It has been known that integron, a naturally occurring gene capture and expression element, plays an important role in the development and dissemination of multidrugresistance. In this study, we investigated the prevalences and molecular characteristics of integrons in NTS clinical strains. Materials and Methods : Between and , a total 26 NTS clinical strains comprising 39 serotypes were collected from clinical specimens. All strains were serotyped, and the MICs of ampicillin, chloramphenicol, streptomycin, sulfamethoxazole, tetracycline, and trimethoprim were determined by agar dilution method. Integrons were detected by PCR amplification of integrase genes, and gene cassettes were determined by PCR and sequencing. Conjugation experiments were performed using E. coli J53 as a recipient. The clonal relationship was analyzed by pulsedfield gel electrophoresis (PFGE). Results : Of the 26 strains tested, class integrons were present in 2 strains (.0%). Class 2 and class 3 integrons were not found. The integronpositive rate was higher in (2.2% [/33]) than in S. Enteritidis (% [3/53]). Overall rates of antimicrobial resistance were higher in integronpositive strains. dhfr2orffaada2 gene cassette was detected in 5 strains, aada2 in strains, dhfr7orffaada5 in 2 strains, and adda in strain. Ten integronpositive transconjugants were successfully selected. Among integronpositive strains of, 7 had similar PFGE patterns. Conclusion : This study suggests that integrons are already playing a significant role in antimicrobial resistance in NTS. Continuous monitoring is needed to detect the emergence and spread of integronmediated antimicrobial resistance. Integron 은주로그람음성세균에서발견되며그내부에하나또는그이상의항균제내성관련유전자를포함하는이동가능한유전자집단 (mobile gene cassette) 이다 (6). 2 반에이루어졌지만 (7), 항균제내성과관련되기이전부터존재했던것으로추정되며세균간의유전자전달을통해세균의진화과정에큰역할을해온것으로추정된다 (). Integron은재조합효소 (recombinase) 를생산하는 integrase 유전자 (int) 의염기서열에따라 class 에서 class 5 까지다섯종류로구분되는데 class 와 class5 는 Vibro species에서만발견된다 (). Class, class 2, class 3는주로그람음성세균에서발견되고항균제내성과관련이있는데, 그중 class integron 이가장흔하며임상적으로의미를가지는경우가많다 (9). Class integron 에는공통적으로 5 말단불변영역 (5' conserved sequence; 5 CS) 와 3 말단불변영역 (5' conserved sequence; 3 CS) 이있고그사이에가변영역 (variable region) 이존재하는데, 이가변영역내의 atti 유전자위치에내성관련유전자집단 (gene cassette) 이삽입됨으로써그람음성세균의항균제내성에관여하게된다 (9, 0). Integron 내에는삽입과절단된유전자의순서에따라다양한형태와수가존재하고이에따라여러가지조합형의다제내성균출현이가능하게되며세균간에이동을통해쉽게전파될수있는특징을가진다 (9, ). 따라서 integron의분포와특징에대한연구는그람음성세균에서의항균제내성의출현과전파를추적하는데있어매우중요한역할을한다고하겠다. 본연구는우리나라환자들의임상검체에서분리된 NTS 균주들을대상으로하여항균제내성의분포와변화, integron의보유율과유전자학적특성을살펴보고자하였다. 재료및방법. 대상균주임상검체에서분리된 NTS 26 균주를대상으로하였다. 균주는서울아산병원과삼성서울병원에서전향적으로수집되었으며 년사이에 5 균주, 년사이에 207 균주가수집되었다. 검체별로살펴보면대변에서분리된경우가 209 균주 (0.0%) 로가장많았으며, 혈액에서 23 균주 (.%), 기타검체에서 29 균주 (.%) 가분리되었다. 한환자당 균주만을대상으로하였다. 혈청형구분은 KauffmanWhite scheme에따라결정하였고 (2) Salmonella enterica serovar Enteritidis (S. Enteritidis) 와 Salmonella enterica serovar Typhimurium () 로분류된균주들은 O항원은슬라이드응집법으로, H항원은시험관응집법으로결정하여파지형을결정하였다 (3). 총 26 균주는 39개의혈 청형으로분류되었다. S. Enteritidis 가가장많았고 (5.6 %, 53/26), (2.6%, 33/26), S. London (5.%, /26), S. Sinstorf (3.%, /26) 의순이었다. 그외 S. Hadar가 6 균주 (2.3%), S. Montevideo 균주 (.5%), S. Heidelberg와 S. Infantis가각각 3 균주 (.%), S. Concord, S. Ngili, S. Braenderup, S. Dublin, S. Brandenburg, S. Geraldton 이각각 2균주 (0.%), S. Bareilly, S. Mbankada, S. Bukuru, S. Tennessee, S. Haifa, S. Schwarzengrund, S. Pakistan, S. Mkamba, S. Larochelle, S. Hadar, S. Manhattan, S. Bardo, S. Blockley, S. Eschweiler, S. Nchanga, S. Gallinarum, S. Onireke, S. Bradford, S. Essen, S. Cuckmere, S. Othmarschen, S. Suberu, S. Stanley, S. Kedougou or S. Pisa, Salmonella enterica subspecies salamae가각각 균주씩 (0.%) 였다. S. Enteritidis 53 균주는 9개의파지형으로분류되었고, PT2 (2.5%, 65/53), PT (23.5 %, 36/53), PT (20.3%, 3/53) 의순이었다. S. Typhimurium 33 균주는 개의파지형으로분류되었는데 DT0 complex (2.2%, 7/33; 0B 균주, 0L 3 균주 ), DT9B (.2%, 6/33), DT90 (9.%, 3/33) 순이었다. 저자들은이들균주들을대상으로하여 nalidixic acid 내성의빈도와내성기전에대한연구결과를보고한바있다 (). 2. Integron 유전자에대한중합효소연쇄반응검사 (polymerase chain reaction, PCR) DNA를추출하여 integrase 유전자 (Int) 에대한 PCR 을시행하였다. PCR을위한 primer 로는 hep 35 (5 TGC GGG TYA AAR GAT BTK GAT TT3 ) 와 hep 36 (5 CAR CAC ATG CGT RTA RAT3 ) 를사용하였다 (9). PCR 반응혼합물 50 µl에는 DNA 20 ng, 각각의 primer 0 pmole씩, Taq polymerase (TaKaRaKorea Biomedical, Inc., Seoul, Korea).25 U, 0 buffer 5 µl, 0 mm dntp, 증류수 37 µl을포함시켰다. PCR 반응의조건은 9 에서 5분간 회반응후 9 에서 0초, 57 에서 0초, 72 에서 분반응을 30 회반복하고최종 72 에서 0분간증폭하였다. 이렇게얻은증폭산물을.0% agarose gel에서 00V로 20분전기영동후 ethidium bromide로염색하여자외선아래에서확인하였다. 3. Integron의유전자형분류 Integron 의유전자형을보기위하여제한효소인 RsaⅠ (Roche Applied Science, Mannheim, Germany) 와 HinfⅠ 66 Infection and Chemotherapy : Vol., No.3, 2009

3 (Roche Applied Science, Mannheim, Germany) 를이용하여 restriction fragment length polymorphism (RFLP) 검사를시행하였다. PCR product 5 µl와 0 restriction enzyme buffer 2 µl, 제한효소 RsaⅠ또는 HinfⅠ U 를섞고멸균된증류수로최종제한효소반응액을 20 µl으로맞추고 37 시간반응시킨후.5% agarose gel에전기영동하였다. 전기영동상에서보인산물들의크기를기준으로 integron 의종류를결정하였다 (9).. 항균제감수성검사항균제감수성검사는 MuellerHinton 배지를사용하여한천희석법으로 ampicillin (Sigma Chemical Co., St. Louis, USA), chloramphenicol (Sigma), streptomycin (Sigma), sulfamethoxazole (Sigma), tetracycline (Sigma), trimethoprim (Sigma) 에대한최소억제농도 (minimal inhibitory concentration, MIC) 를결정하였다. Nalidixic acid에대한 MIC 검사는 Etest (AB biodisk, Solna, Sweden) 를사용하였다. 표준균주로는 Escherichia coli ATCC 25922를사용하였고모든검사를 2회이상반복하여 Clinical and Laboratory Standard Institute (CLSI) 의기준에따라각항균제에대한내성여부를결정하였다 (5). 5. Gene cassette 염기서열분석 Gene cassette 에있는유전자를알아보기위해 PCR과염기서열분석을시행하였다. Primer로는 hep 5 (5 TCA TGG CTT GTT ATG ACT GT3 ) 과 hep 59 (5 GTA GGG CTT ATT ATG CAC GC3 ) 를사용하였고 (6), 염기서열결정은 ABI Prism BigDye Terminators Cycle Sequencing Kit와 ABI Prism 3700 Automated DNA Analyzer (Applied Biosystem, Foster, USA) 를사용하였다. 염기서열분석은 BLAST 프로그램 (National Center for Biotechnology Information, nih.gov) 을이용하였다. 6. 접합 (conjugation) 에의한내성전달시험 Integron 을포함한 plasmid 가다른세균으로전달가능한지알아보기위해접합 (conjugation) 실험을하였다. Sodium azide에내성인 E. coli J53을수용자 (recipient) 로사용하였다. Integron 양성균주와 E. coli J53을 trypticase soy broth (TSB) 에하루배양한후, 각 0. ml씩 TSB ml와섞어 6 시간배양하였다. 이를 sodium azide (Sigma) 00 µg/ml와 ampicillin (Sigma) 00 µg/ml 또는 streptomycin (Sigma) 00 µg/ml 을포함한 trypticase soy agar (TSA) 배지에접종하여피전달접합균주 (transconjugant) 를선별하였다. 선별된균주들을대상으로 integrase 에대한 PCR을시행하여 integron 을확인하고 ampicillin, chloramphenicol, streptomycin, sulfamethoxazole, tetracycline, trimethoprim에대한 MIC 를측정하였다. 7. Pulsedfield gel electrophoresis (PFGE) 대상균을 5 ml의 TSB 배지에서하룻밤배양하여균침사를 PIV 완충용액 (0 mm Tris, ph 7.6, M NaCl) 에부유시켜.3% InCert Agarose (FMC Biopro, Rockland, USA) 에동량을섞은후즉시주형 (mold) 에넣어 plug을만들었다. Plug을준비된 lysis 용액 (6 mm Tris ph 6.0, M NaCl, 00 mm EDTA ph 7.6) 에넣어 37 항온수조에잠기도록하여하룻밤반응시켰다. 그후 ml ESP 용액 (0.5 M EDTA ph.0, 2% sodium lauryl sarcosine, 20 µg/ml proteinase K) 를넣어진탕항온수조에서 55 에서 6시간처리후, 37 항온수조에서증류수로 2회, TE 완충용액 (0 mm Tris ph 7.6, mm EDTA ph 7.6) 으로 2회세척하였다. 제한효소 XbaⅠ (Roche Applied Science, Mannheim, Germany) 을 37 에서 시간동안처리하였다. 반응이끝나면 plug을 % SeaKem GTG agarose gel (Cambrex Bio Science, Rockland, USA) 의 well에넣고 CHEFDRⅢ system (Biorad Laboratories, Inc., USA) 을이용하여 22시간동안전기영동 (6 volt/cm, 20, initial time 2.2 sec, final time 5.2 sec) 하였다. Gel을 0.5 µg/ml ethidium bromode로 30분간염색후증류수로 2시간탈염색하고자외선에서확인하였다. 균주간의유연관계는 Tenover 등이제안한기준 (7) 으로비교분석하였다. 결과. Integron 보유율과분류 26 균주중 2 균주 (.0%) 가 integrase 검출을위한 PCR에양성이었다. 2 균주중에서는 가 균주 (3.0%) 로가장많았고, S. Enteritidis 3균주 (.2 %), S. Heidelberg와 S. Montevideo가각각 2 균주 (9.5 %), 그외에 S. Dublin, S. Sinstorf, S. Cuckmere, S. Othmarschen, S. Haifa, Salmonella enterica subspecies salamae가각각 균주였다. 혈청형별로 integron 보유율을살펴보면 2.2% (/33), S. Enteritidis % (3/53), S. Montevideo 50.0% (2/), S. 4 Heidelberg 66.6% (2/3) 이였다. RFLP 결과 2 균주모두에서 9 bp의 band만이관찰되어모두 class integron으로확인되었다. 2. 약제내성양상항균제감수성검사결과는 Table 에정리하였다. 전체균주에서의내성률변화를살펴보면 trimethorprim 을제외하고감수성시험을시행한모든항균제의내성률이 년균주에비해 년분리균주에서증가하였다. 주요혈청형인 S. Enteritidis와 의경우에는 S. Enteritidis에서 trimethoprim 의내성률이 3.2% 에서.6% 로감소한것을제외하고는내성률이증가하였다. S. Enteritidis와 를제외한균주들의경우 ampicillin, streptomycin, tetracycline, trimethoprim의내성률은감소하였으나 년에분리된균주의수가 9 균주로적어해석에주의를요한다. Integron 존재유무에따라항균제 MIC를비교해보았 는데 (Table 2), 전체균주에서의내성률변화를살펴보면 nalidixic acid를제외한모든항균제에대한내성률이 integron 양성균주군에서높았다. 다제내성 ( 세개계열이상의항균제에내성 ) 인경우도 integron 양성균주들에서 integron 음성인균주에비해많았다 (5.7% [/2] vs. 32.9% [79/20]). Integron 양성이었던 균주의경우 chloramphenicol, streptomycin, tetracycline에대해 00%, ampicillin, sulfamethoxazole에대해 7.5% (7/) 가내성이었다. 하지만, S. Enteritidis의경우 streptomycin에대한내성률은변화가없었고 (33.3% vs. 33.3%) tetracycline에대한내성률은 integron 음성균주들에서더높았다 (9% vs. 66.6%). 3. Gene cassette 에대한염기서열분석 Integron 양성 2 균주의 gene cassette에대한 PCR 결과는 Table 3에정리하였다. 7 균주에서약 2 kb, 5 균주에서약 kb 크기의 PCR band가발견되었다. 6 균주에서 Table. 5 는 integron 내에 PCR band가발견되지않았고, 3 균주는여러 band와 DNA 증폭이약하게되어염기서열분석을할수없었다 (Fig. ). Integron 내 gene cassette 에대한염기서열분석결과 PCR 산물이약 2 kb 크기인 7 균주중에서 5 균주 (S. Enteritidis PT, RDNC, DT90, S. Sinstorf, S. Haifa 각각 균주 ) 에서 dhfr2orffaada2, S. Heidelberg 2 균주에서 dhfr7 orffaada5가존재하였다 (Table 3). PCR 산물이약 kb 크기인 5 균주중 균주 ( DT0L, S. Dubulin, S. Cuckmere, S. Haifa 각각 균주 ) 가 aada2, S. Enteritidis PT 균주가 aada 를가지고있었다.. 접합에의한내성전달 Integron을가지고있는 2 균주중 0 균주 (7.6%) 에서접합에의한내성전달이확인되었다. 이들균주와상응 Figure. Polymerase chain reaction fragments from the cassette regions of class integrons. 7 되었으며그중 S. Enteritidis 는서로연관성이없었고 S. Typhimurium 균주중 7 균주, S. Heidelberg 2 균주가유사했다 (Fig. 2). 혈청형이다른균주들은모두다른유전자형이었다. 고찰본연구에서는국내임상검체에서분리된 NTS 균주들을대상으로전달성내성 (transferable antimicrobial resistance) 에중요한역할을하는것으로알려져있는 integron 의빈도와유전학적특성에관한연구를수행하였다. 현재까지우리나라에서사람에게서분리된 Salmonella enterica에서의 integron 에관한연구는매우제한적이다. NTS를대상으로하였던연구결과를살펴보면, Oh 등은 999년에서 2002년사이에서울시보건환경연구원에검사의뢰되었던식중독환자의분변검체에서분리된 S. Typhimurium 9 균주중 균주 (.2%) 에서 class integron이존재함을보고하였고 (), Sohn 등은 2003년에서 200년사이에경북지역의설사환자에서분리된 S. Typhimurium 7 균주중 3 균주 (7.6%) 에서 class integron 이존재함을보고하였다 (9). 최근, Kim 등은서울지역설사환자에서분리된 33 NTS 균주중에서 균주 (2.%) 에서 class integron이존재함을보고하였다 (20). 이와같이지금까지의국내연구는소규모로일부혈청형에서만진행되었다. 본연구는임상검체에서연속적으로분리된다양한혈청형의 NTS 균들을대상으로 integron의분포율을살펴본대규모연구라는점에서의미가있겠다. 본연구에서전체균주의.0% (2/26) 가 integron 을가지고있었고 nalidixic acid를제외하고는항균제들에대한내성률이전반적으로 integron 보유균주들에게서높았고다제내성과도유의한관련을보였다. 이는우리나라에서도 integron 이항균제내성발현에이미중요한역할을하고있음을의미한다. 혈청형별로 integron 양성률을살펴보았을때는 에서 integron 양성률이높았는데이는외국에서의보고와일치한다 (2, 22). Integron 내유전자를분석한결과 dhfr2orffaada2 가 5 균주, dhfr7orffaada5가 2 균주에서나타나 dhfrorfaada 형태가가장흔했다. dhfrorfaada는 dhfr와 aada 유전자가재조합된형태로각각 trimethoprim 과 aminoglycoside에대한내성유전자로 7 균주모두 trimethoprim 과 streptomycin 에내성이었다. 이와같은형태는 Salmonella 와다른장내세균을대상으로한기존의연 구에서흔히보고되어있는데 (23, 2), 주목할만한것은 99 년에서 2000 년사이에우리나라남서부지역에서대규모로발생했던 Shigella sonnei 유행시분리된여러균주들과 (25) 임상검체에서분리된 Escherichia coli 균주들에서 (26, 27) class integron과 integron 내부에포함된 dhfr2orffaada2이이미보고되었다는점이다. 이는 Salmonella를포함한우리나라의주요장내세균들사이에 gene cassette 들이전파되고있을가능성을시사한다. 앞에서언급한국내연구들에서는 Sohn 등이보고한 integron 양성균주 3 균주모두에서 dhfr0 orffaada2 이발견되었고 (9), Kim 등의연구에서는 aada2 가 2 균주에서발견되었고 aada, aad2orffaad2 가각각 균주에서발견되었다 (20). 6 균주 (2.6%) 에서는 integron 내에유전자가존재하지않았는데, integron 내부에삽입된유전자가없어 5 CS 와 3 CS 영역이서로인접해있는경우로생각된다 (2). Integron이존재하는균주들을대상으로한접합 (conjugation) 연구에서는 2 균주중 0 균주에서양성이었고이는 plasmid 를매개로내성유전자들이전달되었음을의미한다. Table 에정리한바와같이내성양상 (resistance profile) 은대체적으로일치하는경향을보였다. 내성여부가공여균주와차이를보인 7 균주의경우를살펴보면 tetecycline에대해차이를보인경우가 3 균주로가장많았고그다음이 sulfamethoxazole 2 균주, 그리고 streptomycin과 chloramphenicol이각각 균주였다. 본연구에서는 plasmid 에대한분석이나항균제내성기전에대한분석은따로하지않았으나향후연구가필요하겠다. PFGE 결과 2형으로분리가되었다. 균주중 7 균주가 C형으로유사한형태를보였으나 phage type과 gene cassette까지같은경우는없어동일클론으로보기는힘들며서로다른혈청형에서같은형태의 gene cassette들이확인된점을볼때 NTS 균들간에 integron을매개로 gene cassette들의전파가있었음을시사하는것이라하겠다. 본연구에서는우리나라의임상검체에서분리된 NTS 균주들에서의 integron의분포와미생물학적 유전학적특징을살펴보았다. 다양한혈청형에서 integron 이발견되었고 S. Typhumurium에서가장흔했다. Integron을가진균주에서의항균제내성률이의미있게높았고 integron을매개로한 NTS 균들간의항균제내성의전파가의심되어이미우리나라에서도 integron 관련내성이중요한부분을차지하고있음을확인하였다. 향후우리나라에서도 integron에관련한연구가지속적으로필요하겠다.
