They are frequently misdiagnosed due to subtle and variable clinical presentations. The aim of management of facial nerve schwannoma is the complete removal of tumor, combined with preservation or restoration of facial nerve continuity. The site, size of the tumor, age, preoperative hearing level and facial nerve function are consider as decision making factors. But, it is management dilemma to decompress or resect the tumor. The purpose of this paper is to discuss to surgical management of complete removal of tumor and successful facial nerve reconstruction. Recently, we experienced three patients with facial nerve schwannoma with relatively good surgical result. Korean J Otolaryngol 40 7, July 1997 db의 청력 역치를 보였고, 좌측은 정상 이었다. 방사 증 례 2 선학적 소견상 측두골 전산단층촬영에서 안면신경의 60세 여자환자가 1996년 1월 9일 2개월간 지속 슬상신경절에서 수평 분지에 이르는 종물이 주위 측 된 우측 이하선 종물을 주소로 본원에 내원하였다.

종물의 침범 부위는 방사선 검사 의 소견을 보였다. 청력은 정상이였고, 전산단층촬영 에서 확인한 것과 동일하였다(Fig. 3). 종물을 모두 에서 3 4cm 크기의 조영제에 음영이 증가되고, 주 제거한 후 2.5cm 정도의 안면신경 결손을 보여 대이 위 조직과 경계가 명확한 종물이 이하선 심엽부에서 개신경을 이용하여 신경이식술을 시행하였다.

종물은 안면신경의 측두골외분지에서 기 원하여 경유돌공을 통하여 수평분절까지 침범한 소견을 보이고 있었다(Fig. 7). 종물제거후 안면신경은 3.5cm Fig. 1. Temporal CT, axial view(caseⅠ).

2 Korean J Otolaryngol 40 7, July 1997 db의 청력 역치를 보였고, 좌측은 정상 이었다. 방사 증 례 2 선학적 소견상 측두골 전산단층촬영에서 안면신경의 60세 여자환자가 1996년 1월 9일 2개월간 지속 슬상신경절에서 수평 분지에 이르는 종물이 주위 측 된 우측 이하선 종물을 주소로 본원에 내원하였다. 이 두골의 골미란과 함께 발견되었고, 자기공명영상촬영 학적소견상 무통성의 딱딱하고 고정된 종물이 3 4cm 에서 내이도의 porus내로 종물이 침범한 소견을 보였 크기로 우측 이하선 부위에서 촉지 되었고, 안면신경 다(Fig. 1, 2). 1995년 4월 11일 경미로 접근법으로 마비는 보이지 않았다. 세침세포흡입검사상 양성 종양 종물을 제거하였다. 종물의 침범 부위는 방사선 검사 의 소견을 보였다. 청력은 정상이였고, 전산단층촬영 에서 확인한 것과 동일하였다(Fig. 3). 종물을 모두 에서 3 4cm 크기의 조영제에 음영이 증가되고, 주 제거한 후 2.5cm 정도의 안면신경 결손을 보여 대이 위 조직과 경계가 명확한 종물이 이하선 심엽부에서 개신경을 이용하여 신경이식술을 시행하였다. 술후 발견되었다. 종물에 의해 경유돌공 부위의 측두골이 34개월이 지난 현재 안면신경마비는 HB grade IV의 파괴된 소견을 보이고 있었다(Fig. 5, 6). 1996년 2월 소견을 보이고, 재발 소견은 보이지 않고 있다(Fig. 4). 7일 경유양동 접근법과 경이하선 접근법으로 종물을 제거하였다. 종물은 안면신경의 측두골외분지에서 기 원하여 경유돌공을 통하여 수평분절까지 침범한 소견을 보이고 있었다(Fig. 7). 종물제거후 안면신경은 3.5cm Fig. 1.
