1 Hounsfield unit을 이용한 임프란트 식립부위의 골질 평가 조선대학교 치과대학 구강악안면외과학교실 문성용, 정미경, 김수관, 김학균 I. 서론 최근 임프란트 무치악의 재건을 위해 많이 사용되어 지고 있으며, 그 성공률은 환자의 전신적 건강 상태, 임프란트 재료의 생체 적합성, 술자의 술식, 골의 질 과 양에 따라 좌우된다 ). bone quantity"는 임프란트 식립시 유용한 골의 양을 나타내고 bone quality"는 골의 생리, 광화작용의 정도 등의 다양한 면을 종합적 으로 평가하는 용어이다 ). Lekholm and Zarb는 피질 골과 해면골의 양을 비교하여 가지 형태로 bone density를 분류하였고 ) 더 나아가 Misch는 드릴링 시 술자가 느끼는 감각에 따라 가지 골질을 소개하였다 ). 술전에 임프란트 부위의 골량과 골질을 평가 하는 것 은 매우 유용하게 사용될 수 있다. 왜냐하면 임프란트 실패율은 좋지 않은 골질에서 훨씬 높게 나타나기 때 문이다. 골질과 임프란트 안정성의 상관관계를 조사 한 Turkyilmaz와 Tumer의 논문에서 hounsfield unit(hu)과 quality가 좋지 않은 경우에 implant stability quotient(isq)가 낮은 것으로 보고하였다 5,6). 97년에 Godfrey Hounsfield는 computerized axial transverse scanning을 이용하여 치밀골과 해 면골의 상대적인 분포정도를 파악하는 방법을 제시하 였으며, Hounsfield unit(hu)으로 조직의 attenuation coefficients를 직접적으로 측정할 수 있다고 하였다. Hounsfield scale은 공기, 물, dense bone의 density value를 기준으로 하고 있으며 각각 -000, 0, +000이 다 7). Schwarz 등 은 computerized tomography(ct) scan을 이용하여 술전에 환자의 골 량에 대한 평가를 처음 소개하였다 8,9). 이후 CT를 이 용하여 골의 형태와 골밀도에 대한 연구가 이루어졌 다. Fanuscu등은 사체의 상,하악 치조골에서 HU을 측정하여 골 밀도를 조사한 결과 하악에서 5-59HU, 상악에서 86-89HU의 값을 보였다고 보 고하였다 0). 본 연구에서는 매식체 주위 골조직의 HU를 간편하 게 제공하는 Vimplant.0 TM software(cybermed, Seoul, Korea)를 이용하여 임프란트 식립부의 골밀 도를 HU 스케일로 측정하고, 식립 부위에 따른 골질을 HU의 값에 따라 비교하고, 술중에 평가된 Lekholm and Zarb의 주관적 골질 분류법 ) 과 비교하여 분석함 으로써 상관관계에 대해 연구하고자 하였다. II. 연구대상 및 방법 008년 월부터 6월까지 조선대학교 치과병원에 임프란트를 위해 내원한 환자들 중 cone beam CT(CBCT)영상이 있는 명의 환자, 개의 임프란트 식립 부위를 대상으로 하였다. 그 중 여성이 8명, 남성 이 6명이었으며 상악 전치부 부위, 상악 구치부 부위, 하악 전치부 6부위, 하악 구치부 0부위였다. Cone beam CT의 촬영 장치로 CB Mercuray TM (Hitachi, Tokyo, Japan)을 이용하여 DICOM.0 format의 5매 횡단면 영상을 얻었으며 관전압 0kVp, 관전류 5mA, 노출시간 9.6초의 조건하에 촬영하였다. 임프란트 식립부위에 Vimplant.0 TM software (Cybermed, Seoul, Korea)를 이용하여 0mm.5mm의 가상 임프란트를 시뮬레이션 하여 식립하 였다(Fig.,). 그리고 가상 임프란트 주위 mm 골

2 조직의 골밀도를 HU 평균치로 계산하였다(Fig. ). 이를 토대로 임프란트 식립부위 즉, 상악 전치부, 상 악 구치부, 하악 전치부, 하악 구치부의 네 부위에 대 한 HU을 비교하였다. 또한 술중에 임프란트를 식립 한 한명의 술자가 평가한 골질과 비교하여 Mann- Whitney test를 통해 유의성을 검정하였다. 술자는 Lekholm과 Zarb의 분류에 따라 각각의 bone quality 를 분류하였다 ). Implant placement with Vimplant.0 software III. 결과 개의 임프란트 각각의 HU와 골질을 측정한 후 (Table ) 임프란트 식립 부위를 부분으로 나누어 HU와 Lekholm & Zarb에 의한 bone quality를 비교 한 결과 상악의 전치부에서는 HU의 평균값이 66., 상악 구치부에서는 66., 하악 전치부에서는 6., 하악 구치부에서는 8.으로 나타났다(Table ). 골질에 따라 나누어 비교한 결과 Q의 평균 HU값은 96.9 Q는 06.5, Q은 9., Q는 7.0으로 나타났으며 Q는 모두 upper posterior area이었다 (Table ). 또한 술중에 기록된 골질과 술전에 측정된 HU값 사이에는 HU값이 높을수록 bone quality가 좋 음을 보여주었으며, 이는 통계학적으로 유의한 상관 관계를 보여주었다(Fig. ). 4 5 IV. 총괄 및 고찰 불량한 골질에서 임프란트의 실패율이 높은 것으로 많은 문헌에서 보고하고 있으며 5,6,,) 그러므로 시술 전 골의 상태를 평가하는 것은 임프란트의 성공률을 높일 수 있는 한 방법이다. CT를 이용했을 경우 골의 양과 질 모두를 평가할 수 있는 장점이 있으며 HU를 직접적으로 측정할 수 있다. Quantitative conebeam computerized tomography(qcbct)와 quantitative computerized tomography(qct)사이 의 HU를 비교한 논문에서 Aranyarachkul 등은 전반 적으로 QCBCT의 HU가 높았다고 보고하였다 ). Vimplant.0 TM software를 이용한 결과에서 하악 전 치부 대부분이 Q이었고 Q의 전부가 상악 구치 부 였다. 하악 전치부와 구치부에서 대부분 골질이 우수 한 것으로 나타났으며 Hounsfield unit의 평균치도 높 았다. 본 논문의 결과와 유사하게 Rubelisa등도 type은 00HU 이상, type와 은 00에서 00 HU, type는 00이하인 것으로 보고하고 있다 ). 이 번 연구에서 부위별 HU 평균치의 순서는 하악 전치 부>하악 구치부>상악 전치부>상악 구치부로 나타났 으나 악골의 부위별로 나누어 조사한 Turkyilmaz등 의 논문에서는 하악 전치부가 9.9 HU(mean) >상 악 전치부 75.8 HU>하악 구치부 67. HU>하악 구치부 55. HU의 순서를 보여주고 있다 ). Cadaver를 대상으로 한 여러 연구 중에 Shahlaie등 은 9구의 human cadaver의 6개 임프란트 식립 부 위를 조사한 결과 8-65 HU 범위의 bone density가 나타났다고 보고하였다 ). Norton과 Gamble의 연구에서, 개의 CT영상을 대 상으로 Hounsfield unit을 이용하여 골질을 평가하였 다. 가상 임프란트 주변 mm의 골질을 평가하였으며 HU는 77에서 까지 나타났다. 전치부와 구치부 사이의 큰 차이는 없었으며 시술시 type 와 type 두 그룹의 bone quality 차이를 느끼는 것이 어렵기 때문에 한 그룹으로 보았다 5). 또한 Shapurian등은 HU와 골질을 비교한 논문에서 HU와 type 와의 관 계가 가장 유의할 만 하다고 밝혔다 6). 상악과 하악의 피질골과 해면골의 HU을 평가한 Park등의 논문에서, 상악은 견치와 소구치 부위에서 가장 높고, 상악 결절부위에서 가장 낮은 density를 나타냈으며 하악에서는 상악보다 높고, 전치부에서 후구치 부위로 갈수록 높아지는 density를 나타낸다 고 보고하였다 7). Bassi등은 골의 흡수에 따른 bone density를 조사하였는데, 무치악 부위보다 치아가 있 는 부위가, 여성보다 남성에서 density가 높다고 밝혔 다. 그러나 흡수된 비율과 density와는 상관관계가 없 는 것으로 보고하였다 8). Lee등은 삽입토크와 HU사 이에는 높은 상관관계가 있으며 drilling시 bone density를 구별하기 위해서는 80 HU 이상의 차이가 있어야 한다고 하였다 9). 본 연구에서 골질에 따른 HU의 값으로 술전에 골질 을 평가하여 술중에 측정된 골질과의 비교에서, 통계 학적으로 유의한 차이를 보였으며, 이는 술전 CT를 통하여 골량 및 골질에 대한 평가가 이루어질 수 있음 을 보여주었다. 추후 더 많은 데이터를 통하여 HU값 에 대한 기준치의 범위를 제시할 수 있는 연구가 필요 하며, 술전 불량한 골질부위에 임프란트 매식시 불량한 골질의 회복을 위한 외과적 술식 후에 변화된 HU값 측정의 연구가 더 이루어지면 좋을 것으로 사료된다. V. 결론 임프란트 성공률이 많이 향상된 현재에도 실패는 여 전히 존재한다. 그 중 골질이 불량한 상악구치부위의 실패가 많은 것은 사실이다. 본 연구에서 술전 CT를 이용한 골질의 평가에서 술중에 기록된 골질과 상관 관계를 이루는 것을 알 수 있었으며, 또한 술전 CT상 에서 측정된 HU의 값이 골질에 따라 차이가 있음을 알 수 있었다. 이를 바탕으로 골질이 좋지 못한 부위 에 임프란트 매식을 해야 하는 경우에는 술전에 양호 한 골질을 확보하기 위한 외과적 술식을 계획하고 계

Materials & Methods : Thirty-three implant sites of thirteen patients were selected, and bone quality was measured with V implant imaging software(cybermed, Seoul, Korea). Objective scale of the bone density based on the Hounsfield unit and evaluation of correlation between bone density value and subjective quality score when implant was installed. Results : Mean density of Hounsfield unit of Upper anterior area was 66. HU, upper posterior area was 66. HU, lower anterior area was 6. HU, lower posterior area was 8. HU. Mean Hounsfield units according to bone quality was Q-96.9 HU, Q-06.5HU, Q-8.HU, Q-7.0HU. Conclusion : Measurement of Hounsfield unit can be helpful as a valuable supplement to bone density evaluation before implant installation.
