Hence, this study was designed to compare the hemodynamic changes during the induction with propofol or etomidate in geriatric patients. Methods: Sixty ASA physical status class I or II geriatric patients, who were scheduled for elective surgery, were randomly assigned to two groups (n = 30 each). Induction was performed with either propofol (2 mg/kg mixed with lidocaine 40 mg, Group P) or etomidate (0.2 mg/kg, Group E). Both groups received a bolus dose of remifentanil (1 g/kg), followed with continuous administration (0.1 g/kg/min). An additional bolus dose (50 g) was repeated, if needed. The systolic, diastolic, mean arterial blood pressure, heart rates and cardiac index were measured before induction (baseline vital signs), after propofol or etomidate administration, before intubation, immediately after intubation and at 1, 3, 5 and 10 minutes after intubation. Results: Patient characteristics and baseline vital signs were similar in both groups. (Anesth Pain Med 2014; 9: ) Key Words: Cardiovascular response, Etomidate, Propofol, Remifentanil, Vigileo-FloTrac device. 서 Propofol은근래임상에서널리사용되고있는정맥마취유도제로지질용해도가크고제거반감기가짧으며부드러운마취유도, 술후회복시간의단축, 오심및구토의빈도감소등의장점을갖고있다 [1]. 그러나 propofol은정맥투여시심한통증을초래하며직접적인심근억제와전신혈관저항을감소시키고압반사기전을변화시켜심박수도감소시킬수있다. 한편, 전신마취시후두경에의한기관내삽관은교감신경계를자극하여혈압과심박수증가, 혈중카테콜아민농도의상승을유발할수있으며, 기존심질환이있는환자에서는심근허혈을유발시킬수있는데 [2], 이러한혈역학적인변화를줄이기위하여마취유도시에보조제로 remifentanil이함께사용되고있다 [3]. 그러나 remifentanil 역시정맥의확장, 심장충만의감소, 심근의억제, 교감신경계의활성저하등으로혈압하강을일으킬수있으며중추신경계매개기전으로서맥을유발시킬수있다. 따라서 propofol과 remifentanil을병행투여할경우저혈압과서맥이발생할수있으며, 이는저혈량증환자나전신상태가불량한환자, 노인환자, 심혈관계질환이나뇌혈관계질환이있는환자등에서는심각한심혈관계허탈을유발할수있다 [4]. 특히노인환자의경우장기기능의저하와자율신경계의변화, 약력학및약동학의변화로이러한심혈관계부작용이심각하게나타날수있다 [5]. 론 179

2 180 Anesth Pain Med Vol. 9, No. 3, 2014 Etomidate는 1964년에처음합성된탄소기가붙어있는이미다졸유도체로, 1972년에임상에소개되어꾸준히사용이증가되었다. 마취유도용량에서심혈관계나호흡계에대한억제작용이경미하며심박수, 심박출량의변화가적어심장예비율이감소된경우나저혈량등이있는환자의마취유도에적합하다고한다 [6-8]. 부작용으로일시적인부신피질기능억제, 정맥염, 불수의적골격근운동, 딸꾹질, 수술후구역과구토등이보고되고있으나 [9], 심혈관계에미치는영향이적어심장수술및심장질환자의마취유도및유지에비교적안전하게사용되어왔다 [8]. 본연구는심혈관계변화가민감할수있는 60세이상 80세미만의노인환자들의경우, 전신마취중후두경에의한기관내삽관으로인한교감신경계의자극을방지하기위하여 remifentanil를보조적으로투여하고자할경우 propofol 과 etomidate 투여시나타나는혈역학적인변화를 FloTrac 압력계 (Edwards Lifesciences Corporation, USA) 와 Vigileo 모니터 (Edwards Lifesciences Corporation, USA) 를이용하여서로비교하였다. 대상및방법이연구는병원내임상연구심의위원회의승인을얻은후환자에게사전에연구의목적과방법을충분히설명하고동의를구한후실시하였다. 전신마취가예정된환자중미국마취과학회신체등급분류 1 또는 2에해당되는 60세이상 80세미만의성인중무작위로 propofol을투여하여마취를유도한군 (group P) 과 etomidate를투여하여마취를유도한군 (group E) 으로나눴으며, 과거력상심혈관질환이있거나술전항고혈압제를복용중인환자 (14명), 수술실입실후마취유도전측정한평균심박수가분당 45회이하이거나 100회이상 (3명), 또는수축기혈압이 180 mmhg 이상이거나 90 mmhg 이하인환자 (4명), 기관내삽관이예상외로어려워기관내삽관을 2회이상시도한환자들 (7명) 을제외시켰다. 본연구팀이시행한 pilot study를근거로해서 (unpublished data) 두그룹의 Cardiac index의차이가 600 L/min/m 2 를 effect size로설정을하고, alpha = 0.05, beta = 0.2 즉 power를 0.8로설정하여계산된표본수 (sample size) 가각군당 28명이었는데, loss를고려하여군당 30명으로정하였다. 마취전처치는 midazolam 3 mg과 glycopyrrolate 0.2 mg을근육주사후, famotidine 0.5 mg을정맥주사하였다. 수술실에도착후환자에게앙와위를취하게하고평균동맥압, 심박수, 경피적산소포화도측정및심전도를감시하였다. 환자의요골동맥에동맥캐뉼라 (20G, mm) 를거치한후 FloTrac 압력계를연결하여 Vigileo 모니터를통해환자의심박출량 (cardiac output) 과심박출계수 (cardiac index, CI) 를측정하였다. 환자를안정시킨후혈압, 심박수, 심박출계수를측정하여마취유도전기준값 (baseline vital signs) 으로하였다. 환자가안정된후분당 5 L의 100% 산소로보조호흡을실시하며마취유도를위하여 remifentanil 1 g/kg 을정주한후에계속해서 0.1 g/kg/min의속도로정주하였으며, remifentanil 투여 1분후 Group P에속하는환자에게 lidocaine 40 mg과 propofol 2.0 mg/kg를, Group E에속하는환자들에게는 etomidate 0.2 mg/kg를각각정주하였다. 환자가구두명령에반응하지않고안검반사가소실된후 rocuronium 0.6 mg/kg를투여하고 100% 산소로보조호흡을실시하다가, 자발호흡이소실되고근이완이충분히된후에기관내삽관을하였다. 삽관전에수축기혈압이 140 mmhg, 또는심박수가분당 120회이상으로증가되어있는경우에는 remifentanil 1 g/kg을추가적으로 1회정주하였다. 기관내삽관후마취유지를위하여흡입산소분율 40% 와 vol% 의 sevoflurane으로유지하였고 remifentanil 0.1 g/kg/min 을지속적으로정주하였다. 각군별로평균동맥압, 심박수, 심박출량, 그리고심박출계수를마취유도전기준치 (baseline vital signs) 와 remifentanil 투여후, 정맥마취제투여후, 기관내삽관직전, 그리고삽관직후, 기관내삽관후 1분, 3분, 5분그리고 10분에각각측정하여비교하였다. 또한각군에서추가적으로 remifentanil을사용한환자수와부정맥발생등을관찰하였다. 모든측정치는평균 ± 표준편차로표시하였으며, 통계분석은 SPSS (version 12.0 SPSS Inc. Chicago, IL, USA) 를이용하였다. 환자군의성별은카이제곱검정을이용하였으며나이, 체중, 마취유도전기준혈압과심박수, 그리고심박출계수에대해서는각각독립표본 T 검정을실시하였다. 두군에서각시점에따른수치의변화에대해서는기준치와각각대응표본 T 검정을실시하였고, 각시점에서두군간의차이를비교하기위하여독립표본 T 검정을실시하였다. 두군에서추가적인 remifentanil의투여여부에대해서는 Fisher s exact test를실시하였다. 각각의검정에서 P값이 0.05보다작은경우에유의한차이가있다고판단하였다. 결과각군의성별, 연령, 체중, 마취유도전기준심박수와혈압, 그리고심박출계수에서통계적으로유의한차이는보이지않았다 (Table 1). Remifentanil과 propofol을투여하여마취를유도한군 (group P) 에서수축기혈압과평균동맥압은정맥마취제투여후부터기관내삽관 10분후까지기준치에비해통계적으로유의한감소가있었으며, 이완기혈압은정맥마취제투여후, 기관내삽관직전, 기관내삽관후 3분, 5분, 10분에유의한감소가있었다. 한편 Remifentanil 과 etomidate로마취유

HR: Heart rate, SBP: Systolic blood pressure, DBP: Diastolic blood pressure, MAP: Mean arterial pressure, CI: Cardiac index. 도를시행한군 (group E) 에서는수축기혈압, 이완기혈압, 평균동맥압이정맥마취제투여후에기준치보다유의한감소를보이다가, 이후부터기관내삽관 3분후까지유의한감소를보이지않았다. 두군을비교하였을때수축기혈압, 이완기혈압, 평균동맥압모두정맥마취제투여후부터기관내삽관후 5분까지통계적으로유의한차이를보였다 (Fig. 1A C). 심박수의변화는두군모두에서정맥마취제투여후기준치보다유의한감소가있었으나기관내삽관후부터기준치와유의한차이를보이지않았다. 두군간의통계적으로유의한차이는없었다 (Fig. 1D). 심박출계수는정맥마취제투여후두군모두에서유의하게감소하였으나, E군에서는기관내삽관이후회복되었지만 P군에서는계속하여유의하게감소하였다. 이러한감소는두군간에도통계적으로유의하였다 (Fig. 1E). 추가적으로 remifentanil 1 g/kg을투여한환자의수는 P군에서 30명중 5명, E군에서 30명중 14명으로두군간에통계적으로유의한차이가났으며 (P = 0.025) 부정맥이발생한환자는없었다. 고 Propofol의용량에따른마취유도성공율은 propofol 1 mg/kg와 2 mg/kg에서각각 17% 와 100% 라고보고되었으며 [10], propofol 1 mg/kg와 2 mg/kg, 그리고 3 mg/kg으로마취유도시에동맥압의감소와마취성공율에대한연구 [11] 에서는 2 mg/kg가평균동맥압의감소가심하지않으면서마취유도성공율이 100% 로보고되어본연구에서는마취유도를위해 propofol은 2 mg/kg을사용하였다. Propofol에의한동맥압의감소는직접적으로말초혈관 찰 저항감소와동시에아직확실히밝혀진바는아니나심장수축력감소로인한심박출량감소에의해일어날것으로추측되며, Larsen 등에의하면 propofol이전부하를감소시켜서동맥압을감소시킨다고보고하기도하였다 [12]. 심박수의변화를보면, propofol은일회박출량, 심박출량보다는전신혈관저항감소에의해혈압을감소시키나심박수에는큰영향이없다고한다. 이러한심박수의안정성은압수용체의재조정에기인한다고생각했으나 [13] 최근연구에서는압수용체반응의저하와뇌간으로부터교감신경성유출의감소에기인한다고보고되었다 [14]. 이러한이유로인해 propofol 이저혈압을일으킬수있지만비교적안정된심박수의유지로심근관류에큰손상없이관상동맥질환에서도사용될수있다고생각되고있다 [15]. Etomidate는작용시간이짧으면서마취유도효과가우수한마취제중하나로현재까지임상에서이용되는정맥마취제중심혈관계에미치는영향이가장적어심장수술및심장질환자의마취유도와유지에비교적안전하게사용되고있다 [8]. Etomidate 0.3 mg/kg의일회정맥주사후 10초이내에의식이소실되고 3 5분간지속되며제거반감기는 시간으로정맥주사후볼수있는빠른의식회복은재분포와간대사에의해일어난다고알려져있다 [6]. Etomidate와 remifentanil의병용에대한연구는 propofol에비해상대적으로보고가적다. Toklu 등에 [16] 의한연구에의하면 remifentanil과함께 etomidate 혹은 propofol을각각병용하여대장내시경을시행하는경우 etomidate를사용한군에서상대적으로안정적인혈역학적반응을보였다고하였다. 본연구의결과에따르면, propofol을투여한환자들의경우 etomidate를투여한환자들에비해정맥마취제투여후부터혈압이유의하게감소하는소견을보였으며이러한소견은기관내삽관후 5분까지지속되었다. 그후에는두군모두에서기준치에비해유의한혈압의감소가있었는데이는주마취제로투여한 sevoflurane의효과와수술전환자혈액부하정도와관련이있는듯하다. Etomidate를투여한환자의경우에는기관내삽관직전에는혈압이감소하였지만기관내삽관후에는상승하여기준치로회복됨을관찰하였다. 심박수의경우에는앞선연구에서밝혀진바와같이 propofol 또는 etomidate를투여한환자들모두에서유의한변화가없었다. 심박출계수는기관내삽관직전및직후에 propofol을투여한환자들에서유의하게낮게측정되었다. Remifentanil은 -수용체에작용하는강력한아편유사제로정맥주사후 30초에서 90초이내에효과가나타나고, 다른아편유사제와는달리조직과혈장내비특이적인 esterase에의해빠르게분해되는특징이있어투여를중단하면 3 5분내에농도가 50% 로감소하여마취회복이신속히일어나게된다 [17,18]. 이와같은약리적인특징때문에기관내삽관시나타나는혈역학적반응을효과적으로차단하면서도삽

P < 0.05 compared with the corresponding values of group E. 관으로인한자극이사라진후에는저혈압이나서맥이지속적으로나타나지않아삽관시혈역학적변동을차단하는데적합한약제의하나로사용되고있다 [19]. 그러나 propofol과함께사용할경우혈압강하현상이심하여승압제를투여하는경우가흔하다. 마취유도시 propofol과병용할때의적절한 remifentanil 용량에대한연구에따르면, remifentanil 0.5 g/kg 또는 1 g/kg을투여후 0.2 g/kg/min 으로지속주입한경우모두기관내삽관에따른혈역학적변화를완화시킬수있었다고한다 [20]. 본연구에서는 propofol을투여하기전에 remifentanil 1 g/kg을정주하였으며, 이후수술을시작하기전까지자극이적음을고려하여 0.1 g/kg/min으로지속적으로주입하면서기관내삽관자극

5 정우석외 4 인 :Comparison of propofol and etomidate 183 에의해심한혈압및심박수의상승이예상되는환자의경우필요에따라 remifentanil 1 g/kg을추가로투여하였다. 결과적으로 E군의환자들이 P군의환자들에비해서정맥마취제투여후상대적으로혈압이높게유지되었으며, 추가적인 remifentanil이요구되는경우가유의하게많았다. 정맥마취제투여후본연구에서심박출계수의측정은 FloTrac 압력계와 Vigileo 모니터를이용하였다. 폐동맥카테터를사용한열희석법은침습적이며시술자체만으로도출혈, 감염, 부정맥등의부작용을유발할수있어, 그사용에있어매우제한적이며, 경흉부심초음파검사도좌심실용적및심박출량의평가에있어서측정자간의오차가있을수있으며, 숙련된평가자의필요로인해일반적인연구에적용하기가어렵다. 임피던스심장도검사방법 (Impedence cardiography, ICG) 은대동맥혈류량의변화에따른교류저항의변화를이용한검사로, 혈역학감시에있어서열희석법을이용하여측정한심박출량과일치도가매우뛰어난것으로보고되고있으며비침습적이라는장점을지니고있다. 본실험에서사용한 Vigileo 모니터를통한측정방법은맥압이일회박출량에비례한다는원리를이용하여심박출량을계산하는장비로, 동맥압의감시가필요한수술에서기존의동맥캐뉼라를사용하면서 FloTrac 압력계만연결하면사용할수있으며사용이간편하다는장점이있다. 그러나폐동맥카테터를이용한방법과비교했을때, 상대적으로높은심박출량수치가나타났다는보고도있다 [21]. 본실험에서 propofol을사용한환자들의경우기준치와비교하였을때정맥마취제투여후부터기관내삽관직후까지심박출계수가유의하게감소하였으며, etomidate를사용한환자들의경우는정맥마취제투여후부터기관내삽관직전까지심박출계수가유의하게감소하였으나기관내삽관후에는기준치에비해유의하게감소하지않았다. 특히두군을비교하였을경우기관내삽관직전과, 직후에 propofol 을사용한군에서심박출계수가유의하게낮게측정되었다. 그러나기관내삽관직후심박출계수의경우, 본실험에서사용한 Vigileo 모니터가 20초간의데이터를수집하여심박출계수를계산한다는점을고려해야하는한계가있다. 또하나의고려할점은마취유도시 propofol 주입시에만통증완화를위해 lidocaine 40 mg을정주하였다는사실이다. Lidocaine은 Na + 펌프억제하여심근의활동에영향을미칠수있어이번연구에서도영향을줄가능성이있으나이전연구에서 lidocaine 40 mg을 propofol과함께주입할경우혈역학적변화에영향이없다고보고된바있다 [22]. 이연구의한계점으로는먼저 propofol과 etomidate의용량을대표용량한군씩으로만설정했다는점으로다양한용량에대한반응을비교해보면더욱정확한비교가될수있을것으로예상된다. 또한본연구에서사용한 FloTrac 압력계를통해측정한혈압이비침습적인커프를이용하여 측정한혈압과차이를보이는경우가있다는점을들수가있다. 본연구에서는기준치를동맥캐뉼라를거치한후에측정하여측정값간의오차를줄이고자하였다. 결론적으로, remifentanil과병용하여마취유도를할때에 propofol보다 etomidate를사용하는것이마취유도시발생할수있는저혈압에의한합병증들을예방하는데에도움이될것으로사료된다. 참고문헌 1.
