The arachnoiditis of various causes, leptomeningeal meastasis, lep tomeningeal Iymphoma, hypertrophic polyneuritis, etc. are known to have a similar radiologic manifestations The authors reviewed myeolographic findings of 20 patients with spinal arachnoiditis retrospectively to discovery any specific findings helpful to make a differential diagnosis. The causes of spinal arachnoiditis were tuberculous origin in 9 cases, pyogenic in ι postoperative in one, Iymphoma in 3 and leptomeningeal metastasis in 5 Myelographic findings of tuberculous spinal arachnoiditis were block of CSF (89%), especially at the conus medullaris level, multiple fine and/or coarse nodular filling defects (78%), nerve root thickening (56%), focal ir regular adhesive filling defects (44%) and irregular or indistinct thecal sac margin (44%). lrregular adhesive bandlike filling defects and/or multiple fine nodular filling defects seem to be characteristics of the arachnoiditis Myelographic findings showing coarse nodular filling defects without fine ones are suggestive of leptomeningeal metastasls 르긍 1. 서 척추지주막염 (spinal a r ac hn o iditi s) 의원인으로는 척추마취, 척추마취에쓰이는주사기에남은세정제 ( d etergents), 뇌악의강영 ( 화농성, 애독, 결핵등 ), 신생물 ( n eop la s i a), 척수강내 ( intrathecal ) 혈청또는 항암제주입, 척수강조영에쓰이는조영제, 외상 ( 수 술과외부원인둥 ) 및 없는것둥이있다 1.2 ) 비외상성출헐파원안을알수 이논운은 년 8 월 1 8 일에성수하여 년 9 윌 6 일에채택되었음 척추지주막염의척수강조영술소견 A로는뇌척수 액 (cerebrospinal fluid ) 의 완전혹은일부차단, 신경 근소매 ( nerve root sle e v es) 의소설파신경근의비후 ( thickening ), 지주악하공간 (s ub a ra c hn o id space) 의 좁아짐, 조영제의 불규칙한분포와소방형성 (J ocul ation ), 가낭종 ( p se ud ocys t ) 형성파지용성조영제의 항구적흡착 ( fi xa ti o n ) 둥이았다 1) 그러나이렌소견 들은비특이석이어서원인뱅밴들을강별진단하는데 는큰도움이되지 않는것으로알려져있다. 저자들은각뱅변플에서의척수강조영술소견을비 교분석하여서로의 강별진단에도웅이되는소견들 을얻고자하였으여, 그결파결핵성지주악엽의예들

2 - 大韓放射線醫學會誌 : 第 24 卷第 5 號 1988 에서비교석특정석인소견들이판찰되었기에문헌있기전에뇌와흉부에각각화농성뱅변이있었 A 며, 고찰과함께보고하는바이마예의수술후지주막염은추간판탈출증 ( herni atio n 2. 대상및방법 1981 년부터 1988 년에걸쳐서울대학교뱅원에내원 하여척추지주막염으로진단받은 20 예를대상무로하 였다. 척추지주막염의원인들은결핵성 (9 예 ), 수막 전이종 ( l ep tomeningeal metastasis, 5 예 ), 임파선종 ( lympho ma, 3 예 ), 화농성 ( pyogenic 2 예 ), 수술후유 증 ( postoperative, 1 예 ) 등이었마. 수막전이의원말종양들은소세포성폐암 ( small cell 1 ung cancer ), 뇌의핍지교종 ( oligodendrog lioma), 다형성교아종 (g li ob l asto m a multiforme) 의 각 1 예와송파선종 (p in ealoma) 2 예였마. 환자들은남 자 11 명, 여자가 9 명이었으며연령분포는 6 세에서 66 세까지였다. 진단기준은파거력과임상소견, 뇌척수액검사소 견, 척수강조영숭소견퉁이었으여, 임상소견과척 수강조영술소견은 20 여 l 모두에서척추지주막염에 f intervertebral di sc) 으로 2 언의척추수술을받은 과거력이 있고척수강조영술상병변이수술부위에 국한되어있어수술후지주막염무로진단할수있었 다. 척수강천자 ( s pinal pun c ture ) 의과거력이있는예 는, 수술후지주막엽과천이성뱅변의각 1 예와, 2 예 의화농성지주막영, 6 예의결핵성지주악영둥총 10 여 1 였으냐, 척추수술이나척수강조영술, 척수강내 투약 (i ntrathe ca l drug in jection ) 의과거력이있는예 는수술후지주막염의 1 예뿐이었마. 척수강조영술의조영제로 1 4 예에서 180 mg 1 I ml 의 metrizamide(amipaque<!ll, Winthrop ) 을사용하였는 데그중 13 예에서는요추천자 (J umba r puncture ) 로 시행하였고, 1 예에서는요추천자하여뇌척수액이나 오지않아경추천자호시행하였다 ( case 2 ). 4 예 (case 9, 11, 15, 20 ) 에서는요추천자로 300 mg I/ ml 의 Iopa midol (Niopam<!ll Me r ck) 을주입하여시행하였으여, 2 예 (case 1 2, 1 7) 에서는 pant opaq u e <!ll 로시행하였는 석합한소견을보였다. 9예의결핵성척추지주막염환자중뇌척수액에서산내성간균 (ac id-fa st b ac illi ) 이냐결핵균이나온예가 2예 (case 5,9) 있었마. 수술을한 2예 (case 2, 3) 의결핵성환자중 1예에서는영리학적으로확잔되였으나 (case 3), 1예에서는비특이석만성염증소견안을보였으며 (case 2) 9예모두가항결핵제치료후임상증상의호천이있었다. 9예의결핵환자중, 척수강조영술전후 1 개월이내에뇌선산화단층촬영을시챙한 7예중 4예에서기저조 ( basal c i ste rn ) 의조영증강을보이는뇌막염의소견을보였다 (c ase 4,5,6,7 ). 다른 3예는뇌 기저조의증강이없었고교통성뇌수종 (comm unicating hydro. ce phalu s ) 의소견만있었다 (case 2,3,9). 뇌전산화단충촬영을시행하지않은 2예는 27년파 2년전에각각결핵성뇌막염무로치료를받은파거력이있었다 (case 1,8 ) 수막전이와임파선종의 8예에서척수강의병벤자체가병러학석으로확진되지는않았지안전이종의경우원말종양이확잔되었고임파선종은임파철 (J y mph node ) 의생검으로확진되었다. 2예의화농성지주막염에서는지주막염의증상이 데그중 l 예는요추천자로 (case 경추천자 (case 17 ) 로시행하였다. 다. 3. 성 7':1 --, 12), 냐머지 1 예는 임상소견과척수강조영숭소견은 Table 1,.2 와같 결핵성지주막엽은 9 예중 6 예가 20 대였고, 수막전 이종은 1 0 세이전부터 60 대까지다양했 A 여, 염파선 종은 30 대가 1 예, 40 대가 2 예이며, 화농성지주막염은 20 대와 30 대가각각 1 예였고, 수술후지주막염의 1 예 는 40 대였다. 결핵성지주막염의 9 예중 8 예에서지주막영의진안 전후에결핵성뇌막염의앵력이있었고냐머지 1 예는 뇌막염의과거력이분영치않았마. 결핵성척추지주악엽의가장흔한척수강조영술 소견은뇌척수액의차단 ( block ) 으로 8예에서보였고 그중 4 예는완전차단이였으며, 4 예는불완선차단이 었다 (Fig. 1 ). 차단부위는척수원추 (co nu s medu llaris) 부위가 4 예, 7 벤째흉추부위가 2 여 1. 흉추부위가 2 예였다 번 차안된경계의모양은불규칙했 고척수의전위 ( displacement ) 를보이는예는없었

S ummary of Clinical and Myelographic Features Tbc (n=9) Block of CSF 8 lrregular band-like filling 4 defects lrregular indistinct dural 4 sac margm Deformity of dural sac 0 T otal interroot adhesion with 0 apparent rootless dural sac Multiple nodular, filling defects Fine unifo rm sized 3 Coarse variable sized Combined 3 Root thickening Diffuse L ocalized 4 Pyogenic Post-op Mets Lymphoma (n=2) (n= l ) (n =5) (n =3) l 다. 결절성음영결손 ( n od ul a r filling defec t ) 에는, 1 mm 정도의크기이연서 다발성으로나타냐는미세한 ( fine ) 것파, 3-10 mm 정도의다양한크기이연서석 은숫자로보이는굵은 (coa r se) 것의두가지종류가 있었는데 9 예의결핵성지주막염에서는마세결절성 음영결손이 3 예에서보였고, 굵은결절성음영결손 은 1 예에서보였 A 며 3 예에서는두가지마보였다 (Fig.l ). 그밖의소견들은신경근의비후 (5 예 ), 불규 칙한따모양의 ( band-like) 음영결손 ( 4 예 ), 척수강의 모양이불규칙하거나불분명하게보이는경우 ( 4예둥 이었으며불규칙한외형은특정방향의경막낭 ( dur a l sac) 에국한되어있지는않았다. 그러고미세결절성 음영과따모양의음영 결손은수솔후지주악염과수 막천이종의각 1 예외에는보이지않아결핵성지주막 엽의특징적인소견으로생각되었마. 수막전이는 5 예모두에서뇌척수액의차단이있었 으며, 그중완전차단은 3 예였고, 1 예는불완전차단 이었으여, 1 예에서는완전차단파불완선차단이각각 마른부위에동시에보였마. 또한수막전이의 5 예모 두에서결절성음영결손이보였는데그중 4 예는굵은 결결성음영결손이었고 1 예는굵은결절성음영결손 파이세결절성음영결손이같이관찰되었마 ( Fig 2). 그외에 3 예에서경막낭의불규칙한모양이보였 고, 2 예에서는신경근비후가있었다. 그러고경막낭 내에미세결절성음영결손이동반되지않으연서굵 은결절성음영결손이보이는것이수막전이종의특 정석인소견으로생각되었다. 후, 다. 임파선종에서는불완전뇌척수액차단파신경근 "1 경막낭이불분영한소견둥이 3 예모두에서보였 수술후지주막염에서는뇌척수액의차단, 불규칙한 띠모양의음영결손, 신경근의비후, 미세결절성음 영결손둥이수술부위에국한되어보였다 (F i g.3). 2 예의화농성지주막염에서는신경근의유착 로 인해경막낭내에신경근이판찰되지않는 ( ro o tl ess) 소견이 2 예에서있었고, 양이각 1 예에서보였다 ( Fig 4 ). 경막낭의기형파불규칙한모 4. 고 ~. A(case 1), B(case 4), and C(case 8) Lumbar myelograms show fine nod ular(small outlined arrow) Q.nd coarse nodular(large outlined arrow) defects, band-like defect(large black arrow) and root thickening(small black arrow) 20 대였다. 척추지주막염은입상적으로대마비 (paraplegia), 사지마비 ( quadriplegia ), 통증또는다른신경근증상둥이냐타나여 3, 9 ) 저자들의 20예중 1 3예가진챙성대부천마비 (parapa r es i s) 냐대마야로척수강조영술을시행하게되었다. 핸재까지는척추지주막염의원인감벨이어려운것으로알려져있었 A 냐, 저자들의예에서굵은결결성음영결손의유무에판겨 l 없이보인미세결절성음영결손파불규칙한띠모양의음영결손퉁이결핵성지주막염의특징적소견으로생각되었으며, 이러한소견들은수용성조영제을사용한척수강조영울에서판철펼수있을것 A로생각되고, 이러한소견들이과거 의운헌에서보고되지않은것은파거의결핵성지주 막염에판한운헌들이지용성조영제를사용한척수강 조영술소견을대상으로하였기혜문으로사료된마. 미세결결성음영결손파띠모양의음영결손은결 핵성외에수술후지주막엽의 l 예에서도보였지만수 술후지주막염은영변이수술부위에국한펀특징이 있어쉽게감 t 셜되었고 ( Fi g. 3 ), 1 예의수막전이종에 서도미세결절성음영결손이보였으나 ( Fig.2B) 원 발성악성종양의과거력이있어갇벨진단에어려움은 없었다. 이외에결절성음영결손은마릎여러원인의 지주막염, 흑색종증 ( m e lanomatosis ) 13 ), 임파선종 14 ) 수막전이 12, 1 5, 16, 1 7) 와마딸성원발성척수종양 (multicentric primary spinal tumor) 둥에서도보일 727

A(case 18), B(case 20) Lumbar myelograms reveal coarse nod ul ar(white arrow) and fine nodular( black arrow) filling defects, and root th ickening B 수있으며, 척수동정액기형 (a 다 e ri ove n o u s malfo rmation ) 이나혈판아세포종 ( he mang ioblastoma) 같 은혈판뱅변에서는다수의마세결절성음영결손이 보일수있우나이때에는사행성 (se rpigin o u s) 결손이같이있어 1 5 ) 은어렵지않마. 지주막영의원인무로서 음영 결핵성지주막염과의감별진단 빈벤한척수강천자얀으로 도기계석 (me 'cha n ica l ) 지주막염이생걸수있다는 것을고려하연척수강조영숭상에보이는변화가과거 의척수강천자에의한것인지, 원래질환에의한지 주막의밴화인지확안할수는없다. 그러나결핵성지 주막엽의경우에는척수강천자의과거랙이있는 1 예 를제외하고모두결핵성뇌막엽의파거력이있었으 며, 또한척수강의결핵성명변은결핵성뇌막염의이 차적소견이라고일반적으로알려져있마 8 ) 결핵감염을받은척수는부종이생기게되고 (e d e mat o 떠 ) 척수막 (s pin a l m e nin ges) 을따라삼출 액 (e x ud ate) 이생긴다고알려져있으여 9, 10 ) 이러한척 수막의울혈 ( c o n ges tion ) 과결핵성상출액 로인해 척추지주막염이생긴다고생각된다 7, 9, 11 ) 결핵성지 주막영의예에서결절성음영결손으로보이는경막내 육아종 (intradural gra nul oma) 은원형이나난원형이 며수 m m 에서부터뇌척수액의차단을일으킬만큼큰 것도있다 7) 결핵성명변의신경근비후는결핵종 ( tu b erculoma) 의이차석변화에의한점착성섬유성 변화 ( adh es i ve fi broti c c h a n ge) 로생각되고, 불규칙 한점착성의혹은짧은때모양의음영결손은염증성 점착성지주막염으 특징우로보이며아마도삼출성염 증의지엽적인섬유성점착화파정에의해생기는것 으로보안다

Deformed dural sac and apparent rootless dural sac are shown in lumbar myelogram 전이성종양은, 특정석소견이라고보기는어렵지만, 미세결절성음영결손이동반되지않은, 굵은결절성음영결손이다른지주막엽과의차이로생각되며, 수막전이 , 1 7) 나임파선종 14 ) 에서보일수있는결절성음영결손과신경근비후는, 악성세포자체의덩어리와악성세포의신경근챔식 (i n f i ltration) 에의한것으로알려져있다. 화농성지주막엽은대상이적지만, 신경근의유착으로안해경막낭내에신경근이보이지않는소견이강별에도움이될것무로보인다. 문헌에의하면염증성지주막엽은조영제의불규칙한분포, 척수캉의각형성 (angulation ), 지용성조영제의통파장애 (little or no free flow ) 등정상적척수강조영울소견이완전히없어진다고하며이러한점이다른원안의지주막염, 특히전이성종양파의감별점이된마고하냐 1 2 ) 수용성조영제를주로쓴저자들의예에서는이러한소견이보이지않았다. 료르 5. 결 저자들은 9 예의결핵성척추지주막염, 5 예의수막 전이종, 3 예의임파선종, 2 여 l 의화농성척추지주막 염, 1 예의수술후지주막염둥 20 예의척추지주박영 환자의척수강조영술소견을검토하여마음파같은 결론을얻었마. 1. 결핵성지주막염은대개뇌의결핵성뇌막염을 앓고있거나앓은파거력이있고 (9 예중 8 예 ), 20 대가 않았으며 (9 예중 6 예 ) 불규칙한따모양의음영결손과 미서 l 결절성음영 결손이특정으로판찰되었마. 2. 수막전이종은미세결절성음영결손이동반되 지않은, 3-10 mm 정도크기의굵은결절성음영결 손이특징적안소견으로보였마. 지 3. 화농성지주막엽은신경근이정막낭내에서보이 않는것이주된소견이었으며, 수숭후지주악영파 업파선종은지주막영의비특이적소견들을보였다 - 729
