The bone marrow in the healthy adults consisted of a combination of hematopoietic and fatty marrow, which showed age-related dirtribation, that is the component of fatty marrow was increased and the marrow vascularity was dereased with age. Avascular necrosis (AVN) showed a decreased bone marrow sign따 within an normal appearing femoral head on T1 and T2-weighted images. In addition, we could see inhomogenous low signal intensity (31 cases), a ring of low intensity with central normal signal intensity (25 cases), focallow signal intensity (12 cases), or a band of low signal intensity (4 cases). MRI findings were abnormal in 10 cases with normal radiographic findiags as well as in 며 1 the cases with abnormal ones. In conclusion, MRI should be the choice of the imaging modality for the evaluation of early bone marrow chages of A VN. Index Words: Femur, necrosis 443,44 Femur, MR studies 443,1214 Femur, radiography 443,1299 서론대퇴골두무혈성괴사는 20-50세의연령에서호발하며, 둔부의골관절증 (osteoarthrosis), 동통 (pain) 과함께무능력 (disability) 을초래하는질환이다. 초기에는중심감압술 (core decompression), 골이식 (bone g raft), 회전골절술 ( rotational osteotomy) 등으로치료가가능하지만, 병이진행됨에따라고관절치환술 (tota- 1 hip replacement) 을시행하여야하는진행성질환이므로치료에있어서조기진단이매우중요하다 (1). 진단에는단순방사선촬영, 단층촬영, 전산화단층촬영, 골주사등이이용되어왔으며 (2), 최근자기공명 영상 ( 이하 MRI 라약함 ) 의도입으로단순방사선에서의변화전에나타나는골수변화에의하여조기진단이가능하게되었다 (1 8) 저자들은본연구에서정상과비정상대퇴골두의 MRI 소견을기술하고대퇴골두무혈성괴사에서나타나는변화를단순방사선소견및임상소견과비교하여기술하고자한다. 대상및방법 1988년 10월부터 1992년 12월까지 5년간본원에서 M RI를시행한환자중대퇴골두무혈성괴사로진단된 45 이논문은 1992 년 11 월 25 일접수하여 1993 년 3 월 17 일에채택되었음

2 대한방사선의학회지 1993; 29(5) : 105 1"'1056 명환자의 73예의대퇴골두중 MRI 시행이전에고관절치환술을시행한 1예를제외한 72예및단순방사선과 MRI에서정상소견을보인 20명환자의 40예의대퇴골두를대상으로하였다. 비정상소견을보인 72예중 38예에서는조직학적으로진단이이루어졌고, 나머지는임상증상, 단순방사선소견및 MRI 소견으로진단하였다. 조직학적진단이이루어진 38예의대퇴골두는고관절치환술 (29예 ) 또는중심감압술 (9예 ) 을시행하였다. 환자의연령은 19세에서 70세까지다양했고, 평균연령은 48세였으며, 남자가 40명으로절대적으로많았다. 원인은장기간의음주 ( 22명 ), 비골절성외상 ( 7명 ), 골절성외상 ( 6 병 ), 스테로이드남용 (2명), 특발성 ( 8명 ) 등이었다. 6명에서우측, 12명에서좌측을침범했으며, 27명에서는양측에병변이동반되어있었다. 정상소견을보인 20명의경우, 50세이하가 8명, 50세이상이 1 2명이었으며, 남자가 14명이었다. 사용된 MR 기기는 0.2Tesla 영구자석형 Hitachi M RP 20-2 이고, pulse sequence는스핀에코 (spin ech 이방법으로 TR msec/ TE 38msec의 T1 강조관상및시상영상 ( corona l and sagittal i mage) 과 TR 1500msec/ TE 70msec의 T2 강조횡단영상 ( axia l image ) 을얻었다. 시야 ( field of v i ew) 는 350mm 로하고, m atrix numb er는 256X256으로하였으며, 절편두께 (slice thickness) 는 5 또는 7.5mm로하였다. 단순방사선소견은 MRI 시행전후 1개월이내에촬영한것을대상으로, Standard classification descr ibed by Ficat and Arlet (1 0) 에따라 Stage 1은정상 소견, Stage 2A 는낭종성 (cystic) 혹은경화성 (sclerotic) 병변, Stage 2B 는반월형증후 (c rescen t sign), Stage 3 은연골하허탈 (subch ond ral collapse), Stage 4 는관절강 ( j o int space) 이좁아지는경우로분류하였 다. MRI 소견은 T1 강조관상영상을기준으로정상대 퇴골두의고신호음영외에비정상적인저신호음영을 분석하였다. MRI 소견을비교하였다. 또한모든환자에서단순방사선소견과 3 예에서는중심감압술을시 행한후 2, 4, 6 개월에각각추적 MRI 를시행하였다. 고관절활액은양에따라 Grade 0 은없는경우, Grade 1 은소량, Grade 2 는대퇴경 (femoral neck ) 을둘 러싸기에충분한양, Grade 3 은관절막 (j oi nt capsule) 이팽창되는많은양으로분류하였다 ( 11 ). SAS package 를사용한 MH Chi-Square test 로결 과를분석하였다. 결 대퇴골두무혈성괴사로진단된전예에서 MRI 소견 상정상대퇴골두의골수음영내에비정상적인저신호 음영을보였다. 저신호음영은네가지유형으로분류하 였다. 31 예 (43%) 에서불규칙 (inhomgenous) 하며광범 위한저신호음영을보였고 ( Fi g. 1a), 25 예 (35%) 에서 중심부의정상음영을둘러싸는주변의저음영의 ring 형 a c b d Fig. 1. a. Relationship between MRI Pattern and Amount of Toint Fluid 인저신호음영을보였다 ( Fig.1c ). 국소적인병변은모 J 두대퇴골두의전상방에있었고범위가클때에도반드 시전상방은포함되어있었다. 4 예 ( 5% ) 에서띠 ( band) 모양의저신호음영을보였다 ( Fig. 1d). 단순방사선소 견에서는 Stage 1 이 8 예 ( 11 %), 2A 가 2 8 예 ( 39% ), 2B 가 19 예 ( 26% ), 3 이 10 예 ( 14% ), 4 가 7 예 ( 10%) 였다. 관절내 Fluid 는 Grade 2 이상의많은양의활액을보 인경우가 45 예 (63%) 였고, Grade 1 이 20 예 (28%), Grade 0 이 7 예 (9%) 였다. 본연구의 MH Chi-Square 에의한분석에서 MR I 양상에따른활액의양의변화는 통계적으로유의하지않은것으로나타났다 ( Table 1). 정상고관절의경우 40 예중 18 예 ( 4 5%) 에서 Grade 1 의 활액을보였으며 Grade 2 이상은없었다. 72 예중 1 2 예 ( 1 7%) 에서는뚜렷한자각증상을인지할 수없었고, 60 예 ( 83% ) 에서는병변부위의고관절동통이 나보행장애등의증상을호소하였다. 자각증상이없었 던 1 2예중에서국소적인병변이 6예 ( 50% ) 로가장많았 MRI Pattern Grade of Fluid o (n=7) 1 (n=20)2 (n=31) 3 (n= 14) Inhomogeneous (n=31) Ring (n=25) Focal (n = 12) Band (n= 4) * MH Chi-Square (Ridit Scores) test P=0.774 Table 2. Focal (n= 12) 본연구에서 72 예중 8 예 ( 11% ) 에서는단순방사선소 Band (n= 4) 견상에서정상으로나타났으나, 저신호음영을보였으며 ( Fi g. Fatty marrow is seen throughout the femoral head, neck, and acetabulum bilaterally. 고 ;>;: ~ E므 대퇴골두무혈성괴사는대퇴골두에서골수로의혈류 가차단되어생기는질환으로서, 외상, 스테로이드남용, 장기간의음주등여러원인에의하여발생한다. 이질환 은주로젊은연령층에호발하고, 적절한치료에의하여 병의진행을방지흑은억제할수있으므로, 치료에있어 서조기진단이매우중요하다 (1, 12 ). 대퇴골두에서골수로의혈류가차단되면, 6-12 시간 에조혈세포 ( hematopoietic ce l!) 가손상되고, 시간에골돌기 ( osteo ph y t e) 가손상되며, 반변에골수 지방세포 ( marrow fat ce ll) 는 유지되는것으로알려져있다. 2-5 일까지비교적오래 혈류가차단된직후에단 순방사선소견상에서는특별한변화를보이지않으며, 이시기에골주사상에서동위원소의집적이감소된다. 회복기에는혈류증가, 섬유아세포 ( fibrobla s t) 증식, 골 밀도감소에의해단순방사선소견에서경색 ( infarc ti o n) 부위주변에골감소 ( osteo p e ni a) 를보이고, 골주 사에서는골흡수 ( res o rption ) 로인한통위원소의집적 이증가된다. 그후시간이지남에따라단순방사선소 견에서골감소 ( osteopenia ) 와골경화 ( osteosclerosis) 가혼합된병변을보이게되고결국대퇴골두의지주 ( trabeculae) 의약화로인한미세골절 (m icrofracture) 이연골하허탈 (subchondral collapse) 이나반월형증 후 (c r esc e nt s ign) 로보이게된다 ( 13 ). 대퇴골두무혈성괴사의특정적인병변은저음영의 띠로둘러싸인내부의고음영이보이는 nng 형태로알 려져있고 (1 4), 본연구에서도 7 2 예중 25 예 ( 38%) 에서 같은병변을보였다. 조직학적으로지음영의띠는비후 된지주골 (trabecular bone), 간엽조직 (mesenchymal tissue), 섬유조직 (fibrous tissue) 및무형 (amorphous) 의세포파편 (ce llu a r debris) 을, 내부의고음영은괴사 (necrosis) 된골 (bone) 과골수 (marrow) 를나타낸다 (1). Coleman 등은대퇴골두무혈성괴사환자의초기의 자각종상은고관절활액의증가에의한다고보고하고있 으며 (12), Mitchell 등은 84% 의정상고관절에서활액을 볼수있었고그중 5% 에서는 Grade 2 의활액을보인다 고보고하고있다 (2). 대퇴골두무혈성괴사를가진고 관절에서는 Grade 2 이상의병적인많은양의활액을보 인예가 65% 였다고하며, 단순방사선소견상에서는 stage 1, 2 의조기병변에서는단지 43% 인반면 stage 3, 4 의진행된병변에서는 82% 로병이진행될수록활액의 양이많아지는것으로보고하고있다. 특히 Grade 3 의 다량의활액은대퇴골두의함몰이일어나는 stage 3 이 상의병변들중에서는 5 0% 나되는반면, 병변들중에서는단지 5% 만보였다고하며, stage 2 이하의 그이유로 대퇴골두의초자양연골 ( hya line cartilage) 의허탈 (c o ll a pse) 과이에따른퇴행성변화가활막 ( sy nov ium) 을자극하고 pannus 형성을자극하기때문이라고설명하 고있다 (1 1, 15, 16). 본연구에서는 Grade 2이상의활액 을보인 45예중 17예 ( 38% ) 에서 Stage 3 이상의진행된 병변을보였으며, MRI 양상에따른활액의양의변화는

Totty 등은단순방사선소견에서위음영을보인 2 예 와골주사에서위음성을보인 5 예에서 MRI 상양성을 나타낸다고보고하였고 ( 12), 본연구에서도단순방사선 소견상위음성을보인 8예 ( 11% ) 에서의 MRI상병변을찾 을수있었다. 단순방사선소견에서변화는골무기질 ( minera I) 의양을반영하는것이며, 따라서치유과정에 서골의홉수가일어나기전에는변화를보이지않는다 (17), 골주사에서위음성이나오는이유로는첫째, 양 측성으로침범시정상대퇴골두와의비교가불가능하기 때문이며, 일례로 Hungerford 등은 27 예의대퇴골두무 혈성괴사환자중 14 예에서골주사소견상위음성을보 였고, 이중 13 예가양측성으로침범한경우였다고보고하 고있다. 둘째, 골주사는초기병변에서는동위원소의 흡착이없다가맥관재생 (revascularization) 으로인한 충혈이나타나면동위원소의흡착이증가되며, 이두시기 의중간단계에서는정상소견을보일수있기때문이다 (9, 18), 본연구에서사용된저자장 MRI 의경우스캔방법에 따르면 T1 강조관상영상이진단에가장유용하였고, 는 T1 강조영상이 signal- to- noise ratio 가적고, 시간이단축되는장점이있기때문으로생각된다. 이 스캔 또한 관상영상에서단순방사선소견과의비교가가장용이했 고, 대퇴골두상부의피질하해면골 ( subcortica l can. cellous bone) 을잘볼수있었다. 이질환의초기에는체중부하 ( weight- bearing) 를줄 이거나, 회전골절술, 중심감압술등의보존적 (conser. v ativ e) 인방법으로치료가가능하지만, 진행된경우에서 는고관절치환술을시행하여야한다 (1, 1 2), 중심감압술 은염증 (i n flammation) 이나부적절한정맥혈배농 (venous drainage) 으로인한수질압 ( intramedullary pressure ) 을감소시키고, 정맥혈의배농과맥관재생 (revascularization) 을촉진시키며, 이방법에의해서 임상적으로호전을보이지만방사선소견에서는대퇴골 두무혈성괴사의변변이계속진행되는것으로알려져 있다 ( 1 8, 19), 본연구에서는중심감압술을시행한 3예 모두에서 MRI 소견상병변의진행을보이지않았다. 결론적으로대퇴골두무혈성괴사는 MRI 에서불규칙 하며광범위한저신호음영을보이거나, Ring 이나띠 모양과, 보이며, 국소적인모양의다양한형태의저신호음영을 이중불규칙한처신호음영이가장진챙된병 변으로생각된다. 또한이러한병변을조기에발견하여 치료하는것이예후에매우중요할것으로생각되고, 히 MRI 가이질환의고위험성을가진경우에서비침습 특 적 (noninvasive) 인선별검사 ( screening test) 가될수 있을것으로사료된다. 차 τ1 므허 C그 L.!...
