The aims of this study were to determine trends in the isolation and antimicrobial resistance patterns of enteropathogenic bacteria over the last 1 years. Methods: We analyzed stool cultures of Salmonella spp., Shigella spp., Plesiomonas shigelloides, Yersinia spp., Vibrio spp., and Campylobacter spp. collected at Severance Hospital between 1 and 1. Antimicrobial susceptibility testing was performed using the disk diffusion method for nontyphoidal Salmonella (S) and Campylobacter. Results: The number of specimens for stool culture significantly increased from 1,41 during to 6,714 over the past 1 years, whereas the ratio of positive specimens significantly decreased from 1.9% (1,7) to 1.1% (648). The proportion of Salmonella Typhi decreased from 97.% in to.8% in 1-1, whereas the proportion of S increased from.8% to 99.%. The proportion of Shigella among all enteric pathogens was over 5% from 1969 to 1, while only seven strains were isolated from 1 to 1, with the exception of one outbreak. Campylobacter is the second most prevalent organism. The rates of susceptibility to ampicillin and cotrimoxazole were 61% and 9%, respectively, for S isolated from 6 to 1. The ciprofloxacin susceptibility rate was 79.5% for Campylobacter between 6 and 1. Conclusion: The number of isolates of Salmonella Typhi and Shigella significantly decreased, while the proportion of S and Campylobacter increased. Continuous monitoring of ciprofloxacin-resistant Campylobacter isolates is necessary. (Tel) , (Fax) 8--1-, ( ) leekcp@yuhs.ac 론 명이상의 Escherichia coli O14:H4 집단감염의사례처럼선진국에서도감염성설사질환이돌발할수있다 []. 감염성설사의원인중바이러스성장염은대부분이보존적치료로호전되지만, 세균성장염은환자의상태나균종에따라항균제치료가필요할수있다 [4]. 전세계적으로흔한장염세균은 Vibrio, E. coli 등이며, 침습적균종으로는 Shigella가전체 1-% 를차지할만큼빈도가높다 [4]. 최근우리나라의급성설사질환의감시사업의보고 [5] 에따르면, 8년급성설사질환의원인균으로 E. coli 4%, Salmonella 4%, Camphylobacter 15%, Shigella.7% 가분리되었다. 또, Shin 등 [] 이보고한자료에따르면, 년에장염세균양성환자중에 Shigella 5.1%, Salmonella Typhi 16.7% 가분리되었고, 이후점차감소하여 년에는 Shigella 1.5%, Salmonella Typhi 1.5% 가분리되었다. 지속적인보건위생의개선에따라감염성장염의발생률은줄어들고있는추세이나, 장염세균의항균제내성률 45

2 46 Ann Clin Microbiol 1;16(1):45-51 은증가하고있다. 또한해외여행의증가는오염된음식이나물을통해발생하는감염성장염의유입이나유행가능성을높이고있다 [,4]. 본연구에서는최근 1년간세브란스병원환자에서의장염세균분리현황과주요항균제내성양상을분석을통하여, 장염환자의원인균을추정함으로써장염환자의적절한치료에도움을주고자하였다. 재료및방법 1-1년에장염세균분리가의뢰된세브란스병원환자검체를대상으로하였다. 변검체는수송용기 (mini sputum, 모아엠피, 부천, 한국 ) 에담아서, 직장면봉검체는채취후 Stuart 수송배지 ( 마이크로미디아, 부산, 한국 ) 에넣어서미생물검사실로수송하였다. 배양대상세균은 Enterobacteriaceae에속하는 Salmonella spp., Shigella spp., Plesiomonas shigelloides, Yersinia spp. 와그외에 Vibrio spp. 및 Campylobacter spp. 였다. Vibrio 배양은매년 5월에서 1월사이에만시행하였다. Enterobacteriaceae에속한장염세균의배양을위해서는 MacConkey agar (BBL, Becton Dickinson, Cockeysville, MD, USA), Salmonella Shigella agar (BBL), Selenite broth (Difco Laboratories, Detroit, MI, USA) 를, Vibrio 배양을위해서는 Thiosulfate citrate bile sucrose agar (Difco) 를, Campylobacter 배양을위해서는 Tryptose blood agar base (Oxoid, Hampshire, United Kingdom) 에 Campylobacter selective supplement (Oxoid) 를첨가하여제조한 Butzler 배지를사용하였다. Enterobacteriaceae와 Vibio는공기환경에서 5 o C로 4시간배양하였고, Campylobacter 배양을위해서는검체를접종한배지를혐기성단지에넣고, 혐기성상자를이용하여미호기성조건을만든후, 4 에서 48시간배양하였다 [6]. 세균동정은대부분집락의형태와 triple sugar iron agar 배지등을이용한생화학적방법을이용하였고, 필요에따라 Vitek GN 카드 (biomériux, Marcy-l Etoile, France) 를사용하였다. Salmonella나 Shigella의혈청학적동정을위해서는 Salmonella 및 Shigella군진단혈청 ( 국립보건연구원 ) 을써서슬라이드응집반응을시행하였다. Campylobacter의균종동정은그람염색소견, oxidase 시험및 hippurate 가수분해시험에의하였다. Salmonella의항균제감수성을 1년 6월이전에는 Clinical and Laboratory Standard Institute (CLSI) 디스크확산법으로 [7], 이후에는 Vitek AST 카드 (biomériux) 로시험하였다. Campylobacter의 erythromycin과 ciprofloxacin 감수성검사는 CLSI 디스크확산법, 즉 5% 혈액을포함한 Muller-Hinton agar배지에접종하고, 5 o C 미호기성상태에서 48시간배양후판독하는방법으로시행하였다. 이때, 표준균주는 S. aureus ATCC 59 을사용하였다 [8]. 결 1-1년에의뢰된검체수는 6,714개였으며배양양성률은 1.1% 였다. 이를과거 년간의장염세균의분리현황과비교했을때의뢰된검체수는 년 1,41개, 년 7,846개, 19-19년 51,441개로과거에비해의뢰검체수의증가를보였다 (Table 1). 그러나, 배양양성률은 년 1.9% 로가장높았으며이후점차감소하여 1-1 년에는 1.1% 의배양양성률을나타내었다. 분리된균주중 Shigella와 Salmonella 분리수는 년에는 Shigella가 1,9 주, Salmonella 가 54 주, 년에는 Shigella 1,58 주, Salmonella 1,1주, 19-19년에는 Shigella 51주, Salmonella 1,186주였으나 1-1년에는집단발생건을제외한분리수가 Shigella 7주, Salmonella 59주로감소하였다. 1-1년장염세균이분리된 648검체는중복분리주를제외하면 565명환자에서의뢰된것으로, 한균종이분리된환자는 56명, 두가지장염세균이동시에분리된환자가 5명이었다. 두가지의장염세균이동시에분리된경우는서로다른혈청군의 Salmonella가분리된 예, Campylobacter속 균종이분리된 예, Campylobacter와 Salmonella 가분리된 1예가있었다. 한균종이분리된 56 명환자중 Salmonella 는 54 명 (6.%) 에 Table 1. 3 In Joo Cho, et al. : Trends in Isolation and Antimicrobial Susceptibility of Enteropathogenic Bacteria 47 서, Shigella 는 명 (17.5%) 에서, Campylobacter 는 8 명 (14.%) 에서분리되었다. 이밖에 V. parahaemolyticus가 7명 (4.8%), Y. enterocolitica는 1명 (.%) 에서분리되었다. 분리된 Salmonella 중 Salmonella Typhi는 1명, Salmonella Paratyphi-A는 명이었으며, 나머지 51명에서 nontyphoidal Salmonella (S) 가분리되었고, S 중 D혈청군이 155명 (44.1%) 으로가장많았다. Shigella가 1년에는집단발생한환자 91명에서분리되었으나, -1년에는 7명에서만분리되었다. 1년에집단발생한환자에서분리된균주모두는 D혈청군인 S. sonnei였다. 분리수가비교적많았던 S와 Campylobacter 균종의월별 Table. (.4) 1 (1.) 4 (4.7) 6 (7.1) 85 (1) 분리율을보면 S는 5-9월에, Campylobacter는 5-8월에높았다 (Table ). 주요균종의분리율을환자연령군별비교했을때 S는 5세이하에서가장높았고, Campylobacter의분리는 5 세이하와 -9세사이의연령군에서높았다 (Table ). 항균제감수성률은 1-5년과 6-1년분리균주로구분하여분석하였다 (Table 4). S의 1-5년과 6-1년의각항균제감수성률은 ampicillin 65% 와 61%, cotrimoxazole 9% 와 9%, cefotaxime 99% 와 96%, ceftazidime 1% 와 %, levofloxacin 1% 와 % 였다 년분리 S의항균제감수성률은 ampicillin 76%, cotrimoxazole %, cefotaxime %, ceftazidime %, levofloxacin 1% 였다. 1-5년과 6-1년분리 Campylobacter의감수성률은 eyrthromycin에대하여각각 95.1% 와 87.% 였으며, ciprofloxacin에대해서는 5.8% 와 79.5% 였다 년분리 Campylobacter의감수성률은 erythromycin 9%, fluoroquinolone (ofloxacin, ; levofloxacin, ) 74.% 였다 (Table 5). 고찰본연구에서는 1-1년의장염세균분리결과를 년의분리결과와비교하여장염세균의분리추세변화를분석하고, 또한항생제내성양상을검토하고자하였다. 본연구기간의변배양검사의뢰수는 년의 1,41검체와비교할때 4.5배증가한반면에, 배양양성인검체수는 1,7 ( 양성률 1.9%) 에서 648 ( 양성률 1.1%) 로현저히감소하 Table. Abbreviation:, not tested. 였다. 이는병상수의증가에따른환자수의증가, 원내감염감시배양의증가및보건위생의향상때문이라고생각되었다 []. 개발도상국에서는장염세균중에 Shigella, Vibio, Salmonella 가주로분리되나, 선진국에서는 Shigella와 Salmonella Typhi가대단히드물어졌다. 선진국에서는 Campylobacter와 S가주요분리균종이되었고, 미국의한연구는장염세균원인균중 Campylobacter 4%, S %, Shigella 1% 가분리되었다고보고하여이러한분리균종의변화를뒷받침하였다 [9,1]. 특히 Salmonella 혈청형 D군에속하는 Salmonella enteritidis는세계각지에서유행하는가장흔한 Salmonella 혈청형중에하나이며, 식품유래살모넬라증의주된원인균으로알려져있다 [11]. Jin 등 [1] 은우리나라의소아장염환자에서분리된 S혈청중에서 Salmonella enteritidis가 47.6% 로가장많았다고보고하였다. 본연구기간인 1-1년에는 S가 56명환자에서발생하였고, 분리된장염세균중에 S가가장많이분리되어 6% 를차지하였다 년에분리된 Salmonella 중 9.6% 가 Salmonella Typhi였으나, 1-1년에는 1명 (.%) 에서만분리되었다. 또한, Salmonella Paratyphi-A 양성환자수도 년에는 11명이었으나 1-1년에는 명으로현저히줄었다. 반면에 S가차지하는비중은점차증가하여 년에는 14명에서분리되었으나 1-1년에는 56명에서분리되었다. 과거에는 S 중에 B혈청군이많았으나, 1-1년에는 D혈청군이많았다. 이러한추세는 199년후반이후외국과국내다른지역에서의경향과비슷하였다 [1,1,14]. Shigella는세균성이질을일으키며, 장외감염은극히드물다. Shigella는 년에는총,547명환자에서분리되어, 전체분리수의 5% 이상을차지하는가장흔한장염세균이었으나, 19년이후에현저히감소하였다. 본연구에서는 1-1년에총 명의환자에서분리되었으나그중 91명은 1년에집단발생한환자였고, -1년에는 7명에서만분리되어최근의분리감소를반영하였다. 집단발생환자에서분리된 Shigella는모두 S. sonnei였는데후진국에는 S. dysenteriae와 S. flexneri 감염이많으나, 선진국에는 S. sonnei 감염이많다 [4]. 비교적근래에우리나라에는 S. dysenteriae 감염은드물었고, 1년대초반에는 S. flexneri 분리수가많았으나, 199년중반부터는감소하였다. 그러나, 199년후반이후에도 Shigella의집단감염이종종생기고있어서계속적인배양이필요하다 [15]. 미국에서 Campylobacter는 Salmonella 다음으로흔히분리되는장염세균으로 1년에 백만명의환자가발생하였으나, 1996년이후점차감소하여미국 Centers for Disease Control and Prevention (CDC) 의 FoodNet 1년자료에의하면 1만명당 1명의환자가발생할정도로매우줄었다 [16]. 세브란스병원에서는 11년부터 Campylobacter 분리를시작하였고, 19-19년에는 9명의환자에서, 1-1년에는 8명의환자에서분리되었다. 8명의환자에서분리된 85균주중에 C. jejuni (77주) 가가장많았고, C. coli 주, 균종이확인되지않은 Campylobacter 5주가있었다. Cho 등 [17] 은설사환자의대변에서다중중합효소연쇄반응검사법을이용한 Campylobacter 검출이 9.5% 로가장높은빈도를나타냈음을보고하였다. Campylobacter는배양이어려워임상검사실에서배양을통한분리에어려움이있으나, 분자유전학적인방법으로검출한비율은높음을고려할때, 우리나라의 Campylobacter 분리율이높을것으로판단되었다. V. parahemolyticus는조사기간중 7명 (4.8%) 에서분리되어, 그수가많지않았는데, 이장염환자는감염 -일후에자연회복되므로 차의료기관에서이세균을분리할기회가적었기때문으로생각한다. 우리나라에서는흔히어패류를생식하므로감염환자수는많을것으로추측되는데, 실제로 19년인천에서는 V. parahemolyticus 장염환자가게장이원인이되어 5,명이상발생한것으로추정된보고가있었다 [18]. Y. enterocolitica는북유럽과캐나다등지에서, P. shigelloides 는주로열대및아열대지방환자에서분리가보고되어있다 [19]. 우리나라의경우는몇차례의산발적인발생보고가있었

5 In Joo Cho, et al. : Trends in Isolation and Antimicrobial Susceptibility of Enteropathogenic Bacteria 49 고 Shin 등 [] 은 년에 Y. enterocolitica 47예와 P. shigelloides 15예를보고하였으나, 본연구에서는해당기간에는 Y. enterocolitica가 1주만분리되었다. S와 Campylobacter는 5세이하환자에서분리가많았는데, 이는다른국내외연구자들의결과와비슷하였다 [9,]. 또한 Campylobacter는 5세이하에서뿐아니라, -9 세연령에서도다수분리되었다. 본연구기간중에두가지의장염세균이동시에분리된경우는 5건 (.9%) 으로 년에 9건 (1.%) 과비슷하였고, 핀란드, 바르셀로나등국가의 % 에비하면매우낮은양상을보였다 [1]. 본연구에서 S의 ampicillin과 cotrimoxazole에대한내성률이과거에비하여높아졌으나, cefotaxime, ceftazidime 및 fluoroquinolone에내성인균주는여전히드물었다. 우리나라에서 세대 cephalosporin에내성인 extended-spectrum β-lactamase (ESBL) 생성 Salmonella는 년에처음보고되었고, 그후점차증가하고있으며, 주로 CTX-M-type ESBL에의한것으로추정된다 [,]. Salmonella에대한항균력이좋은 ciprofloxacin 내성률은매우낮으나, ciprofloxacin에내성인 Salmonella가외국에서증가된보고가있으므로계속적인감시가필요할것으로생각한다 [4-6]. 국내소아에서는장염의경험적항생제로치료제로사용되는 ampicillin/sulbactam, cefotaxime 에대한내성률은각각 19%,.9% 라고보고되었다 [1]. Campylobacter 장염치료에는 erythromycin과 ciprofloxacin 이권장되는데, 근래이들약제에대한내성증가가보고되고있다. Campylobacter의항균제감수성검사해석기준변경에따라직접적인비교는어렵지만, 1-5년에 ciprofloxacin 감수성률은 5.8% 였고 [], 6-1년에감수성률은 79.5% 로내성률의증가는관찰되지않았다. Ciprofloxacin 내성률은나라에따라서차이를보이는데, 유럽은 -5% 의낮은내성률이, 태국에서는 8% 이상의높은내성률이보고되었다 [7-9]. 반면본연구에서 erythromycin과 tetracycline 감수성률은이전기간과비교하여 6-1년분리주에대해서각각 87.% 와 % 로점차낮아지는추세였다. 국내다른연구에서 erythromycin 감수성률은 1%, tetracycline 감수성률은 % 로보고하였는데, 지역적, 대상자수의차이로본연구와 erythromycin 감수성률의차이를나타낸것으로생각한다 []. 페루에서 6-1년에분리된균주를대상으로한연구에서 erythromycin 내성률이지역에따라.-14.9%, ciprofloxacin 은 % 라고보고되었다 [1]. 장염환자의경험적항생제로널리사용되는 ciprofloxacin, erythromycin 항균제에대한계속적인내성률감시가필요할것으로생각한다. 참고문헌 1. 7 In Joo Cho, et al. : Trends in Isolation and Antimicrobial Susceptibility of Enteropathogenic Bacteria 51 = 국문초록 = 국내한대학병원의 1-1 년장염세균분리추세와항균제감수성양상 1 연세대학교의과대학진단검사의학교실, 세균내성연구소, 국민건강보험일산병원진단검사의학과조인주 1, 임지숙 1, 이양순 1,, 김명숙 1, 서유경 1, 정혜선 1, 용동은 1, 정석훈 1, 이경원 1, 정윤섭 1 배경 : 우리나라의환경위생이개선됨에따라장염세균의분리경향도달라지고있다. 본연구에서는최근 1년간장염세균의분리양상과항균제내성률을분석하였다. 방법 : 1-1년에의뢰된환자의변배양검사결과를분석하였다. 검사대상은 Salmonella spp., Shigella spp., Plesiomonas shigelloides, Yersinia spp., Vibrio spp. 와 Campylobacter spp. 였다. 분리된장염세균중 nontyphoidal Salmonella (S) 와 Campylobacter에대하여디스크확산법으로항균제감수성을시험하였다. 결과 : 년과비교하여지난 1년간의뢰된검체수는 1,41건에서 6,714건으로현저히증가한반면, 양성검체비율은 1.9% (1,7 검체 ) 에서 1.1% (648 검체 ) 로현저히감소하였다. Salmonella 중 Salmonella Typhi의비율이 년에는 97.% 였으나, 1-1년에는.8% 로줄었다. 반면 S의비율은.8% 에서 99.% 로점차증가하였다 년에는 Shigella가전체장염세균의 5% 이상이었으나, 1-1년에는집단발생을제외하고 7주만분리되었다. Campylobacter가 Salmonella 다음으로많이분리되었다. 6-1년분리 S의 ampicillin과 cotrimoxazole 감수성률은 61% 와 9% 였고, 같은기간에분리된 Campylobacter의 ciprofloxacin에대한감수성률은 79.5% 였다. 결론 : 병원내감염관리를위한감시배양이활발해짐에따라의뢰된검체수는증가한반면, 환경위생의개선으로장염세균의분리비율은감소하였다. Salmonella Typhi와 Shigella의분리수는현저히감소한반면, S와 Campylobacter의분리비율이증가하였으며 ciprofloxacin 내성 Campylobacter가증가하여이에대한감시가필요하다.
