Based on TRUS staging, decisions regarding treatment strategy would be based on whether local excision of the tumor is feasible or not. TRUS can differentiate between the different layers of the rectal wall. In recent years, it has become the diagnostic modality of choice for preoperative staging of rectal tumors. The ability to image tumor invasion through the different layers of the rectal wall led to development of an ultrasonographic staging system, whose degrees of invasion correspond to the pathologic stages of the TNM classification. The reported accuracy of TRUS for rectal cancer staging in evaluation of depth of invasion varied from 69% to 91% and accuracy in evaluation of lymph node metastasis varied from 61% to 84%. Although the accuracy of restaging after preoperative chemoradiation of rectal cancer showed a decrease up to 40%, it provides a large amount of useful information in terms of the choice of treatment modality in rectal cancer. We should understand the nature of TRUS and its limitation in diagnosis of rectal cancer. Keywords: Transrectal ultrasonography, Rectal cancer, Staging, Re-staging, Limitation 서 론 정상초음파해부학을중심으로기술하고자한다. 직장항문질환의진단을위해서는전통적으로직장수지검사가널리사용되어왔다. 그러나직장수지검사는검사자의경험에의존하게되며, 비록정확도는높게보고되고있으나객관성이떨어지는단점이있다. 경직장또는경항문초음파검사 (transrectal or transanal ultrasonography) 는다른초음파검사와마찬가지로검사자중심의검사방법이지만직장암의병기결정뿐아니라변실금, 치루와같은양성항문질환의진단에중요한역할을하며, 치료방침결정에도도움을주는유용한검사방법이다. 본문에서는경직장초음파검사법의실제적인적용과 본론 1. 검사방법경직장초음파혹은경항문초음파검사를위해서는먼저검사전 30분에관장을실시하여대변이나분비물을제거해야정확한영상을얻을수있다. 직장암의경우장폐쇄정도에따라관장이불충분한경우가있으며, 이때에는추가관장이필요하다. 경항문초음파검사의경우에는관장이반드시필요한것은아니다. 환자는관장후좌측위 (Left Lateral Position) 로누운 Journal of Surgical Ultrasound is an Open Access Journal. 2 J Surg Ultrasound Vol. 1, No. 1, 2014 상태에서검사하게된다. 경직장혹은경항문초음파검사를실시하기전에반드시직장수지검사를통해직장의주행방향과종양의위치등을검사자가먼저파악하는것이필요하다. 경직장초음파검사는직장수지검사후직장경검사 (proctoscopy) 를통해직장암의위치및모양, 직장폐쇄정도등을관찰한뒤직장경을직장암의상부까지전진시킨다. 직장암의병기결정을위해직장암의상부부터검사하는것이중요한데, 직장암의침윤정도는일정하지않은것이대부분이어서가장깊이침윤된곳을초음파검사를통해서찾는것이중요하기때문이다. 또한직장암은상부의하장간동맥 (Inferior mesenteric artery) 방향으로림프절전이가생기므로종양상방에서림프절전이여부를판단하는것이필요하다. 일부폐쇄를동반한직장암의경우에는검사를중단하거나경항문초음파검사에사용되는기구로교체한후검사하는것이좋은데, 광원 (light source) 이포함되어있는 ESI 직장경 (Electrical Surgical Instrument, Rochester, NY) 을사용한다. 직장경의총길이는 20 cm이며초음파탐침자 (BK 1850 probe) 를직장경을통해통과시킨후직장경을후방으로당겨서초음파탐침자의물을주입하는주입구 (spigot) 에닿게하면초음파검사영역이항문연상방의어느위치인지를정확히알수있다. 즉초음파탐침자는직장경을통해삽입하였을때 7 cm 상방을스캔하게되므로직장경에표시된높이에 7 cm를더하면직장암의위치가어디인지를알수있게된다. 직장경이직장암을통과한후직장경속으로초음파탐침자를삽입하여끝까지밀어넣은후물주입구를상방으로향하게한후직장경을뒤로당겨물주입구에닿게한후주입구를통해 cc 정도의증류수를주입한다. 검사를하는동안초음파탐침자가직장벽의정중앙에위치하도록유지하는것이좋은영상을얻는데필수적이다.(1) 3차원입체영상을얻을수있도록고안된경직장초음파탐침자 (BK 3D 2052 probe) 는직장경을삽입한후끝부분의길이가 6 cm이며, 주입구를아래로하여야 12시방향이앞쪽인영상을얻을수있다. 정확한초음파검사를위해서는대변이나직장내의점액을제거하는것이중요하며, 주입하는증류수도가능하면공기제거증류수 (de-gased distilled water) 를사용하면공기방울로인한초음파영상잡음을줄일수있다. 2. 정상직장벽과직장주위지방 (perirectal fat) 의해부학 정상직장벽은 5층으로구별할수있는데, 고해상도의초음파검사로는 7층까지구별할수있다는주장도있다. 연구자에따라초음파로보이는각층의구조에대한이견은있으나 Beynon 등 (2) 이제안한모델이가장많이이용되며, 각층의해부학적구조는다음과같다 (Fig. 1). Inner white layer: interface between balloon and mucosa Inner dark or hypoechoic layer: mucosa or muscularis mucosae Middle white layer: submucosa Outer black layer: muscularis propria Outer white layer: interface with perirectal fat 중간의흰색선 (Middle white layer) 은점막하층을의미하며조기직장암의병기결정에가장중요한층이므로, 만약종양이중간의흰색선을침범했다면 ut1으로판독한다. 직장의융모종양 (villous tumor) 의경우점막하층으로침윤되는경우는드문데 inner dark layer 가확장되면 ut0로판독하고, 만약종양이 outer black layer 까지침윤했다면 ut2로판독하며, outer white layer의침범이확인되면 ut3 로판독한다. 또한, 종양이주변의장기 ( 전립 Fig. 1. 4 J Surg Ultrasound Vol. 1, No. 1, 2014 직장암의치료방침결정에도움을준다. 경직장초음파검사의직장암의침윤도에대한정확도는 69-91% 로보고되고있으며,(3-9) 림프절전이의정확도는 61-84% 로보고되고있다.(3,4,9,10) 특히조기직장암의직장벽침윤도결정은자기공명영상이나컴퓨터단층촬영검사에비해우수하므로환자의치료방침설정에매우중요한검사법으로외과의사가반드시시행할수있어야할것으로생각된다. 4. 경직장초음파검사의제한점경직장초음파검사가직장벽의해부학적층별구조를가장잘보여줄수있는검사지만, 진행성직장암으로직장벽을둥글게침윤한경우 (encircling mass) 에는초음파탐침자를직장암상방으로진행할수없으므로정확한검사를진행할수없다. 조기직장암의경우, ut1과 ut2 종양의감별이매우중요하지만미네소타대학의연구결과에의하면 ut1 종양의정확도는 47%, ut2종양은 68% 에불과하다.(3) 그러므로경직장초음파검사에근거하여경항문국소절제술을시행한경우에는반드시조직검사의결과를확인해야하며, 결과에따른추가적인치료가필요한지판단해야한다. 그러나 ut2 종양은경항문국소절제술의적응증이아니므로임상적용에는문제가없을것으로생각된다. 최근진행성직장암에서수술전방사선화학요법이많이시행되고있으며, 방사선화학요법후경직장초음파를통한병기결정의정확도는 40% 이하로보고되고있는데, 염증작용과종양을구별할수없는단점으로인해과병기화 (over-staging) 되는경향이있다.(11) 다른초음파검사와마찬가지로검사자의주관적인판단이많이개입되는검사방법이므로일정한기간의학습기간이필요하며, 경험이축적되면검사의정확도도높아진다. 5. 3차원초음파검사최근들어 3차원경직장초음파및경항문초음파검사가가능하게되었다. 3차원초음파검사는일반적인 2차원초음파검사에비해항문과직장주변을 3차원적으로영상을재구성하여볼수있으므로직장암의병기결정에도움을줄수있으며내괄약근의손상길이를측정가능하고, 복합치루의경우치루의경로를탐색할수있는장점이있다.(12) 결 론 경직장초음파검사는직장수지검사를대체할수있는중요한검사법으로써직장암의병기진단을정확하게할수있으며, 이를통한치료방법결정에도움을준다. 그러나검사자에따라검사결과의판독이달라질수있는주관적인검사법이며, 또한수술전방사선항암화학요법후병기결정을할때에는과병기화되는경향이있으므로주의하여검사를하여야한다.
