The aim of this study is to share our recent experience with the management of this difficult infectious disease. Materials and Methods: The authors reviewed retrospectively the clinical records of a series of 10 patients with Fournier s gangrene between the years 2004 and 2007 who, after initial treatment. The patient s age, predisposing etiological factors, interval between onset of symptoms and diagnosis, lesion site, results of bacteriologic cultures, treatment and reconstructive procedures, length of hospital stay, treatment and outcome were analyzed. Results: The patients' ages ranged between 46 and 84 years (mean 59 years). Of the 10 patients, 1 (10%) died and 9 (90%) survived. The predisposing factors included diabetes mellitus (10 cases, 100%), hypertention (3 cases, 30%), liver cirrhosis (2 cases, 20%), CVA (1 case, 10%). All 10 patients had positive culture results, with 9 (90%) of these being polymicrobial. The most common organisms isolated were Escherichia coli (n=6), Staphylococcus aureus (n=4) and Streptococcus agalactiae (n=2). The important finding wass the fact that quinolone-resistant extended spectrum beta-lactamase (ESBL) Escherichia coli (E. coli) was detected in two cases (20%). The mean length of hospital stay was 41.1 days (7-70). Conclusions: There is no differences between current study and previous, butit is important that ESBL producing E. coli was appeared. It may have influence on length of hospital stay and wound healing. However, more studies are required to conclusively prove the effect of ESBL on prognosis of Fournier s gangrene. 2 204 대한요로생식기감염학회지 : 제 2 권제 2 호 2007 년 10 월 서론 Fournier 괴저는회음부와생식기관부위의괴사성근막염으로특징지워지는질환으로작은피하혈관의혈전증을일으켜피부괴사를가져오게된다. 1 Fournier 괴저는 1764년 Baurienne에의해질환에대하여처음으로기술되었지만, 성병전문의사였던 Jean Alfred Fournier 2 에의해 1883년에현재병명을가지는이름의시조가되었다. 3 당시에는 건강한젊은남성의회음부에갑작스럽게발생하여빠른진행을보이는특발성의괴사 라고정의하였으나, 그이후질환의역학과병인등에대한이해는전연령대의남녀에서발생하고음낭뿐만아니라회음부와항문주위로도파급되는괴사성근막염이라는것을알게하였다. 또한질환의이해와함께치료의발전도계속되었다. 치료원칙은적극적혈역학적안정, 광범위항균제요법과적극적외과적시술이며, 최근고압산소치료, 벌꿀, 과산화수소수등을이용한시험적치료도시행하고있다. 4-6 그러나 Fournier 괴저는여전히 15-50% 의높은사망률을보이며비뇨기과영역의응급질환이다. 7 국내에서도몇편의논문이발표되었고최근두논문은 1990년부터 10년간, 1992년부터 10년간치료와예후에대해조사한논문이었다. 8,9 저자들은최근 3년간본원에내원한 Fournier 괴저환자 10례를대상으로그특성을파악하였고지금까지보고된과거의 Fournier 괴저의특성에비추어최근질환의경향을파악하고자본연구를계획하였다. 대상및방법 2004년 1월부터 2007년 6월까지본원비뇨기과에서 Fournier 괴저로진단및치료를받았던 10례를대상으로의무기록지를통하여후향적으로분석하였다. 진단은환자의병력, 신체검사와필요한경우방사선학적영상을통하여시행하였다. 괴사성감염이없는단순음낭또는회음부의농양은연구에서제외하였다. 환자의나이, 기저질환, 증상발현에서내원까지의기간, 병소부위, 요로 / 대장전 환술유무, 세균배양검사결과, 괴사조직제거술의시행횟수, 음낭재건술의시행여부, 재원일수, 치료결과등을조사하였다. 모든환자에서내원시 3세대 cephalosporin, aminoglycoside, clindamycin 등을이용한 3종항균제치료를시행하였으며, 괴사조직의광범위외과적제거를시행하였다. 필요시국소또는전신마취하에추가적인괴사조직제거를시행하였다. 배뇨에장애가있거나음경또는항문주위로괴저가침범한경우에는요로또는대장전환술을시행하였고, 지속적으로베타딘을이용한소독및괴사부위의반복적제거를시행하였다. 조직에서세균배양검사가음성으로전환되고육아조직이생성시음낭재건술을실시하였다. 결과환자들의평균연령은 59세 (46-84) 였으며, 여성에서도발생한다는보고도있지만 10명모두남자환자였다. 치료결과전체 10례중 9명모두생존하였고사망한환자는 1례에서있었다. 기저질환으로모든환자에서당뇨병 (10례, 100%) 를가지고있었고, 동반질환으로고혈압 3례, 간경화 2례, 뇌졸중 1례등이있었다. 증상발현에서내원까지의평균기간은 5.9일 (3-14) 이었고, 임상증상으로음낭부종, 동통, 홍반, 피부괴사등은모든환자에서있었으며 38 이상의발열은 7례 (70%), 패혈증 2례 (20%) 등이있었다. 병소부위는 10례모두에서음낭부위의병소를가지고있었으며항문주위 5례, 서혜부 4례, 음경 2례, 후복막강 1례에서있었다 (Table 1). 내원시 5례에서치골상부요로전환술을시행하였고, 1례에서요로전환술및대장전환술을시행하였다. 세균배양검사결과에서는전체 10례중 10례에서균이동정되었으며, 그중복합감염은 9례에서관찰되었다. 동정된균으로는 Escherichia coli 6례였으며그중 2례에서 quinolone 내성인 Extended spectrum beta-lactamase (ESBL) 가검출되었다. Staphylococcus aureus는 4례, Streptococcus agalactiae 2례등의순이었다 (Table 2). 음낭재건의방법으로단순봉합은 7례에서시행하였고, 근피판이나부분식피술을이

Fournier 괴저는남성에서주로발생하며평균연령은 50세이상으로알려져있으며, 10,11 본연구에서도 59세로큰차이는없었다. 또한기저질환은 Fournier 괴저를유발하는선행요인이되는것으로당뇨, 알코올성간질환, 악성종양, 자가면역감소등의질환에서흔히나타나는것으로보고되고있고, 이중당뇨가가장중요한요인으로알려져있다 저자들의경우에도 10례모두에서당뇨병을가지고있었으며다른연구에서보다는당뇨병의비율이높게나타났다. 10례중 2명을제외하고는당조절을하지않은환자에서발생하였다. 최근 대사성증후군이증가됨에따라향후 Fournier 괴저의유병률에도영향을줄수있을것으로생각한다. 임상증상의시작에서내원까지의시간은 5.9일로다른연구와큰차이를보이지는않았다. 가장흔한임상증상으로는동통과부종으로종종발열과동반되기도한다. 동통은보통초기에뚜렷하게나타나다가피부신경의압박및파괴로인해점차로무감각해진다. 16 본원에내원한 10례중 7례에서발열이동반된동통및부종이있었으며내원즉시임상증상으로진단가능하였다. Fournier 괴저의급격한진행은신속한진단을필요로하며, 패혈성쇼크로의진행을막을수있을것으로생각한다. 10례중사망한 1례에서유일하게내원시패혈성쇼크증상을보이고있었다. 일반적으로 Fournier 괴저의원인균은호기성균과혐기성균의혼합감염이흔한것으로알려져있다. 각각의균주들의독성은약하고항생제치료에효과적으로반응하나, 혼합감염으로인한상승작용으로더욱심한감염을일으키는것이특징이다. 그리고이로인한환자의면역력저하는감염상태를더욱악화시키는요인이된다. 17 병리조직학적기전

5 진명헌외 : Fournier 괴저최근 10 례의특성 207 은작은혈관들이혈전에의해막혀서발생하는폐색성말초혈관염과이로인해발생하는피부의괴저이다. Fournier 괴저의원인균들은이말초혈관염의발생에영향을주는데, E. coli, Proteus, Klebsiela 등그람음성균은모세혈관의혈전형성과관련되는 lipopolysaccharide endotoxin을만들며, 17 Staphylococcus는 hyaluronidases를만들어탐식작용을억제하고결합조직을파괴한다. Beta-streptococci는 coagulase를만들어모세혈관의혈전형성을유발하며, hyaluronidase도생성한다. 혐기성균인 Bacteroides는 collagenase 와 hyaluronidase, heparinases를생성하여혈전의생성을유발하며, 탐식작용도억제하여환자의면역력을저하시킨다. 호기성균은혈소판응집과보체의고정을일으키는것으로알려져있다. 대부분의연구에서 E. coli, Proteus, Klebsiella, Bacteroides, Streptococcus, Staphylococcus 등이가장흔히동정되는균으로알려져있다. 18 본연구에서도다른연구들과마찬가지로 E. coli가 6례에서가장많았으며다음으로 Staphylococcus 순으로나타났다. 한편균의배양검사결과혐기성균이나오지않아도항생제치료는반드시 3종항균제치료를시행해야한다. 왜냐하면혐기성균은배양검사에잘나타나지않는경우가많으며, 이것의존재가병의악화에관여하기때문이다. 18 그러나한가지주목해야할점은 E. coli가 10례중 6례에서발생을하였는데, 그중 2례 (20%) 에서 quinolone-저항성 ESBL E.coli가동정되었다는것이다. 최근항생제내성균이병원성감염에흔하며질환의치유를방해하는주된요인으로주목을받고있다. ESBL 양성을보인 2례의환자에서사망례가없어서예후에영향을주는지는알수없으나다른 8명의환자군에비해유의하게재원일수및음낭재건시봉합에영향을주는것으로나타났다. 향후 ESBL 양성 E. coli는 methicillin-resistant Staphylococcus aureus (MRSA) 등과마찬가지로증가될것이며이는 Fournier 괴저의예후에도영향을줄수있을것으로생각한다. 결론 Fournier 괴저 10례의환자에서나이, 기저질환, 임상증상, 내원까지의기간, 배양균주의종류에따른빈도등에서는기존의연구들과큰차이가없었으나, 세균배양검사결과최근병원감염에서문제가되는 ESBL 양성 E. coli가출현하였다. 이균주는환자의재원기간및음낭재건에영향을줄수있을것으로생각하며향후예후인자로서의영향에대한연구도필요할것으로생각한다.
