However, it is often challenging to develop a drug for neonates due to distinct characteristics and limitations of clinical trials, making it often unavoidable to use off-label drugs in neonates. The objective of this study was to investigate the use of off-label drugs in neonatal intensive care unit (NICU) in a single institution in Korea. Methods : All neonates admitted to NICU of Seoul National University Children s Hospital between January and June 2017 were retrospectively studied. Demographic data and patient medication profile were collected for the study. 2 JKSHP, VOL.36, NO.3 (2019) ment route, duration of treatment, contraindications and warnings specified in the specific marketing authorization, and classified as licensed or off-label according to their licensing status. Results : 226 neonates were subjected to the study. Of the 226 neonates, 137 (60.6%) were preterm infants and 89 (39.4%) were term infants. Of the 1,797 prescriptions, 1,366 (76.0%) were used off-label drug, 155 (8.6%) were Approved for neonatal and pediatric period but off-label use for dose and/or frequency, 651 (36.2%) were Off-label for neonatal period but on-label for pediatric period, 481 (26.8%) were Undetermined approval state for neonates and pediatric, and 79 (4.4%) were Contraindicated for neonates and pediatric. Conclusions : Off-label drug use rates were as high as 76% of total prescriptions. Thus, the NICU pharmacist should be alert to this, and put more efforts to examine prescriptions, provide appropriate information through various literature reviews, and closely monitor drug side effects. In addition, further research should be done to obtain clinical evidence for the use of 'off-label drugs' in neonates. [Key words] Infant, Newborn, Intensive care unit, Neonatal, Off-label use 신생아는출생후첫4주동안의아기를뜻하며, 신생아기에는급격한성장과발달을통해많은생리학적변화가일어난다. 신생아는신체가미성숙하고생리학적인변화로인해성인과는다른약동학적특성 ( 흡수, 분포, 대사, 배설 ) 을나타낸다. 그러므로, 신생아에서약물사용시에는이러한약동학적특성을고려하여약물을선택하고용량및투여간격이결정되어야한다. 1),2) 신생아에서주의깊은약물사용이필요하나, 실제신생아진료현장에서사용되는많은약물들이신생아에대해안전성과효능이입증되지않은채사용되고있다. 신생아를대상으로한임상시험을찾아보기어려운이유는신생아및소아를대상으로한임상시험이취약계층으로여겨지는대상을피험자로한다는윤리적인문제가있고, 소아가소수집단이어서제약회사에서수익상의문제로임상연구투자에주저하는경향이있기때문이다. 결과적으로신생아와소아들이배제된채약물이승인되고있는실정이다. 3),4) 한편, 여성의출산연령이높아지고난임시술이증가되면서신생아중미숙아및다태아가차지하는비율이증가하고있다. 5) 이에따라, 신생아를대상으로한약물의안전성과효능에대한연구의중요성은더욱커지고있다. 이러한문제를인식하고해외에서는신생아에대한약물사용현황을분석하는연구가활발히수행되고있다. 해외의여러보고에따르면, 신생아중환자실에서비승인 (unlicensed) 약물이처방되거나허가외 (offlabel) 로사용된처방비율이 34~80% 였고, 신생아중환자실에입실한신생아의 70~97% 가적어도하나이상의비승인약물또는허가외로사용된약물을처방받은적이있다고보고하고있다. 6)-11) 국내에서는신생아대상의임상시험활성화를위한대책등의논의는있어왔지만, 12) 국내신생아에대한약물사용현황을직접적으로조사한연구는거의없다. 이에본연구는단일상급종합병원신생아중환자실에서사용되는약물의사용현황을파악하고, 신생아에대한허가외사용약물의빈도를분석해보고자하였다. 연구방법 1. 연구대상및기간 2017년 1월부터 2017년 6월까지서울대학교어린이병원신생아중환자실 (neonatal intensive care unit,

3 배혜정, 신윤주 : 단일기관신생아중환자실에서의허가외약물사용현황 Fig. 1 Decision tree to classify licensing status of the drugs NICU) 에입실하여 1가지이상의약물을처방받은환자를대상으로하였다. 대상자중 48 시간이내에사망또는전동된경우는제외하였다. 이연구는서울대학교병원연구윤리의원회의승인을받아진행하였다 (IRB No. H ). 2. 조사방법대상환자의전자의무기록자료를후향적으로분석하였다. 환자기본특성으로재태기간, 출생시체중, 성별, 1분및 5분아프가점수 (Apgar score) 를수집하였고, 처방약물관련정보로는유효성분, 투여간격, 허가적응증, 제형, 투여경로, 용량, 치료기간, ATC 코드 (anatomical therapeutic chemical classification system), 투여시작나이및체중등을수집하였다. 각환자에서동일한목적으로 2회이상처방된약물은 1 건으로기록하였다. 정맥영양제제, 혈액제제, 산소치료, 광선치료, 수술중사용된약물, 진단을위한약물 은분석에서제외하였다. 대상군은재태기간에따라 37주이전에출생한신생아들을미숙아군으로, 37 주이후에태어난신생아들을만삭아군으로분류하였다. 신생아중환자실재원기간동안미숙아는생후연령 44주까지, 만삭아는생후 31 일까지의약물사용을분석하였다. 연구기간동안사용된모든약물은적응증, 투여경로, 용량및용법등에대해식품의약품안전처온라인의약도서관 13) 의허가사항과비교하여 (1) 신생아사용에대한허가약물, (2) 신생아, 소아에대한허가는있으나용량 / 용법에대한허가외사용, (3) 소아에대한허가는있으나신생아에대한허가가없는약물, (4) 신생아와소아모두에게허가가없는약물, (5) 신생아, 소아에대한금기약물, 총 5가지로분류하였다 (Fig. 1). 신생아에서신독성을일으킬수있는약물들은여러문헌을검토하여다음과같이선정하였다. 14)-17) : antibacterials (ampicillin, amikacin, gentamicin, piperacillin, vancomycin), antifungal

8 JKSHP, VOL.36, NO.3 (2019) 및 off-label 포함 ) 처방비율이 55~80% 로나타났으며, 80~97% 환자에서허가외약물이사용되었다. 10) 비교적최근에유럽에서수행된연구들에서도허가외약물의처방비율은여전히높게나타났다. 프랑스에위치한 2개의신생아중환자실에서수행된연구에따르면, 허가외사용약물의처방비율이 64.7% 로나타났으며, 94.8% 환자에서허가외약물이사용되었다. 8) 이탈리아에서수행된다기관 (36개신생아중환자실 ) 연구에서는허가외사용약물의처방비율이 73.5% 로나타났으며, 87.7% 환자에서허가외약물이사용되었다. 11) 이연구에서는항감염제가가장흔하게처방되는허가외사용약물이었으며, 나이와투여횟수에서허가외로사용하는비율이높았다. 한편, 독일의한연구에서는허가외사용약물의처방비율이 34% 로다른연구들에비해상대적으로다소낮게나타났는데, 이연구에서는나이와용량, 적응증에대해서만허가상태를확인하였다고보고하고있다. 6) 본연구에서단일기관의신생아중환자실에서의전체처방 1,797건을분석한결과 76.0% 가허가외로사용되고있었다. 대다수 (99.1%) 의환자에서신생아중환자실재원기간중 1가지이상의허가외사용약물을투여하였다. 이는기존의해외연구에서의결과와유사하며, 국내신생아중환자실에서도약물의허가외사용이광범위하게이루어지고있음을보여준다. 본연구에서조사된신생아, 소아에대한금기약물은총 18 종이었고처방비율은 4.4% 로나타났다. 국내허가사항과미국 Food and Drug Administration (FDA) 허가사항 18) 을비교해보았을때 18 종중 2종 [methylprednisolone sodium succinate (IV), lorazepam (IV)] 만 FDA에서미숙아사용금기로되어있었으며, 금기사유는첨가제로미숙아에게치명적인호흡곤란을발생시킬수있는 benzyl alcohol을포함하기때문이었다. 실제본원에서사용되었던신생아, 소아에대한금기약물 18종중 5종 [methylprednisolone sodium succinate (IV), lorazepam (IV), dexamethasone (IV), phenobarbital (IV), amiodarone (IV)] 에 benzyl alcohol이포함되어있었으며, 나머지의경우는특정연령금기약물이었다. 허가외사용약물을 ATC 코드로나누어보았을때기존의해외연구와유사하게전신작용항감염제가가장 다빈도로처방되고있었다. 6) 전신작용항감염제의허가상태를분류하였을때신생아, 소아에대한허가는있으나용량 / 용법에대한허가외사용이 45.5% 로가장비율이높았고, 전체용량 / 용법에대한허가외사용의 98.1% 를차지하였다. 이는전신작용항감염제에서허가사항이실제임상적으로사용되는것과차이가있음을의미한다. 실제 vancomycin, aminoglycoside계약물인 amikacin, gentamicin 등은신기능과약물의배설과의연관성이크고, 신생아는월경후연령 (postmenstrual age, PMA) 에따라신기능이크게달라지기때문에이러한특성을고려하여용량및용법을결정하여야하나, 국내허가사항에는이들약물에대해월경후연령에따른세부적인용량및용법을제시하지않고있다. 따라서실제임상현장에서는 Micromedex Drugdex TM (Thomson, Green wood, CO, USA) 와같은약물관련데이터베이스나최신논문등을참고하여처방하고있기때문에국내허가사항과실제사용에차이가발생하는것으로사료된다. 아일랜드의신생아중환자실에서수행된선행연구에서는재태기간이 32주, 28 주로짧아질수록허가외약물의사용비율이높게나타났다. 9) 프랑스에서수행된연구에서는재태기간에따라환자군을보다세분하여허가외약물의처방빈도를조사하였는데, 만삭아에비해미숙아그룹 (28주미만, 28~36주미만, 32~37 주미만 ) 에서허가외약물의처방비율이 45.4% vs 67.9~73.6% 로높게나타났으며, 허가외약물을처방받은환자의비율도 82.6% vs 98.6~100% 로높게나타났다. 8) 한편, 본연구에서만삭아와미숙아에서의허가외약물처방빈도를비교하였을때 77.7% vs 75.2% 로두군에서유의한차이가없었다. 이는국내와해외의허가사항과다르고, 신생아에게허가된약물도월경후연령이나미숙아및만삭아를구분하여용량및용법이구체적으로제시되어있지않은경우가많기때문으로사료된다. 본연구에서조사한신생아에서신독성발생가능약물들은동맥관개존증 (patent ductus arteriosus, PDA) 의치료제로허가받은페데아주 ( 수입판매원 : 삼오제약, 제조자 : Merckle GmbH, Blaubeuren, Germany) 를제외하고는모두허가외사용약물이었다. 신독성발생가능약물의유지기간은대부분 7일이내

9 배혜정, 신윤주 : 단일기관신생아중환자실에서의허가외약물사용현황 로길지않았으나, 미숙아에서경구이뇨제의사용시평균 24 일로장기간사용하는경향을보였다. 이는미숙아에서다빈도로발생하는질환들의치료와연관지을수있다. 미숙아에서발생빈도가높은 PDA나기관지폐이형성증 (bronchopulmonary dysplasia, BPD) 등에서보존적치료로체내순환하는혈액량을줄임으로폐혈류의과부하를줄여호흡기능을개선시키기위해경구이뇨제를장기간사용한다. 19)-21) 미숙아에서경구이뇨제사용시약물의유지기간에유의하고, 장기간사용시부작용모니터링에특히주의를기울여야할것으로사료된다. 본연구는단일기관에서수행되었고대상자의수가충분하지않았다는한계점이있다. 신생아에서허가외사용과신독성발생가능약물의사용현황에대해서조사하였으나, 실제신생아에서약물사용의안전성과관련된평가는이루어지지못하였다. 그러나, 본연구는국내신생아를대상으로허가외사용약물의현황을분석한초기연구라는점에서의의가있다. 또한, 허가외사용약물을세분화하여신생아, 소아에대한허가는있으나용량 / 용법에대한허가외사용, 소아에대한허가는있으나신생아에대한허가는없는약물, 신생아와소아모두에게허가가없는약물, 신생아, 소아에대한금기약물, 4가지로구분하여분석함으로써기존연구에비해더욱구체적인자료를제시하였다. 결론국내신생아중환자실에서허가외사용약물의처방빈도가높았으며대다수의환자에서허가외약물이사용되었다. 신생아에게안전하고효과적인약물요법을위해서, 신생아에대한약동학적특성을이해하고, 국내와다른해외허가사항에대한비교와다양한문헌고찰을통해보다전문적인약료서비스를제공하는것이필요할것으로사료된다. 더나아가, 신생아중환자실의다빈도사용약물을중심으로향후다기관네트워크를통한후향적연구를통해유효성과안전성을입증함으로써신생아에게질높은치료를제공할수있는인프라구축이필요할것으로생각된다. 참고문헌 1) Heimann G.
