Clinical diagnostic criteria for MEN1 include the presence of two or more endocrine tumors such as pituitary, parathyroid, and pancreatic islet tumors. Treatment is needed for tumors accompanied by symptoms or having malignant potential. Malignant neuroendocrine tumors (NETs) are the major cause of MEN1-related death, and pancreatic NETs account for 30-80% of MEN1 cases. Surgery is the mainstay curative treatment, and endoscopic intervention is a treatment option when patients are poor candidates for surgery. 33-year old female patient with MEN1 was treated via endoscopic ultrasonographyguided ethanol injection for a pancreatic NET. 6 kg, 신장 163 cm, 체질량 33세 여자 환자로, 내원 1개월 전 타원에서 폐렴으로 치료 지수 31.47 kg/m2로 비만하였다.

의식은 명료하였고, 갑상 영상검사에서 부신과 췌장 및 갑상선에 종양이 확인되어 내 선 및 복부 촉진상 이상소견은 없었다. 24시간 소변 유리 코티 원하였다.

2 Sujong n, et al. sonography-guided ethanol injection, EUS-EI)을 시행한 경 중이었다. 어머니와 삼촌이 MEN1 진단받은 가족력이 있으 험에 대해 문헌과 함께 보고하는 바이다. 며, 어머니는 부갑상샘절제술 및 먼쪽 이자절제를 시행받았 고, 삼촌은 췌장을 전절제하였다. 증 례 활력 징후는 혈압 127/94 mmhg, 심박수 86/min, 호흡수 18/min, 체온 36.7 였고, 체중 83.6 kg, 신장 163 cm, 체질량 33세 여자 환자로, 내원 1개월 전 타원에서 폐렴으로 치료 지수 kg/m2로 비만하였다. 달덩이얼굴이었으며, 팔다 중에, 멍이 잘 들고, 털이 많아 쿠싱증후군이 의심되어 시행한 리 및 복부에 다모증이 확인되었다. 의식은 명료하였고, 갑상 영상검사에서 부신과 췌장 및 갑상선에 종양이 확인되어 내 선 및 복부 촉진상 이상소견은 없었다. 24시간 소변 유리 코티 원하였다. 고혈압으로 irbesartan, dichlozid, nifedipine 복용 졸은 277 µg/day (20-70 µg/day)로 상승해 있었으며, 한밤중 C D Fig. 1. () The pancreatic neuroendocrine tumor (NET) has a typical plasmacytoid appearance (propidium iodide, 10).

5 cm 크기의 뇌하수체 선종으 증후군에 합당하였다. 혈장 부신피질 자극호르몬(adrenocor- 로 확인되었다. ticotrophic hormone, CTH)은 16.5 pg/ml (6-76 pg/ml)로 환자는 혈액검사에서 혈장 칼슘이 11.4 mg/dl (8.7-10.2 억제되지 않았기에 CTH 의존 쿠싱증후군을 시사하였다.

또한 부갑상샘 스캔에서 우측하부 부갑상샘에서 흡 사에서 1 cm의 뇌하수체 종양이 확인되었다(Fig. 1).

3일 뒤 시행한 전산화 tophysin, chromogranin 및 e-cadherin 양성으로 확인되어 췌 단층촬영에서 급성 췌장염 소견과 함께 췌장 두부 및 경부의 장 NET로 진단되었다(Fig. 2-D). EUS-FN는 선형배열 초 시술 병변에서 리피돌에 의한 고음영이 병변이 관찰되었다.

(F) Leakage of lipidol to the main pancreatic duct (red arrowheads) and peripancreatic infiltration with diﬀuse swelling of the pancreas are shown. 과 함께 다발성의 췌장종양(Fig. 1D)이 관찰되었다. 부신 종 53 cm 부위에서 췌장의 경부를 관찰하였을 때, 양은 호르몬 검사에서 24시간 소변 메타네프린 40.7 µg/day 1.3 cm의 병변이 관찰되어 에탄올과 리피돌 1:1 혼합용액을 ( µg/day), 24시간 소변 노르메타네프린 µg/day 3.8 ml 주입하였고, 이후에도 주변부 조영 증강이 확인되어 ( µg/day), 디히드로에피안드로스테론 황산염(dehy- 추가로 3.3 ml 주입하였다(Fig. 3, ). 시술 중 NET와 췌관 droepiandrosterone-sulfate) 31.0 µg/dl ( µg/dl)로 정 과의 연결이 의심되어 시술 후 췌장염 발생 가능성이 있을 것 상 범위를 보여 비기능성 종양 진단 하에 경과 관찰하기로 하 으로 판단하였다. 추가적으로 상절치에서 60 cm에서 갈고리 였다. 췌장종양은 두부에서 미부까지 분포하였고, 이 중 체부 돌기를 관찰했을 때 cm의 병변이 관찰되어, 이에 대 의 2.4 cm 종양에 대해 초음파 내시경 유도하 흡인생검(en- 해 에탄올과 리피돌 혼합용액을 3.0 ml 주입하였다(Fig. 3C, doscopic ultrasonography-guided fine needle aspiration, D). 시술 당일 복통 및 발열과 함께 아밀라제 806 U/L, 리파제 EUS-FN)을 시행하였다(Fig. 2). 면역형광염색에서 synap U/L로 급성 췌장염이 발생하였다. 3일 뒤 시행한 전산화 tophysin, chromogranin 및 e-cadherin 양성으로 확인되어 췌 단층촬영에서 급성 췌장염 소견과 함께 췌장 두부 및 경부의 장 NET로 진단되었다(Fig. 2-D). EUS-FN는 선형배열 초 시술 병변에서 리피돌에 의한 고음영이 병변이 관찰되었다. 음파 내시경(GF-UE206; Olympus Medical System, Tokyo, 이와 함께 십이지장 및 원위부 췌장과 주췌관 내부에도 시술 Japan)과 22 gauge 세침(Echotip ProCore; Cook Endoscopy, 용액의 누출에 의한 고음영 부위가 확인되었다(Fig. 3E, F). 지 않아서 추가로 2.7 ml 주입하였다(Fig. 4). 두 번째 시술 후 및 췌장 미부의 종양의 크기도 변화가 없었으며, 수술에 적극 에도 발열 및 복통과 함께 아밀라제 733 U/L, 리파제 1106 U/L 적이지 않아 경과 관찰 중에 있다.

췌장절제는 환자가 동의하지 않아 정기적 내분비 조직에서 선종이나 과증식증을 유발한다.1 제1형은 뇌 으로 영상검사를 시행하기로 하였다.
