Leakage of polymethylmethacrylate into spinal canal resulted in compression of thecal sac. 부 환자는 적절한 치료에도 불구하고 오랫동안 통증을 경험 상되는 경우에 주된 치료법이나 추경나사를 이용한 내고정 하게 되며 특히 입원할 정도로 심한 경우에는 통증완화에 시에는 기구보강목적으로도 가능하다. 신경학적결손과 골절 만 약 2주 정도 걸린다5). 골다공증성 골절환자의 50~ 60% 편의 후방전위에 의한 척추관의 협착이 없거나 경미하여야 정도는 증상이 없을 수도 있으나, 많은 환자가 다발성척추 하며, 골절이 심하여 추체높이의 2/3 이상이 소실되었거나 분절에 골절이 있어 고질적인 만성요통, 흉요추부 움직임과 척추경과 추체후면골피질이 파괴된 경우에도 상대적금기가 활동의 제한, 척추기형, 보행장애, 진행성마비와 감각이상 된다. 또한 다발성분절 침범이 흔하고 노화에 따른 추간판 등의 심각한 이환율을 보인다12)14). 골다공증, 고령, 불량한 과 후관절의 퇴행성변화 등이 동반되어 통증을 유발하는 주 전신상태 및 합병된 질환 등으로 기구를 이용한 척추골융 된 척추분절과 원인를 옳게 인지해야 한다7)15). 본 연구에 합술은 불가능한 경우가 많아 근 중재적 방사선학적시술로 서도 5례의 환자가 다발성이었으며 인접 추체의 퇴행성추 인조골시멘트를 추체에 직접주입하여 골절에 의한 통증을 간판병변이 심했던 2례는 많이 호전되지 않았다. 동시에 주 완화하고 골다공증으로 약해진 척추를 보강하기 위한 방법 입 혼합물 누출에 의한 경막외 또는 추간공협착으로 시술 후 4)8)9)16). 이 소개되었다 신경학적장애가 가능하므로 즉각적인 응급수술이 가능해야 5)7) 6)8) 에 의한 추체압 한다. 저자들의 경우 초기시술에서 과량의 골시멘트혼합물 박골절의 치료법으로 많이 시행되고 있으며 예후결정 요소, 주입으로 술 후 전산화단층촬영에서 경막외강의 협소와 압 부작용, 장기성적 등에 대한 연구가 진행중이다. Tohmeh 박이 관찰되었으나 다행히 신경학적 악화는 없었으며 최근 경피적척추성형술은 외상 이나 종양 17) 등 은 사체척추에 골시멘트를 주입한 후의 강도와 경도의 변화를 측정하여 척추성형술의 생체역학적인 근거를 제시 2) 에는 가급적 소량을 주입한다. 경피적척추성형술은 경척추경 후방접근법5)16), 후측방 추 하였고, Botrom 등 은 골절위험이 예상되는 골다공증성추 경외15), 추체전방경로3)로 시술가능하며, 본 연구에서처럼 체 환자에서의 예방적 보강 및 안정화로 추체의 압박진행 흉요추병변은 경척추경법이 적당하므로 전산화단층촬영을 과 기형을 예방할 수 있다고 하였다. 본 시술의 대상이 되는 세밀히 분석하여 척추경의 각도와 넓이를 확인한 후 영상증 주된 척추병변은 척추혈관종, 다발성골수종, 골용해성 척추 폭장치을 이용하면 정확히 시술할 수 있고, 척추경이 손상 전이암, 골다공증성 골절등이며, 시술 결정시에는 병소의 되었거나 척추융합술을 시행한 경우는 후측방으로 접근해야 정도와 범위, 통증정도, 신경학적 상태, 전신건강, 기대여명 한다. 척추체 정맥조영술은 시술자에 따라 이견13)14)은 있 등를 충분히 고려하여 다각적인 팀접근이 필요하다3)5)18). 으나 삽입된 주사침의 위치와 추체내외의 정맥유출경로를 특히 고령의 골다공증성 추체골절의 환자의 경우에는 충분 파악하고 주입혼합물의 점도를 결정해야 하므로 반드시 시 한 약물과 보존적치료에 실패하였거나, 장기간의 입원과 침 행하여야 한다. 추체피질골이 심하게 파괴된 경우에는 추 상안정으로 욕창, 심부정맥혈전, 근위축 등의 합병증이 예 체측면이나 추간판으로 누출되어 일시적인 통증을 호소할 J Korean Neurosurg Soc/ Volume 30 / February,
