We investigated the prevalence of Hepatitis B surface antigen (HBsAg) and Hepatitis B core antibody (anti-hbc) in HIV positive groups (n=430) and HIV negative groups (n=434) from January 2014 to October When comparing the prevalence of anti-hbc between the two groups, HIV positive group (36.0%) showed a higher prevalence than HIV negative group (24.7%), the result was statistically significant (p < 0.05). However, there was a little difference on HBsAg and not significant. Therefore, by showing a clear difference in anti-hbc between the two groups of HIV in Korea, it was confirmed to be associated with co-infections between Hepatitis B and HIV. Key Words: Hepatitis B, HIV, HBsAg, Anti-HBc 전세계적으로 B형간염바이러스 (Hepatitis B Virus, HBV) 는만성감염을일으키는바이러스로환자에게는치명적인질병이다. B형간염바이러스는전세계적으로 2억 4천만명이감염된것으로추정된다 (1). 대부분의 B형간염은사하라이남아프리카와동아시아에서성인인구의 5~10% 정도만성화되어있다. 중부유럽과동유럽의남부지역에서높은유병률을보이며, 유병률은 2~5% 정도이다. B형간염표면항원 (HBsAg) 양성유병률이 8% 이상인지역인아시아, 아프리카및태평양도서지역, 중동지방, 아마존유역에서는출산때수직감염이되거나소아기때가족내감염또는수평감염을통해감염이된다. 이러한사례대부분은감염시기에따라만성화되며, 영아기에감염된경우 80~90%, 6세이전의경우 30~50%, 성인의경우 5% 미만으로만성화된다. 이중성인감염자의 20~30% 는간경화나간암으로발전하게된다 (1). 반면북미나서유럽은 B형간염표면항원양성인구가 2% 미만으로유병률이낮으며, 이와같은지역에서는성접촉, 주사기재사용을통한오염된혈액으로부터의감염또는상처를통한체액감염이며, 주로성인시기에감염된다. 인간면역결핍바이러스 (Human Immunodeficiency Virus, HIV) 는 B형간염바이러스와같은감염경로로성접촉을통하거나, 주사기및수직감염등과같은공통된감염경로를가지고있다. HIV의경우전세계적으로약 3천7백만명의사람들이 HIV에감염된것으로추정되며, 2015년 1백1십만명의사람이 HIV 감염과관련되어사망하였고 2백1십만명이새로이감염되었다. 그중전세계 HIV 감염자의 54% 인, 약 1천9백만명이아프리카동부와이남에집중되어있다 (2). 국내의경우, 질병관리본부의 2015년 HIV/AIDS 신고현황통계연보에따르면, 2015년기준총내국인감염자수는 10,502명이며, 신규감염자수는 1,018명으로, 해마다점진적으로증가추세에있다. 2 284 H-H Lee, et al. 최근 HBV 및 HIV 연구가진전되면서, 이에대해세계각국에서의동시감염사례가지속적으로보고되고있으며, 더불어동시감염은환자의생존율에치명적결과를나타내기에이에다양한연구가필요한때이다. 전세계적으로 HIV와 HBV 동시감염자는 2~4백만명으로추정하고있으며, 아프리카사하라이남지역의경우는 HIV 유병률뿐만아니라, 수직감염을통한 B형간염유병률이높아 HIV와 HBV 동시감염유병률이가장높을것으로추정하고있다. 서유럽및개발도상국의경우수직감염을통한 B형간염의전파보다는성접촉및주사기재사용을통한감염이많다. 미국과서유럽의경우 HIV 양성자에게서 B형간염유병률은 6~14% 로나타났다 (4). 국내의경우 2003년국립보건원에서 HIV 양성환자 200명에대하여, B형간염표면항원 (HBsAg) 을조사한결과동시감염유병률이 7.5% 였다 (4). 이에본연구는 B형간염바이러스와같은감염경로를가지고있는 HIV에대해, HIV 양성자군과음성자군간에 B형간염표면항원 (HBsAg) 및핵심항체 (anti-hbc) 혈청유병률을비교함으로써국내에서의그연관가능성을확인하고자하였다. 시료는 2014년 1월 ~2015년 10월, 경기도내보건소, 병원, 지방병무청, 혈액원및민간검사기관에서의뢰된혈청 864건을 2014~2015년질병관리본부 HIV/AIDS 관리지침에따라, 항체검사에서는입자응집법을이용한 Serodia HIV1/2 kit (Fujirebio Inc, Tokyo, Japan) 과 Enzyme Linked Fluorescent Assay (ELFA) 원리를이용한 VIDAS HIV DUO Ultra kit (biomeriux, Marcy-l'Etoile, France) 으로 minividas (biomeriux) 장비를이용하여그결과를확인하였다. 참고로 ELFA는 Enzyme Linked Immunosorbent Assay (ELISA) 와같은원리인항원항체반응을이용하며, 반응시흡광도가아닌형광이발현되어검출하는방법으로 ELISA보다감도가높은특징이있다. 이와같은방법으로 HIV 항원 (P24) 검사를위해 VIDAS P24 kit (biomeriux) 을이용하여, 같은장비로분석하였다. 마지막으로 HIV blot 2.2 (Medical Technology Promedt Consulting GmbH, Ingbert, Germany) 를사용하여, 웨스턴블럿결과로최종확진하였다. 이후 HIV 양성으로판정된 430건혈청그룹과음성으로판정된 434건혈청그룹을대상으로, 혈청학적 B형간염표지자는현재 B형간염바이러스의직접적확인지표인 B형간염표면항원 (HBsAg) 과감염후평생지속되는항체지표로 B형간염핵심항체 (anti-hbc) 를선택하였으며, 이를검출하기위하여, 각각 VIDAS HBsAg Ultra kit (biomeriux) 과 VIDAS anti-hbc Total II kit (biomeriux) 로역시위와같은원리 (ELFA) 를이용, 같은장비로결과를확인하였으며, 통계처리는 SPSS를이용하여분석하였다. 각항목에따라백분율을구하였고변수간의유의성검증은 χ 2 -test를실시하였으며, probability value는 p < 0.05 수준에서해석하였다. HIV 검사완료된혈청 864 샘플중 HIV 항체양성으로분석된 430건과음성으로분석된 434건에대하여, B형간염표면항원과핵심항체에대하여각각혈청유병률을조사한결과, 항원의경우 HIV 양성자그룹 430건중 16건 (3.7%) 이며, 음성자그룹 434건중 10건 (2.3%) 이양성으로, 두그룹에서근소한차이가있었으나, 통계적으로유의하지는않았다. B형간염항원 (HBsAg) 은급성 B형간염이나만성감염자를검출하는데가장많이사용하는지표이다. 이것은노출된후빠르면 1~2주, 늦으면 11~12주에검출이가능하다. 이항원이존재하면감염형태가급성혹은만성간염이든상관없이타인으로감염력이있음을의미한다. HIV 양성자에서의 B형간염바이러스표면항원은국내 2003년국립보건원결과 7.5% 로, 12년이지난지금은예전에비해절반가까이 B형간염표면항원유병률이줄어들었음을확인할수있었다. 그이유는 1995년이후 B형간염백신국가예방접종에포함되면서, 이후 B형간염표면항원유병률이 3% 이하로내려가면서그룹간큰유의적차이를보이기어려운것이큰원인일것으로추정 Table 1. 4 286 H-H Lee, et al. 미국의경우 2016년질병통제본부 (CDC) 에따르면 HIV 양성자에게서 B형간염항원유병률을 10% 로추정하고있다. 그밖에중국은 16.22%, 태국은 11.9% 아프리카말라위 20.4%, 에티오피아 22.2% 로높은편이다. 특히마약투약자의경우 HIV, HBV 및 HCV(C 형간염바이러스 ) 동시감염자가 40% 로매우높다 (5~10). 반면 B형간염핵심항체유병률은 HIV 양성자그룹에서는 155건 (36.0%), 음성자그룹에서는 107건 (24.7%) 로각그룹간 11.3% 의유의적차이를확인하였다 (p < 0.05). B형간염핵심항체는 (anti-hbc) 는모든 B형간염바이러스감염에서형성되는데, 급성질환의혈청에서 HBsAg 가생성된다음나타나며, 일반적으로평생지속된다. 따라서과거에인지하지못했던 HBV 감염을의미하기도한다. 또한 anti-hbc 는바이러스감염후에만생기기때문에백신에의한면역으로형성되지않는다 (Fig. 1). Anti-HBc 의유병률에대해다른국가의상황을살펴보면, 남미의경우칠레 HIV 양성자에게서 42.7%, 브라질 29.3%, 아프리카의경우수단 26.8%, 르완다 42.9%, 아시아의경우인도 33.9%, 중국 36.5% 이다 (11~16). B형간염표면항원및핵심항체결과조합을통한, 분석에서는 HBsAg(+) anti-hbc(+) 의경우 HIV 양성자그룹 3.7%, HIV 음성자그룹 2.3% 로작은차이는있었으나유의적이지않았다. HBsAg(-) anti-hbc(+) 의경우는각각 32.3%, 22.4% 차이가났으며, HBsAg(-) anti-hbc(-) 인경우각각 64.0%, 75.3% 로역시유의적차이를확인하였다 (p < 0.05). 반면 HBsAg(+) anti-hbc(-) 인경우즉항원은있으나항체가형성되지않은, 항체미형성기간에해당하는시료는없었다 (Fig. 2). 결국 B형간염표면항원에대해두그룹간유의적인차이를보이지않았을지라도, 국내에서 B형간염핵심항체의혈청보유율에대해서두그룹간유의적차이가있음을처음으로확인하였고, 그차이는 HIV와 B형간염이공통된감염경로인성접촉, 수직감염및수혈등일것으로추정되고, 특히국내 HIV 감염자의경우대부분이성접촉으로인한감염인 (64.0%) 것과 2013년질병관리본부전국예방접종률조사결과, 국내소아 B형간염예방접종률이 99.4% 이상인것을생각해보면, 현재수직감염에의한 B형간염감염기회는줄어든반면성접촉을통한기회가많아진것으로추정된다. 참고로국내 B형간염표면항원의시대에따른유병률조사결과를보면, B형간염백신이사용되기이전인 1970년대및 1980년대초는전인구의 7~8%, 가임기산모연령층은 5%, 학동기연령층은 4.8% 이었으나, 1983 년에 B형간염백신이우리나라에처음도입되고 1995년부터국가예방접종사업영유아대상정기예방접종이실시된이래로 B형간염표면항원은지속적으로감소되고있음을확인할수있다 (Table 1) 년에시행된국민건강 영양조사결과에의하면 10세이상인구의 B형간염항원양성률은남자 3.4%, 여자 2.6% 로현저히감소되었다. 최근본연구의 HIV 음성그룹 434건에서의 B형간염표면항원유병률 2.3% 을확인하여, 지속적으로감소되고있음을알수있다 (21, 23~28). 이와같이국내 B형간염백신접종률 (99.4%) 이아주높은가운데, B형간염표면항원을이용한 HIV 그룹간차이는확인할수없었지만, B형간염핵심항체에대한두그룹간의차이를확인할수있었고, 국내 HIV의양성자그룹의행동특성을생각해보면수혈, 주사기공동사용등의감염보다는안전하지않은성접촉의비중이높을것으로추정된다. 아쉬운점은분석한그룹의나이, 성별, 직업, 소득, 지역및성 ( 性 ) 기호도등과같은기타정보를확인할수있었으면, 좀더다양하고유익한예방의학적자료가만들어질수있을것으로생각되나, 국내후천성면역결핍증예방법에따라개인정보는익명으로관리되어추가적인정보를확인할수없었다.
