Clozapine is underutilized in the management of treatment-resistant schizophrenia. To understand contributing factors, we analyzed the time course and causes of clozapine discontinuations that occurred over a 20-year period in a clinical setting. Methods : The reasons for discontinuation and duration of clozapine treatment from a retrospective database of 138 patients with schizophrenia who had prescribed clozapine at least a month were reviewed, with the motives for discontinuation coded. The causes for termination were analyzed. Results : Over two-thirds of the patient had ceased clozapine. The two most common causes for discontinuation were side-effects (50%), and own decision (30%). Somnolence accounted for 34% of all side-effects induced discontinuations. Hematological problems accounted for 23% of side-effect. The Maximal treatment dose of clozapine was higher in continuation group ( mg) than in discontinuation group ( mg). The CGI-S score when prescribing clozapine last was higher in discontinuation group than in continuous group. The patients who took atypical antipsychotics before clozapine tended to cease clozapine because of side-effects than who took typical agent. Conclusion : Future studies should seek various methods to relieve side-effects of clozapine. Prospective researches using more objective tools are needed to clarify the reason for clozapine discontinuation. (Korean J Schizophr Res 2016;19:17-24) Key Words : Schizophrenia Clozapine Antipsychotics Treatment discontinuation Side-Effect. 서 론 약 1% 의평생유병률이보이는조현병환자중약 30% 는 1 세대혹은 2 세대항정신병약물치료에반응을보이지않는 다. 1) 기존의항정신병약물에효과가없는치료저항성조현병 환자에서클로자핀 (clozapine) 은양성증상및음성증상의 유의한호전을가져오며, 2-6) 추체외로부작용, 지연성운동장 애, 고프로락틴혈증과같은항정신병약물에의한부작용이 매우적다. 2,7) 2009 년에진행된대규모의환자를대상으로한 연구결과, 클로자핀사용을지속할때입원율이감소하고, Received: February 3, 2016 / Revised: April 20, 2016 Accepted: May 23, 2016 Address for correspondence: Chul-Eung Kim, Department of Psychiatry, Inha University Hospital, 27 Inhang-ro, Jung-gu, Incheon 22332, Korea Tel: , Fax: kce320@inha.ac.kr 강제적인치료과정이줄어든것이관찰되었다. 8) 또한, 클로자핀을사용하는환자는, 다른항정신병약물을사용하는환자에비해독립적이며, 직업활동을하고있는경우가많았다. 9) 이밖에도수많은연구에서, 자살률및공격적인행동에효과를나타내는것이입증되었다. 10) 클로자핀은뛰어난장점에도불구하고, 여러가지부작용을가지고있어처방이지연되고, 11,12) 충분히사용되지않고있다. 13) 클로자핀은심한졸림, 저혈압, 고용량사용시경련유발, 대사부작용등과밀접하게연관되어있다 ) 또한, 1% 이하로드물지만치명적인부작용인무과립구혈증의가능성이있어서, 주기적인피검사를통한혈액학적모니터링을해야한다. 16) 이러한부작용과번거로운검사들이클로자핀중단율을높이며, 임상가들로하여금클로자핀처방을꺼리게한다. 17) 환자의 50% 가량이클로자핀의높은부작용에부담을느껴 Copyright 2016 Korean Society for Schizophrenia Research 17

2 치료에잘따르지않는것으로보고된바있다. 18,19) 하지만기존연구는대부분 1년이하의연구이므로 20) 실제임상현장에서클로자핀중단과관련된요인들을보여줄수없다는한계가있다. 이에따라, 저자들은일대학병원에서 20년간의의무기록을후향적으로조사해서, 클로자핀의중단시점과그이유를분석해보고자한다. 연구방법 연구대상 1996년 5월부터 2015년 6월까지인하대병원정신건강의학과에서클로자핀을처방받은외래및입원환자 243명의의무기록을분석하였다. 이중, 조현병으로진단받고, 최소 1개월이상클로자핀을투여받은환자만을연구대상으로하였다. 클로자핀을처방받은 243명중, 48명은진단상의이유로, 56명은클로자핀처방기간이 1개월미만이어서, 2명은의무기록이미흡하여제외하였다. 최종대상환자는 137명이었으며이들중남자는 73명여자는 64명이었다. 발병평균연령은 22.3세, 클로자핀을처방받기까지걸린시간은평균 7.2년이었다. 의무기록분석종료시점인 2015년 6월을기준으로클로자핀을계속해서사용하고있는경우를클로자핀유지군으로정의하면, 잠재적치료중단군이클로자핀유지군에포함되어바이어스 (bias) 가발생할수있어, 조사종료시점을기준으로이전 2년이상클로자핀을처방받아온대상자들을클로자핀유지군 ( 이하유지군 ) 으로정의하였다. 어떠한부작용이었는지조사하였다. 클로자핀중단원인은기존연구들을참고하여 6개의범주로나누어조사하였다 ; 약물부작용, 환자의결정, 전원, 치료실패, 임신, 사망이범주에대한정의는표 1에서술하였다. 9,17,20) 클로자핀을마지막으로처방하였을때, 부작용을호소한대상자를약물부작용을나타낸것으로분류하였다. 부작용호소가없었으나환자가중단을요청하거나전원한경우에는환자의결정또는전원으로, 클로자핀사용해도증상이지속되어의료진의결정에의해클로자핀을중단한경우를치료실패로정의하였다. 중단원인이부작용에해당되는경우에, 8가지원인으로나누어추가로분류하였다 ; 혈액학적이상, 졸림, 경련, 심혈관계이상, 자율신경계이상 ( 어지럼증, 침분비과다 ), 소화기계문제 ( 구역, 변비, 간수치이상 ), 신경학적이상, 기타. 자료분석클로자핀유지군과중단군사이의사회인구학적요인과임상적요인의차이는독립표본 t 검정 (independent samples t-test) 과카이제곱검정 (Chi-square test) 을통해검증하였다. 또한중단원인및그중단원인이약물부작용일때, 각각클로자핀사용기간에따라어떠한차이를나타내는지분석하였다. 클로자핀으로치료를받는동안클로자핀중단이얼마나발생되었는지분석하기위하여 Kaplan-Meier 생존분석을실시하였다. SPSS window용 version 21.0을이용하여자료를분석하였다. 모든통계적유의수준은양측검정에서 0.05 미만으로하였다. 방법연구대상의성별, 학력, 결혼상태, 동거유무, 직업유무, 경제상태, 동반사용된항정신병약물, 클로자핀의용량및투여기간등인구학적및임상적특성은정신건강의학과의사와간호사가작성한진료기록및간호정보조사지를검토하여후향적으로이루어졌다. 또한클로자핀을중단한경우, 중단사유에대해분석하고그사유가약물부작용인경우, 결과 사회인구학적특징의무기록분석종료시점인 2015년 6월을기준으로클로자핀을계속해서사용하고있는대상자의수는 41명이고, 중간에중단한대상자의수는 96명이었다. 클로자핀유지군의평균사용기간은 126.5개월이었다. 중단군과유지군의사회 Table 1. 3 조진우등 인구학적특징에는통계적으로유의한차이가없었다 ( 표 2). 조사종료시점에여타의항정신병약물이클로자핀과함께처방되었고, 클로자핀을처방받은기간의 50% 이상동반해서사용된경우를병용요법으로보고, 이를분석하여클로자핀중단에영향을주었을가능성에대해고려하였다. 단, 항정신병약물을클로자핀으로교체하는경우는제외하였다. 유지군의 68% 에서클로자핀과병용한항정신병약물이있었으나, 중단군에서는 37% 만이항정신병약물을동반사용하였다 ( 표 2). 유지군에서 13명 (32%) 이 aripiprazole을병용하였으며, quetiapine, amisulpride, risperidone 을병용한경우가각각 3명 (7%) 이었다. 그외에도 blonanserine, olanzapine, paliperidone, paliperidone palmitate 등이동반사용되었다. 클로자핀중단까지걸리는기간초기 12개월동안 33% 의환자가클로자핀을중단하였다. 50% 의환자가클로자핀을중단하게되는기간은 43개월로, 클로자핀을처방받은환자들의절반이상이 4년안에클로자핀을중단하였다. 10년이상클로자핀을계속유지하는환자수는 35% 였다 ( 그림 1). 클로자핀중단원인과약물부작용클로자핀중단원인의결과는다음과같다. 약물부작용으로인해중단하게되는경우가 50% 에해당하였고, 그다음 1-proportion of patients discontinuation Time (month) Fig. 1. 4 으로환자자신의결정 (30%), 전원 (15%), 치료에대한반응미흡 (3%) 이뒤따랐다. 임신및사망으로인한약물중단이각각 1례 (1%) 가있었다. 클로자핀중단에이르게한부작용으로는졸림 (34%, 전체중단가운데 16%) 이가장흔했다. 백혈구감소증, 호중구감소증등혈액학적이상 (23%), 자율신경계이상 (17%), 소화기계증상 (8%), 신경학적이상 (6%), 경련 (4%), 심혈관계이상 (4%) 등의순서로나타났다. 기타는 4% 였다. 클로자핀사용기간별중단원인과부작용의관계클로자핀사용중단까지걸리는기간을 1~12 개월, 13~43 개월, 44개월이상으로나누어분석하였다 ( 그림 2). 모든구간에서, 약물의부작용에따른중단이가장많았다. 특히 12 개월이전이 23명으로, 투약초반에상당수의환자들이약물 25 부작용때문에클로자핀을중단했다. 클로자핀중단원인이부작용인경우에, 위의구간별로나누어각각의부작용을조사하였다 ( 그림 3). 8가지항목을재분류하여, 중추신경계이상 ( 졸림 + 경련 ), 혈액학적이상, 자율신경계이상, 신경학적이상, 기타 ( 심혈관계이상 + 소화기계증상 + 기타 ) 항목 5가지로묶어서분석하였다. 12개월이전에졸림등중추신경계이상 (13명) 으로중단하는경우가가장많았으며혈액학적이상으로인한중단은구간에상관없이비교적일정한비율을보였다. 클로자핀을중단한군과유지하는군의차이클로자핀을중단한군과그렇지않은군에서, 어떠한임상적차이가있는지를조사하였다 ( 표 3). 발병연령, 클로자핀 months months >43 months months months >43 months Hematilogical CNS Neurological Other Death Hematilogical CNS Neurological Other Fig. 2. Clinical characteristics of the subjects Total Side effect, n (%) Other reason, n (%) χ 2 p-value Number of previous antipsychotics (40.7) 16 (59.3) (58.5) 17 (41.5) (50.0) 8 (50.0) (42.9) 4 (57.1) (33.3) 2 (66.7) (100.0) 0 (0) Previous antipsychotics Typical Antipsychotics 20 4 (20.0) 16 (80.0) <0.001 Atypical Antipsychotics (68.0) 16 (32.0) 시작연령, 클로자핀사용전후입원횟수에는차이가없었다. 클로자핀최대용량을 300 mg 이상사용한대상자만을비교하였을때, 클로자핀최고용량은유지군이 mg, 중단군이 mg으로유의한차이를보였으나, 클로자핀중단시처방용량의차이는보이지않았다. 전반적임상인상심각도 (Clinical Global Impression scale-severity : CGI- Severity) 를사용하여측정한클로자핀시작당시의증상심각도는중단군과그렇지않은군의차이는없었으나마지막클로자핀처방시평가한증상심각도는중단군이유지군에비해유의하게높았다. 부작용으로인해중단한군과다른이유로인해중단한군의차이클로자핀의부작용으로약물중단한군과다른이유로중단한군의임상적차이를분석하였다 ( 표 4). 클로자핀이전에비정형항정신병약물을처방받았던경우, 부작용에의한중단 (68%) 이많았으며, 정형항정신병약물을사용했던군은부작용에의한중단 (20%) 이그밖의다른이유로인한중단 (80%) 에비해적었다. 고찰 클로자핀을처방받은환자중 6개월이내에클로자핀을중단한비율은 23%, 12개월이내에클로자핀을중단한비율은 33% 였다. Pai 등은 6개월안에 50%, 12개월에는 66% 가중단한것으로보고하였다. 9) 그러나, 그에앞서시행된연구에따르면치료시작 1년안에 35% 환자가클로자핀을중단한것이관찰되어, 본연구와비슷한결과를보였다. 21) 가장최근발표된한논문에서는 1년내에클로자핀을중단한비율이 24% 로나타나기도하였다. 17) 이러한중단율은클로자핀외의다른항정신병약물연구 에서밝혀진중단율에비해낮은것으로생각된다 명의만성조현병환자를대상으로진행된대규모연구에서, 클로자핀을제외한비전형항정신병약물및 perphenazine 을임의로처방받은대상자의 74% 가 18개월안에치료를중단하였다. 22) 모든종류의비전형항정신병약물연구를조사한 meta 분석에서, 7~12 개월사이에중단된비율이 38% 였다. 23) 또한초발조현병환자 498명을대상으로한 olanzapine, haloperidol 약물비교연구에서도 12개월내에중단된비율이각각 33%, 72% 로서본연구에비해비슷하거나높은결과를나타냈다. 24) 초발환자들은치료반응이높은것으로알려져있기때문에, 25) 대상자의다수가만성환자인본연구의클로자핀중단비율이상대적으로더낮다고생각할수있다. 이전연구결과 26-29) 와마찬가지로, 클로자핀투여초반에중단되는경우가상당수였다. 그리고 5년가량클로자핀을처방받고있는경우, 대부분이 10년이상투약을지속하는것으로나타났다. 본연구에서약물부작용에의한중단은 50% 로나타났는데, 이전연구에서약물부작용에의한중단비율은 5% 30) 에서 50% 이상 31,32) 까지다양하게보고되었다. 환자의결정에의해중단된비율은 30% 로, 다른연구에서25~50% 를나타낸것에비교하면, 비교적낮은수치이다. 9,33,34) 약물부작용에의한중단과환자의결정에의한중단은각연구마다상당한차이가있었다. 약물을중단하고자환자가결정을내릴때에는약물부작용에의해내리게되는경우가매우많아, 약물부작용 과 환자의결정 을구분하는것은어려운문제이다. 더구나의무기록에의존해야하는후향적연구의특성상더욱그러하다. 이는기존의자연적인연구 (naturalistic study) 에서반복적으로제기되는한계점이다. 17) 또한기존연구에서약물중단의이유를지나치게환자의결정으로돌리려한다는지적이있어왔다. 35,36) 따라서환자의결정과약물부작용에의한클로자핀의중단을보다명확하게알수있는 Korean J Schizophr Res 2016;19:

6 대규모의전향적연구가필요하다. 치료실패로인한클로자핀의중단이 3% 로매우낮았는데, Kane 등은치료저항성조현병환자의 70% 에서, 클로자핀에반응을보이지않았다고보고하였다. 37) 실제임상현장에서는치료반응이부족하여도클로자핀이외의다른항정신병약물로변경하거나중단하는것이매우어렵기때문에클로자핀을중단하지못하고사용하는경우가상당수존재한다. 이러한상황을고려할때, 본연구에서의치료실패로인한중단비율은매우적은결과를보인것으로생각할수있다. 실제로이전에발표된여러클로자핀중단연구들은, 치료실패에따른약물중단을약 3% 로본연구와비슷한결과를보고하였다. 9,17,27) 약물부작용으로인한중단가운데는졸림 (34%) 이가장흔한이유로밝혀졌다. 다른연구에서는졸림항목을따로조사하지않은경우가많았다. 이에대해고찰해보면, 졸림은향정신성약물을복용하는사람들이흔히불편감을느끼는항목이다. 이러한차이는, 졸림을 환자의결정 으로볼것인가, 아니면 약물부작용 항목에포함시켜야할것인가에대한연구자의관점차이로생각된다. 혈액학적이상에의한약물중단은전체중단원인의 12% 였다 ( 부작용의 23%). 여러연구에서, 혈액학적이상으로인해클로자핀을시작한지 6개월이내에중단하는비율이높다고보고하였다. 21) Young 등에의하면, 95% 의혈액학적이상에따른약물중단이 6개월이전에일어난다고하였다. 38) 그러나이번연구에서는혈액학적이상으로인한중단비율이구간에상관없이비교적일정했다. 클로자핀을사용한지 5년이상된환자에서지연성무과립구혈증이나범호중구감소증이발생한증례가상당수보고되어있다. 따라서, 수년간클로자핀에의한혈액학적부작용이없었더라도모니터링을소홀히하면치명적인결과를가져올수있으므로유의해야한다. 클로자핀최대용량을 300 mg 이상사용한대상자만을비교하였을때, 클로자핀최고용량은유지군이 mg, 중단군이 mg으로유의한차이를보였으나, 클로자핀중단시처방용량에는차이가없었다. 클로자핀의혈중농도를분석하지않았기때문에클로자핀이조현병환자개개인의치료농도까지충분히쓰였는지는분명치않다. 그러나유지군의마지막처방시전반적임상인상척도점수가중단군에비해낮은것을고려해볼때, 유지군에서클로자핀에의해증상이더많이호전되었고이는최고용량의차이에서기인했다고추정해볼수있다. 한편, 기존에사용하던항정신병약물의종류에따라중단 하는이유가다른것이관찰되었다. 비정형항정신병약물을사용하다가새로클로자핀을처방받은환자들은약물부작용에의한중단 (68%) 이, 그밖의다른이유로인한중단 (32%) 에비해높았다. 반면에정형항정신병약물을사용한환자들은약물부작용으로인한중단 (20%) 이적고, 다른이유로인한중단 (80%) 이많았다. 정형항정신병약물은비정형항정신병약물에비해부작용이많아 39) 정형항정신병약물을처방받다가클로자핀으로교체한군이부작용에대한불편감이적어클로자핀중단에영향을미쳤을수있다. 40) 유지군의 68% 에서클로자핀과병용한항정신병약물이있었으나, 중단군에서는 37% 만이병용요법을사용하였다. 병용요법은단일요법에비해치료효과를향상시키며, 이는치료실패로인한약물중단의가능성을줄인다. 41) 또한유지군에서병용요법을사용한경우의 32% 가 aripiprazole 을사용하였는데, aripiprazole 은약물기전상클로자핀의부작용을감소시키는효과를나타낼수있다. Aripiprazole 은 D2 부분효현제로서, 클로자핀의작용결과로인해발생한저도파민성환경을개선시켜서부작용을줄인다. 42) 클로자핀과 aripiprazole 병용요법연구에서, 병용요법군이단일요법군에비해추체외로부작용이현저히적게관찰되기도하였다. 43) 이러한부작용감소가클로자핀유지와연관이있을가능성이있으며, 이에대한연구가더필요하다. 본연구는몇가지제한점을지닌다. 첫째, 후향적연구로중단의이유가불분명한경우가다소포함되었다. 가령, 약물에대한부작용을호소하다가갑자기추적관찰이안되는경우, 이를부작용으로볼것인가아니면환자의결정으로볼것인가결정하는데에어려움이있었다. 특히 1개월이내에클로자핀을중단한경우는의무기록에서그이유를정확하게파악하기어려워대상에서제외하였는데, 차후에는구조화된면담등을사용한전향적연구설계에따라이러한환자군을포함하여분석하는것이의미있을것으로보인다. 둘째, 단일병원의데이터를바탕으로진행된연구이기때문에, 본연구에서도출된경향성을일반적으로적용하기에어려움이따른다. 다른병원및다른지역의데이터를포함한연구를진행한다면임상적으로더큰의미를지닐수있을것이다. 이번연구는치료저항성조현병에서최선의선택약제인클로자핀을중단하는이유와그원인에대한이해를제공한다. 특히실제임상에서, 어떠한강요나불필요한과정없이자연적으로진행된치료과정을검토하였다는이점이있다. 22 Korean J Schizophr Res 2016;19:17-24

7 조진우등 결 론 본연구에서가장흔한클로자핀중단사유는부작용이었 다. 부작용중졸림, 혈액학적이상, 자율신경계이상, 소화기 계증상, 신경학적이상, 경련, 심혈관계이상순으로많았다. 클로자핀의부작용은클로자핀의감량이나부작용을감소 시키는다른약물에의해많은경우해결될수있다. 12) 클로 자핀의중단을줄이기위해부작용을경감시킬수있는다 양한방법및약제들에대한연구가더필요하다. 또한, 클로 자핀의중단사유에대하여보다더객관적인도구를이용한 전향적인연구가이루어져야할것이다. 중심단어 : 조현병 클로자핀 항정신병약물 치료중단 부작용 REFERENCES 1) Levine SZ, Rabinowitz J, Faries D, Lawson AH, Scher-Svanum H.
