45.6, 16.6 ± 48.9 ug*h/l, AUC inf 는각각 17.9 ± 48.5, ± 54.2 ug*h/l ( 평균 ± 표준편차 ) 이었다 (Table 3). 시험약과대조약투여간 AUC last, C max, T max 및 t½ 는개인에따라다양한양상을보였으며, 일정한경향을보이지는않았다 (Fig. 2). 기존제형대비변경제형의 S-amlodipine 의약 동학적파라미터를비교하기위해주요파라미터인 AUC last 와 C max 의수치를로그변환하여평균치차의 9% 신뢰구간을산출하였다. 그결과 AUC last 의신뢰구간은.93~1.8, C max 의신뢰구간은.9~1.3 이었다 (Table 3). 2) 안전성평가연구기간동안총 24명의피험자중 2명의피험자에서총 48건의유해사례가발생하였다. 그중두통 6건, 복부불쾌감 1건, 어지러움 1건, 졸림 1건의증상이임상시험용의약품과인과관계가있다고판단되었으나 11,12, 모두기존 S-amlodipine 과 Table 3. Choi et al.: Bioequivalence study of S-amlodipine gentisate conventional versus new formulation 관련하여보고된예측가능한유해사례였고, 경증에서중등증의이상반응이었으며후유증없이자발적으로소실되는양상을보였다. 유해사례발생률에있어서유의수준.5 로검정하였을때, 투여군간의통계적으로유의한차이는관찰되지않았다 (Table 4). 기타안전성평가항목인활력징후, 심전도검사, 임상실험실검사, 심체검진의결과에있어임상시험용의약품투여후의미있는변화는관찰되지않았다 ( 결과생략 ). 고찰 하였고 8, 이외의약동학파라미터를비교한결과, T max 는중앙값이 6 시간 (2~12 시간 ), 소실반감 기의평균은 29~52 시간으로약제간에유사하였으며, 기존에보고된결과와도유사하였다. 본연구는건강한성인남성을대상으로하여, 사람에서최초로변경제형의 S-amlodipine 의안전성을및기존의제형의 S-amlodipine 과비교하여약동학적특성을확인하였다. 본임상시험결과를미루어보아기존제형의 S-amlodipine 은변경제형의 S-amlodipine 과다르지않은약동학적양상을보이는것으로생각된다. 본연구에서는기존의 S-amlodipine 제제와부형제를변경한변경제형의제제를 24명의건강한성인남성에게단회교차경구투여한후안전성을검토하고, 두약제가유사한약동학적특성을보이는지비교평가하였다. 임상시험기간내발생한모든유해사례는모두경증에서중등증으로예측가능한부작용이었으며, 특이사항은발견되지않았다. AUC last 의신뢰구간은.93~1.8, C max 의신뢰구간은.9~1.3 으로 1 을포함하였으며, 이는식품의약품안전청의생물학적동등성을평가하기위한항목즉, 대조약과시험약의 AUC last 및 C max 를대상으로로그변환하여통계처리하였을때, 로그변환한평균치차의 9 % 신뢰구간이.8~1.25 이내여야한다는기준을만족시켰다 13. 채혈시간이임상시험용의약품의약물동태학적인특성에따라적절하게설정되었는가를평가하기위해 AUC extra, 즉 AUC inf 와 AUC last 의차이를 AUC inf 에대한백분율로계산하였다. 그결과 AUC extra 는 1.9~16.9% 로최종채혈시간의설정 은적절하였음을확인할수있었다. 두약제의 AUC last 와 C max 의개체간변이는 2~3% 정도로기존에알려져있는결과와유사 * 본연구는 SK케미칼의지원을받은위탁연구이며, 저자들은어떠한이해갈등관계 (conflict of interest) 도없다. 참고문헌 1.
