Longer therapies may become more popular if there are better results. Recent eradication rate should be examined and proper recommendation should be timely done according to the result. Methods: We examined recent eradication rates of first-line triple regimens and compared eradication rate according to the duration of this first-line therapy for H. pylori infection. The 976 patients received first-line triple therapy at the single center from November, 2005 to October, 2009 were retrospectively analyzed. Urea breath test was done at four to six weeks after completion of eradication therapy. Results: Overall eradication rate of triple regimen by intention to treat analysis was 80.4% (785/976). One week eradication rate has decreasing trend and below 73.3% during recent 2 years. Eradication rate in 2 weeks group (83.3%, 165/198) was significantly higher than 1 week group (73.3%, 198/270) from November, 2007 to October, 2009 (p<0.01). Conclusions: Our results showed that 2 weeks triple regimen should be preferred as first-line therapy for H. pylori eradication. (Korean J Gastrointest Endosc 2010; 41:5-9) Key Words: Helicobacter pylori, Eradication 교신저자. 임윤정동국대학교일산병원소화기내과 ( ), 경기도고양시일산동구식사동 814 전화 : 팩스 : 이메일 : limyj@duih.org 접수 년 2 월 17 일승인 년 6 월 21 일 서론 Helicobacter pylori (H. pylori) 는 1983년 Warren과 Marshall 등에의해위점막에서처음배양된이후소화성궤양, 위염, 위암등의질환을일으키는것으로알려져있다. 1-4 현재까지반흔을포함한모든소화성궤양, 조기위암의내시경점막 ( 하 ) 절제술후, 초기병기의저악성도점막연관림프조직형위림프종에서제균치료를권고한다. 1-5 제균치료로 protonpump inhibitor (PPI), amoxicillin, clarithromycin 1 2주요법 을일차적으로권고한다. 3-5 H. pylori 제균요법으로적합하기위해서는 per protocol analysis에서 90% 이상, intention to treat analysis에서 80% 이상제균율을권고하나최근국내뿐아니라전세계적으로 1차삼제제균요법의제균율감소가보고되고있다. 5-9 이에 H. pylori 제균치료의기간연장, 연속치료 (sequential therapy), 새로운제균항생제조합의개발등을시도하고있다 H. pylori 제균율은지역, 인종에따른숙주의면역차이, 항생제내성균주의발생률차이, 환자의순응도의차이, 위내 H. pylori 균밀도의차이, 제균효과판정방법의차이등의원인으로지역, 국가, 인종등집단에따라차이가 Vol. 41, No. 1 July, 2010 (5-9) 5

2 나며시간이지남에따라항생제내성률이증가하여시기별지역별제균율조사가적절하게이루어져야한다 저자들은경기북서부지역의단일기관의최근 4년간의 1차삼제요법의제균율과치료기간에따른차이, 1주와 2주제균치료의제균율을조사하였다. 대상및방법 1. 대상 2005년 11월 1일부터 2009년 10월 31일까지경기북서부대학병원을내원하여상부위장관내시경검사를받고위점막조직검사또는, 신속요소분해효소검사 (rapid urease test, CLO test R ) 로 H. pylori 감염양성을진단받고 PPI, amoxicillin, clarithromycin으로구성된삼제요법을 1주일혹은 2주일받은후요소호기검사 (urea breath test) 로제균후판정을받은환자 976명을전자의무기록을토대로후향적으로조사하였다. 2. 방법 1) H. pylori 양성기준 : 상부위장관내시경을시행후 H. pylori 감염여부를확인하기위해위점막조직검사또는신속요소분해효소검사를시행하였다. 위점막조직검사또는신속요소분해효소검사중어느하나라도양성이나오면 H. pylori 양성으로판정하였다. 위점막조직검사는병변부위또는 H. pylori 감염여부를확인하기위해위축성위염의변화가적은부위에서생검후 hematoxylin & eosin 염색과 Giemsa 염색을시행하여 2명의병리의사가 H. pylori 유무를판정하였다. 신속요소분해효소검사는상부위장관내시경검사를통해위축성위염의변화가적은부위에서한군데생검후 24시간까지변색여부를최종확인후판정하였다. 2) H. pylori 제균요법 : 치료약제는 PPI로 lansoprazole 30 mg, pantoprazole 40 mg 중한가지와 amoxicillin 1,000 mg, Table 1. General Characteristics of 1 Week and 2 Weeks Group 1 week group (n=270) 2 weeks group (n=198) p-value Duration Age (years) < (79.3%) 157 (79.3%) (20.7%) 41 (20.7%) Sex Male 165 (61.1%) 145 (73.2%) Female 105 (38.9%) 53 (26.8%) GU 64 (23.7%) 52 (26.3%) DU 134 (49.6%) 82 (41.4%) CG 72 (26.7%) 64 (32.3%) GU, gastric ulcer; DU, duodenal ulcer; CG, chronic gastritis. clarithromycin 500 mg을하루에 2회씩일주일또는이주일동안투약시켰다. 3) H. pylori 제균판정과제균율판정 : 제균치료 4 6주이후에요소호기검사를시행하여제균후성공여부를판정하였다. 요소호기검사전에 PPI는 2주전, H 2 blocker는 1주전, antacid는 3일전, 항생제는 4주전에끊도록하였다. 제균후판정을위해요소호기검사를시행한환자를대상으로전자의무기록을통해후향적으로분석하여정확한약복용실태를확인하지못하여 intention to treat 분석으로 H. pylori 제균율을조사하였다. 4) 통계분석 : 통계분석은 SPSS, version 12.0 for Windows (SPSS, Inc., Chicago, IL, USA) 을이용하였다. 비연속변수는 χ 2 검정을이용하여비교하였고, 연속변수에대해서는 student t test를이용하였다. 통계유의수준은 p값이 0.05 미만일때통계적으로의미있다고판정하였다. 결과 1. 대상환자의특성 2005년 11월 1일부터 2009년 10월 31일까지이번연구에참여한인원은총 976명이며, 연령범위는 13 79세로평균연령은 48.5세이었고, 남자는 622명 (63.7%), 여자는 354명 (36.3%) 이었다. 상부위장관내시경에서위궤양은 338명 (34.6%), 십이지장궤양은 412명 (42.2%), 만성위염은 226명 (23.2%) 이었다 년 11월 1일부터 2009년 10월 31일까지같은기간동안의 1주일간의삼제요법과 2주일간의삼제요법의대상자특성은성별을제외한나이, 기저질환에따른차이를보이지않았다 (Table 1). 2. 삼제요법의제균율 1) 연도별 7일, 14일제균요법의제균율의변화 : 2005년 11 월 1일부터 2009년 10월 31일까지삼제요법의제균율은 80.4% (785/976) 였다. 최근 4년간 1주일간의삼제요법의제균율은 79.7% (620/778) 였으며연도별 1주일제균율은 년은 82.6% (180/218), 년은 83.4% (242/290), 년은 74.6% (91/122), 년은 72.3% (107/ 148) 였다. 최근 2년간 2007년 11월 1일부터 2009년 10월 31일까지 1주일제균율은 73.3% (198/270) 로 2005년 11월 1일부터 2007년 10월 31일까지 1주일제균율 83.1% (422/508) 보다통계적으로유의하게감소하였다 (p<0.01)(fig. 1). 2007년 11월 1일부터 2009년 10월 31일까지 2주일제균율은 83.3% (165/198) 였고연도별제균율은별다른차이를보이지않았다 (Table 2). 2) 제균기간 (7일, 14일 ) 에따른제균율비교 : 2007년 11월 1일부터 2009년 10월 31일까지 1주일, 2주일제균요법의제균율은각각 73.3% (198/270), 83.3% (165/198) 로 2주일제균요 6 The Korean Journal of Gastrointestinal Endoscopy

Two Week Eradication Rate of Helicobacter pylori according to Year During 2 Years p-value Eradication rate 83.6% (61/73) 83.2% (104/125) 법이 1주일보다통계적으로유의하게높음을알수있었다 (p< 0.01)(Fig. 2). 고찰 H. pylori 감염은만성위염과소화성궤양의원인인자로 H. pylori 제균치료는궤양의완치를유도하고재발을방지하는데있어중요한역할을한다. 1-5 우리나라에서는 H. pylori 감염의 1차치료로 PPI에기초한 amoxicillin과 clarithromycin을투여하는삼제요법이가장널리쓰이고있다. 16,17 PPI와두가지항생제를병용하는삼제요법은현재사용되고있는 H. pylori 제균요법들중비교적제균율이높고부작용이적어 1차치료제로권장되었다. 1990년대초반기에는제균율이 90% 이상으로매우효과적인치료법으로인정받았으나, 최근에는제균율이점차감소하는추세이며, 평균 80% 전후의제균율을보고하고있다 년부터 2005년까지 525명의소화성궤양환자에서일주일간의표준삼제요법에따른제균율의변화를관찰한한연구에서는 1998년 83.7% 에서 2005년 77.6% 로통계적인유의성은없었으나전반적으로감소하는추세에있었으며, 특히 2000년이후의제균율은 80% 미만인것으로나타났다. 17 그러나, 2001년에서 2006년까지의제균율변화에대한다른국내보고에서는제균율의저하가뚜렷이관찰되지않았 다. 19 H. pylori 제균치료에사용하는항생제를다른질환때문에처방하는경우가많아지면서항생제내성률의증가로제균율은감소하고있어국내외적으로이를극복하기위한 2차제균치료, 연속치료, 보조요법등의여러가지치료법이시도되었다 차제균치료의제균율향상을위해 1998년 1차삼제요법의치료기간을 1주에서 2주까지폭넓게제시하였지만현재까지 H. pylori 제균치료기간에대해서는정확한합의가이루어지지않은상태이다. 5 최근아시아, 유럽, 미국등에서 H. pylori 삼제요법 1주제균치료한경우제균율이 1990년대중반의초기제균율에비해저하되고있다는보고들이나오고있고, 제균율향상을위해치료기간의연장이필요한지에대한연구가진행되었다. 25 치료기간을 2주일로연장했을때특히 clarithromycin 내성이비교적낮은국가에서제균율이향상되었다. 18,20 American College of Gastroenterology 가이드라인에서도 7일치료는너무짧다고주장하고있고 10 14일치료를권장하고있다. 26 치료기간이짧아 1차치료에서충분한제균율을얻지못하는것보다는치료기간이길어지더라도충분한제균율을얻는것이적절하다. 24,25 우리나라는의료보험급여기준의제약으로현재 1주일치료가주로처방되고있다. 시기별로지역과인종에따라제균율은다르게나타나므로각지역의 1차치료에서의제균율을주기적으로검토할필요가있다. 지금까지우리나라에서지역별 H. pylori 제균율이다르다는보고는없으며제균요법의제균율의차이를우리나라와같이면적이좁고식생활이비슷하고혈통이같은단일민족의경우지역에따른제균율의차이보다는특정기관별항생제내성률의차이에따른제균율의차이로설명하였다. 27 경기서부단일기관의 1997년 1월부터 2007년 12월까지 1 차삼제요법제균율은유의한감소추세를보였으나 1주치료와 2주치료간의제균성공률이나재활성화율은차이가없음을 Vol. 41, No. 1 July, 2010 (5-9) 7

4 보고하였다. 28 본연구는경기북서부지역의단일기관의 1차삼제요법의최근 2년간의 1주일제균율이 73.3% (198/270) 로 80% 이하로나타나 1차제균요법의 1주일치료가부적합함을알수있었다. 2주일로연장한제균치료는 83.3% (165/198) 로 1주일보다높게나타나 1차삼제요법의 2주일치료의타당성을보여주었다. 본연구의제한점은전자의무기록을통한후향적분석이라제균율에영향을미칠수있는환자의기저질환, 음주, 흡연여부, 약물순응도등은대상환자의일부에서기록부재로분석하지못했다. 약제내성의증가등으로 2주일연장한삼제요법도제균율저하가우려되고치료기간연장에따른약제비상승과복약순응도감소등단점을고려해볼때새로운항생제조합의개발이필요할것으로생각된다. Levofloxacin을포함한삼제요법은 2차제균치료에서 bismuth를포함한 4제요법의제균율과차이는보이지않으며오심, 구토, 복부불쾌감등부작용이적었다. 12 지금까지 1차제균치료의제균율을높이기위한방법으로는 4제요법의도입, 새로운항생제의조합, probiotics 추가, 제균율을높일수있는동반치료약물에대한연구등이제시되고있으나그결과는만족스럽지못하다 본연구의결과에서 PPI, amoxicillin, clarithromycin 1주일제균치료효과가감소하고있고최근제균율이 80% 이하로부적합하여국내보험인정기준인 PPI 포함삼제요법 1주일치료의타당성에대한재검토가이루어져야한다. 아울러본연구결과에서 2주일제균율도 83.3% (165/198) 로높지않아제균치료기간을 2주로연장하는것에대한환자순응도에대한검토가필요하다. 국내에서항생제내성에따른제균율감소에적절한새로운제균요법의개발, 투여기간의결정을위해서는단순하게제균율만고려하지않고치료기간에따른순응도, 비용-효과면, 부작용, 항생제내성등을고려한신뢰성있는연구가필요할것으로생각된다. 결론적으로경기북서부의 PPI, amoxicillin, clarithromycin 삼제요법 1주일제균율은최근 2년간 80% 이하로나타나 1주일제균치료가적합하지않게나타났고같은기간내 2주일치료가 1주일보다통계적으로유의하게높게나타나일차적으로 PPI, amoxicillin, clarithromycin 삼제요법 2주일치료가타당하다. 요약 목적 : 최근 Helicobacter pylori (H. pylori) 1차삼제요법의제균율이감소하는경향을보이고있어치료기간의연장을권고하고있다. 시기별지역과인종에따라제균율의차이를보이고있어시기적절하게그지역의제균율조사가이루어져야하고그결과에따라적절한제균요법을권고해야한다. 경기 북서부지역의단일기관의최근 4년간의 1차제균요법 (proton pump inhibitor (PPI), amoxicillin, clarithromycin) 의제균율을알아보았다. 대상및방법 : 2005년 11월 1일부터 2009년 10월 31일까지단일기관, 대학병원에서 PPI (lansoprazole 60 mg or pantoprazole 80 mg), amoxicillin 2,000 mg, clarithromycin 1,000 mg 삼제요법 2주일과 1주일처방을받은환자들의제균율을조사하였다. 제균후판정은제균치료 4 6주후에 13 C-요소호기검사로하였다. 결과 : PPI, amoxicillin, clarithromycin 제균율은 80.4%(785/ 976) 였다. 1주일제균요법의제균율은 2005년 11월 1일 2007 년 10월 31일동안 83.1%(422/508) 이나, 2007년 11월 1일 2009년 10월 31일은 73.3%(198/270) 으로제균율감소를볼수있었다 (p<0.01). 2007년 11월 1일부터 2009년 10월 31일까지 2주일제균율은 83.3%(165/198) 로 1주일제균율보다통계적으로유의하게높게나타났다 (p<0.01). 결론 : 경기북서부지역의 1차 1주일제균요법의제균율은 73.3%(198/270) 로나타나만족스럽지못하고 2주일로치료기간의연장하였을때제균율이증가해삼제요법 2주처방이바람직하다. 색인단어 : Helicobacter pylori, 제균치료 참고문헌 1.
