1 전립선비대증에서 5 알파환원효소억제제 (Finasteride) 가미세혈관밀도, 혈관내피세포성장인자및 5 알파환원효소의발현에미치는영향 The Effect of Finasteride on Microvessel Density and Expression of Vascular Endothelial Growth Factor and 5α-Reductase in Prostatic Hyperplasia Jae-Ho Hyun, Kun-Hyun Cho, Dong-Seok Han, Jin-Bum Kim, Young-seop Chang From the Department of Urology, College of Medicine, Konyang University, Daejeon, Korea Purpose: Vascular endothelial growth factor (VEGF), a potent stimulator of angiogenesis and microvessel density (MVD), which is an important indicator of neoangiogenesis, were independently evaluated to elucidate the mechanism of decreased bleeding observed in patients treated with finasteride, an inhibitor of 5α-reductase (5AR). We evaluated MVD and the expression of VEGF and 5AR type II in patients with benign prostatic hyperplasia (BPH) treated with finasteride. Materials and Methods: The study included 6 patients undergoing transurethral prostatectomy (TURP) for BPH. Among these patients, 29 had well-preserved paraffin blocks, 3 of whom were given finasteride for a minimum of 3 weeks before surgery; the remaining 6 patients served as controls. MVD was calculated by counting the number of positively stained blood vessels on 5 random, high-power fields within the prostatic section. Expressions of VEGF and 5AR type II were analyzed with a confocal laser scanning microscope and an image analyzer. Results: Prostatic MVD was significantly lower in the finasteride-treated group (p<0.05). The expression of VEGF and 5AR type II at the level of the prostatic glandular epithelium and stroma was not significantly different between the 2 groups. VEGF and 5AR type II were more strongly expressed in the epithelium of both groups than in stromal smooth cells (p<0.05). Conclusions: Finasteride treatment had no clear effect on the expression of VEGF or 5AR type II. It is possible, however, that finasteride improves blood loss after TURP and BPH-induced hematuria by reducing MVD. Further study on the mechanism of MVD reduction is needed. 2 948 Korean Journal of Urology vol. 50, , October 2009 만, 남성호르몬차단치료가전립선혈류및혈관형성을감소시키는것이밝혀지면서이러한출혈의치료방법중의하나로제안되고있다 [3,4]. 남성호르몬차단치료의대표적인약제인 finasteride는 5 알파환원효소억제제의길항저해제로 testosterone을좀더강력한형태인 dihydrotestosterone (DHT) 으로변환되는것을막으며세포고사억제유전자인 survivin과 bcl-2의발현을감소시킨다 [5]. 감소된 DHT는 insulin-like growth factor (IGF) 와상호작용하여전립선상피의증식을억제하며 [6], 전립선상피세포의고사를통해전립선의크기를줄일수있다 [5]. 이로인해요류가개선되고전립선비대증으로인한증상이경감하게된다 [7]. MTOPS study에서 finasteride 의사용은요폐를줄이고비대된전립선으로인한수술적치료의필요성또한감소시킨다고보고되고있다 [8]. Miller와 Puchner의보고에서 finasteride를복용한환자들에서평균 3개월간의장기추적결과전립선비대증으로인한혈뇨가 6명중에서 4명이개선되었으며 [9], Puchner와 Miller는 finasteride를복용한군이비복용군에비하여전립선비대증에의한출혈의재발이의미있게감소되는것을확인하였다 [4]. Foley 등은 finasteride를복용한군과복용하지않은군으로나누어복용한군에서육안적인혈뇨가적었다는결과를제시하면서 [2], 이전의 Miller와 Puchner [9] 와 Puchner와 Miller [4] 의 전립선비대증에의한혈뇨의예방에 finasteride가효과가있다 라는연구결과를통한가설을확인하게되는계기가되었다. Finasteride가혈뇨의치료에사용되는기전으로전립선요도하미세혈관밀도 (microvessel density; MVD) 및혈관형성을자극하는혈관내피성장인자 (vascular endothelial growth factor; VEGF) 의발현을감소시킨다는것이일부에서기술되었다 [0,]. 그러나, Pareek 등에의하면전립선요도하 MVD 및 VEGF는감소시키지만, 전립선비대부위에서 finasteride는아무런영향이미치지않는다고밝혀진바있다 []. 또한, Canda 등도쥐의전립선에서 finasteride가 MVD와 VEGF의발현을감소시키지않았다는실험결과를도출하면서 finasteride의전립선출혈에대한치료에대한여러가지이견이제시되고있다 [2]. 또한, finasteride가전립선내 5알파환원효소의발현을실제로감소시키는지는명확히밝혀진바가없다 [3]. 이와같이 finasteride의전립선출혈에대한치료효과에대해여러가지논란의있어저자들은전립선비대증에서 finasteride를사용하였을경우 MVD 및 VEGF의발현에어떤영향을미치는지, 더불어 5알파환원효소 (5α-reductase; 5AR) 의전립선내발현이실제로감소되는지를확인해보고자한다. 대상및방법 2004년 월부터 2008년 4월까지본원에서전립선비대증으로인한심한하부요로증상, 급성요폐및약물치료에효과가없었던경우에대하여경요도전립선절제술을시행하였고이중수술후조직병리검사결과전립선비대증으로진단되었던 6명의환자를비교하였다. 이중파라핀블록으로고정된조직절편이비교적잘보관되어있는 29명의대상환자의대한전립선조직에대한 MVD, VEGF, 5AR의발현정도를검사하였다. 29명의환자는수술전 finasteride 5 mg/day를복용한군 3명, 대조군 6명으로구분하였다. Finasteride의복용여부및기간에관계없이일괄적으로모든환자에대해서문진및직장수지검사, 요검사및요배양검사, 혈액검사로는전혈구감별계산검사, 프로트롬빈시간, 활성화부분트롬보플라틴시간, 혈액요소질소, 크레아티닌, 전해질등을기본적인검사실검사로시행하였다. 또한, 전립선비대증에대하여국제전립선증상점수 (International Prostate Symptom Score; IPSS), 요류검사및잔뇨량검사, 전립선특이항원 (prostate-specific antigen total/ free; PSA), 경직장초음파검사 (8 MHz, Logiq9, GE) 를시행하였다. 과거력에서당뇨및고혈압, 폐결핵및간염등이있는환자에대하여내과적협의진료후에수술후합병증에대한위험이없을경우수술을시행하였다. CD34 면역조직화학염색및 MVD 측정은경요도전립선절제술을통해얻어진전립선조직을 0% 중성포르말린에하루동안고정시키고흐르는물에수세하여고정제를제거하고탈수하였다. 투명화과정후에파라핀에포매하였으며조직블럭을 4μm 두께로절편을제작하여코팅슬라이드에부착시켰다. 탈파라핀화및함수화를위해 58 o C 오븐에 시간동안방치한후 xylene으로파라핀을제거하고, 00% 에탄올에 2분씩 3번, 95% 에탄올에 2분씩 2번반응시킨후에증류수로세척하였다. 0.0 M Sodium citrate buffer (ph 6.0) 용액에넣어극초단파오븐으로 20분간전처리하고슬라이드를식힌후에증류수로세척하고, 이후 Tris buffer (DAKO) 로 5-0분간처리하였다. 3% H 2O 2 를첨가한메탄올에 0분간실온에서처리하고다시 Tris buffer로 5-0분간세척하였다. 일차항체인 CD34 (:00, Santa Cruz Biotechnology, Inc., Santa Cruz, USA) 로 시간반응시킨후에 Tris buffer 용액에 5-0분세척하였다. Dako EnVision R +system- HRP (Dako Corporation, Carpinteria, USA) 에 60분간반응시켰다. Tris buffer로 5-0분간세척후 0.05% diaminobenzidine (DAB) 에약 -2분간반응시킨후에증류수로충분하게세척하였다. Mayer's hematoxylin (Muto) 으로 30초정도대조

MVD의측정은 Bigler 등의보고 [4] 를참조하여먼저차례대로 40배와 00배시야에서관찰하여미세혈관이가장풍부한부위 (microvascular hot spot) 5개를선택하였다. 이후이들 5부위를 200배시야에서관찰하여 DAB 에의해뚜렷하게염색되는미세혈관의수를측정하였으며, 이를 mm 2 면적당수치로환산하였다. 각표본별로가장미세혈관분포가많은 5부위의 mm 2 면적당혈관의평균수치를미세혈관밀도로정의하였다. 혈관의크기나내강의존재및내강내적혈구의존재여부에상관없이다른혈관과구별되어혈관내피세포가관찰될때는하나의독립된미세혈관으로평가하였다. VEGF 및 5AR의면역형광염색및영상분석은절제된전립선조직을 0% 중성포르말린에하루동안고정시키고흐르는물에수세하여고정제를제거하고탈수하였다. 투명화과정후에 paraffin에포매하였으며조직블럭은 slide glass에 4μm 두께로절편을제작하여부착시켰다. 탈파라핀화및함수화를위해 xylene I, II에각 0분씩, 00%, 95%, 90%, 80%, 70% 알코올에순차적으로각각 5분씩반응시켰다. 증류수로 5분간세척한후 0.0 M (ph 7.4) PBS로 0분씩 3번세척후항원을노출시키기위하여 x sodium citrate buffer 용액에담가 4분간극초단파오븐으로전처리하고, 0.0 M (ph 7.4) PBS로 0분씩 3번세척하였다. 비특이적인반응을없애고자 0% normal donkey serum으로 시간동안 37 o C에서항온처리하였다. 그리고 차항체인 mouse VEGF monoclonal antibody (:00, Santa Cruz Biotechnology, Inc., Santa Cruz, USA) 와 rabbit 5α-reductase type II polyclonal antibody (:00, Santa Cruz Biotechnology, Inc., Santa Cruz, USA) 로 4 o C에서 6시간반응시킨뒤, 0.0 M (ph 7.4) PBS 로 0분씩 3번세척하였다. 2차항체로 37 o C의암실에서 시간동안항온처리하였고, 2차항체로는 VEGF는 Donkey antimouse FITC (:00, Santa Cruz Biotechnology, Inc., Santa Cruz, USA), 5α-reductase type II는 Rabbit texas red (:00, Santa Cruz Biotechnology, Inc., Santa Cruz, USA) 를사용하였다. 0.% Triton X-00 (Sigma-Aldrich Corp., St. Louis, USA) 가함유된 0.0 M PBT로 30분씩 2회세척한후 PBS로 20분씩 5회세척하였다. 염색이끝난슬라이드는 mounting media (Dako, Carpinteria, USA) 를이용하여봉입하였다. 영상분석은전립선비대증조직에서 VEGF 및 5AR의면역염색강도의평가는한장의슬라이드에서최소한 3군데이상전립선상피세포와간질의 VEGF 및 5AR의면역염색강도를측정하였으며그평균값을구하여환자의 VEGF 및 5AR의면역염색강도로하였고공초점레이저주사현미경 (Zeiss LSM, Jena, Germany) 과영상분석장치를이용하여측 정하였다. 모든실험결과는평균 ± 표준편차 ( 최소값-최대값 ) 로표시하였으며, finasteride 복용군과대조군의기본적인특성과 MVD의비교, 각슬라이드에서측정된 VEGF와 5AR의발현강도는 Mann-Whitney test를사용하여분석비교하였다. 통계분석은 SPSS version 3.0을이용하였고 p값이 0.05 미만일경우통계학적으로유의한차이가있는것으로간주하였다. 결과 Finasteride 복용군의평균복용기간은 5.92±6.29개월이었으며복용기간은최소 3주부터 28개월까지분포하였다. Finasteride 복용군과대조군사이에수술당시평균연령은 69.±6.38세, 7.7±7.57세, 국제전립선증상점수는 25.68± 6.5, 22.26±9.04, 삶의질점수 5.00±.20, 5.00±.5 로양군간에차이가없었다. 두군의과거력의분포에대한차이도없었다. 전립선용적은 finasteride 복용군이 66.08±36.6 g, 대조군이 6.33±28.68 g, 수술전 PSA수치는 total PSA 3.4±2.90 ng/ml, 4.66±3.97 ng/ml, free PSA 0.95± 0.93 ng/ml,.45±.89 ng/ml로통계적으로유의한차이는없었다. 또한, 수술전요류검사에서배뇨량에있어서만유의한차이를보였으며최대요흐름속도와잔뇨량및수술시절제된전립선의용적도 finasteride 복용군과대조군사이에차이가나타나지않았다 (Table ). Finasteride 복용군의 MVD는 6.02±4.43, 대조군의 MVD 는 25.76±4.76으로 finasteride를복용한군에서유의하게낮게나타났다 (p<0.05) (Table 2, Fig., 2). VEGF의발현강도는전립선상피세포에서비교했을때 finasteride 복용군은 24.3±9.69, 대조군은 26.26±8.4로대조군에서더높게나타났지만통계적으로의미있는차이를보이지않았으며 (p=0.06), 간질의평활근에서는 4.56±7.0, 5.57±7.06으로발현의강도가비슷하게관찰되었다 (p=0.7) (Table 2, Fig. 3). 그러나, VEGF 발현강도를전립선상피세포와간질평활근에서서로비교하면, finasteride 복용군이나, 대조군모두에서전립선상피세포에서의 VEGF 발현이간질평활근에비해의의있게높았다 (p<0.05). 5AR의발현강도는전립선상피세포에서는 finasteride 복용군에서 30.0±8.04, 대조군에서 3.06±4.65로두군간에차이가없었으며 (p=0.), 간질의평활근에서도 7.4±0.89, 8.04±9.03으로발현강도의차이가없었다 (p=0.22). 그러나, 5AR의발현강도를전립선상피세포와간질평활근에서서로비교하면 finasteride 복용군이나대조군모두에서전립선상피세포에서의발현이간질평활근에서의발현보다높았다 (p<0.05) (Fig. 4).

Foley 등은전립선비대증으로인한혈뇨에대하여 finasteride로치료받은환자들에서수술의필요성이매우감소하였다고하였다 [2]. 이들의연구에서 57명의만성간헐성혈뇨환자를 finasteride 치료를받는군과대조군으로나누어 2개월동안관찰하였고, 대조군의 7명 (63%) 이혈뇨가재발하였고그 중에서 7명 (26%) 에서수술이필요할정도로심했으며, finasteride 치료군은 4명 (4%) 에서재발하였고수술적치료가필요한환자는없었다. 이연구에서는 finasteride 치료를시작하고 4주이내에혈뇨가호전되었다. Carlin 등은전립선비대증으로인한육안적혈뇨를보이는환자 2명에대하여전향적으로 finasteride 치료를시행하였고, 2주이내에혈뇨가개선되는것을보고하였다 [6]. 또한, 혈뇨치료에대한 finasteride의효능은전립선의크기와반비례한다고하였다. Kearney 등은급성또는재발성출혈에대해 finasteride 치료받은환자들을후향적으로분석하였으며, 급성출혈이있는군에서출혈이멈추는시간이평균 2일이었고, 전립선크기가 40 g 미만일경우최소 2.7일에서 50 g 이상일경우 45일

Pareek 등은 경요도전립선절제술 시행 전 최소 6주 이상 finasteride를 복용한 군과 대조군의 MVD를 비교하였으며 전립선요도하 MVD는 실험군에서 의미 있게 감소하였으나, 전립선비대부위의 MVD는 차이가 없었다는 결과를 보고하였다 []. 그러나, Sandfeldt 등은 경요도전립 선절제술 전 finasteride를 하루 5 mg씩 3개월 동안 복용한 군 25명과 대조군 29명에 대한 전립선요도하 MVD를 비교 하였고, finasteride를 복용한 군은 24, 대조군은 22로 양 군 Fig. 2. MVD: microvessel density. 간에 전립선요도하 MVD에 별다른 차이가 없었다고 보고 하여 [9], Pareek 등의 실험 결과 []에 반하는 결론을 내렸 다. Puchner와 Miller가 제안한 기전은 좀 더 구체적으로 finasteride가 testosterone이 DHT로 변환되는 것을 막아서 혈 관 형성을 관장하는 남성호르몬에 의해 조절되는 성장 인 이상까지 길어진다고 하였다 [7]. 전립선비대증으로 인한 자의 활성이 감소하고, 이로 인해 전립선 출혈이 감소하게 혈뇨의 치료에 이러한 finasteride가 효과가 있다는 것이 증명 된다는 것이다 [4]. 다른 의견으로, Hagerty 등은 finasteride 되었지만, 정확한 기전은 아직 명확히 밝혀지지 않은 상태 에 의한 전립선 크기 감소 효과로 전립선의 대사요구량이 이다. 이전의 여러 연구에서 수술 전 finasteride 복용기간은 감소하여 이차적으로 혈류량이 감소하게 된다고 하였다 다양했는데, 본 연구에서도 최소 3주, 최장 28개월 등 다양한 [20]. 또한, Canda 등은 쥐의 전립선을 사용한 실험을 통하 복용 기간에 관계없이 finasteride 투여군에서 대조군에 비해 여 finasteride가 전립선비대증에서 출혈을 감소시키는 효과 MVD가 낮게 나타났다. 이러한 결과를 토대로 finasteride 투 는 전립선의 증식부위가 위축되면서 전립선의 전체적인 혈 여군은 경요도전립선절제술 후 또는 전립선비대증 관련 출 관분포도를 감소시키기보다는 혈관벽의 안정성을 증진시 혈 경향이 어느 정도 낮아진다는 것을 다시 한 번 확인할 키기 때문이라고 하였다 [2]. 이렇듯 finasteride가 전립선비 수 있다. 대증에서 출혈을 감소시키는 효과를 일으키는 기전에 대해 전립선비대증에서 나타나는 혈뇨는 전립선의 혈관 분포 여러 가지 견해가 있다. 저자의 실험에서 전립선 전체의 의 증가와 연관되어있을 가능성이 높다 [3,4]. 이러한 혈관 MVD에 대한 조사 결과 finasteride 복용군에서 대조군에 비 형성은 finasteride로 치료받은 개의 전립선에서 전립선 내 해 MVD가 감소되어 있었으며 이러한 결과는 이전의 보고

VEGF는생리적, 병적혈관형성의가장기본적인조절인자이다 [24]. 배아기발달과출생후성장동안에 VEGF의정상혈관형성에대한역할은이미증명되었다 [25]. VEGF 의병적혈관형성은종양이증식하면서 VEGF mrna가과발현되면서발생하며, 이미사람에서발생하는많은종류의암에서밝혀졌다 [26,28]. VEGF에대하여전립선에서는상반되는의견이존재하지만, 최근의자료에서전립선비대

7 Jae-Ho Hyun, et al:the Effect of Finasteride on Vascularity in Prostatic Hyperplasia 953 증과전립선암모두에서 VEGF의발현이증가된다는것이밝혀졌다 [25]. 특이하게, 전립선상피세포에서간질의평활근과비교하여 VEGF 발현이더높게나타났다. Pareek 등의연구결과에서는 finasteride를최소 6주이상복용한군에서대조군에비해요도하부위의 VEGF와 MVD의발현을유의하게낮추었으나, 전립선비대부위에서는이러한영향이없었다고하였다 []. 또한, Lekas 등은최소 7.주이상 finasteride를복용했던군에서대조군에비하여요도하부위의 VEGF 발현및 MVD가낮게나타났으며이러한경향이경요도전립선절제술후출혈을감소시키는데영향을주었다는결과를제시하였다 [27]. 그러나, Weisser와 Krieg는 finasteride를 3개월복용했을경우 VEGF의발현을 Western blot으로측정하였으며, 대조군에비해 VEGF의발현은감소하였으나, MVD는차이가없었다고보고하였다 [28]. 이러한여러가지자료를근거로볼때 VEGF와 MVD는 finasteride에의해변화되는경향이꼭비슷하게나타나지는않았다. 본연구결과에서도 MVD와 VEGF의발현이 finasteride 복용유무에따라같은경향을보이지않았다. 특히 VEGF의발현은본연구에서같은군에서전립선상피세포와간질의평활근에서 Ravindranath 등이제시한바 [25] 와같은경향을보였으며, 이러한경향은 finasteride의복용여부와관계없었다. 그러나, 저자들의결과에서는전립선상피세포및간질의평활근을각각 finasteride의복용여부에따라 VEGF의발현을비교했을때실험군이비교적낮은발현강도를보였지만통계학적유의성은없었다. 이러한이유는실험군과대조군의집단의수가적고, finasteride의복용기간이 3주에서부터 28개월까지분포하였으며표준편차가 6.29개월로차이가많았기때문일것으로생각한다. Finasteride는 5AR의아형중에서주로 II형에작용한다. 앞에서기술한것처럼, finasteride는간질평활근보다주로전립선상피세포에서 DHT를감소시키지만, 실제로 5AR type II가전립선상피세포와간질평활근에서어떻게발현의차이가나는지명확히밝혀진바는없다. Weisser와 Krieg는 finasteride가간질평활근보다전립선상피세포에서 5AR의활성도를더많이억제하며, 전립선상피세포와기질에서 finasteride는용량의존성이며경쟁적으로 5AR 활성도를억제한다는결과를제시했다 [28]. Vaarala 등은 finasteride를지속적으로사용하면서 3, 6, 2개월동안 5AR type II mrna 의발현을 in situ hybridization을통하여비교한결과, 5AR type II는주로전립선상피세포에국한되어있었고, finasteride가전립선상피세포및간질의평활근에존재하는 5AR type II의발현에아무런영향을주지못했다고결론지었다 [3]. 본실험에서는 Vaarala 등의결과 [3] 와유사하게 finasteride 복용여부에따라전립선상피세포및간질평활근 에서 5AR type II의발현강도에차이가없었으며, 같은군내에서는 5AR type II가전립선상피세포에서간질의평활근보다발현강도가높게나타났다. 즉, finasteride가전립선에작용하는기전이 5AR type II를억제하여테스토스테론이 DHT로전환되는것을억제하지만, 직접적으로 5AR type II 의발현까지감소시키는것은확실치않다고할수있다. 5알파환원효소억제제인 finasteride는전립선비대증에서사용했을경우전립선요도하부위에국한되서 MVD를감소시키는것이아니라전체적인 MVD 감소효과를나타냈다. 이러한전체적인 MVD 감소효과는경요도전립선절제술을시행할경우출혈을감소시키는결과를예측할수있으며, 또한전립선비대증과관련된혈뇨를치료하는한가지방법으로사용할수있을것이다. MVD 감소효과의기전으로알려져있던 finasteride의 VEGF 억제효과는본연구에서통계학적유의성을찾을수없었지만, 실험군에서대조군에비해 VEGF의발현이더감소되어있었다. 또한, VEGF의발현이전립선상피세포에서더높게나타났다. 이러한결과는 finasteride가전립선상피세포에서 DHT의감소효과가더크다는것을고려할때, 전립선상피세포의 VEGF 발현을감소시켜이에따라 MVD를감소시킬수있다고생각할수있다. Finasteride는 5AR type II의길항적억제제로작용하지만전립선에서 5AR type II의발현자체를직접억제하는효과를나타내지는않았다. VEGF와마찬가지로 5AR type II의발현은전립선상피세포에서간질평활근세포에비해높게나타났다. 본연구에서관찰된소견을종합해보면, finasteride가혈뇨를치료하는기전은요도하부위의 VEGF 를직접적으로억제하여요도하부위의 MVD를줄이는것보다는전체적인전립선의크기를감소시켜혈관성을낮추는것이며, 이러한효과가요도하부위에국한되지않고전립선전체에걸쳐서나타난다는것을알수있다. 또한, finasteride 가 VEGF를억제하는것은 MVD를감소시키는원인중의하나일뿐이며, 더복잡하고다양한기전으로 MVD를전체적으로감소시킨다고생각할수있다. 그러므로, 아직까지 finasteride가혈뇨의치료및경요도전립선절제술후출혈을감소시키는기전이어떤것인지명확히밝혀지지않았기때문에여러가지다양한연구가필요할것으로생각한다. 결론 Finasteride는전립선비대증에서전체적인 MVD를감소시키며, VEGF와 5AR의발현에는아무런영향을주지못했다. 그러나, 전립선의전반적인 MVD 감소가경요도전립선절제술후출혈및전립선비대증과관련된혈뇨를어느정도낮출수있을것이라고생각할수있다. 향후, finasteride
